Waldenström's macroglobulinemia. Report of an autopsy case presenting with a pulmonary manifestation.

An autopsy case of Waldenström's macroglobulinemia is reported, in whom an abnormal pulmonary shadow had already existed 2 years before the diagnosis of the disease and was proved to be pulmonary involvement. Immunoelectrophoresis demonstrated a monoclonal increase in immunoglobulin M with kappa light chain. A chest X-ray film showed a reticulo-nodular shadow in the right lower lobe of the lung. A bronchial biopsy specimen revealed a diffuse and dense lymphocytic infiltration. Bone marrow aspirate revealed no remarkable change except for a slight increase in plasma cells (1.7%) and an appearance of atypical lymphocytes (0.5%). At autopsy, more than half of the right lower lobe of the lung was occupied by a pale whitish, viscid and glossy tumour mass. Heptosplenomegaly and lymph node enlargement were not observed. Histological findings of the tumour tissue were similar to those of the biopsy specimen. Lymphocytic infiltration was observed also in the liver, kidneys, spleen, bone marrow and lymph nodes, but was of minor degree. Other reported cases of Waldenström's macroglobulinemia accompanied by pulmonary involvement are reviewed.

A small temporal lobe glioma detected only by magnetic resonance imaging.

The authors were able to demonstrate a very small glioma (1.0 x 0.8 cm in size) of the temporal lobe using MRI. They discuss the relative merits of CT and MRI in the diagnosis of gliomas in terms of their size. CT fails to detect tumours less than 2.0 x 1.5 cm in size which are not calcified. So far, MRI has revealed a tumour as small as 1.0 x 0.8 cm. Therefore, MRI is superior to CT for detecting small gliomas.

Intercapillary distance in the proliferating area of human glioma.

Tissues from 22 patients with glioma and 7 patients with normal white matter were used in this study. First we determined the proliferating area in the tumor tissue by evaluating the growth fraction (GF). Second we determined the mean number of cells and measured the intercapillary distance in a modified tumor cord in the proliferating area by using the texture analyzing system and Voronoi's method. Average GFs of tumor cells and tumor endothelial cells were 27.7 +/- 5.3% (SD) and 36.8 +/- 8.1% in many malignant gliomas and 6.5 +/- 3.1% and 17.0 +/- 11.9% in many benign gliomas. The mean number of cells within a tumor cord was 15.9 +/- 6.4 in the proliferating areas of all gliomas and 22.3 +/- 6.8 in the control group. The mean intercapillary distances were 77.8 +/- 15.9 micron in the proliferating area of all gliomas and 123 +/- 4.2 micron in the control group. There were significant differences in the number of cells and the intercapillary distances between the proliferating group and the control group (t value; P less than 0.05--less than 0.001). It has been suggested that GF is high in glioma cells and tumor endothelial cells in malignant gliomas and that GF of endothelial cells is always greater than that of the tumor cells. It is suggested that it is possible to use Voronoi's method for the interpretation of biological action in human gliomas.

Tumor cord and growth in human brain tumors based on mathematical morphology.

The growth of human brain tumor was quantitatively investigated by using the anti-bromodeoxyuridine (BrdU) monoclonal antibody and the texture analyzing system (TAS). Tissues from thirty eight patients consisting of 22 gliomas, 8 brain metastases, 1 cerebellar hemangioblastoma and 7 normal white matters were applied in this study. Mean values of growth fractions (GFs) were 29.4% in group 1, 14.8% in group 2, and 6.5% in group 3. The higher value of GFs showed a significantly larger degree of perivascular aggregation of S-phase cells (DPAS). The mean number of cells in each tumor cord was approximately 20, which is not significantly larger than that of the normal group. But there was a significant difference between the non-selected area of the histologically malignant or benign gliomas and the proliferating area of the biologically malignant or benign brain tumors (t-value; p less than 0.001). A significant difference was also noticed in the mean area and diameter of tumor cords (t-value; p less than 0.001). From the point of view of biological behavior in human brain tumor, it is suggested that one tumor cord contains approximately 20 viable cells in the proliferating area in both benign and malignant brain tumors, and that if viable cells increase to more than 20 cells in one TC, necrotic cells and the diameter and area of the TC will increase, because the vessel-density is decreased by the occlusion of the vessels and the death of the endothels.

The measurement of serum immunoreactive pancreatic secretory trypsin inhibitor in gastrointestinal cancer and pancreatic disease.

The clinical usefulness of serum pancreatic secretory trypsin inhibitor (PSTI) in pancreatic disease and gastric and colorectal cancer has been examined. The results showed that serum PSTI in acute pancreatitis was significantly higher than in normal subjects and it was also raised in acute exacerbations of chronic pancreatitis. Although the sensitivities of serum PSTI, amylase and elastase I were similar, serum PSTI in necrotizing hemorrhagic pancreatitis was 2.7 times higher than in mild acute pancreatitis. Only a few patients with chronic pancreatitis showed increased concentrations and the mean value was near normal. The mean PSTI in patients with pancreatic and colorectal cancer was higher than normal, although that of gastric cancer was within normal limits. The sensitivity of serum PSTI measurements in patients with these three malignant diseases was only about 30%. The results suggested that the measurement of serum PSTI could be useful in the diagnosis of acute pancreatitis, but of limited value in the diagnosis of other disease which we examined.

[Investigation of marking procedure for markers in deep-seated organs by proton beam therapy].

To obtain, an accurate outline of a tumor lesion in deep-seated organs with X-ray images in proton therapy, an implantation procedure of markers to determine the margins of the lesion has been developed. Nonradioactive Au grains were used as the markers and produced sufficiently clear images. The procedure consists of implanting the markers with a specially designed injector for endoscopic, palpative and intraoperative approaches to the lesions, which was performed without any adverse effects. Due to application of this procedure, the accuracy of treatment planning and pinpointing the site of the treatment field has improved greatly.

Biliary penetration of cefbuperazone in the presence and absence of obstructive jaundice.

In thirteen patients with normal liver function, the mean concentrations of cefbuperazone in hepatic bile, gall bladder bile and gallbladder tissue 30 min after injection were 1134.8 +/- 36.8 (mean +/- S.E.M.) mg/l, 6.6 +/- 3.0 mg/l and 26.1 +/- 7.6 mg/l, respectively. In patients with obstructive jaundice, cefbuperazone concentrations in bile were 99 +/- 29.2 mg/l (mean +/- S.E.M.) 1 h post-dose and decreased to 13.9 +/- 5.1 mg/l 6 h post-dose. In both groups of patients biliary concentrations of cefbuperazone were higher than the MICs of most organisms causing biliary infection.

Late radiation effects in the mouse brain. A quantitative study on PAS positive deposits.

Mice received whole brain irradiation (250 kV roentgen rays) with single doses ranging from 13 to 25 Gy. PAS positive deposits, microscopically observed in the dorsal part of the ventral thalamic nucleus, were quantitatively scored at different intervals between 20 and 85 weeks after irradiation. A significant dose-dependent inhibition of the formation of these deposits was observed in the irradiated brains.

[Usefulness of esophageal prosthesis in inoperable esophageal cancer].

Radiotherapy of inoperable esophageal cancer provided palliation for dysphagia in only 40% of the patients treated over the past 5 years at our hospital. The remaining patients were unable to eat a regular diet even after radiotherapy and to provide them with nourishment, a gastrostomy and/or IVH were required. We feel that the release of the patient from a medical environment will lead to a better quality of remaining life. From our experience with a prosthesis tube for palliation of a malignant esophageal stricture, 4 patients showed excellent results with this way of improving their oral intake.

Isolation of Vibrio fluvialis, and unusual pathogen in acute suppurative cholangitis.

We report a case of acute suppurative cholangitis, from which were isolated Enterobacter aerogenes and Vibrio fluvialis. There have been no previous reports of the isolation of V. fluvialis in this disease. It is usually found in seawater and various seafoods and may cause acute diarrhea, but this is the first report in which it has been associated with acute suppurative cholangitis.

Quantitative histology of peritumoural low density area associated with glioma on CT; image analyzing study of peritumoural low density area after steroid-osmotherapy.

In this study, an invasive site of the tumour cells was quantitatively investigated in the low density area (LDA) on computerized tomography (CT) after steroid and osmotic drug therapy (SOT) by using a texture image analyzing computer system (TAS). Twenty six patients with gliomas (10 benign, 16 malignant) were applied in this study. According to the effect to reduce the volume of LDA, patients were classified into three groups as follows; good, poor, and failed. The reduction effect of the low density volume was not significantly different between the benign and malignant gliomas. Cells including those with a large nucleus with 6.8-9.1 microns in diameter were not observed in the control group, but they were observed in the adjacent and distant parts from the centre of the enhanced tumour element in the peritumoural LDA with 0.87-0.99 percent of all cells in both the poor and failed groups. But in the good group they were significantly rarely observed in the peritumoural LDA after SOT (p less than 0.05). Furthermore, they were significantly frequently observed in the adjacent part from the centre of the enhanced tumour element in the peritumoural LDA with 0.9 percent of all cells after SOT in the malignant gliomas in comparison with the benign gliomas (p less than 0.05). Conclusively it was suggested that the treatment of glioma with peritumoural LDA on CT must be extended to the adjacent and distant part from the centre of the enhanced tumour element in the peritumoural LDA as well as the treatment of the enhanced tumour mass on CT in the poor and failed groups.
