[Clinical study on treatment 82 cases of chronic primary glomerulopathy with deficiency of yin with damp-heat symptom with dan shao tang].

OBJECTIVE: To investigate the treatment effect of Dan Shao Tang on chronic primary glomerulopathy with deficiency of Yin with damp-heat symptom. METHODS: 132 cases of chronic glomerulopathy with deficiency of Yin with damp-heat symptom, which divided into two groups at random, the 82 cases which were the therapy group using Dan Shao Tang and west medicine, and the other 50 cases which treated only by west medicine. We observed the effect and indexes change including renal function, proteinuria, hematuria. RESULTS: The effective rate of the therapy group is 90.24%, and the effective rate of the control group is 66% (P < 0.05), The therapy group is superior to the control group in improving renal function, decreasing proteinuria and hematuria in evidence(P < 0.05). CONCLUSION: Dan Shao Tang is effective to treatment on chronic primary glomerulopathy with deficiency of Yin with damp-heat symptom.

Characterization of human rotavirus serotype G9 isolated in Japan and Thailand from 1995 to 1997.

Serotyping of human rotavirus was conducted in 396 Japanese and 100 Thai rotavirus-positive fecal specimens collected from 1995 to 1997. Serotype G9 was found to be the third most common serotype with frequency of 16.2% in Thailand from 1996 to 1997. It was also detected in Japan with a low frequency (0.7%) in this year. The genetic analyses of VP4 and NSP4 genes of these G9 strains showed that 1 strain from Japan possessed P[8] genotype and NSP4 Wa-group with long electropherotype (e-type). In contrast, 5 strains from Thailand belonged to P[6] and 1 strain belonged to P[4]. All of the Thai strains were in the NSP4 KUN-group with a short e-type. Sequence analysis of their VP7 gene revealed that there was the highest homology among fecal G9 strains (> 96.3%, amino acid identity) and a relatively high degree of homology to standard viruses, F45 from Japan (95.4-96.3%, amino acid identity) and 116E from India (92-92.3%, amino acid identity). However, immunological analysis using G9 specific monoclonal antibodies (Mabs) against VP7 protein showed that the G9 strains isolated from the two countries had different antigenic specificity. It was confirmed further by intraserotypical phylogenetic analysis of VP7 amino acid. These results indicated that the prevalence of G9 rotavirus in 1996-1997 in Thailand was relative to the continuing recent emergence of it on a worldwide basis, while the Japanese G9 strain isolated in this survey was identified to have progenitors common to the F45 strain that was prevalent in 1985 in Japan. Phylogenetic analysis of VP7 amino acid of G1-14 prototype rotavirus showed that the G9 strains were most closely related to the equine G14 rotavirus FI23 strain but G3 strains, interserotypically. These findings suggest that G9 rotaviruses might be divided into two or more subtypes.

[The effect of composite Radix Salviae Militiorrhizae injection on myocyte apoptosis in acute myocardial infarction of rabbits].

OBJECTIVE: To study the effect of composite Radix Salviae Militiorrhizae injectiion on myocyte apoptosis in experimental acute myocardial infarction of rabbits. METHODS: The models of acute myocardial infarction were made with rabbits. The method of terminal deoxynucleotidyl transferase mediated dUTP nick end labeling was used to explore myocardial cell apoptosis in different injury area of acute myocardial infarction. The activities of plasma superoxide dismurase and the contents of serum malonic aldehyde were determined. RESULTS: When rabbits were in the condition of acute myocardial infarction, there were myocyte apoptosis in the area of ischemia reperfusion injury and the area of infarction. The number of myocyte apoptosis and the myocyte rate in control group and composite Radix Salviae Militiorrhizae injectiion treated group were higher than in sham-operated group (P < 0.001). In the area of ischemia reperfusion injury the number of myocyte apoptosis and the myocyte rate in composite salvia militiorrhiza-treated group were lower than in control group (P < 0.05). The activity of plasma superoxide dismurase in composite salvia militiorrhiza-treated group was higher than in control group (P < 0.05), but lower than in sham-orerated group (P < 0.001). The content of serum malonic aldehyde in composite Radix Salviae Militiorrhizae injectiion treated group were lower than in control group (P < 0.05), but higher than in sham-operated group (P < 0.01). CONCLUSION: Composite Radix Salviae Militiorrhizae injectiion could reduce the number of myocyte apoptosis in the area of ischemia reperfusion injury, which maybe explained by the increase of the activity of plasma superoxide dismurase and the decrease of the contents of serum malonic aldehyde.

[Quantitative analysis of the expression of insulin receptor and tyrosine protein kinase of patients with hepatic cirrhosis].

OBJECTIVE: To investigate the relationship between the expression of insulin receptor (IR) and the content of tyrosine protein kinase (TPK) in patients with hepatic cirrhosis. METHODS: Glucose tolerance test (GTT), insulin, c-peptide, insulin antibodies in serum, the expression of IR and TPK were quantitively analysed in patients with hepatic cirrhosis. The amount of IR and TPK was evaluated by immunohistochemical quantitative analysis using image analyzer in 12 patients with positive HBV marks and cirrhosis. Insulin and c-peptide in serum were determined by immunoenzymemetric assay (IEMA) and insulin antibodies in serum determined by RIA. RESULTS: IR amount in normal and abnormal GTT patients with hepatic cirrhosis was signifficantly less than that in control group (P<0.01). TPK amount in liver of patitents with abnormal GTT was less than that of normal GTT (P<0.01). CONCLUSIONS: IR decreases in cirrhotic patients. Abnormal GTT has relation to reducing TPK activity.

Anti-human platelet tetraspanin (CD9) monoclonal antibodies induce platelet integrin alpha IIb beta 3 activation in a Fc receptor-independent fashion.

OBJECTIVES: To characterize the activation of platelet integrin alpha IIb beta 3 induced by two anti-human platelet tetraspanin monoclonal antibodies (mAbs), HI117 and SJ9A4, and investigate their potential mechanism of action. METHODS: Using 125I-labeled human fibrinogen (Fg), specific Fg binding to human platelets induced by HI117 and SJ9A4 was measured. RESULTS: HI117 and SJ9A4 (10 micrograms/ml and 20 micrograms/ml) induced specific Fg binding to human platelets, suggesting that the two mAbs evoked activation of platelet integrin alpha IIb beta 3. Further study indicated that HI117 and SJ9A4 induced integrin alpha IIb beta 3 activation independent of platelet Fc-receptors, and that HI117 and SJ9A4-induced integrin alpha IIb beta 3 activation was inhibited by pretreatment of platelets with sphingosine, aspirin, apyrase, and/or PGI2. CONCLUSIONS: Anti-platelet tetraspanin (CD9) mAbs, HI117 and SJ9A4, can induce platelet integrin alpha IIb beta 3 activation independent of Fc-receptors. Three signaling pathways, namely thromboxane, secreted ADP, and cAMP pathways, may be involved in the process, with protein kinase C activation presumably being the common step of the three pathways.

Arsenic trioxide induces multiple myeloma cell apoptosis via disruption of mitochondrial transmembrane potentials and activation of caspase-3.

OBJECTIVE: To investigate the response of multiple myeloma (MM) cells to arsenic trioxide (As2O3) and their possible mechanisms. METHODS: Two MM-derived cell lines RPMI8226 and U266 cells were used as in vitro models. Cell apoptosis was assessed by morphology, flow cytometry, and DNA gel electrophoresis. Mitochondrial transmembrane potentials (delta psi m) were evaluated by measuring cellular Rhodamine 123 staining intensity. Protein expression was analyzed using Western blot. RESULTS: Zero point one to 0.5 mumol/L As2O3 inhibited cell proliferation and 2.0 mumol/L As2O3 induced cell apoptosis, while 1.0 mumol/L As2O3 inhibited proliferation with a weak degree of apoptosis induction in RPMI8226 and U266 cell lines. As2O3-induced apoptosis was accompanied by mitochondrial transmembrane potentials (delta psi m) collapse and caspase-3 activation in the presence of intact membrane. Glutathione depleter buthionine sulfoximine enhanced, while disulfide bond-reducing agent dithiothreitol partially antagonized As2O3-induced delta psi m collapse and apoptosis in MM cells. All-trans retinoic acid (ATRA) could also induce apoptosis in RPMI8226 cells, but it did not show any cooperative effects with As2O3. CONCLUSION: As2O3 exerts apoptosis-inducing and growth-inhibiting effects on MM cells, and mitochondrium is a pivotal and common target of As2O3 for apoptosis induction.

Hepatic angiomyolipoma: a clinicopathologic study of 10 cases.

OBJECTIVE: To study the clinicopathologic features of hepatic angiomyolipoma (AML) and to investigate the feasibility of a new antibody-A103 as a diagnostic aid for AML. METHODS: Ten cases of AML were retrieved from hospital records and analyzed morphologically. Immunohistochemistry was performed on paraffin-embedded tissues with a panel of antibodies, including antibody-A103. RESULTS: There were eight women and two men, with ages ranging from 38-58 years (median 45.7). Clinically, nine cases were asymptomatic and found by imaging techniques. None of the patients had associated tuberous sclerosis. All tumors were sharply demarcated from the surrounding liver parenchyma. Histologically they were composed of a heterogeneous mixture of three components: thick-walled blood vessels, spindle or epithelioid smooth muscle cells and adipose tissue. All tumors showed a strong immunoreactivity to A103, HMB-45 and smooth muscle actins. Follow-up information on all 10 cases showed a benign course with no signs of recurrence. CONCLUSIONS: Hepatic AML is a rare mesenchymal tumor of the liver. A103 is a promising marker for a pathologic diagnosis of hepatic AML.

Molecular basis for the effect of lipid lowering drugs on growth factors after de-endothelialization.

OBJECTIVE: To study the mechanism and effect of lipid lowering drugs in arresting the development of arterial restenosis after angioplasty. METHODS: De-endothelialization injury of rabbit aortae, common iliac and femoral arteries using balloon angioplasty and the expression of growth factors such as platelet derived growth factor-B (PDGF-B), transforming growth factor beta-1 (TGF beta-1), and fibroblast growth factors (bFGF) were investigated. Total serum cholesterol (TC) and triglycerides (TG) were analyzed during and after the treatment using either simvastatin combined with gemfibrozil or simvastatin alone for 6 weeks. RESULTS: Serum total cholesterol and triglycerides were only slightly to moderately increased after high cholesterol ration intake lasting for 6 weeks in rabbits of two therapeutic groups (simvastatin plus gemfibrozil or only simvastatin). A positive correlation was found between TC and intimal/medial ratio (r = 0.5873, P < 0.05). PDGF-B detected by immuno-histochemistry and RT-PCR analysis showed that the release of PDGF-B was inhibited by simvastatin and gemfibrozil after de-endothelialization. RT-PCR analysis showed that TGF beta-1 was increased in the neointima in two treatment groups but no definite change was seen in the mRNA of bFGF in the smooth muscle cell (SMC) of the balloon-injured arteries even under lipid lowering drug treatment. CONCLUSION: In addition to the lipid lowering effect, both simvastatin and gemfibrozil also influence the release of PDGF-Band TGF-1 in the neointima after de-endothelialization.

[Pathological changes between HBeAg and anti-HBe in patients with chronic hepatitis B].

OBJECTIVE: To observe the differences of pathological changes between HBeAg and anti-HBe in chronic hepatitis B. METHODS: Liver biopsy was performed in patients with chronic hepatitis B. The histopathological grading and staging were carried out. Serum markers of viral hepatitis were tested. RESULTS: (1) The histopathological grading and staging were closely correlated with the appearance of serum HBeAg and anti-HBe. The number of G(3-4) and S(3-4) in anti-HBe cases was significantly higher than that of G(1-2) and S(1-2), respectively. The number of G(1-2) and S(1-2) in HBeAg cases was significantly higher than that of G(3-4) and S(3-4), respectively. Significant differences were found between the group of anti-HBe cases and the group of HBeAg cases (P<0.005). (2) HBV DNA was detected in 126 of 151 HBeAg positive patients (83.4%), and the positive rate of HBV DNA in anti-HBe positive patient was 29% only. CONCLUSIONS: (1) Severe inflammatory, necrosis and fibrosis of hepatic tissue are observed in patients with anti-HBe positive chronic hepatitis B. (2) Although HBV DNA is positively correlated to HBeAg, HBV-DNA can be detected in part of anti-HBe positive patients.

[Mechanisms of arsenic trioxide-induced apoptosis in myeloma cells].

OBJECTIVE: To investigate the possible mechanisms of arsenic trioxide (As2O3)-induced apoptosis in multiple myeloma (MM) cell line, and the interactions between As2O3 and all-trans retinoic acid (ATRA) or interferon(IFN)-alpha. METHODS: Multiple myeloma cell lines RPMI 8226 and U266 were treated with As2O3 in combination with dithiothreitol (DTT), buthionine sulfoximine (BSO), ATRA and IFN-alpha. Cell viability was counted by trypan-blue exclusion. Apoptosis was assessed by cell morphology and flow cytometry. Mitochondrial transmembrane potentials (delta psi m) were measured by cellular rhodamine 123 staining intensity on flow cytometry. RESULTS: Glutathione depleting agent BSO at 1.0 mmol/L enhanced, while disulfide bond-reducing agent DTT at 0.2 mmol/L partially antagonized As2O3-induced decline of delta psi m and apoptosis in MM cells. ATRA also induced RPMI 8226 cell apoptosis, but it could not synergize with As2O3. On the other hand, As2O3-induced apoptosis did not occur in U266 cells. In addition, IFN-alpha itself did not inhibit the growth and viability of MM cells, nor did it influence the effects of As2O3 on MM cells. CONCLUSION: As2O3-induced apoptosis of RPMI 8226 multiple myeloma cells involves sulphyhydryl groups. ATRA and IFN-alpha do not synergize with As2O3 in the induction of apoptosis of MM cells.

[Differential diagnosis of pulmonary nodules by using enhanced CT with iodinated contrast material].

OBJECTIVE: To evaluate pulmonary nodules by using enhanced CT. METHODS: 89 patients with pulmonary nodules included 63 pulmonary carcinomas and 26 benign pulmonary nodules. Thin-slice CT was performed before injection, 35 s, 2 min and 5 min after injection of 100 ml of contrast material at 2 ml/s. The maximum level of enhancement and the enhanced patterns were recorded. RESULTS: Lung carcinomas enhanced significantly more than benign nodules. 27 lung carcinomas showed the sign of enhanced spots and twigs; 51 lung carcinomas and 4 benign nodules showed inhomogeneous enhancement; 3 lung carcinomas and 4 benign nodules had homogeneous enhancement; 5 tuberculomas and 1 inflammatory pseudotuma appeared ring enhancement; 12 tuberculomas showed nonenhancement. CONCLUSION: Enhanced CT is a valuable method to differentiate pulmonary carcinomas from benign pulmonary nodules.

[Preliminary study on orthodontic treatment before and after distraction osteogenesis].

OBJECTIVE: To get some guidelines for the orthodontic treatment before and after distraction osteogenesis (DO). METHODS: DO was performed in 4 patients maxilla and two for mandible. The patients were aged from 19 to 25 (average 21.5). We try to sum up the procedures and clinical experiences of pre- and post-surgery orthodontics. RESULTS: The procedures and experiences obtained are the followings: 1. Pre-surgery orthodontics for DO is simplified as alignment of dental arch in a short period. 2. Post-surgery orthodontics for DO is more complicated and peculiar. The main task is to make the opened posterior teeth occlude as soon as possible by light force vertical elastics. At the same time, some cross elastics may be needed for correction of posterior occlusal relationship. 3. Expansion of upper arch may be needed in some patients because of constricted dental arch. 4. Class II or III elastics may be used for prevention of relapse after DO. CONCLUSIONS: DO is a new technique in orthognathic surgery. Post-surgery orthodontics is more complicated and important in the treatment course.

[Pre- and post-surgical orthodontic treatment for skeletal open bite].

OBJECTIVE: To Study the principles and rules of pre- and post-surgical orthodontic treatment for skeletal open bite patients. METHODS: Thirty-two surgically treated open bite cases were analyzed, of which 9 were males, and 23 were females, aged from 16 to 38. Open bite was from 1 to 8.5 mm, average was 4 mm. 31 patients were Class III malocclusion, while 1 patient was Class II malocclusion. RESULTS: 1. Totally 21 patients were treated with orthodontics before and after orthognathic surgery, while 8 patients had pre-surgical orthodontics only, and other 3 had post-surgical orthodontics only. The duration for pre-surgical orthodontics was from 4 to 33 months, average was 12 months. The duration for post-surgical orthodontics was from 3 to 17 months, average was 8.5 months. 2. Presurgical orthodontic treatment included: Alignment of arches, decompensation of incisors, avoiding extrusion of incisors, and slight expansion of arches for coordination of arches. 3. Post-surgical orthodontic treatment included: Closure of residual spaces in the arches, realignment of arches, vertical elastics and Class II or III intermaxillary elastics. CONCLUSIONS: Skeletal open bites require combined orthodontic-orthognathic surgery for optimal and esthetical pleasing results.

[Ecophysiological response of Pinus koraiensis seedlings to elevated CO2].

Studies on the ecophysiological response of Pinus koraiensis seedlings were conducted in open-top chambers which were controlled with CO2 concentration of about 500 and 700 mumol.mol-1. The result shows that there were different responses in net photosynthetic rate and stomatal conductance at high CO2(500 and 700 mumol.mol-1) and ambient CO2 concentrations. Under 500 mumol.mol-1 CO2, the carboxylase activity of RuBP was the highest, the photosynthesis showed "up-ward", the average daily net CO2 assimilation was the greatest, and the contents of chlorophyll and soluble sugars were the highest. While under 700 mumol.mol-1 CO2, the photosynthesis showed "down-ward" and was lower than that of the control, and the enzyme activity and the contents of substances were the lowest.

Effects of nimodipine on changes of endothelin after head injury in rabbits.

OBJECTIVE: To investigate the effects of nimodipine on changes of endothelin after head injury. METHODS: Sixty-five adult rabbits were randomized into an injury group (IG, n=30), a nimodipine-treatment group (NTG, n=30) and a control group (CG, n=5). With their heads unfixed, the animals in IG and NTG were injured in the frontal-parietal zone with BIM-II bioimpact. Blood samples and brain tissue were collected before and after injury. The endothelin level was measured with RIA. RESULTS: The endothelin level in plasma and brain tissue was significantly increased 24 hours after injury. At the 8th or/and 24th hours postinjury, the endothelin level was significantly lower in NTG than that in IG. CONCLUSIONS: Nimodipine can prevent endothelin from elevation and act as a practical endothelin antagonist after head injury clinically.

[Clinicopathological significance of grading on thymic epithelial tumors].

OBJECTIVE: To study the clinicopathologic relevance of a thymic epithelial tumor (TET) grading standard with the WHO classification. METHODS: A grading system for TET was proposed based on the application of WHO histological typing of thymic tumors and analyzed in relation to clinical therapy results and follow-up data of 200 TET cases. RESULTS: In this series, 8 patients (4.0%) belonged to type A, 68 (34.0%) were type AB, 17 (8.5%) were type B1, 39 (19.5%) were type B2, 27 (13.5%) were type B3 and 36 (18.0%) were type C. The remaining 5 cases were rare thymomas. The overall postoperative survival data showed highly significant differences among the histological subtypes (P < 0.001). Type A & type AB thymomas showed excellent prognosis, none of these patients died of tumor; in type B1, only 1 case (5.9%) died at 22 months postoperatively. Types B2, B3 and C thymomas shared the bad, worse and worst prognosis. Ninety-six patients (48.0%) were in stage I, 26 (13.0%) in stage II, 65 (32.5%) in stage III and 13 (6.5%) in stage IV. Clinical stage is also highly significant in predicting survival (P < 0.001). It was found that tumor histology could predict survival expectancies well in stage I and stage II cases. It was also found that type B2, B3 and C thymomas had a statistically significant worse prognosis than type A, AB and B1 thymomas (P < 0.001). According to the histology, clinical data, biological behavior and prognosis, it is proposed that thymomas be divided into 4 grades: grade I, II, III and IV. Follow-up is the best strategy for grade I & II patients after radical surgery. In this series, the 30 patients (15.0%) presenting clinical signs of myasthenia gravis were mostly in type B2 and B3 groups (P < 0.01). CONCLUSIONS: The WHO classification for TET provides good pathological definitions and criteria for diagnosis, which can independently predict the invasiveness and prognosis of TET. TET grading is of use in unifying pathological and clinical findings, in selection of proper therapy and in predicting prognosis.

[An experimental study on systemic inflammatory response syndrome induced by subeschar tissue fluid].

OBJECTIVE: To investigate the effects of subeschar tissue fluid (STF) on normal rats and their peritoneal macrophages in vitro, so as to explore its relationship with inflammatory mediators. METHODS: Eighteen Wistar rats were evenly and randomly divided into three groups, i.e. STF, normal human serum control and blank control groups. The changes of the heart and respiratory rates of rats in every group were recorded by two channel physiological recorder. Blood samples were collected for the determination of hepatic and renal function. The hepatic, pulmonary, renal and cardiac tissues were harvested for routine morphological examination. The changes of the secretion of TNFalpha, IL-1 were also determined. RESULTS: The heart and respiratory rates increased after the infusion of STF, and leukocyte count increased along with different degrees of injury to lungs, liver and kidneys. It was indicated in in vitro experiment that STF could activate macrophage and stimulate the secretion of TNFalpha and IL-1 from the macrophage. CONCLUSION: STF might induce the development of SIRS in rats, which further lead to the development of MODS.

[The combination of cyclosporin A and androgen in the treatment of chronic aplastic anemia].

OBJECTIVE: To explore the therapeutic effectiveness of combination of cyclosporin A (CsA) and androgen in the treatment of chronic aplastic anemia (CAA). METHOD: Androgen alone or combined with CsA for the treatment of CAA was compared by a randomized controlled clinical trial. RESULT: The efficacy of androgen combined with CsA (87.9%) was higher than that of androgen alone (57.1%). Therapeutic effectiveness of the combination treatment between the patients with positive and negative peripheral blood mononuclear cells (PBMNCs) inhibiting normal colony formation unit-granulocyte and macrophage (CFU-GM) test showed a significant difference (P < 0.005). CONCLUSION: Androgen combined with CsA had a much better efficacy than that of androgen alone in the treatment of CAA. The patients' PBMNCs inhibiting normal CFU-GM growth test can be used as an index of the treatment outcome. The side effects of the combination therapy are low and tolerable.
