[A report of 136 cases of upper eyelid fold formation together with medial canthoplasty].

OBJECTIVE: To find a new method to perform medial canthoplasty and upper eyelid fold formation at one stage. METHODS: Based on the principle to release the skin tension and minimize incision scarring around the medial canthus, an operation was designed for medial canthoplasty together with upper eyelid fold formation. 136 patients with mild or moderate epicanthus underwent this procedure. Postoperative follow-up was as long as 34 months. RESULTS: Based on the follow-up of 67 cases, the appearances of the upper eyelid fold and medial canthus were evaluated. The upper eyelid fold was the parallel type. The epicanthus was corrected completely or mostly. CONCLUSION: This new method for medial canthoplasty together with upper eyelid fold formation is suitable to all the simple epicanthus except the reverse epicanthus. The operative results were effective and satisfactory.

[AT1 receptor autoantibodies and essential hypertension].

OBJECTIVE: To observe the relationship between AT1 receptor autoantibodies and clinical features in essential hypertension, and to compare the therapeutic effect of losartan between the positive and the negative group. METHODS: Clinical data were collected in patients with essential hypertension (n = 123). AT1 receptor autoantibodies were checked. Twenty-four patients from either AT1-positive or AT1-negative group were treated with losartan for 12 weeks. RESULTS: The average age in the positive group was significantly higher than that of the negative one (P = 0.023). Logistic regression analysis indicated that age was an independant risk factor of AT1 receptor autoantibody. Antihypertensive efficiency of losartan showed no obvious difference between the two groups. CONCLUSION: Positive response of AT1 receptor autoantibody is related only to age. Anti-hypertensive therapy with losartan in the positive group shows no advantage over the negative one, indicating that AT1 receptor autoantibodies possibly have no vascular activity in vivo.

Localization of lens intrinsic membrane protein MP19 and mutant protein MP19(To3) using fluorescent expression vectors.

PURPOSE: [corrected] MP19 is the second most abundant major intrinsic protein of the lens fiber cell membrane. A specific heritable mutation at amino acid 15 in the MP19 protein, termed MP19To3, results in total cataract and microphthalmia in the mouse. The goals of this study were to determine the specific localization of MP19 in the cell membrane and to determine whether the mutant MP19To3 protein migrates to the cell membrane in a similar fashion to normal MP19. METHODS: MP19 and MP19To3 cDNAs were cloned into two different sets of expression vectors. The first set was composed of two vectors, pEGFP-N1 and pDsRed2-N1. The first vector expressed green fluorescent protein and the second expressed a red fluorescent protein when transfected into mammalian cells. The two lens membrane protein cDNAs were separately cloned into the vectors so that the cDNA was at the 5'-end of the fluorescent protein coding DNA. These vectors expressed each of the lens proteins fused to the fluorescent protein upon transfection into mammalian cell cultures. The second vector set was a single vector, pcDNA4/TO which must be induced in the transfected cells by tetracycline in order to express the cloned cDNAs. Each of the membrane cDNAs coupled to the fluorescent protein coding region was cut out of the first vector set and cloned into pcDNA4/TO and stable clones were isolated. Each of the prepared plasmids was transfected into human and chick embryo lens epithelial cells and human T-RexTM-293 cells. The fluorescent cells were viewed using confocal and episcopic-fluorescence microscopy. RESULTS: Each of the transfected plasmids expressed fluorescent protein in all three cell lines. MP19 was observed to transport to the cell membrane. When compared to the distribution of another, separate fusion protein consisting of a signal peptide that targets to cell membranes fused to EGFP, MP19 did not distribute uniformly on the membrane, but appeared to localize into "spots" or pools of fluorescent material around the cell membrane. In contrast, MP19To3 protein appeared to not distribute to the cell membrane; it instead appeared to collect in a particular subcellular compartment within the cell. CONCLUSIONS: The distribution of MP19 and MP19To3 in the cell appeared to be quite distinct. MP19 was observed to distribute to the cell membrane while MP19To3 did not. The fact that the MP19To3 did not traffic to the membrane, instead appearing to be trapped within a subcellular compartment within the cell sheds further light on the cause of the cataract and microphthalmia observed in the MP19To3 mutation, and further sheds information on the pathway of MP19 transport to the cell membrane.

[Characteristics of the sleep apnea syndrome in the aged].

OBJECTIVE: To study the characteristics of the sleep apnea syndrome in the aged. METHODS: Forty-two males who were suspicious of the sleep apnea syndrome (SAS) were detected with 7-hour polysomnography (PSG). RESULTS: The body mass index (BMI) and neck circumference showed no significant difference in the aged and middle aged groups. The apnea/hypopnea index (AHI) and microarousal index (MAI) were significantly lower, and the minimum oxygen saturation (SaO2) significantly higher in the aged than those in the middle aged (P < 0.05). Age was negatively correlated to AHI (r = -0.329, P < 0.05). The BMI and neck circumference were positively correlated to AHI (r = 0.326, P < 0.05; r = 0.306, P < 0.05, respectively). CONCLUSION: SAS patients are less severe in the aged excluding the affection of the BMI and neck circumference.

[Cloning of cDNA fragments of human defensins].

In order to acquire the cDNA of a small molecular peptide with biological activity, the cDNA encoding human defensins was isolated from total RNAs of peripheral leukocyte in a patient with chronic myelogenous leukemia by the RT-PCR method, about 280 bp in length. The RT-PCR products were cloned into the pUCm-T vector, and may be used to construct a novel fusion protein, which contains human defensins and human bactericidal permeability increasing proteins.

Flame ionization detection after splitting the water effluent in subcritical water chromatography.

The coupling of subcritical water separation with flame ionization detection (FID) in the split mode has been investigated in this study. In order to keep the FID system stable during subcritical water separation, a Tee union was connected between the separation column and the FID system to split the water flow. The ratio of the water flow to the FID system over the flow-rate to a waste bottle varied depending on the dimension of capillary tubings and the total water flow-rate used. Separations of several carbohydrates, carboxylic acids, and amino acids were performed on commercially available columns using a laboratory-made subcritical water chromatography-FID system. The FID system was very stable in this split mode even at total flow-rate as high as 1.24 ml/min. The linear dynamic range was up to three orders of magnitude and the limit of detection (LOD) ranged from 38 to 111 ng (306-925 ng/microl injected) with split ratios of approximately 1:10 to approximately 1:17 (FID/waste bottle) for several analytes studied. However, the LOD can be significantly lowered by adjusting the dimensions of the restrictors to allow a higher percentage of the total flow to the FID system.

Anchorage-independent phosphorylation of p130(Cas) protects lung adenocarcinoma cells from anoikis.

The regulation and function of the signaling adaptor protein p130(Cas) in tumor cell anchorage-independent survival, or anoikis resistance, were investigated in human lung adenocarcinoma cells. The tyrosine phosphorylation and function of p130(Cas) during cell detachment were analyzed in tumor cells and compared with that of normal epithelial cells. Cell detachment trigged rapid dephosphorylation of p130(Cas) in the nontumorigenic and anoikis-sensitive normal epithelial cells, but had no effect on the tyrosine phosphorylation of p130(Cas) in the anoikis-resistant lung adenocarcinoma cells. Further analysis revealed that the total tyrosine kinase activities associated with p130(Cas) in the lung tumor cells are anchorage-independent and are significantly higher than that in the normal cells, in which the p130(Cas)-associated tyrosine kinase activities are anchorage-dependent. Analysis of two known p130(Cas)-associated tyrosine kinases FAK and Src indicated that the regulation of tyrosine phosphorylation of FAK and Src are altered in the tumor cells. Inhibition of Src specifically abolished phosphorylation of p130(Cas) and induced anoikis. Furthermore, overexpression of dominant-negative forms of p130(Cas) also induced apoptosis. Taken together, these data suggest that p130(Cas) mediates a cell survival signal from cell-matrix interaction. Alterations in tumor cells that lead to constitutive phosphorylation of p130(Cas) can prevent cells from anoikis, hence contribute to tumor cell anchorage independence and metastasis.

Expression of cytokeratin 8 in lung cancer cell lines and measurement of serum cytokeratin 8 in lung cancer patients.

It has been reported that cytokeratin 8 (CK8) can be expressed in several cancers and expression of CK8 is correlated with increased invasiveness of the tumor in vitro and in vivo. In the present study, we investigated expressions of CK8 in human lung cancer cell lines. In addition, we also evaluated the clinical significance of CK8 measurements in sera of patients with lung cancer. Expression of mRNA for CK8 was semi-quantitatively evaluated by the competitive reverse transcriptase-polymerase chain reaction (competitive RT-PCR), using human lung cancer cell lines. The level of CK8 protein in culture supernatants of lung cancer cell lines and 70 sera of patients with lung cancer was measured by enzyme-linked immunosorbent assay (ELISA). Levels of serum CK8 according to clinical parameters were also examined. The level of expression of CK8 mRNA in non-small cell lung cancer (NSCLC) cell lines was significantly high compared with that of small cell lung cancer (SCLC) cell lines (P<0.05). The level of CK8 in culture supernatants in NSCLC was significantly high compared with that of SCLC. The level of serum CK8 in patients with NSCLC was significantly high compared with that of normal non-smokers and compared with that of SCLC (P<0.05). Patients with a CK8 value of 50.0 ng/ml, or higher, had a statistically significant diminished survival compared with those patients whose CK8 values were lower. In conclusion, CK8 was preferentially expressed in NSCLC. Increasing values of CK8 were significantly associated with tumor progression and decreased survival in patients with NSCLC. Therefore, CK8 in sera may become a novel tumor marker in patients with lung cancer.

Assessment of myocardial viability in prior myocardial infarction by intravenous bolus microbubble injection: a new time domain index to estimate regional relative myocardial blood volume.

We tested whether the duration of myocardial opacification by harmonic power Doppler imaging after intravenous bolus microbubble injection (with a definition of "the end of opacification") would reflect the remaining vascular bed in infarcted segments. In 28 patients with previous myocardial infarction and 20 control patients, we performed harmonic power Doppler imaging after intravenous bolus injection of 1.5 g of Levovist. Using multiframe trigger mode in which 4 consecutive frames were imaged at every sixth end systole, which formed 1 "burst," we recorded anterior/septal and inferior/posterior walls separately on the center of each apical view with individual boluses. The duration of segmental opacification was measured as the number of "bursts" in which color signals persisted until the fourth frame. The duration was similar between the anterior/septal and inferior/posterior walls (13 +/- 3 vs 13 +/- 3 bursts, not significant) in the control group. In myocardial infarction patients, the duration was significantly shorter in the infarcted than in the control segments (6 +/- 6 vs 14 +/- 3 bursts, P < .001) and their ratio and difference exhibited significant correlations (r = 0.82, P < .001 and r = 0.91, P < .001, respectively) with the activity ratio on thallium Tl 201 single-photon emission computed tomography at rest. Thus, the duration of opacification by harmonic power Doppler imaging after intravenous bolus microbubble injection, the measurement of which was standardized by using multiframe trigger mode, may be useful in assessing regional myocardial viability in patients with previous myocardial infarction.

Temperature effect on peak width and column efficiency in subcritical water chromatography.

Subcritical water has been recently employed as the mobile phase to eliminate the use of organic solvents in reversed-phase liquid chromatography. Although the influence of temperature on retention in subcritical water chromatography has been reported, the temperature effect on peak width and column efficiency has not yet been quantitatively studied. In this work, several polar and chlorinated compounds are separated using pure subcritical water on Zorbax RX-C8, PRP-1 (polystyrene-divinylbenzene), Hypersil ODS, and ZirChrom-polybutadiene columns. Isothermal separations are performed at temperatures ranging from 60 degrees C to 160 degrees C. The retention time and peak width of analytes are reduced with increasing temperature. However, the column efficiency is either improved or almost unchanged with the increasing temperature in the low-temperature range (lower than the 100 degrees C to 120 degrees C range), but it is decreased when temperature is further raised in the high-temperature range (higher than the 100 degrees C to 120 degrees C range). Therefore, a maximum in column efficiency is obtained at temperatures within the 100 degrees C to 120 degrees C range in most cases.

Evaluation of the clonality of multilobar bronchioloalveolar carcinoma of the lung: case report.

A case of multilobar bronchioloalveolar carcinoma (BAC) is reported. To investigate the clonality of BAC, immunohistochemical staining as well as genetic analysis were performed. To investigate point mutations of the p53 gene, we used the polymerase chain reaction and fluorescence-based single strand conformation polymorphism analysis method. The BAC tissues of the right upper lobe, right lower lobe, and the other lobes were considered to be multiclonal. This case suggests that multilobar BAC might occur with multiclonality.

Chronological evaluation of the onset of histologically confirmed interstitial pneumonia associated with polymyositis/dermatomyositis.

OBJECTIVE: The present study was designed to determine the chronological sequence of interstitial pneumonia, skin involvement, and muscle involvement associated with polymyositis/dermatomyositis (PM/DM). METHODS: We examined our own cases of histologically confirmed interstitial pneumonia associated with PM/DM. In addition, a review of the literature was done to evaluate other cases of histologically confirmed interstitial pneumonia associated with PM/DM. Lung involvement was the first clinical symptom for all of our 8 patients. RESULTS: Including the literature review and our 8 cases, there were 94 patients with PM/DM in whom interstitial pneumonia was histologically confirmed (36 PM, 50 DM, and 8 amyopathic DM). Chronological evaluation between the diagnosis of PM/DM and lung involvement demonstrated that most lung involvement occurred just before or just after the diagnosis of PM/DM. Interstitial pneumonia was preceded in 35 of 87 evaluable patients [21 cases with PM (61.8%), 14 cases with DM and amyopathic DM (40.2%)]. In 60 of 87 evaluable patients (69.0%), lung involvement occurred as a clinical manifestation at the diagnosis of PM/DM. CONCLUSION: The present data demonstrate that interstitial pneumonia was frequently the initial onset in patients with PM/DM.

[The clinical effects of percutaneous lumbar discectomy combined with sodium hyaluronate in the treatment of lumbar intervertebral disc herniation].

OBJECTIVE: To investigate the therapeutic effect of percutaneous lumbar discectomy (PLD) combined with sodium hyaluronate (SH) injection in the treatment of lumbar intervertebral disc herniation. METHODS: Forty-eight patients suffered from lumbar disc herniation were divided into two groups and treated by PLD combined with SH injection into epidural cavity (treatment group) or single PLD (control group) respectively. All patients were followed up for 24 months. The therapeutic effects in both groups were assessed and compared according to Macnab's criterion. RESULTS: The patients in the treatment group got much more significant improvement than those in the control group, with shorter therapeutic course and more safety. CONCLUSION: PLD combined with SH injection into epidural cavity is more effective and safety in the treatment of lumbar disc herniation than of pure PLD.

[Restriction endonucleases fingerprinting-single strand conformation polymorphism: an efficient method to screen mutations in long segments].

OBJECTIVE: To develop a simple, cheap and efficient restriction endonucleases fingerprinting-single strand conformation polymorphism(REF-SSCP) method applied to screen for mutations in long segments. METHODS: The genomic DNA of Cx26 gene segment of the patients with deafness was amplified. The amplification products were screened with SSCP and REF-SSCP technique and DNA sequencing to evaluate and compare the effect on detection of mutations in long segments. RESULTS: No different band was found in 724 bp segment in SSCP examination. Three kinds of different bands were discovered in REF-SSCP examination and the 79 G -->A mutation detected by DNA sequencing were accorded with the REF-SSCP bands entirely. The rate of detection was 100%. CONCLUSION: The present REF-SSCP method is applicable to screen mutations in long segment DNA of mass specimens.

Relationship between sleep apnea hypopnea syndrome and cardiovascular events in elderly Chinese snorers.

OBJECTIVE: To investigate the relationship between sleep apnea hypopnea syndrome (SAHS) and some cardiovascular abnormalities in elderly snorers, as well as the effectiveness of nasal continuous positive airway pressure on those with SAHS. METHODS: With the use of polysomnography, 73 elderly snorers (older than 60 years) were examined and placed into either the SAHS group or the control group. Using ambulatory electrocardiogram (ECG) and blood pressure measurement, daily nocturnal rhythm of blood pressure, hypertension, heart rate variability, some arrhythmia and angina pectoris of coronary heart disease (CHD) were monitored and compared between the two groups before and after 5 - 7 days of treatment with nasal continuous positive airway pressure on the SAHS group. RESULTS: This study indicated a higher incidence (47.9%) of sleep apnea syndrome in elderly snorers and demonstrated that there was a significantly higher incidence of hypertension, disappearance in daily nocturnal rhythm of blood pressure, poor effectiveness of nitrate on angina pectoris of coronary heart disease, decreased heart rate variability during sleep, increased arrhythmia and lower pulse oxygen saturation (SpO(2)) levels in the SAHS group than in the control group. After nasal continuous positive airway pressure treatment during sleep, snoring control, significantly higher SpO(2) levels and lower index of apnea/hypopnea were achieved in the SAHS group; heart rate variability (HRV) and blood pressure day nocturnal rhythm were returned to normal levels. CONCLUSION: This research suggests that there is a close relationship between the development of sleep apnea syndrome and some cardiovascular diseases. Continuous positive nasal airway pressure is effective not only on SAHS but also on coexisting cardiovascular disorders.

Structure and Function of a Novel Silencer in 5' Flanking Distal Region of Human beta Globin Gene.

A novel silencer fragment(310 bp) was recently discovered and identified to locate between -2 132 bp and -1 822 bp upstream from the cap site of beta-globin gene by gel retardation assay and luciferase reporter gene expression analysis. DNA footprinting assays were performed to determine the interaction between its DNA sequence and binding proteins from the nuclear extract of Hela cells. The results showed that there were two nuclear protein binding sites in thissilencer, one was the -2 017--2 011 bp sequence CTTCCGC" and the other was the -2 006--1 997 bp sequence "CACTTTATTT". Two sequences were mutated into "CTTAAGC" and "CACTTAAGTT", respectively by two mutagenic primer pairs, in order to construct two mutation types of the 310 bp fragment. The competitive gel retardation assays showed that two mutation types of the 310 bp fragment and their four smaller DNA fragments, which were formed respectively by the digestion of restriction enzyme BspTI, all lost their competitive ability against the wild type of 310 bp fragment probe for DNA-binding proteins without exception. Furthermore, the double-strand oligonucleotides, which contained both the sequences of "CTTCCGC" and "CACTTTATTT", were synthesized, and the competitive gel retardation assays showed that they competed ability against wild type 310 bp fragment probe for DNA-binding proteins. The results suggest that two binding sites of the nuclear proteins are involved or associated with a potential DNA-DNA interaction. Moreover, the specific DNA-binding proteins were purified from the nuclear extract of Hela cells by using "DNA-binding protein purification kit" for magnetic isolation. In order to identify the purified DNA-binding proteins, a SDS-PAGE was performed. By using the silver staining, the PAGE electrophoretogram showed that these two nuclear proteins specifically bound to these two sites of the silencer, appearing as two definite bands. The molecular weight of each protein was determined to be 37 kD or 81 kD, respectively.

Function and Identification of a Novel Silencer in 5' Flanking Distal Region of Human beta Globin Gene.

In order to inquire into the possible function of the sequence in the 5' flanking distal region and/or delta-beta intergene of human beta-globin gene several recombinant retrovirus vectors containing beta-globin gene with different length of 5' flanking sequences were constructed and transfected into Hela cells. The levels of beta-globin gene expression determined quantitatively by Northern blotting and RT-cPCR revealed that there is a strong silencer activity in a DNA sequence(723 bp) between-2282 bp and -1559 bp upstream from the cap site of beta-globin gene. The silencer activity was further identified to be possibly located mainly on a 310bp DNA restriction fragment by using the competitive gel retardation assay(cGRA) and many known regulatory elements were found present in it as well. Luciferase reporter gene expression system was adopted to study the function of this 310bp fragment and data showed that the fragment exhibited significant inhibitory effect on each of the recombinant vectors with alpha, beta or delta gene promoters. The above results suggest that this silencer may be involved in a potential regulatory network between delta and beta globin genes and may play an important role in the regulation of globin gene expression.
