Rehabilitation of failing dentition with immediate denture prostheses: technique for a predictable esthetic and functional outcome.

The goal of this article is to describe the clinical and laboratory procedures used in the fabrication of immediate complete and removable partial denture prostheses. In this type of treatment, achieving optimal esthetic and functional outcomes is complicated because of the absence of the try-in stage. To overcome this limitation, a technique that predictably transfers the information obtained during esthetic dentofacial analysis from the patient to the laboratory in the form of an incisal-occlusal plane guide is described. Stability, support, and retention of the immediate partial denture prosthesis can be improved with the incorporation of cast clasp assemblies, and its fabrication technique is presented in this article.

Implant-retained overdenture design for the malpositioned mandibular implants.

In the rehabilitation of the edentulous mandible, implant-retained overdenture represents a viable and cost-effective treatment. Successful prosthetic integration of this treatment relies on the accurate 3-dimensional position and angulation of the implant fixtures. If these requirements are not fulfilled, implant malposition occurs and negative effects on the function, esthetics, and structural integrity of the prosthesis may follow. Implant malposition commonly results from poor planning, surgical error, inaccuracies in the radiographic/surgical template fabrication, and the instability of the template during surgery. Potential solutions for implant malposition include surgical re-treatment or prosthetic compensation in the form of an altered design of the final prosthesis. This article presents a prosthetic design of an overdenture for malpositioned implants in the anteroposterior plane. Prosthetic compensations included relocating the attachment mechanism into the bulk of the overdenture with the help of an interconnecting implant bar and a metal reinforcing framework for the overdenture.

Fatigue resistance of endodontically treated teeth restored with three dowel-and-core systems.

STATEMENT OF PROBLEM: The successful restoration of endodontically treated teeth is enhanced by a crown design employing the ferrule effect. However, it is unclear which dowel-and-core system most effectively supports successful treatment. PURPOSE: The purpose of this study was to compare the load fatigue resistance of 3 dowel-and-core systems. MATERIAL AND METHODS: Fifteen endodontically treated maxillary central incisors were sectioned perpendicular to the long axis at a point 1.5 mm incisal to the cemento-enamel junction (CEJ). At the level of the CEJ, specimens were then prepared for crowns with 1-mm complete shoulder finish lines and 1.5 mm of axial wall height. The prepared teeth were divided into 3 groups (n=5) and restored with 1 of the following dowel-and-core combinations: Group CG, cast gold dowels and cores; Group TA, titanium alloy dowels (ParaPost XH) with composite cores; or Group FR, fiber-reinforced resin dowels (ParaPost FiberWhite) with composite cores. A dentin bonding agent (OptiBond Solo) was placed prior to the composite cores. Dowel-and-core castings and titanium alloy dowels were cemented with zinc phosphate cement. The fiber-reinforced dowels were cemented with a resin cement (ParaPost Cement). The crowns for all specimens were cast with an incisal notch for applying the fatigue load. The independent variable measured was the number of load fatigue cycles required to cause luting cement failure. The data were subjected to 1-way analysis of variance and the Student-Newman-Keuls test for 3 subsets (alpha=.05). RESULTS: The mean value+/-standard deviation for the cycles to failure for each group was: Group CG: 11,897+/-4080 load cycles, Group TA: 24,384+/-8231 load cycles, and Group FR: 50,696+/-7063 load cycles. Significant differences were found between all groups ( P<.05). CONCLUSIONS: Fiber-reinforced resin dowels and bonded composite cores under fatigue loading provided significantly stronger crown retention than cast gold dowels and cores and titanium alloy dowels with composite cores under fatigue loading.

The edentulous patient and body image--achieving greater patient satisfaction.

UNLABELLED: Aesthetic outcomes in the rehabilitation of edentulous patients play an important role in regard to treatment satisfaction and its acceptance. While the dental literature is replete with techniques and treatment philosophies that allow the fabrication of aesthetic restorations, adhering to such mechanical practices alone does not necessarily guarantee patient satisfaction. Taking into consideration the patient's perspective can significantly enhance treatment success. This article outlines the relevant factors that influence the aesthetic result of complete denture treatment. LEARNING OBJECTIVES: This article describes factors that contribute to patients' satisfaction with denture aesthetics and provides guidelines for clinicians to follow in daily practice. Upon reading this article, the reader should be able to: Understand the role that a patient's body image plays in an aesthetic outcome. Recognize contemporary aesthetic trends that influence denture aesthetics.

Complete sequencing and characterization of 21,243 full-length human cDNAs.

As a base for human transcriptome and functional genomics, we created the "full-length long Japan" (FLJ) collection of sequenced human cDNAs. We determined the entire sequence of 21,243 selected clones and found that 14,490 cDNAs (10,897 clusters) were unique to the FLJ collection. About half of them (5,416) seemed to be protein-coding. Of those, 1,999 clusters had not been predicted by computational methods. The distribution of GC content of nonpredicted cDNAs had a peak at approximately 58% compared with a peak at approximately 42%for predicted cDNAs. Thus, there seems to be a slight bias against GC-rich transcripts in current gene prediction procedures. The rest of the cDNAs unique to the FLJ collection (5,481) contained no obvious open reading frames (ORFs) and thus are candidate noncoding RNAs. About one-fourth of them (1,378) showed a clear pattern of splicing. The distribution of GC content of noncoding cDNAs was narrow and had a peak at approximately 42%, relatively low compared with that of protein-coding cDNAs.

A light-polymerized resin support tray as an aid for recording centric relation.

For patients who require complex fixed prosthodontic oral rehabilitation, recording an accurate centric relation is often a difficult procedure. A minimal number of abutment teeth or implants may not offer adequate support for the occlusal registration material. In the described technique, Triad tray material is used as an occlusal registration supporting tray. This procedure is accurate and convenient for the restorative dentist.

Custom precision attachment housings for removable partial dentures.

Limited space for extracoronal attachments is a serious complication in the design and fabrication of a precision attachment removable partial denture. A custom metal-ceramic housing joined to the partial framework with laser welding offers strength and improved esthetics while requiring minimal space. This article presents the technical aspects of custom housing construction.

A < 1.7 cM interval is responsible for Dmo1 obesity phenotypes in OLETF rats.

1. Dmo1 (Diabetes Mellitus OLETF type I) is a major quantitative trait locus for dyslipidaemia, obesity and diabetes phenotypes of male Otsuka Long Evans Tokushima Fatty (OLETF) rats. 2. Our congenic lines, produced by transferring Dmo1 chromosomal segments from the non-diabetic Brown Norway (BN) rat into the OLETF strain, have confirmed the strong, wide-range therapeutic effects of Dmo1 on dyslipidaemia, obesity and diabetes in the fourth (BC4) and fifth (BC5) generations of congenic animals. Analysis of a relatively small number of BC5 rats (n = 71) suggested that the critical Dmo1 interval lies within a < 4.9 cM region between D1Rat461 and D1Rat459. 3. To confirm the assignment of the Dmo1 critical interval, we intercrossed BC5 animals to produce a larger study population (BC5:F1 males; n = 406). For the present study, we used bodyweight at 18 weeks of age as an index of obesity; this phenotype is representative of the closely associated dyslipidaemia and hyperglycaemia phenotypes. 4. Interval mapping assigned logarithm of odds (LOD) peaks at the D1Rat90 marker (LOD = 9.11). One LOD support interval lies within the < 1.7 cM region between D1Rat461 and D1Rat459. 5. This large intercross study confirms that Dmo1 is likely localized within the interval.

Rehabilitation of the edentulous mandible with a fixed implant-supported prosthesis.

UNLABELLED: Implant rehabilitation of an edentulous mandible presents significant improvements over conventional complete denture therapy with regard to patient satisfaction, and it can improve a patient's quality of life. Several implant prosthetic designs have been developed and successfully utilized in clinical practices. The goal of this article is to describe the prosthetic design and technical steps in the fabrication of a fixed implant-supported mandibular prosthesis. A technique is described in which the positioning of appropriately selected abutments is completed in a predictable manner. LEARNING OBJECTIVES: This article discusses the prosthetic designs and technical steps in fabricating a fixed implant-supported mandibular prosthesis. Upon reading this article, the reader should: Understand the prosthetic design for the treatment of an edentulous mandible. Recognize the significance of the technical steps in this rehabilitation process.
