[Detection of p53 gene alteration in sputum sample and its implications in early diagnosis of lung cancer].

OBJECTIVES: To evaluate the value of detecting point mutation of p53 gene in sputum sample and its validity and reliability as a surveillance index in early diagnosis of lung cancer in suspicious patients. METHODS: Sputum samples were collected from 54 cases identified as lung cancer and 114 cases as pulmonary benign disease. The polymerase chain reaction-single strand conformation polymorphism (PCR-SSCP) was performed for detection of point mutation at exon 5 - 8 of p53 gene and sputum smear also used for each sample. RESULTS: A detection rate of 55.56% (30/54) in the lung cancer group is significantly higher than that of 1.75% (2/114) in the control (P < 0.001). In the lung cancer group, the sensitivity, specificity, and positive likelihood ratio of p53 alterations as a diagnostic marker of lung cancer were 55.56%, 98.25% and 31.75 respectively. Moreover, the detection rates were 35.19% (19/54) by smears and 55.56% (30/54) by PCR-SSCP-silver stain; both the difference and consistency were statistically significant (P < 0.01 and P < 0.05 respectively). Statistic analysis did not show any significant relation between p53 alteration and clinical parameters such as gender, smoking habits, histotypes and stages, but the detection rate of p53 alteration in older patients (>or=60 years old) was significantly higher than that in younger (P = 0.02). One case with p53 alteration at exon 5 in the control group was confirmed to be squamous carcinoma after 4 years of follow-up. CONCLUSION: Detection of p53 gene alteration in sputum sample by PCR-SSCP-silver stain can be used as a follow-up surveillance index for early diagnosis of lung cancer in suspicious patients.

[Alveolar macrophage and airway inflammation in patients with chronic obstructive pulmonary disease].

OBJECTIVE: To investigate the effects of alveolar macrophage(AM) on airway inflammation of chronic obstructive pulmonary disease(COPD). METHODS: Broncho alveolar fluid(BALF) and bronchial mucosa biopsies were performed as usual. The levels of macrophage inflammatory-1 alpha (MIP-1 alpha), gelatinase B (MMP-9), interleukin(IL)-8 in BALF and the culture supernatants of AMs were measured with ELISA. The levels of endothelin (ET) were measured with radioimmunoassay method. The AMs expressing lymphocytic functional associated antigen-1 (LFA-1) in BALF, the AMs in mucosa and the thickness of mucosa were measured with immunocytochemistry and immunohistochemistry technique. RESULTS: The levels of MIP-1 alpha, MMP-9, IL-8 and ET in the culture supernatants of AMs in patients with COPD were (1 446.9 +/- 126.7) microg/L, (16.98 +/- 5.15) microg /L, (43.98 +/- 5.54) ng/L, (168.10 +/- 21.22) microg/L respectively; while those were (1 123.5 +/- 90.6) microg/L, (5.24 +/- 0.93) microg/L, (25.99 +/- 6.05) ng/L, (124.68 +/- 9.96) microg/L in a control group. The number of macrophages in mucosa in patients with COPD was (1.71 +/- 0.48) cells/Hp, while that was (0.59 +/- 0.18) cells/Hp in the control group (0.63 +/- 0.25) x 10(4). Statistical significance was found between the two groups. The number of AMs and polymorphonuclear leucocyte in BALF were inversely correlated with forced expiratory volume in one second as percentage of predicted value(FEV(1)% Pre) (r = -0.511, P < 0.01; r = -0.562, P < 0.01). The levels of MMP-9 and ET in the culture supernatants of AMs were inversely correlated with FEV(1)% Pre(r = -0.678, P < 0.01; r = -0.871, P < 0.01). Both the level of MIP-1 alpha and the positive number of AM expressing LFA-1 in BALF were positively correlated with the number of AMs in BALF(r = 0.572, P < 0.01; r = 0.625, P < 0.01). CONCLUSION: AM may be involved in the process of airway inflammation and subsequent airway obstruction.

[Clinico-pathological features and possible pathogenesis of rifampicin-induced acute renal failure].

OBJECTIVE: To explore the clinico-pathological features and possible pathogenesis of rifampicin-associated acute renal failure (ARF). METHODS: The clinico-pathological features of 4 patients with Rifampicin-associated ARF were analyzed. Rifampicin-dependent antibody in the patient's serum was detected with Coomb's test. RESULTS: 4 cases with rifampicin-associated ARF was distinguished from 202 in-patients with ARF in 5 years(1.98%). Most of them had infection before the occurrence of ARF. Clinical features were characterized by fever, fatigue and gastro-intestinal symptoms after taking rifampicin (especially when it was reused). Oliguria or anuria developed suddenly. Laboratory data showed hemolytic anemia, thrombocytopenia and decline of renal function. Some also had impaired liver function. Renal biopsy revealed acute tubular necrosis in 3 cases and acute interstitial nephritis in 1 case. Serum rifampicin-dependent antibody was positive in all the cases. CONCLUSION: Rifampicin-associated ARF is not rare clinically, especially in those rifampicin re-exposure patients. Renal function should be closely monitored in susceptible patients. Detection of serum rifampicin-dependent antibody may be helpful to distinguish this group of ARF patients.

[Relationship between circulating vascular cell adhesion molecule-1 and vascular complications in type 2 diabetes mellitus].

OBJECTIVE: To investigate the relationship between soluble vascular cell adhesion molecule-1 (sVCAM-1) and type 2 diabetes complicated with vascular diseases. METHODS: In 61 cases of type 2 diabetes with macrovascular or microvascular complications and 36 cases of healthy control, sVCAM-1 levels were measured in duplicate by enzyme-linked immunosorbent assay using the soluble VCAM-1 Kit (R& D Systems Ltd, Ablingdon, Oxfordshire, UK). RESULTS: sVCAM-1 levels in type 2 diabetes [(973.25 +/- 73.38) microg/L] were higher than these of healthy controls [(230.45 +/- 12.47) microg/L], P < 0.001. sVCAM-1 levels of type 2 diabetes with hypertension [(1263.99 +/- 87.00) microg/L] were higher than those of type 2 diabetes with microvascular complications [(726.57 +/- 95.40) microg/L], P < 0.001. CONCLUSION: There was a significant correlation between sVCAM-1 concentration and vascular complications of type 2 diabetes.

[Glucocorticoid receptor and HSP 90 mRNA expression in peripheral blood mononuclear cell from steroid resistant asthmatics].

OBJECTIVE: To investigate the expression of glucocorticoid receptor (GR) and HSP 90 mRNA in peripheral blood mononuclear cells (PBMC) from steroid-sensitive asthma (SS), steroid-dependant asthma (SD) and steroid-resistant asthma (SR) and assess the role of GR and HSP 90 in the pathogenesis of SR. METHODS: With reverse transcription-polymerase chain reaction (RT-PCR), expression of GR and HSP 90 mRNA were detected in PBMC without or with IL-2 and (or) IL-4 stimulation from 10 normal volunteers, 10 SS, 5 SD and 6 SR patients. RESULTS: Expression of GR and HSP 90 mRNA in PBMC was the highest in SR group (0.730 +/- 0.171, 1.122 +/- 0.165 respectively), then in SS group (0.359 +/- 0.350, 0.885 +/- 0.250 respectively), the lowest in SD group (0.017 +/- 0.008, 0.078 +/- 0.039 respectively). The expression of GR and HSP 90 mRNA in PBMC from normal volunteers was 0.052 +/- 0.013 and 0.362 +/- 0.101 respectively. The difference of the expression of GR or HSP 90 was significant between each group (P < 0.05). The ratio of HSP 90/GR in normal volunteers, SS, SD and SR patients was 7.15 +/- 1.84, 8.39 +/- 7.95, 5.51 +/- 3.30 and 1.57 +/- 0.18 respectively. HSP 90/GR in SR was significant lower than those of the others, suggesting that the expression of HSP 90 mRNA was in sufficient in this group. When PBMC were incubated with combination of IL-2 and IL-4, a significant higher over expression of GR mRNA was observed in all asthmatic group and a significant higher expression of HSP 90 mRNA was observed only in SS and SD groups but not in SR group. CONCLUSION: The low expression of HSP 90 mRNA and the lowering ratio of HSP 90/GR mRNA may be related to the pathogenesis of SR. Different response to IL-2 and IL-4 may play a role in the development of HSP 90/GR imbalance.

[Airway inflammation and macrophage inflammatory protein-1alpha, gelatinase B level in patients with COPD].

OBJECTIVE: To investigate the levels of macrophage inflammatory protein-1alpha (MIP-1alpha) and gelatinase-B (MMP-9) in bronchial alveolar lavage fluids (BALF) and supernatants of the cultured alveolar macrophages (AM) in patients with chronic bronchitis (CB) and chronic obstructive pulmonary disease (COPD). METHODS: Bronchial alveolar lavage (BAL) was performed bronchoscopically in CB group, COPD group, and control group. Total cells were counted using hemacytometer. Differential cell counts were made with Wright's stained cell smear. The levels of MIP-1alpha, MMP-9 in BALF and in supernatants of cultured AMs were measured by ELISA. RESULTS: The numbers of AMs and neutrophils in BALF in patients with CB and COPD groups were significantly higher than those in control group (P < 0.05). The levels of MIP-1alpha and MMP-9 in BALF and the supernatants of cultured AMs in patients with CB and COPD groups were higher than those in control group (P < 0.05). The levels of MIP-1alpha and MMP-9 in BALF were positively correlated with those in the supernatants of cultured AMs (r = 0.253, P < 0.05; r = 0.529, P < 0.01). The number of AMs in BALF was positively correlated with the level of MIP-1alpha and MMP-9 in BALF (r = 0.558, P < 0.01; r = 0.405, P < 0.01). Both AMs counts and the levels of MIP-1alpha, MMP-9 in BALF were inversely correlated with FEV(1.0)%pred (r = -0.322, P < 0.05; r = -0.319, P < 0.05; r = -0.616, P < 0.01). The levels of MIP-1alpha and MMP-9 in supernatants of the culture AMs were increased significantly after LPS stimulation (P < 0.05). CONCLUSION: AMs, which may be the most important cellular source of MIP-1alpha and MMP-9 in COPD, accelerate MIP-1alpha and MMP-9 accumulation in the lung, which exaggerates inflammation process in the airway.

[The VIP-immunoreactive nerve fibers in dental pulp-dentin complex of human premolar].

OBJECTIVE: The expression of vasoactive intestinal peptide (VIP) in dental pulp-dentin complex of human premolar was studied to find out the relationship of distribution and function of VIP positive nerve fibers. METHODS: Fresh health premolars were collected, fixed, decalcified, embedded in paraffin and cut into slices. The slices were treated by immunohistochemical reaction of VIP and quantitative image analysis was then carried out. RESULTS: VIP positive nerve fibers in dental root were in bundle and branches of the nerve fibers were few. The nerve fibers extended in a radiative way from cervical line to the coronal pulp and branched a lot. A part of branches surrounded blood vessels, some terminated in the dental pulp matrix, the others passed odontoblast layer and ran into the predentin. In the coronal pulp the integral optical density was 12.7400 +/- 1.8070, the volume density was 0.0192 +/- 0.0127, the length density was 0.0046 +/- 0.0028; in predentin the integral optical density was 13.0700 +/- 1.9270, the line length was (19.6000 +/- 8.5970) microns. CONCLUSIONS: VIP-immunoreactive nerve fibers exist in human dental pulp-dentin complex. Some fibers surround blood vessels, some terminate in dental pulp matrix and predentin, which suggest that in addition to relating the action of blood vessels, VIP-immunoreactive nerve fibers might also relate to sense.

[The inhibitory effect of CLA on mice forestomach neoplasia induced by B(a)P].

OBJECTIVE: To study the inhibitory effect of CLA on the postinitiation phase of forestomach neoplasia formation induced by B(a)P and explore its possible mechanism of anticarcinogenesis. METHODS: Kunming mice were divided into five groups, i.e. salad oil control group, CLA control group, B(a)P group, B(a)P + high dose CLA group and B(a)P + low dose CLA group. The experimental period was 23 weeks, and the endpoints included cell proliferation, expression of Pan-ras P21 and some enzymes. RESULTS: Short term CLA treated significantly inhibited the forestomach neoplasia formation induced by B(a)P at postinitiation phase. The incidence of tumor in the groups of B(a)P, B(a)P + high dose CLA and B(a)P + low dose CLA was 100%, 60% and 69% respectively (P < 0.05). The possible mechanism of this anticarcinogenic effect may be related to the inhibition of cell proliferation and the induction of the activities of GSH-Px, GST and SOD, irrespective of the regulation of expression of Pan-ras P21. CONCLUSION: CLA is a promising chemopreventive agent, and its anticarcinogenic effect may involve its effects on the redox system.

Association between ACE gene polymorphism and diabetic nephropathy in South Indian patients.

OBJECTIVE: To study the association of ACE gene polymorphism and diabetic nephropathy in South Indian subjects. SETTING: Outpatient clinic of a specialized hospital. PATIENTS: The study included 109 South Indian type 2 diabetic patients (72 males and 37 females; age 56.7 plus/minus 9.0 years, mean plus/minus SD). The patients were subdivided into two groups: nephropathic (n=86) and normoalbuminuric patients (n=23). INTERVENTIONS: Genomic DNA was isolated from the peripheral blood leukocytes. To determine the ACE genotype, genomic DNA was amplified by PCR initially using a flanking primer pair and, subsequently when necessary, with a primer pair that recognizes the insertion specific sequence for confirmation of the specificity of the amplification reactions. MAIN OUTCOME MEASURES: ACE genotype distribution in the two study groups. RESULTS: In the nephropathic patients, ID and DD genotypes were present in 52.3% and 27.9% of the patients, respectively as compared to 34.8% and 21.7% respectively in those with normoalbuminuria. The D allele was present in 80.2% of the nephropathic patients and 56.5% of the normoalbuminuric patients (chi-squared=4.28, P=0.039; odds ratio 3.12). Therefore, the higher percentage of II genotype in the normoalbuminuric group was 43.5% as compared to the 19.8% in nephropathic patients. CONCLUSIONS: This study showed a positive association between the D allele (ID and DD genotype) of the ACE polymorphism and diabetic proteinuria in South Indian type 2 diabetic patients. Our findings are in keeping with several earlier studies showing a strong association of the D allele of the ACE gene with diabetic nephropathy.

[Application of triple-color flow cytometry for minimal residual disease detection in acute B-lymphoblastic leukemia].

OBJECTIVE: To set up a sensitive triple-color flow cytometric assay (FCM) for detection of residual leukemia cells. METHODS: Application of triple-color flow cytometry to the patients for detecting CD(10)(+)CD(22)(+)CD(45)(- or dim) leukemic cells in acute B lymphoblastic leukemia (B-ALL). RESULTS: Leukemic cells in B-ALL patients' bone marrow (BM) consistently showed the CD(10)(+)CD(22)(+)CD(45)(- or dim) phenotype. FCM analysis could detect leukemic cells at the level of 1 per 10(4) cells. The CD(10)(+)CD(22)(+)CD(45)(- or dim) cell population was absent in normal BM, regenerating BM, and BM from leukemia patients in continuous complete remission. This assay could identify residual leukemic cells in 50 days after starting induction chemotherapy. Furthermore, a gradual increase of leukemic cell population could be monitored by this assay before morphological confirmation of BM relapse. CONCLUSION: Triple-color FCM was a sensitive method for detecting BM CD(10)(+)CD(22)(+)CD(45)(minus sign or dim) leukemic cells and could be an additional useful tool to monitor MRD in B-ALL patients.

[The diagnostic value of platelet glycoprotein-specific autoantibody detection in autoimmune thrombocytopenic purpura].

OBJECTIVE: To evaluate the clinical significance of platelet glycoprotein-specific autoantibody detection and platelet-associated IgG (PAIgG) measurement in the differential diagnosis between autoimmune thrombocytopenia and nonimmune thrombocytopenias. METHODS: Anti- GP II b/III a and anti-GP I b/IX autoantibodies were detected by a modified monoclonal antibody immobilization of platelet antigens assay (modified MAIPA), and PAIgG by competitive ELISA. RESULTS: Comparing with the measurement of PAIgG, the GP-specific autoantibody assay was less sensitive but much more specific for the diagnosis of autoimmune thrombocytopenia. CONCLUSION: The platelet GP-specific autoantibody detection is considerably better than PAIgG measurement in discriminating immune from nonimmune thrombocytopenia.

[Changes of L-selectin expression on polymorphonuclear leukocytes and its role during polymorphonuclear leukocytes sequestration in lungs of rats with acute lung injury].

OBJECTIVE: To explore the changes of L-selectin protein expression on peripheral blood polymorphonuclear leukocytes (PMN) and their role during PMN sequestration in the lungs of rats with acute lung injury (ALI). METHODS: The model of ALI in rat was established by intravenous injection of E. Coli endotoxin (ET). The protein expression of L-selectin on peripheral blood PMNs was measured by immunofluorescence and flow cytometry. Myeloperoxidase (MPO) enzyme analysis and histology were employed to estimate the quantity of PMN sequestration in lungs of rats during ALI. RESULTS: (1) The protein expression of L-selectin on PMN after injection of ET (7.8 +/- 1.6) was all significantly lower than that in control group (10.5 +/- 2.1, P < 0.05). (2) MPO activity increased significantly after injection of ET in comparison with the control group [0.069 +/- 0.011) U/mg lung tissue, P < 0.01]. The difference was not significant between the fucodin treated 5 min group [(0.391 +/- 0.071) U/mg lung tissue] and ET-treated 5 min group [(0.359 +/- 0.074) U/mg lung tissue]. But MPO activity was significantly lower in fucodin-treated 15 min group [(0.396 +/- 0.061) U/mg lung tissue] than that in ET-treated group at same time point [(0.490 +/- 0.069) U/mg lung tissue, P < 0.05]. CONCLUSIONS: (1) In physiological state, L-selectin are consistently expressed on the surface of PMN membrane. The protein expression of L-selectin on PMNs reduces rapidly after injection of ET and is the lowest at 15 min, then gradually increases. (2) The early stage of PMN sequestration in lungs is not dependent on L-selectin, but L-selectin is still important for maintaining a continual PMN sequestration in lungs of rats with ALI.

[Study on an enhancing agent for removing arsenic from drinking water].

Drinking water contaminated by arsenic for an extended period of time could be detrimental to the health of people. Some preliminary symptoms could be alleviated by drinking water non-contaminated. It is important to develop an arsenic removal agent with a specific property of most efficient, cost-effective and easy for operation. The results showed that the capacity of the agent developed in this study was 10 times higher for arsenic removal than other agent available. The lowest arsenic content of the treated water was 0.05 mg/L. The special function of this agent was arsenic removing without changing other components and the concentrations of other elements in the treated water. The operation and management was simple without adjusting pH of the influent and effluent water. The agent was 5 times cheaper in cost than alumina or activated carbon, because it was a reusable oxidation-catalyst. Therefore, the agent could be widely applied in drinking water plants or used as a purifier at home in the high arsenic areas.
