[Protective effects of prostacyclin on acute necrotizing pancreatitis and its renal damage in rats].

In a rat model with acute necrotizing pancreatitis (ANP), the administration of exogenous prostacyclin (PGI2) significantly increased the pancreatic and renal blood flow, brought the level of 6-keto-PGF1 alpha to TXB2 in renal vein blood back to normal, reduced the severity of pancreatic and renal histolcagic damage, decreased the mortality, and prolonged survival time. The study showed that exogenous PGI2 can increase pancreatic blood flow in rats with ANP, help to prevent ANP, and protect from renal damage following ANP.

[Masticatory efficiency in skeletal class III malocclusion].

In this research, we used the Gunne's spectrophotometry method to measure the masticatory efficiency in 30 skeletal Class III patients and in 55 normal people. After groups comparing and step wise-regression analysis, we found that masticatory efficiency in Class III malocclusion group was much lower than that in normal occlusion group, and the masticatory efficiency in Class III malocclusion was about 60% of that in normal peoples. Masticatory efficiency in male was higher than that in female.

[Detection of hepatitis C virus RNA and C33c antigen in the liver tissue from hepatitis C virus infection patients with chronic liver disease].

To study the distribution of hepatitis C virus (HCV) in liver tissue and its pathogenesis, nonisotopic in situ hybridization with a digoxin labelled cDNA probe to 5'-NC region of HCV and immunohistochemical study with monoclonal anti-body of anti HCV C33c were performed on the liver tissue from 24 HCV infection patients with chronic liver disease. The results were as follows: In the liver tissue of 17 patients with HCV infection (both ELISA anti-HCV and nested PCR HCV RNA positive in serum), the positivity of HCV RNA and C33c were 82.3% (14/17) and 88.2% (15/17) respectively. In 7 patients with anti-HCV positive only HCV RNA was not detected in the liver tissue, but 4 out of these 7 cases were C33c positive. The specific signals of HCV RNA and C33c were mainly localized in the cytoplasm of hepatocytes. The distribution of the HCV infected hepatocytes was spotty, scattered or diffuse. There was no relationship between the distribution and the level of alanine transaminase or pathologic changes in liver. There were many lymphocytes and monoclear cells infitrating the periphery of HCV-infected liver cells. The results suggested that replication of HCV in the cytoplasm of hepatocytes elicits the host immune response in cells infected with HCV and this is important in the pathogenesis of HCV infection.

[Painful symptoms of pelvic endometriosis].

From February 1991 to January 1993, a total of 383 women of child-bearing age who were hospitalized in gynecologic ward were investigated for their painful symptoms, of whom 192 were found to be patients with endometriosis (endometriosis group). The other 191 cases without endometriosis served as the control. The results showed that the frequency of dysmenorrhea, lower abdominal or pelvic pain, dyspareunia, anal pressure pain during menstruation were 65.6%, 44.3%, 30.2% and 32.3% respectively in patients with endometriosis, and 37.7%, 33.0%, 20.9%, and 20.4% respectively in the control, the former were all significantly higher than the later respectively. Whereas the frequency of all the painful symptoms and the severity of dysmenorrhea were not correlated with the stages of endometriosis. It is suggested that patients with endometriosis have more painful symptoms besides dysmenorrhea, which are very useful for better recognization and more accurate diagnosis of this disease.

[Observation on left ventricular diastolic function of patients with heart-deficiency syndrome of cardiovascular diseases].

In this paper, left ventricular diastolic function (LVDF) of 99 Heart-Deficiency Syndrome (HDS) patients was observed with pulse Doppler echocardiogram and was compared with normal group. The results showed that different LVDF abnormalities occurred in the HDS patients. When E, Ei DC were decreased and A, Ai, A/E, Ai/Ei, IRT increased, the degree of HDS deteriorated, the pattern was Heart Qi-Yin Deficiency > Heart Yang Deficiency > Heart Qi Deficiency > Heart Yin Deficiency. In conclusion, the above parameters of LVDF could be important indices for evaluation of HDS.

[Hemofiltration ameliorating reperfusion injury after complete cerebral ischemia: an experimental study].

The effects of hemofiltration on cerebral resuscitation were studied by using resuscitation model of potassemia-induced cardiac arrest and cardiopulmonary bypass. Sixteen rabbits were divided into control group (CG) and hemofiltration group (HFG). Blood lactic acid, lipid peroxidant (LPO), serum iron, middle molecular substance (MMS) were determined. Neurologic deficit score (NDS) system was used to evaluate neurologic function. The level of blood lactic acid was increased in both CG and HFG. It was higher in CG than in HFG (P > 0.05). Serum iron was increased in CG, however, it was decreased in HFG. The difference between the two groups was significant (P < 0.01). LPO was increased significantly in CG and there was no obvious changes in HFG. MMS was increased gradually during reperfusion in CG, whereas it was decreased in HFG. There was significant difference in the corresponding times of the two groups. NDS of CG was higher than that of HFG (32 +/- 12 vs 18 +/- 10) (P < 0.01). These data demonstrate that hemofiltration is able to eliminate ischemic and reperfusional toxic products quickly. Additionally, free iron, agonist factor of catalyzing free radical reaction, does not increase because free radicals and lactic acid are eliminated quickly, inhibiting free radical tissue injury and improving cerebral resuscitation.

Visual deprivation does not affect the orientation and direction sensitivity of relay cells in the lateral geniculate nucleus of the cat.

Visual deprivation in early life profoundly affects the characteristic sensitivity of visual cortical cells to stimulus orientation and direction. Recently, relay cells in the lateral geniculate nucleus (LGNd) have been shown to exhibit significant degrees of orientation and direction sensitivity. The effects of visual deprivation upon these properties of subcortical cells are unknown. In this study cats were reared from birth to 6-12 months of age in total darkness; the orientation and direction sensitivities of area 17 (striate cortex) and LGNd cells were compared. All cells were studied using identical quantitative techniques and statistical tests designed to analyze distributions of angles. The results confirm previous work and indicate that the orientation and direction sensitivities of cells in area 17 are profoundly reduced by dark rearing. In marked contrast, these properties of LGNd relay cells are unaffected. The result is that, unlike in the normal cat, in dark-reared cats the orientation and direction sensitivities of cells in the LGNd and visual cortex do not differ. It is concluded that (1) the orientation and direction sensitivities of cortical cells contribute little, if at all, to the sensitivities of LGNd cells since LGNd cells exhibit normal sensitivities even though the cortical cells projecting to them exhibit greatly reduced sensitivities and (2) during normal development intracortical mechanisms appear to expand upon and/or modify the weak orientation and direction sensitivities of their inputs. These intracortical mechanisms depend upon normal visual experience since in dark-reared cats, but not normal ones, the orientation and direction sensitivities of cells in the LGNd and visual cortex do not differ quantitatively or qualitatively.

Concomitant sensitivity to orientation, direction, and color of cells in layers 2, 3, and 4 of monkey striate cortex.

The receptive field properties of cells in layers 2, 3, and 4 of area 17 (V1) of the monkey were studied quantitatively using colored and broad-band gratings, bars, and spots. Many cells in all regions studied responded selectively to stimulus orientation, direction, and color. Nearly all cells (95%) in layers 2 and 3 exhibited statistically significant orientation preferences (biases), most exhibited at least some color sensitivity, and many were direction sensitive. The degree of selectivity of cells in layers 2 and 3 varied continuously among cells; we did not find discrete regions containing cells sensitive to orientation and direction but not color, and vice versa. There was no relationship between the degree of orientation sensitivity of the cells studied and their degree of color sensitivity. There was also no obvious relationship between the receptive field properties studied and the cells' location relative to cytochrome oxidase-rich regions. Our findings are difficult to reconcile with the hypothesis that there is a strict segregation of cells sensitive to orientation, direction, and color in layers 2 and 3. In fact, the present results suggest the opposite since most cells in these layers are selective for a number of stimulus attributes.

The analysis of disparity evoked potentials by a new form of static random-dot stereograms.

PURPOSE: A new form of static random-dot stereograms free of monocular clues was designed as stimulus to elicit disparity evoked potentials. METHODS: Disparity evoked potentials were recorded in 40 stereo-normal subjects. The stimulator was a white-black static random-dot stereograms generated by a computer and had no monocular clues. Every subject was tested in disparity stimulus, zero disparity stimulus, monocular stimulus and wearing prism condition. RESULTS: A characteristic wide positive wave at about 250 ms was consistently recorded in disparity stimulus, which may be regarded as evidence of the presence of stereopsis. In contrast, recordings for zero disparity stimulus, monocular vision, stimulus and wearing prism condition all demonstrated a markedly difference from recording for disparity stimulus.

[Surgical treatment of primary cardiac tumors].

From Jan. 1962 to Nov. 1994, 130 patients with primary cardiac tumors were performed in Shanghai Chest Hospital. There were 48 male and 82 female patients ranging in age from 16 to 73 (average 43.8 years). 123 (94.6%) had benign and 7 (5.4%) were malignant tumor. Cardiac myoxmas accounted for 91.1% of all benign neoplasias. 5 patients with benign and 1 patient with malignant tumor died early (within 30 days of operation). Of the 4 patients who died late, 3 had had malignant disease.

[Comparative study on pharmacological effects of various species of Pueraria].

Through comparative studies on the acute toxicity of four species of Pueraria, it was found that the toxicity of P. peduncularis was the highest, followed by P. lobata and P. omeiensis, and that of P. thomsonii was the lowest. The inhibitory effects of P. lobata, P. omeiensis and puerarin on the fever induced by 2,4-dinitrophenol in rats were very fast and significant, maintaining for about 8 hours, while the effect of P. thomsonii is relatively weak. As for influence upon the degrees of myocardiac ischemia induced by pituitrin, P. omeiensis appeared to be the most potent, and P. thomsonii the least one, while P. lobata and puerarin were intermediate between the above two. Summarily, the antipyretic and anti-myocardiac ischemia effects of P. lobata and P. omeiensis are stronger than those of P. thomsonii, and puerarin is one of their effective components.

[Multimodality therapy of late stage lung cancer].

A prospective randomized study of 80 cases of stage III lung cancer treated with different multimodality therapy was carried out. Life table and Kaplan-Meir curve were employed to calculate survival rate. Log rank-multivariate analysis and time t test were used to evaluate statistical values of the 80 cases, 40 SCLC were randomly treated with either chemotherapy (CT)-surgery-CT or CT-radiotherapy (RT)-CT. The year survival rates were better in the group treated with CT-surgery-CT, a statistical difference was observed in 2yr survival rate (P < 0.05). Thus, surgical resection for SCLC was better than RT after CT. The remaining 40 cases of NSCLC were randomly treated with either CT-RT-CT or RT-CT. Multivariate analysis showed a better statistical meaning in the 20 cases treated with CT-RT-CT than the other group, their 1, 2 year survival rates were 27%, 40% and 22%, 15%, respectively. Thus preradiative CT was beneficial for survival.

[Surgical treatment of undifferentiated esophageal carcinoma].

From 1961 through 1983, a total of 3,804 cases with carcinoma of esophagus underwent surgical resection of cancer at Shanghai Chest Hospital. Thirty-nine of them were primary undifferentiated esophageal carcinoma (UEC) with an incidence of 1%. In 97% of the cases, the lesions were located at the mid- and lower-esophagus. All cases underwent tumor resection and esophagogastrostomy, with an operative complication rate of 10%, and mortality rate of 13%. Follow-up until December 1994, all patients were dead. 42% of the cases died within 6 months, another 42% of the cases died within 7-24 months, with a median survival period of 7 months only. The postoperative 5-year survival rate was 15%. To improve the therapeutic results, preoperative radiotherapy and postoperative adjuvant therapy are needed.

[Examination and analysis on keratometry and axis of myopia].

In this study, we applied SYSTAT software package to deal with the data, measured the keratometric values and axes of 276 patients with myopia (523 eyes), analyzed on the data of keratometry and astigmatism, correlations between the myopic degree and keratometric value and between the former and length of axis, and pathogenesis of myopia. The results revealed that the vertical corneal refractive power was stronger than the horizontal value, the vertical corneal refractive power in high and superhigh myopia was greater than that in mild and medium myopia, the axial lengths are significantly different among mild, medium, high and superhigh myopia (P < 0.01), and the myopic degree is correlated with the keratometric value and axial length. We can conclude that keratometry and axis are two main factors for the formation of myopia.

[Diagnosis and treatment of postmenopausal uterine bleeding by hysteroscopy].

OBJECTIVES: To evaluate the value of hysteroscopy in diagnosis and treatment of patients with postmenopausal uterine bleeding. METHODS: One hundred and thirty-five patients with postmenopausal bleeding hospitalized in our department from July 1990 to July 1994 were examined by hysteroscopy. The results of 124 cases were compared with pathological findings obtained during uterine curettage. Thirty-nine patients were operated simultaneously under hysteroscopy. RESULTS: Under hysteroscopy, atrophic endometrium, endometrial polyps, cervical polyps, endometrial carcinoma, endometrial hyperplasia, submucous myoma were diagnosed in 62 cases (45.9%), 25 cases (18.5%), 13 cases (9.6%), 13 cases (9.6%), 11 cases (8.2%) and 5 cases (3.7%) respectively. The remaining 6 (4.5%) had IUD insertion. These findings were confirmed pathologically in 115 out of 124 cases (92.7%). There were 9 cases misdiagnosed by hysteroscopy. 6 cases had false negative results during previous curettage. Twenty-two cases of endometrial polyps, 13 cases of cervical polyps and 4 cases of pediculated submucous myoma were removed hysteroscopically without any complication. CONCLUSIONS: Hysteroscopy is an effective method for identifying the causes of postmenopausal uterine bleeding, which is superior to curettage of uterus. For some selected patients hysteroscopic operations can be performed at the same time.

[Effects of nasal continuous positive airway pressure on breathing pattern and respiratory drive in patients with obstructive sleep apnea syndrome].

To examine the influence of continuous positive airway pressure (CPAP) therapy on respiratory center drive in patients with obstructive sleep apnea syndrome (OSAS), 20 normocapnic OSAS patients (group 0) and 20 simple snoring patients were studied. In the first night, diagnostic polysomnography (PSG) was performed. Before and after PSG monitoring, mouth occlusion pressure (P0.1), tidal volume (VT), minute ventilation (VE), respiratory rate (RR), inspiratory time (Ti), expiratory time (Te), total cycle duration (Ttot), inspiratory duty cycle (Ti/Ttot), mean inspiratory flow (VT/Ti) and effective inspiratory impedance (P0.1/VT/Ti, Ieff) were measured while they were breathing room air. In the following night the OSAS patients were treated with nasal CPAP and PSG monitoring and the above mentioned measurements were repeated. The results showed that pre-PSG values of P0.1, RR and P0.1/VT/Ti in the OSAS patients were significantly higher than those in the snoring patients, while VT, Ti, Te and Ttot values were lower. In the first night, the post-PSG P0.1 value in the OSAS patients increased markedly as compared with the pre-PSG. After overnight nasal CPAP therapy, the respiratory disorder index in the OSAS patients decreased markedly, the nadir SaO2 increased markedly, but the post-PSG P0.1 value did not increase significantly. It is concluded that, before sleep, OSAS patients exhibit a higher respiratory drive and a shallow and frequent breathing pattern as compared with simple snoring patients. After nocturnal sleep, the respiratory drive of OSAS patients increases significantly, the breathing pattern becomes more shallow and frequent. Nasal CPAP may effectively relieve the sleep apnea and hypopnea as well as the resulting hypoxemia and therefore correct the changes in breathing pattern and respiratory drive through nocturnal sleep in patients with OSAS.

[Influence of acupuncture on blood pressure, contents of NE, DA and 5-HT of SHR and the interrelation between blood pressure and whole blood viscosity].

Twenty spontaneous hypertensive rats (SHR) with systolic blood pressure (SBP) >or= 20 kPa were chosen and divided randomly into two groups (acupuncture group and control group). The former was acupunctured on bilateral Quchi (L1 11), Zusanli (ST 36). Another 46 immature SHRs which were about 7-week-old with SBP ranged from 16 kPa to 20 kPa were given 10% high molecular weight dextran (HMWD) or normal saline by intervention respectively. The results showed: 1. After three course of acupuncture, SHRs' SBP dropped remarkably from 25.493 +/- 0.73 kPa to 19.547 +/- 0.555 kPa (P < 0.01); 2. After three courses of acupuncture, NE and 5-HT contents in plasma were lowered (P < 0.05, < 0.01) and DA content in plasma was increased (P < 0.05) compared with that in the control group. 3. After three course of acupuncture, NE content in brainstem, hypothalamus, cerebral cortex increased (P < 0.005-0.01) and DA content in brainstem, cerebral cortex increased remarkably (P < 0.005-0.01) compared with that in the control group. 5-HT content in brainstem, hypothalamus, and cerebral cortex increased obviously compared with that in the control group. 4. After 1 or 2 hours of intervention of 10% HMWD to immature SHR, the whole blood viscosity and blood pressure rise remarkably compared with normal saline group. The experiment implies that the mechanism of lowering the blood pressure of SHR by acupuncture lies in that it can adjust NE, DA and 5-HT contents in plasma and central nervous system to adjust the activity of sympathetic, reduce whole blood viscosity etc.
