The double transposition flap for closure of the extremely wide hard palate cleft.

Repair of the extremely wide hard palate cleft can be done effectively with a double transposition flap. All cases referred for closure of cleft palates from 1997 to 2005 were considered for this study. Of these, 6 were classified as extremely wide cleft palates thought not to be treatable with standard closure techniques. A double transposition flap was used in each case. All patients tolerated the procedure well; no flap failure or dehiscence was noted; and none has required secondary repair. All patients tolerated an appropriate diet following flap repair. Two patients who were gastrostomy tube dependent preoperatively no longer required gastrosotomy tubes postoperatively.

Metastatic neuroendocrine tumors of the parotid gland.

OBJECTIVES: To present 2 cases of neuroendocrine tumors of the parotid gland and to review the clinical presentation, histopathologic, and immunohistochemical findings and treatment of these rare tumors. STUDY DESIGN: Case series. METHODS: This study reviews 2 patients who both presented with firm masses in the parotid gland diagnosed as neuroendocrine carcinoma by histopathologic and immunohistochemical examination. The evaluation, diagnosis, and treatment are discussed. RESULTS: Both patients underwent complete tumor extirpation via a total or subtotal parotidectomy. Both patients received postoperative radiation therapy and chemotherapy. CONCLUSIONS: Neuroendocrine tumors of the parotid gland are uncommon lesions; however, they have a favorable prognosis as compared to their pulmonary counterparts with early diagnosis and aggressive treatment. Diagnosis is based on the immunohistochemical characteristics of the tumor. Appropriate treatment includes complete surgical excision with postoperative radiation therapy to the parotid bed with the addition of postoperative chemotherapy in patients suspected to have metastatic disease.

Metastatic breast cancer of the oral cavity.

OBJECTIVE: To present a rare case of breast carcinoma initially presenting as an oral cavity mass. STUDY DESIGN: Case report with review of the literature. RESULTS: A case is presented of a postmenopausal woman with a remote history of treated breast carcinoma who was declared disease-free 2 years before presentation. She presented with an enlarging vestibule mass consistent with metastatic breast carcinoma. Subsequent investigations revealed widely metastatic disease for which the patient was referred for palliative chemotherapy. CONCLUSIONS: Metastatic disease to the oral cavity represents only 1% of all oral cavity malignancies. A high index of clinical suspicion is necessary when evaluating patients with a history of non-head and neck carcinoma.

Frontal sinus hamartomas.

OBJECTIVE: To review the entity of respiratory epithelial adenomatoid hamartoma of the paranasal sinuses using an illustrative case example of frontal sinus hamartoma. METHODS: Case report with review of the literature. RESULTS: Respiratory epithelial adenomatoid hamartoma represents a benign process that is generally cured with conservative surgical resection of the involved area. Nevertheless, as shown in our case example, this can be a locally aggressive process that can lead to significant morbidity if left untreated. CONCLUSION: Hamartomas of the paranasal sinuses represent a rare entity, which should be considered in the differential diagnosis of expansile sinonasal lesions. Treatment of choice is surgical resection. Frontal sinus involvement may extend into the surrounding orbit and intracranial cavities, which may need to be addressed with standard skull base surgery techniques.

Evaluation of fibula free flap donor site morbidity.

OBJECTIVE: To evaluate foot and ankle function in a series of patients undergoing fibula microvascular free tissue transfer. STUDY DESIGN: A nonrandomized, nonblinded analysis was performed using the ankle-hindfoot scale as well as radiographic analysis to determine donor site morbidity. RESULTS: Overall, foot ankle function was deemed to be good in this series of patients. Average ankle-hindfoot score was 84.82 of a possible 100 (range, 55-100). Radiographic analysis of donor and contralateral ankles showed no difference in the syndesmotic space or medial clear space. Only 1 patient had increased talar tilt compared with the contralateral side, although this patient had preservation of stability. CONCLUSION: Fibula free flap harvest appears to be associated with acceptable donor site morbidity and preservation of good foot and ankle function in most individuals.

Temporal bone chondroblastomas.

OBJECTIVES: To review temporal bone chondroblastomas in regards to their presentation, radiographic findings, histopathology, and treatment.Study design Case report and literature review. METHODS: A case report of a 38-year-old man is reviewed who presented with left-sided mixed hearing loss, otalgia, otorrhea, and a left external auditory canal mass. A computed tomography scan revealed a 6-cm mass involving the petrous and squamous portions of the left temporal bone. CONCLUSIONS: Temporal bone chondroblastomas are extremely rare osseous tumors with only 35 cases previously reported in the literature. Presenting symptomatology and pathology may be confused with more common lesions seen in the temporal bone. Diagnostic radiology, including computed tomography and/or magnetic resonance imaging, as well as immunohistochemical staining with S-100 protein may assist in making the diagnosis. Treatment is complete surgical excision with preservation of vital neurovascular structures.

Thyroglossal duct cysts in the elderly population.

Thyroglossal duct cysts may present at any age. However, only rarely do they first present in elderly patients. We outline a series of thyroglossal duct cysts in elderly patients at John Peter Smith Hospital, Fort Worth, TX. There appears to be a remarkable rate of malignant transformation noted in this subset of the population. We would therefore suggest early and definitive removal of all suspected thyroglossal duct cysts presenting in elderly individuals.
