DNA-dependent mono(ADP-ribosyl)ation of p33, an acceptor protein in hen liver nuclei.

A non-histone acceptor protein for hen liver nuclear ADP-ribosyltransferase was purified to an apparently homogeneous state through salt extraction and chromatography on hydroxyapatite, phenyl-Sepharose, carboxy-methyl-cellulose, Sephadex G-75, phenyl 5-PW, mono S and Radial PAK C18. This protein was termed p33. The ADP-ribosylation of p33 was enhanced more than 60-fold by double-stranded DNA. Single-stranded DNA, RNA and poly(L-glutamate), but not deoxyribonucleotide, were partially effective. DNA-dependent ADP-ribosylation was also observed when whole histones were used as acceptor. DNA required for the maximal ADP-ribosylation depended on the dose of the acceptor protein; the optimal mass ratio of DNA to the acceptor protein was 1:1 with both p33 and whole histones. DNA decreased the Km for NAD and concomitantly increased the Vmax value, but did not alter the Km for p33. These results are consistent with the idea that p33 may participate in chromatin processes such as replication or transcription, through modification by nuclear ADP-ribosyltransferase.

Surgical correction of complete bifid scrotum using subcutaneous pedicle flaps; a report of two cases.

A method of surgical correction for the separation and severe transposition of the scrotum associated with perineal hypospadias is reported. Although this is a rare congenital anomaly, we have treated two cases in the past 7 years. A technique has been devised by which the scrotum is divided into two subcutaneous pedicle flaps, enabling complete correction of the transposed penis and scrotum.

Xanthine:acceptor oxidoreductase activities in ischemic rat skin flaps.

Xanthine:acceptor oxidoreductase activities were assayed in free skin flaps following prolonged preservation. In normal rat skin, xanthine dehydrogenase transfers electrons to NAD+ and accounts for 73% of total oxidoreductase activity, and xanthine oxidase transfers electrons to molecular oxygen and accounts for the remaining 27%. Xanthine oxidase activity increased significantly in skin flaps during ischemia: approximately 30 and 100% increases after 6 and 24 hr of ischemia, respectively. Allopurinol inhibited xanthine oxidoreductase activity: free skin flaps obtained from allopurinol-treated animals exhibited a low level of xanthine oxidoreductase activity throughout the period of preservation. Systemic allopurinol significantly improved the survival rate from 32 to 75% of free flaps transferred after 24 hr of preservation at room temperature. These observations suggest that the xanthine oxidase system is a major source of oxygen free radicals following ischemia/reperfusion in skin. The increase in xanthine oxidase is attributable to the conversion of xanthine dehydrogenase to oxidase, a conversion which involves sulfhydryl oxidation in skin flaps.

Sonographic examination of sialolithiasis.

Ultrasound examination for sialolithiasis was carried out in ten cases, and the findings were compared with those from sialography. Salivary stones were recognized as high-level reverberation echoes accompanied by posterior acoustic shadows in eight cases, whereas in the remaining two they appeared only as hyperechoic masses. In a case of sialolithiasis of the parotid gland that was difficult to detect by simple radiographic examination, a salivary stone was detected by both sialographic and ultrasonographic examination. Clearly separated salivary stones were also easily detected in each case by the ultrasonographic method, but multiple, gathered calculi were not precisely detected. The smallest sialolith that could be detected was approximately 2 x 3 mm in size. The rate of detection of salivary stones by using the ultrasonographic method was as high as that from using sialography. Because the examination is simple and noninvasive, its use to evaluate sialolithiasis is probably indicated more frequently, especially when there is acute infection in the gland.

Ethylidene glucose-substituted new analogue of streptozotocin cannot induce diabetes: study on the basis of structure and activity relationship.

4,6-O-Ethylidene glucose (ethylidene glucose), a specific inhibitor at the outer surface of a glucose transporter in the cell membranes, substituted analogue of streptozotocin was newly synthesized. This compound did not induce diabetes in rats and also did not show cytotoxic effect on pancreatic beta cells of neonatal rats in a monolayer culture system. The reasons why such a molecule was designed and why it showed no biological effects are discussed on the basis of a structure-activity relationship. Our results afford positive evidence for the presence of a glucose transport system or a glucose transporter on pancreatic beta cells and its involvement in the action of streptozotocin on beta cells.

Clinical experience in the induction of ovulation and pregnancy with pulsatile subcutaneous administration of human menopausal gonadotropin: a low incidence of multiple pregnancy.

The pulsatile subcutaneous administration of human menopausal gonadotropin (hMG) or follicle-stimulating hormone (FSH) was used for induction of ovulation in 26 patients with hypothalamic/pituitary amenorrhea or polycystic ovary syndrome (PCO). Ovulation was observed in 116 (90.6%) of 128 treatment cycles, and 15 (16 treatment cycles) of 26 patients became pregnant. All 14 fetuses, excluding two pregnancies interrupted spontaneously at weeks 6 and 9, were singleton conceptions. Ovarian hyperstimulation was observed in 15.6% of treatment cycles. Five patients with PCO who failed to conceive on the hMG regimen also received pulsatile FSH administration. Although ovulation rates in PCO patients did not differ significantly between the hMG (88.1%) and FSH (88.2%) regimens, a significant reduction in the average dose of FSH (P less than 0.05) was observed with pulsatile FSH administration. Furthermore, the number of patients who conceived during the FSH regimen was significantly greater than that found with hMG treatment. The present data demonstrate that pulsatile subcutaneous administration of hMG or FSH is effective in induction of successful ovulation and establishment of singleton pregnancy in patients with various types of anovulatory infertility.

Salivary gland function after sialolithiasis: scintigraphic examination of submandibular glands with 99mTc-pertechnetate.

Scintigraphic evaluation of 13 cases of submandibular duct sialolithiasis was performed to determine glandular function and to estimate the justification for sialoadenectomy. Twenty-two per cent of these glands showed no function despite sialolithectomy, which suggests that it would be reasonable to excise submandibular salivary glands that have no recovery of function. However, the remaining 78% of the glands showed varying degrees of recovery of function, indicating that sialolithectomy should be done first, followed by 99mTc-pertechnetate scintigraphic examination to determine the necessity of further sialoadenectomy.

Treatment and therapeutic devices in the management of frontal sinus fractures. Our experience with 42 cases.

Forty-two cases with frontal sinus fractures were treated at the Department of Plastic and Reconstructive Surgery of Fujita-Gakuen Health University Hospital between December, 1978 and December, 1987; this experience and a discussion of our management of these cases are presented herein. Our concept of fundamental treatment is based on not obliterating the sinus, but rather removing the devitalized mucosa and establishing permanent drainage to the nose. During the operation, we use a cold light source, which provides a fine operative field, and a special cutting saw which allows us safely to perform bone grafts. Up to now we have experienced no significant complications and have achieved uniformly good results, both functionally and aesthetically.

Developmental potential of rabbit oocyte matured in vitro: the possible contribution of prolactin.

The present study was undertaken to define hormonal conditions for in vitro maturation that support subsequent fertilization and embryonic development. Follicular oocytes were recovered from nonstimulated rabbit ovaries and cultured for 12 h in Brackett's medium supplemented with or without hormones. Matured oocytes were inseminated in vitro and transferred 12 h later to Ham's F-10 medium supplemented with 20% fetal calf serum. The initial cleavage frequency of matured oocytes in Brackett's medium was comparable to the frequency of development for in vitro-matured oocytes under various hormonal conditions. However, the addition of estradiol (E2, 1 microgram/ml) to incubation medium containing luteinizing hormone (LH) and follicle-stimulating hormone (FSH) increased significantly (p less than 0.001) the percentage of embryos achieving morula or blastocyst formation (16/98, 16.3%), as compared to the mature oocytes in medium containing LH, LH plus FSH, or no hormone. The addition of prolactin (PRL) to the maturation medium increased the percentage of development to organized embryos in a dose-dependent manner. In vitro-matured oocytes in medium containing LH, FSH, and PRL exhibited a significantly (p less than 0.001) lower incidence of developmental competence (5/95, 5.3%) than oocytes matured in the presence of E2 in conjunction with pituitary hormones (43/89, 48.3%). These results demonstrate that hormonal composition in the environment of the oocyte is critical for acquisition of developmental capacity. PRL as well as E2 appears to be an important constituent in the process of oocyte maturation, promoting preimplantation embryonic development.

Effects of dibutyryl cyclic AMP on oocyte maturation and ovulation in the perfused rabbit ovary.

The involvement of cyclic AMP (cAMP) in mammalian oocyte maturation was assessed using cultures of rabbit cumulus-oocyte complexes and in-vitro perfused rabbit ovaries. Rabbit cumulus-oocyte complexes were cultured in Brackett's medium with or without dibutyryl cyclic AMP [Bu)2cAMP) at 10(-3), 10(-4) or 10(-5) M for 4-12 h. At 4 h spontaneous meiotic maturation was significantly inhibited by (Bu)2cAMP (P less than 0.025). With prolonged incubation, spontaneous maturation progressed despite exposure to (Bu)2cAMP. When ovaries were continuously perfused in vitro for 12 h with (Bu)2cAMP (10(-3) M) or medium alone, (Bu)2cAMP stimulated ovarian progesterone production, but did not affect ovulation or maturation of follicular oocytes. When ovaries were perfused in vitro with or without (Bu)2cAMP (10(-3), 10(-4) or 10(-5) M) for the first 2 h and then transferred to medium without (Bu)2cAMP for an additional 10 h, ovulation did not occur, but transient exposure to (Bu)2cAMP stimulated a dose-related increase in maturation of follicular oocytes. Degeneration of follicle-enclosed oocytes and cumulus-oocyte complexes was not affected by exposure to (Bu)2cAMP. These results suggest that transient, but not continuous, elevation of cAMP after the gonadotrophin surge may be required for the initiation of oocyte maturation.

[Balloon occluded arterial infusion (BOAI) therapy in patients with progressive cervical carcinoma].

Ten patients with progressive cervical carcinoma were treated with intra-arterial infusion by BOAI (balloon occluded arterial infusion) in order to secure high concentrations of anti-tumor agents. This study included 9 cases (2 stage IIb, 5 stage IIIb, and 2 stage IV) with squamous cell carcinoma and 1 case (stage IIIb) with adenocarcinoma. Cis-platin (CCDP) 100 mg/body and bleomycin (BLM) 20-30 mg/body and and/or aclacinomycin (ACM) 30-40 mg/body were infused after the bilateral internal iliac arteries were occluded. 1. The serum concentration of free-Pt reached its maximum 15-30 min after BOAI therapy and returned to an undetectable level in 3 hours. 2. The free-Pt concentration following BOAI therapy was 2 to 3 times greater than those found with intravenous infusion. 3. The free-Pt concentration in internal iliac arteries was twice as great as those observed in the femoral arteries or peripheral veins. 4. The tissue concentration of Pt after BOAI was significantly (p less than 0.05) greater than those found with intra-venous infusion. 5. BOAI treatment resulted in 2 cases of partial response, 6 cases of minor response and 2 cases with no change. These results demonstrate that BOAI therapy is effective for the treatment of progressive cervical carcinoma by increasing intratumoral concentrations of the drugs.

[Vincristine, adriamycin, mitomycin-C and UFT (VAM-UFT) therapy in progressive or recurrent breast cancer].

Since June 1984, 23 cases of progressive or recurrent breast cancers were treated with combination chemotherapy of VAM-UFT consisting of vincristine, adriamycin, mitomycin C and UFT. Clinical effects of VAM-UFT therapy were 3 CR, 12 PR, and the response rate was 65.2%. Its effective interval was 3 months. But the patients treated with over 4 cycles of VAM-UFT therapy showed an 85% response rate, with a 5-month effective interval. In each patient's background, a shorter disease free interval tended to be more highly effective, but other factors were not significant. Scirrhous carcinoma of pathology evidenced slightly high response rate. Compared with the survival time of patients treated with under 3 cycles and over 4 cycles of this therapy, the latter was significantly longer. Toxicity involved leukocytopenia (74%), thrombocytopenia (22%), anemia (30%), alopecia (91%), nausea and vomiting (87%) and stomatitis (35%), but cases in which the treatment was stopped were not observed. Therefore VAM-UFT therapy had a highly therapeutic effect, reflected in an 85% response rate, for progressive or recurrent breast cancers.

Ultrastructural and immunoelectron microscopic studies on infiltrating mononuclear cells in lymphocytic submandibulitis in NOD mice.

The ultrastructural relations of the infiltrating mononuclear cells to the parenchymal tissues were studied in the submandibular gland of the female non-obese diabetic (NOD) mouse. In addition, the phenotype of mononuclear cells infiltrating the submandibular gland has been determined by light and electron microscopy by using monoclonal antibodies against T-cell subsets (Thy1.2, Lyt1, Lyt2). Ultrastructurally, lymphoid cells were frequently observed around and in the acini and ducts. Some of the lymphoid cells observed in the acini and ducts were irregular in shape and sometimes sent spike-like projections into acinar and ductal cells. Immunohistochemical study demonstrated that Thy1.2+ cells were predominant among the infiltrating cells, and the majority of these infiltrating T-cells were composed of Lyt1+ cells with a small proportion of Lyt2+ cells. By immunoelectron microscopy, lymphocytes carrying Thy1.2, Lyt1 or Lyt2 antigen were identified, as is demonstrated by an electron-dense reaction product on the entire cell surface, and these immunopositive cells were frequently observed around and in the acini and ducts. Some of the Thy1.2+, Lyt1+ or Lyt2+ cells observed in the acini and ducts demonstrated a close contact with acinar and ductal cells and both Lyt1+ and Lyt2+ cells sent spike-like projections into them. Occasionally, a partial degeneration of acinar cell adjacent to the invading lymphocytes was observed. These observations suggest that T-lymphocytes are involved in the direct destruction of acinar and ductal cells in the NOD mouse submandibular glands.

[A case of glomus tumor of the stomach, including a review of the literature of 52 reported cases in Japan].

Discussed is a 62-year-old male who was admitted to hospital for an examination of the stomach. Physical examination and associated laboratory data uncovered no unusual results. On barium ingestion, however, an X-ray revealed an oval shaped filling defect at the greater curvature of the antrum. Further, endoscopic examination revealed half of a globular tumor with a surface redness and bridging folds radiating from the tumor. Examination of a biopsied specimen led to the determination of a glomus tumor and the patient underwent an operation. On operation, a well circumscribed submucosal tumor, 1.5 x 1.5 x 1.5 cm in size, with lobulation was found in the antral submucosal region, this tumor extending to the subserosa. A histopathological study of this neoplasm revealed that it consisted of irregular, swollen, blood vessel lumens lined with flattened endothelial cells, with surrounding aggregates of glomus cells forming lobules. In addition to this case, 52 cases of glomus tumors reported in the Japanese literature are discussed.
