[Treatment of cancer of the hard palate--analysis of 93 patients].

From Jun. 1958 to Jun. 1977, 128 patients with cancer of the hard palate were treated in our hospital. Of them, 93 (58 male and 35 female), proved by pathology and followed up more than five years, were analysed. The peak age was between 40 approximately 59 years (comprised 48% of all patients). Sixty-six patients were treated by radiation only and 27, by the non-randomized combination of surgery and radiation. The dose for radiation alone was 7,000-10,000 rad (for the neck 4,000-5,000 rad) and for the combined therapy was 5,000 rad. The five year survival rates of radiation alone and the combined therapy were 24% and 59% (P less than 0.05). The factors influencing prognosis were: 1. Age--patients with ages in the fourth and fifth decades gave the best result (51%). 2. Clinical stage--the five year survival rates in stage I, II, III and IV were 92%, 43%, 37% and 17%, respectively. The results of the combined therapy were better than those of the radiation alone, whatever the stage was. 3. Dose--when the dose for radiation alone exceeded 7,000 rad, the results were better. The combined therapy gave more satisfactory results with the dose 5,000-8,000 rad. The final outcome was not influenced by sex and pathological classification. Of 72 patients who died, 55 were treated by radiation alone (64% of patients died of local uncontrol), and 17 by the combined therapy, (died of hematogenous spread 47% and local recurrence 41%).

Effects of cadmium on cadmium smelter workers.

The effects of cadmium on 65 workers in a cadmium-refining plant in the People's Republic of China were investigated. The average airborne cadmium oxide concentrations ranged from 0.004 to 0.187 mg/m3 at most worksites. Except for some vague symptoms and reports of anosmia, no significant abnormalities were found in the clinical examinations. The level of cadmium in the blood and urine was increased in the majority of the cadmium-exposed subjects. Nine cases (13.8%) with suspected or mild renal tubular damage were found in this study.

Antagonists of the luteinizing hormone releasing hormone (LHRH) with emphasis on the TRP7 of the salmon and chicken II LHRH's.

The sequences of four naturally occurring luteinizing hormone releasing hormones (LHRH's) differ only in positions 5, 7 and 8. Salmon and chicken II LHRH's have Trp7; porcine/ovine (P/O) and chicken I LHRH's have Leu7. The receptor for P/O LHRH might effectively bind certain antagonists with Trp7. Thirteen antagonists having Trp7 and eight antagonists with other substitutions in position 7 were synthesized. One of the thirteen antagonists with the natural Trp7, [N-Ac-D-2-Nal1,D-pClPhe2,D-3-Pal3,D-Arg6,Trp7,D- Ala10]-LHRH, not only maintained activity, but had increased potency (ca. 58%; 90% antiovulatory activity/250 ng; rats) in comparison with the companion analog with the natural Leu7 of P/O LHRH. The other twelve Trp7-antagonists had lower potency.

Antagonists of substance P from emphasis on position 11.

Ten analogs of substance P (SP) were designed and synthesized. The agonist and antagonist activities against SP were assayed on the isolated guinea pig ileum. The prime designs were changes of the important Met11 to include Leu11, Thr11, D-Leu11 and D-Ala11. Step-wise designs of changing D-Arg1 and D-Pro2 to the corresponding L-configurations resulted in decreasing antagonist activity. Changing Leu11 to D-Leu11 and D-Ala11 reduced antagonist activity. [D-Arg1,D-Pro2,D-Trp7,D-Trp9,Leu11]-SP is the most potent antagonist of this group of analogs, and required a 100-fold increase in the concentration of SP to give 50% of the maximal response caused by SP.

Quantitative analysis of technetium 99m 2-methoxyisobutyl isonitrile single-photon emission computed tomography and isosorbide dinitrate infusion in assessment of myocardial viability before and after revascularization.

BACKGROUND: Assessment of viable myocardium in territories of hypoperfused myocardium is important for predicting functional recovery after revascularization. This study was designed to evaluate quantitative analysis of 99mTc-labeled 2-methoxyisobutyl isonitrile (MIBI) myocardial perfusion imaging combined with isosorbide dinitrate (ISDN) infusion to detect myocardial viability in patients with chronic coronary artery disease before and after revascularization. METHODS AND RESULTS: Twenty-seven consecutive patients with previous myocardial infarction and left ventricular dysfunction (left ventricular ejection fraction 35.2% +/- 13.5%) referred for coronary artery bypass (CABG) were studied with 99mTc-labeled MIBI single-photon emission computed tomography at rest and during ISDN infusion before CABG followed by resting imaging after CABG. Quantitative analysis was performed with circumferential profiles. Left ventricular function (global and regional) was assessed by radionuclide ventriculography before and after CABG. Out of 212 abnormal perfusion segments with resting 99mTc-labeled MIBI SPECT, 99 segments (47%) showed improved uptake of 99mTc-labeled MIBI during ISDN infusion. The mean ratio of myocardial uptake was 0.58 +/- 0.25 (resting 0.53 +/- 0.23; p < 0.05). After CABG, of 212 segments with hypoperfusion, 108 segments (51%; p > 0.05 vs ISDN) showed improved uptake of 99mTc-labeled MIBI. The mean ratio of myocardial uptake was 0.60 +/- 0.26 (resting 0.53 +/- 0.23; p < 0.05). The concordance between the improvement of post-CABG wall motion and that of pre-CABG ISDN perfusion imaging was 83%, between the improvement of wall motion and perfusion imaging after CABG 94%, and between the improvement of pre-CABG ISDN and post-CABG perfusion imaging 83%, respectively. CONCLUSION: ISDN infusion can improve the uptake of 99mTc-labeled MIBI in hypoperfused myocardium and increase the efficiency of 99mTc-labeled MIBI in the detection of viable myocardium in patients with previous myocardial infarction and left ventricular dysfunction.

High resistance to cucumber mosaic virus conferred by satellite RNA and coat protein in transgenic commercial tobacco cultivar G-140.

A chimeric vector was constructed to express cucumber mosaic virus (CMV) satellite (Sat) RNA and coat protein (CP). Transgenic lines of tobacco cultivar G-140 expressing CP and Sat-RNA were obtained; these lines had high resistance to CMV. Fifty to 70% of the transgenic plants were symptomless 90 days after inoculation with 25-50 micrograms/ml of CMV. Resistance was about twice that conferred by the Sat-RNA or the CP gene alone in transformed plants.
