[An evaluation of the speech therapy methods for patients after pharyngoplasty.].

PURPOSE: Speech therapy was carried out for patients with speech disorders after pharyngoplasty, and the comparison of speech articulation before and after treatment, was done to evaluate the new method. METHODS: 82 patients with the varying age from 4 to 31 years were included in the current study. The patients' speeches including syllables, word, phrase and sentence were compared before and after treatment according to Chinese Speech Intelligibility Testing Form. The systematic speech training was carried out according to the compensatory articulation characteristics. The steps were as follows. Firstly, the function of velopharyngeal closure should be strengthened. Secondly, the patients were trained with an average duration of 3.83 months (range from 1.5-12 months) according to the following three sequences: One was from phoneme, syllables, phrase, sentence to short paragraph or dialogue, another was from anterior to posterior according to the speech position, and the other was from easy to difficult according to the articulation method, such as from aspirated syllables to non-aspirated syllables, from plosive, fricative to affricative. RESULTS: There were 71 patients with compensatory articulation characteristic of velopharyngeal incompetence, the speech articulation of patients with glottis plosive increased from preoperative 46.27% to postoperative 97.16% (P<0.001). The speech articulation of patients with pharynx fricative increased from preoperative 57.19% to postoperative 97.72% (P<0.001). There were 11 patients with compensatory articulation characteristic of velopharyngeal adequacy, and the speech articulation of 11 patients with palatalized and lateral misarticulation increased from preoperative 71.10% to postoperative 98.55% (P<0.001). CONCLUSIONS: The training of velopharyngeal closure should be emphasized on the patients with compensatory articulation resulting from velopharyngeal incompetence after pharyngoplasty, then the correct speech position and method was established. While to the patients with compensatory articulation without velopharyngeal incompetence after pharyngoplasty, the correct speech position and method could be directly established.

Membrane protein folding: beyond the two stage model.

The folding of alpha-helical membrane proteins has previously been described using the two stage model, in which the membrane insertion of independently stable alpha-helices is followed by their mutual interactions within the membrane to give higher order folding and oligomerization. Given recent advances in our understanding of membrane protein structure it has become apparent that in some cases the model may not fully represent the folding process. Here we present a three stage model which gives considerations to ligand binding, folding of extramembranous loops, insertion of peripheral domains and the formation of quaternary structure.

Screening of inhibitors using enzymes entrapped in sol-gel-derived materials.

In recent years, a number of new methods have been reported that make use of immobilized enzymes either on microarrays or in bioaffinity columns for high-throughput screening of compound libraries. A key question that arises in such methods is whether immobilization may alter the intrinsic catalytic and inhibition constants of the enzyme. Herein, we examine how immobilization within sol-gel-derived materials affects the catalytic constant (kcat), Michaelis constant (KM), and inhibition constant (KI) of the clinically relevant enzymes Factor Xa, dihydrofolate reductase, cyclooxygenase-2, and gamma-glutamyl transpeptidase. These enzymes were encapsulated into sol-gel-derived glasses produced from either tetraethyl orthosilicate (TEOS) or the newly developed silica precursor diglyceryl silane (DGS). It was found that the catalytic efficiency and long-term stability of all enzymes were improved upon entrapment into DGS-derived materials relative to entrapment in TEOS-based glasses, likely owing to the liberation of the biocompatible reagent glycerol from DGS. The KM values of enzymes entrapped in DGS-derived materials were typically higher than those in solution, whereas upon entrapment, kcat values were generally lowered by a factor of 1.5-7 relative to the value in solution, indicating that substrate turnover was limited by partitioning effects or diffusion through the silica matrix. Nonetheless, the apparent KI value for the entrapped enzyme was in most cases within error of the value in solution, and even in the worst case, the values differed by no more than a factor of 3. The implications of these findings for high-throughput screening are discussed.

The product of the candidate prostate cancer susceptibility gene ELAC2 interacts with the gamma-tubulin complex.

ELAC2 is a novel candidate cancer susceptibility gene located on chromosome 17p: Carriers of mutations in ELAC2 display a higher risk of developing prostate cancer. Overexpression of ELAC2 in tumor cells causes a delay in G2-M progression characterized by accumulation of cyclin B levels. Consistent with a function in mitosis, further biochemical analysis revealed that ELAC2 physically interacts with the gamma-tubulin complex. This is the first biologic insight into the function of this new putative cancer susceptibility gene, providing clues of how perturbation of ELAC2 might promote tumorigenesis through irregular cell division.

Hepatoprotective and antihyperliposis activities of in vitro cultured Anoectochilus formosanus.

The pharmacological effects of an aqueous extract of the whole plants of in vitro cultured Anoectochilus formosanus were investigated experimentally for hepatoprotective and antihyperliposis activities. The extract showed significant antihepatotoxic activity against carbon tetrachloride induced cytotoxicity in primary cultured rat hepatocytes. In an assay for antihyperliposis using aurothioglucose-induced obese mice, the extract suppressed significantly the increase in the weights of body and liver, ameliorated triglyceride levels in the liver and serum, and also significantly reduced the deposition of adipose tissue.

Ratio of leptin to adiponectin as an obesity index of cynomolgus monkeys (Macaca fascicularis).

Obesity is responsible for inducing various metabolic diseases. Laboratory-bred cynomolgus monkeys exhibit spontaneous onset of obesity. However, to date, no blood chemistry index to identify the state of obesity in cynomolgus monkeys has been determined. In the present study, to determine such an index, we measured the serum levels of two adipocyte-derived hormones, leptin and adiponectin, and evaluated the relationship between these hormones and other serum energy metabolic factors (i.e. insulin, total protein, glucose, total cholesterol and triglyceride) as well as the percentage of body fat (%Fat) in mature cynomolgus monkeys. Both in females and males, leptin was positively correlated with insulin and %Fat, and adiponectin was negatively correlated with insulin and %Fat. In female cynomolgus monkeys, leptin, adiponectin, and glucose were selected as the most important determinants for %Fat in multiple regression analysis, and in male cynomolgus monkeys, leptin was selected. The ratio of leptin to adiponectin (L/A ratio) was significantly elevated in the animals with %Fat over 40 (P < 0.01). The results indicate that L/A ratio is a potential index for comprehensively identifying obesity in cynomolgus monkeys.

[Subjective evaluation of acoustic changes before and after restoration with complete denture].

OBJECTIVE: To evaluate clinic adaptive effect of speech articulation of patients before and after restoration with complete denture. METHODS: Before and after patient's restoration at the beginning and then at 1 week, 2 weeks, 4 weeks and 8 weeks, the patient's self-judgement and the measurement of Chinese intelligibility by speech pathologist were used to analyse the regularity of speech articulation of edentulous patients restored with complete denture. RESULTS: At the beginning, the patients in two groups all felt uncomfortable of using denture. After one week, patients using metal base denture adapted well, but patients using plastic base denture needed 2 weeks to adapt. The data of Chinese intelligibility showed significant difference between two groups after using denture 1 week (P<0.05). In metal base group, there was significant difference before and after using denture a week (P<0.01). The plastic base group showed this change after two weeks. CONCLUSION: Patient's phonetic function recovered quickly when the thickness of the base reduced and palatal rugae of the denture base restored. The chinese intelligibility of them was better.

[The influence of botulinum toxin type A on vasomotor rhinitis and morphological study].

OBJECTIVE: To investigate the effect and mechanisms of botulinum toxin type A(BTA) on vasomotor rhinitis. METHOD: A retrospective study of 32 patients with vasomotor rhinitis who received BTA injection at 4 points in bilateral nasal cavities with 2.5 units at each point had been done. The state of rhinorrhea, nasal obstruction, sneeze and nasal mucosa edema of each patient were recorded and scored before and after the application of BTA respectively with follow-ups of 6-30 months. Twelve guinea pigs were randomly divided into 2 groups: BTA group and control group. Merocel sponge soaked with 10 units (0.2 ml) BTA was put into left nasal cavity under general anesthesia for one hour in BTA group. Saline replaced BTA in control group. The inferior turbinate mucosa of guinea pig ware harvested under general anesthesia at 7, 14, 28 days after BTA treatment and was observed with HE staining and electron microscope. RESULT: Rhinorrhea improved significantly in all 32 patients while sneeze, nasal obstruction and mucosa edema reduced partly. The reducing of rhinorrhea lasted averagely 6 weeks. Twelve patients had an episode of nasal dryness or increased rhinorrhea at the day of injection. No other complications were found. Seven, fourteen days after local BTA application, the degenerations of glandular epithelium and canal epithelium were observed in nasal mucosa of guinea pigs. Most of the glandular cells showed normal at 28 days after BTA treatment. CONCLUSION: BTA can induce the degeneration of glandular cells in nasal mucosa and thus obviously relieve over rhinorrhea of patients with vasomotor rhinitis.
