[HLA-amino acid residue matching in organ transplantation].

OBJECTIVE: To present a new matching policy-HLA amino acid residue matching (Res M) for Chinese population. METHODS: Based on the outcome of genotyping for HLA alleles in massive samples, we presented new Res M according to 10 groups of amino acid residue for class I and, groups of DR/DQ for class II and compared them with HLA six-antigen match program (Ag M). The influence of Res M on early renal function, acute rejection and 1-5 year graft survivals were retrospectively analyzed in 316 cases of first cadaver kidney transplantation. RESULTS: The rate of Res-matched transplants increased greatly. The rate of 0 mismatch (MM) recipients increased from 0.3% by Ag M to 10% by Res M, and that of 0-1 MM transplants from 3.2% to 42.4%. Clinical study showed that 1 - 5 year graft survival with 0 - 1 Res-MM was similar to kidneys with 0 - 2 Ag-MM. Compared to 3 - 4 Res-MM, the early renal function obviously improved (88.8% vs 46.7%), acute rejection decreased (14.9% vs 86.9%), 1 - 5 year survival was increased by 10% - 26% (P < 0.05), and graft survival increased by 14% - 35% (P < 0.01) in recipients with 0 - 1 Res-MM. Cox regression showed that in 10 factors affecting the graft survival, Res-MM was most significant (risk ratio 21.73). CONCLUSION: New Res M has proved to be suitable for clinical application in Chinese population, especially for cadaver kidney transplantation.

[Long-term therapeutic effects of extended radical resection and radical resection of cancer of cardia and stomach fundus].

OBJECTIVE: To study the best range of radical resection in the treatment of cancer of the cardia and fundus of stomach. METHODS: 418 patients with cancer of the cardia and fundus of stomach underwent radical resection. Of them 192 were treated by extended radical resection (extended group), and 226 by radical resection (radical group). The 5-year and 10-year survival rates were followed up and compared in the two groups. RESULTS: Analysis failed to demonstrate significant difference between the two operations for TNM stage I and II (P > 0.05). For stage III, however, the 5-year and 10-year survival rates in the extended group increased by 14.2% and 15.9% as compared with those in the radical group (P < 0.05). The two groups survival rates were similar for stage IV (P > 0.05). CONCLUSIONS: To completely clean the lymph nodes of splenic hilus and artery and improve long-term survival rate, extended radical resection including spleen and body and tail of the pancreas should be recommended for stage III patients with cancer of the cardia and fundus of stomach when their serosa was involved or lymph node metastasis took place.

[Long-term effect on preoperative ulnaris arterial intubation chemotherapy in treatment of stage III breast cancer].

OBJECTIVE: To evaluate preoperative ulnaris arterial intubation chemotherapy as a step of multidisciplinary treatment of stage III breast cancer. METHODS: 109 patients with stage III breast cancer were studied. 64 patients (group A) were treated by preoperative ulnaris arterial intubation chemotherapy and others (group B) were not. The 5 and 10-year survival rates and disease-free survival rates were analyzed. RESULTS: In patients with stage III breast cancer, the overall response rate of group A was 76.6%, and the response rate to histological study was 89.1%. The 5-year and 10-year survival rates of group A were 64.0% and 47.9% respectively. The disease-free survival rate of group A was 59.4% at 5 years and 43.3% at 10 years. All were higher than the 5-year and 10-year survival rates (31.1%, 21.9%) and the 5-year and 10-year disease-free survival rates (22.2%, 12.5%) of group B (P < 0.05). CONCLUSION: Preoperative ulnaris arterial intubation chemotherapy is an effective measure for the treatment of stage III breast cancer. It can improve the long-term effects on stage III breast cancer and reduce local relapse and distant metastasis.

[The treatment of premature arrest of growth plate with a novel engineered growth plate: experimental studies].

OBJECTIVES: To observe the role of the engineered growth plate (EGP) in the treatment of premature arrest of growth plate and to establish a novel treatment method for the premature arrest of the growth plate. METHODS: The engineered growth plates were cultured for the first time by using polylactic acid (PLA) as cell scaffold seeded with growth plate chondrocytes and they were implanted into the medial proximal defects of growth plates of New Zealand rabbit tibia. The degree of deformity of the tibia was evaluated by X-ray and the expression of collagen II mRNA of regenerating growth plate was detected by in situ hybridization technique. RESULTS: Little deformity appeared in the EGP group 8 weeks after operation. Some deformity was seen in the EGP group 16 weeks after operation, whereas it's degree was much less than that of the control group (P = 0.0001). The degree of the angular deformity of the EGP precultured with bFGF and TGF-beta group was less than that of the EGP group (P < 0.05). The cells in the regenerating growth plate arranged in column and were stained blue by in situ hybridization technique. CONCLUSION: The EGP can prevent the formation of bony bridge and restore the growth of the damaged growth plate.

[Study on risk factors of breast cancer among urban women in China].

OBJECTIVE: Breast cancer is the most common malignant neoplasm in women all over the world. It is necessary to establish monitoring methods and to identify risk factors. METHODS: Six cities including Beijing, Shanghai, Tianjin, Chongqing, Wuhan and Guangzhou were chosen as the sample areas. 2 100 cases with 1:1 matched controls were interviewed with a uniformed questionnaire. Conditional logistic regression and cluster analysis were used for multivariate analysis. RESULTS: The common risk factors for the combined data of the six cities were high educational level (OR: 1.17), high body mass index (OR: 1.51), high intake of milk products (OR: 1.49), high intake of eggs (OR: 1.14), psychological depression (OR: 2.78), history of benign breast diseases (OR: 2.24), old age at marriage (OR: 1.42), dysmenorrhea (OR: 1.22), short duration of lactation (OR: 1.17). Among these factors, different cities had different ORs, as high educational level (OR: 1.20 - 1.61), high body mass index (OR: 1.40 - 22.36), high intake of milk products (OR: 1.64 - 3.39), depression (OR: 1.60 - 10.50), history of benign breast diseases (OR: 1.73 - 19.44). Some statistically significant correlations existed in different cities such as early age at menarche, old age at marriage, long menstrual periods, high intake of meat, eggs, sea food and passive smoking were risk factors. Some risk factors varied between premenopausal and postmenopausal women. Beijing, Wuhan and Chongqing were clustered as the same group in cluster analysis, so did Shanghai and Tianjin, but Guangzhou could not be grouped with other cities. CONCLUSION: Some common risk factors for breast cancer did exist in the six cities, but there were varieties in different areas which showed different places would have their own characteristics. Thus, careful study of risk factors, especially in the selection of variables and data analysis need to be carried on.

[A cross-sectional study on the effects of microcystin in drinking water to the health of human beings].

OBJECTIVE: To understand the effects of microcystin (MC) in drinking water on health of human beings. METHODS: In Taixing, a cross-sectional study was conducted in two middle schools with different types of drinking water, from which 111 and 92 students were randomly selected and investigated. Blood samples from all individuals were collected and tested for serum HBsAg, HBeAg, anti-HBe, anti-HBs, anti-HBc with enzyme linked immunoabsorbent assay and for serum alanine aminotransferase (ALT), gamma-glutamyltransferase (gamma-GT) and alkaline phosphatase (ALP) contents by colorimetry. These levels of enzymes in sera were compared after the students were stratified according to the status of HBV infection. RESULT: With the exclusion of the effect of HBV infection, three enzymes levels in sera between two groups of students showed significant differences (P < 0.01), much higher in exposure group than those in control group. CONCLUSION: MC was possibly harmful to human beings in drinking water and induced the contents of some enzymes from the liver of human beings to increase in sera.

[Effects of adenovirus-mediated p16 and p53 genes transfer on apoptosis and cell cycle of lung carcinoma cells].

OBJECTIVE: To explore the synergistic inhibition effect and apoptosis induction of p16 and p53 genes on lung carcinoma cells. METHODS: E1-deficient and replication-defective recombinant p16 and p53 adenoviruses were generated by liposome-mediated co-transfection of recombinant plasmid pAdCMV-p16 or pAdCMV-p53 along with pJM17 and homologous recombination in 293 packaging cell. The lung cancer cell line H358, which had a homozygous deletion of p53 gene and no expression of p16 mRNA and protein, was infected with recombinant p16 and p53 adenovirus either individually or together. RESULTS: Immunohistochemical analysis showed that recombinant adenovirus could transfer p53 gene into tumor cell with 98% efficiency. Western blot indicated that p16 and p53 proteins were expressed at a high level in infected H358 cell. Inhibition effect of p53 gene on proliferation of H358 cell was weaker than that of p16 gene, and the combined use of both genes could completely prevent the proliferation of H358 cell. In situ end-labeling and flow cytometry indicated that p16 could result in G(1) arrest of cell cycle and did not induce H358 cells to undergo apoptosis; p53 also induced apoptosis of few cells besides G(1) arrest; and the simultaneous use of p16 and p53 genes could induce marked apoptosis of H358 cells. CONCLUSION: p16 and p53 genes possess the synergistic inhibiting effect on growth of lung cancer cells and can cooperate to induce apoptosis of H358 cell. The combined application of recombinant p16 and p53 adenoviruses can be used as a new strategy for cancer gene therapy.

[Changes of vWF in vascular endothelial cells with acute hypoxia].

OBJECTIVE: To investigate changes of vWF in vascular endothelial cells with acute hypoxia and assess its significance in the study of vascular injury and hypercoagulability. METHODS: vWF in vascular endothelial cells and in blood were measured by immunohistochemical staining image analysis and ELISA technique. Fibrin was displayed by phosphotungstic hematoxylin staining. RESULTS: The amount of vWF in vascular endothelial cells decreased progressively, while in blood increased progressively with hypoxia. Fibrin prominently increased with hypoxia. CONCLUSION: Acute hypoxia caused severe injury of vascular endothelial cells which resulted in release of vWF into blood from endothelial cells. The vWF amounts in blood may reflect the hypercoagulability, and may serve as a useful indicator of vascular injury and hypercoagulability.

[A three-dimensional finite element analysis of the correlation between lengths and diameters of the implants of fixed bridges with proper stress distribution].

OBJECTIVE: The study is designed to investigate the correlation between lengths and diameters of the implants of fixed bridges with proper stress distribution. Meanwhile, the correlation between the diameter of the implants and the size of a mandible bone is evaluated by stress analysis. METHODS: According to the stress around the implants of an implant fixed bridge with proper stress distribution, especially peak stress, a three-dimensional finite element method was used to determine the correlation between diameters and lengths of implants with their diameters or lengths changing. RESULTS: The peak stress surrounding the implants in fixed bridge supported by clinical implants that were commonly used was figured out, and the correlated amount of diameters or lengths of the implants were achieved. Furthermore, a correlation curve of diameter and length of the implants was made. But the correlated amount of diameters or lengths of the implants were not found in this model. CONCLUSION: The proper stress distribution of implants with different implant sizes can be achieved by adjusting diameters or/and lengths of implants. The selection of the implant size is related to the limited model size. Being a preliminary theory, the results will be proved to be clinically acceptable to the size selection of implants.
