Lymphoepithelial cysts in the mediastinum with an opening to the trachea.

A case of lymphoepithelial cysts (LECs) in the mediastinum, manifested radiographically by paratracheal air collection, is reported. Percutaneous injection of radiopaque material and indocyanine green into a cyst showed an opening to the trachea. During surgical resection, three well-circumscribed cysts were isolated. Pathologically the cysts were lined by a ciliated columnar epithelium and surrounded by lymphoid tissue with germinal centers. Judging from the level of the duct opening onto the airway, these LECs might have derived from a remnant of the fourth branchial pouch. To the authors' knowledge, this is the first case described.

[Experimental study on effects of a high-sugar diet and physical exercise on serum phospholipids in young adults].

The life style of young adults has been receiving attention with a view to its improvement to prevent coronary heart diseases (CHD) later in life. In this study, to determining the influence of different life styles on the serum phospholipids level, we carried out experimental studies on the effects a high sugar diet and physical exercise on serum phospholipid levels. Considering the trend for young people to consume large quantities of carbonated drinks, in which most of the sugar is sucrose, we tested the effect of a high carbohydrate diet on one group of six healthy male subjects and did not find that it caused a significant increase in the serum phospholipids level. In another group of 108 healthy young female subjects, a mild exercise regimen results in a slight, but significant decrease in the serum phospholipid level (p < 0.01). The serum total cholesterol level of the group did not exhibit a significant change.

Changes in lipid peroxide and antioxidant enzyme activities in corpora lutea during pseudopregnancy in rats.

This study investigated the involvement of lipid peroxidation and antioxidant enzymes in the regulation of luteal function in pseudopregnant rats. The activities of superoxide dismutase (SOD), a specific scavenger of superoxide radicals, and glutathione peroxidase, a scavenger of hydrogen peroxide, and lipid peroxide concentrations were measured in the corpus luteum on days 1, 3, 5, 7, 9, 11 and 13 of pseudopregnancy. The activity of SOD in the corpus luteum gradually increased until day 9 of pseudopregnancy and decreased thereafter, in a similar manner to serum progesterone concentration. Glutathione peroxidase activity significantly increased from day 1 to day 3 and remained high until day 11 of pseudopregnancy. The concentrations of lipid peroxides in the corpus luteum increased from day 3 to day 13 of pseudopregnancy. The involvement of prostaglandin F2 alpha (PGF2 alpha) in the production of lipid peroxides in regression of the corpus luteum was investigated by administering PGF2 alpha (3 mg kg-1, s.c.) or saline on days 7, 9 and 12 of pseudopregnancy. Each group of rats was autopsied 2 h later, and SOD activity, glutathione peroxidase activity and the concentration of lipid peroxides in the corpus luteum were determined. PGF2 alpha significantly increased lipid peroxide concentrations in the corpus luteum on days 7, 9 and 12 of pseudopregnancy (approximately twofold increases on days 7 and 9, and a fivefold increase on day 12, compared with the control that received saline). The activity of SOD in the corpus luteum was significantly increased by PGF2 alpha on days 7 and 9, but not on day 12, of pseudopregnancy. PGF2 alpha did not cause any significant changes in glutathione peroxidase activity in the corpus luteum on days 7, 9 and 12 of pseudopregnancy. It is concluded that lipid peroxides play an important role in regulating luteal function in pseudopregnant rats.

The mechanism of cold-induced platelet aggregation in the presence of heparin.

Low temperature induces platelet aggregation, but this phenomenon is slight and poorly reproduced. However, heparin potentiated the reaction in a dose dependent manner. The degree of aggregation increased as the temperature at which the platelet-rich plasma was chilled was lowered, and as the time of chilling lengthened. Acetylsalicylic acid, a cyclooxygenase inhibitor, and staurosporin, an inhibitor of protein kinase C, partially inhibited cold-induced platelet aggregation (CIPA), suggesting that at least part of the reaction mechanism involves production of thromboxane A2 and activation of protein kinase C. Prostaglandin E1 (PGE1), which inhibits platelet responses through elevating platelet cyclic AMP, completely blocked CIPA, suggesting that PGE1 dependent pathway in platelets plays an important role for CIPA. The inhibition of CIPA by these inhibitors suggests that CIPA is aggregation with platelet activation but not platelet agglutination. Extracellular Ca2+ is essential for CIPA because ethylene glycol-bis (beta-aminoethylether) N, N, N', N'-tetraacetic acid (EGTA), extracellular Ca2+ chelating agent, completely inhibited CIPA. Monoclonal antibodies against glycoprotein (GP) IIb/IIIa (10E5, P2) and Ang-Gly-Asp-Ser-peptide (RGDS-peptide) which inhibit fibrinogen binding to GPIIb/IIIa completely blocked CIPA but monoclonal antibodies against GPIb (6D1, SZ2) partially. In addition, CIPA occurred only when fibrinogen was added to washed platelets suspension. These results indicate that CIPA is dependent on binding of fibrinogen to GPIIb/IIIa and GPIb is partly related with the reaction.

Evaluation of the effects of various factors on the serum alpha hydroxybutyrate dehydrogenase activity in young females.

In this study, for determining the influence of different life style on the serum alpha hydroxybutyrate dehydrogenase activity (alpha HBD), we carried out surveys and laboratory studies on the relationships of the nutritional intake, physical activity, cigarette smoking and alcohol consumption of young females with their serum alpha HBD activity. There are significant positive correlation between alpha HBD and GOT, GPT, LDH, CPK, phospholipids, total cholesterol, and HDL-cholesterol, and significant negative correlation between alpha HBD/LDH and GOT, GPT, and gamma GTP. There are significant positive or negative correlation between alpha HBD and systolic pressure, diastolic pressure, cardiac index, stoke volume and total periperal resistance, and significant negative correlation between alpha HBD/LDH and vital capacity in young females. The serum alpha HBD activity levels of nonsmokers was significant higher than those of smokers (p < 0.05). There are significant positive correlations between serum alpha HBD activity and beverage consumption, and between alpha HBD/LDH and protein, calcium, vitamin A, B1, B2, C, pulses, vegetables and milk in young females. The finding shows that the group of serum alpha HBD activity level of 240 or more units exhibited higher level of rate of those who exercised sometimes+usually than the other group in young females. However, the serum alpha HBD activity level did not show significant correlation with energy expenditures per day. Exercise at 50 and 70% VO2 max may produce a significant increase in their serum alpha HBD activity. There is a possibility that serum alpha HBD activity may serve as an indicator for ones health condition. It is undeniable that observation of variation of serum alpha HBD activity is highly significant in health maintenance.

Anatomical studies on the extensor pollicis et indicis accessorius muscle and the extensor indicis radialis muscle in Japanese.

Anatomical studies on the extensor pollicis et indicis accessorius and extensor indicis radialis muscles were conducted on 952 upper limbs from 476 Japanese adult cadavers. Each anomalous muscle occurred between the extensor pollicis longus proprius and extensor indicis proprius muscles in the dorsum of the forearm and hand. The extensor pollicis et indicis accessorius muscle was present in 13 of the limbs (1.4%). It was not coexistent with another anomalous muscle of the extensor digitorum profundus mass in 9 cases, and it was coexistent with that muscle in 4. The extensor indicis radialis muscle was present in 34 limbs (3.6%). It was accompanied by another anomaly of the extensor digitorum profundus mass in 5 cases, and not accompanied by that muscle in 29. The extensor pollicis et indicis accessorius and extensor indicis radialis muscles were innervated by the posterior interosseus nerve of the radial nerve. It seems that the former anomalous muscle supplies the extensor control of the thumb and index finger, and the latter one of the index finger only. Each anomalous muscle was considered to be differentiating on the radial side of the extensor digitorum profundus mass in humans.

Distribution of NADPH diaphorase-reactive nerves in the human female genital organ.

OBJECTIVE: This study was designed to histochemically clarify the presence of nerves containing NADPH diaphorase, representing the catalytic activity of nitric oxide synthase, in the human female genital organ. STUDY DESIGN: Female genital organs were isolated for NADPH diaphorase staining by extensive or simple hysterectomy from ten patients. RESULTS: Paracervical ganglia contained many nerve cells intensely stained. The nerve from the ganglia contained many axons intensely stained, which selectively innervated blood vessels distributing to the whole genital organ. NADPH diaphorase-reactive nerve fibers around the vascular wall distributed in the adventitia, closely contacting to the medial layer. The distribution of the axons was much denser in the arterial wall than in the venous wall. CONCLUSION: The NADPH diaphorase-reactive nerve fibers, possibly originated from paracervical ganglia, appear to play an important role in the regulation of uterine vascular tone by liberating nitric oxide, and such a neurogenic control may be predominant in the artery over the vein.

Effects of long-term treatment with recombinant human granulocyte-macrophage colony-stimulating factor in patients with myelodysplastic syndrome.

We examined the effects of long-term treatment with recombinant human granulocyte-macrophage colony-stimulating factor (rhGM-CSF) in 61 patients with myelodysplastic syndrome (MDS). Patients were randomly assigned to receive daily subcutaneous injection of 60 micrograms/m2, 125 micrograms/m2 or 250 micrograms/m2 for 8 weeks. A significant increase in granulocyte counts including neutrophils and eosinophils was shown from one week after the start of the treatment in all three dose groups. The increase in granulocyte counts reached a plateau at the 4th week and was sustained during the treatment period. However, no consistent change in other cell lineages including monocytes, lymphocytes, reticulocytes and platelets were observed. Nevertheless peak-levels of these cells were significantly higher than the pre-treatment levels. In higher dose groups, the number of patients developing infections was reduced. There was no significant difference in the incidence of adverse events among the 3 dose groups, and the toxicity was generally well-tolerated. These observations indicate that treatment with rhGM-CSF can be of potential therapeutic benefit to patients with MDS.

[Rest and exercise performance of allografts, Bicer valves and St. Jude Medical valves in the aortic position].

Cardiac catheterization was performed to measure pressure gradients and hemodynamic parameters at rest and during supine bicycle exercise in 37 patients with aortic valve replacements, using allograft valves. Bicer prosthetic valves and SJM prosthetic valves of 19 to 27 mm diameter. Fourteen patients had an allograft valve replacement, 17 a Bicer valve, and 6 a St. Jude Medical valve. Patients were comparable for age, valve size, body surface area and left ventricular systolic function. There were no statistical differences among three groups for body surface area, left ventricular end-systolic and end-diastolic volume indices, exercise cardiac index, exercise heart rate, or workload achieved. In all 37 patients left ventricular and ascending aortic pressures were measured simultaneously according to the Brockenbrough method. In a 21 mm valve size, the mean pressure gradient was higher for Bicer and St. Jude Medical prosthetic valves than for allograft valves, both at rest (8.0 +/- 3.5 mmHg for Bicer, 5.0 +/- 1.4 mmHg for St. Jude Medical versus 0 +/- 0 mmHg for allograft; p < 0.05) and during exercise (11.0 +/- 4.9 mmHg, 10.0 +/- 2.8 mmHg versus 0 +/- 0 mmHg, respectively: p < 0.05). In a 23 mm size, there were no significant differences among three groups for resting mean pressure gradient, but during exercise the mean pressure gradient was higher for Bicer and St. Jude Medical prosthetic valves than for allograft valves (8.4 +/- 9.3 mmHg, 5.5 +/- 4.4 mmHg versus 0 +/- 0 mmHg; p < 0.05).(ABSTRACT TRUNCATED AT 250 WORDS)

[A case of liver metastasis of gastric cancer with portal vein tumor thrombosis responding to chemotherapy with 5-FU and epirubicin].

A 67-year-old male patient having liver metastasis from gastric cancer with portal vein tumor thrombosis was treated by chemotherapy with 5-FU and EPI. A dose of 500 mg/body/day of 5-FU was continuously administered via the central venous catheter. Anorexia and hepatic dysfunction were reduced. In addition, 20 mg/body/week of low-dose EPI was added to the chemotherapy. This treatment produced marked regression of portal vein tumor thrombosis on CT. The side effect observed was slight nausea, but no bone marrow suppression was found. Thus, chemotherapy with 5-FU and EPI appears to be a useful and safe treatment for liver metastasis with portal vein tumor thrombosis.

[Two surgical cases of tectal glioma].

Tectal glioma is rare and difficult to diagnose, and the tumor has been known as the tumor that gives no indication of the need for direct surgery because of its anatomical location. At present, MR imaging is available to detect the tumor and its location. We present two patients who underwent direct surgery. Case 1: A 18-year-old female was admitted with headache and nausea. MRI showed signs of stenosis of the aqueduct and a tumor of the tectal region. The tumor was removed partially. Histological examination of this tumor demonstrated a low grade astrocytoma. After surgery, hydrocephalus improved. Case 2: A 12-year-old boy was admitted with hydrocephalus detected by CT. MRI demonstrated a tumor of the midbrain without confirmation. He underwent a V-P shunt for hydrocephalus, and was discharged without any complaints. After 2 years, he was admitted again with shunt malfunction. After shunt revision, his consciousness recovered. However, Parinaud's sign appeared and patency of the shunt was unstable. MRI revealed a tumor of the tectal region and signs of stenosis of the aqueduct of Sylvius. The tumor was removed directly by the occipital transtentorial approach. The aqueduct was opened and a catheter was inserted from the 3rd to the 4th ventricle. Histological examination revealed a low grade astrocytoma. The patient followed a satisfactory postoperative course except for slight Parinaud's sign, and the V-P shunt was no longer necessary.

[Endocrine mechanism of placental circulatory disturbances induced by microwave in pregnant rats].

Effects of microwaves on fetus and female genital organs remain to be elucidated. To demonstrate the placental circulatory disturbances induced by microwaves and to clarify the endocrine pathogenesis, placental blood flow and five endocrine indicators, i.e., corticosterone (CS), estradiol (E2), progesterone (P), prostaglandin E2 (PGE2) and prostaglandin F2 alpha (PGF2 alpha) were measured in rats exposed to whole-body microwaves with an intensity of 10 mW/cm2 at a frequency of 2,450 MHz. The placental blood flow at 45-90 min after exposure was significantly decreased in the rats exposed to the microwaves. Placental blood flow at 15 and 30 min was increased by pretreatment with intraperitoneal administration of angiotensin II (AII). In contrast, no significant change in placental blood flow was recognized in the AII pretreated rats exposed to the microwaves. An increase in CS and a decrease in E2 were induced by the microwave exposure independent of pretreatment with AII. P was increased by microwave exposure in the rats without pretreatment with AII. PGE2 was not changed by the microwave exposure in the case of either nonpretreatment or pretreatment with AII. PGF2 alpha was increased by the microwave exposure in the rats without pretreatment with AII. The present results indicate that excessive exposure to whole-body microwave disorders pregnancy in terms of placental circulatory dysfunction. The data suggest the involvement of endocrine mechanisms in the decrease in placental blood flow which is induced via a detrimental effect of microwaves on PGF2 alpha and on pituitary functions such as general emotional stress.

[Health locus of control and acquisition of health-related information].

Relationships between acquisition of health-related information and Health Locus of Control (HLC) were investigated in 204 women aged 29-45 years, using Horige's Japanese version of Health Locus of Control (JHLC) Scales. The acquisition behavior of health-related information was obtained from the question "How often do you get health-related information by newspaper, television or magazine?" The results were as follows: 1) Women who perceived the information related directly to their personal health, acquired health-related information more often than those who did not perceive personal application of the information. 2) The technical school or junior college graduate group had the most health-related information acquisition scores. 3) The university graduate group had fewer Chance HLC beliefs and Supernatural HLC beliefs. 4) Family HLC mean scores were significantly higher in the "high family value" group than in the "high health value" group. 5) Internal HLC mean scores were significantly higher in those self-rated as "very healthy" than in those self-rated as "healthy" group. 6) Women acquiring health-related information the most had Internal HLC mean scores significantly higher than the others. Health-related information acquisition scores correlated significantly with Internal HLC mean scores and Family HLC mean scores.

[A case of eosinophilic pneumonia presenting as hemoptysis, with epithelioid cell granuloma on lung biopsy].

A 60-year-old woman was admitted because of hemoptysis and dyspnea on exertion, which began one month before admission and gradually increased. Chest X-ray film taken on admission showed an infiltrative shadow in and around the left posterior basal segment. Chest CT disclosed peripheral and patchy consolidation with some air bronchograms and interstitial thickening in the left basal segments. Bronchoalveolar lavage fluid contained many red blood cells and abnormally high numbers of eosinophils and lymphocytes. The specimen obtained by CT-guided lung biopsy revealed epithelioid cell granulomas, hemosiderin-laden macrophages, diffuse infiltration of small round cells with a few eosinophils and monocytes in the alveolar septa, and many eosinophils in the intraalveolar exudates. No pathogens of pulmonary eosinophilia were identified and there was no secondary pulmonary eosinophilia. Therefore, we diagnosed eosinophilic pneumonia with no obvious cause. Hemoptysis may have been due to alveolar hemorrhage of eosinophilic pneumonia. The hemoptysis and chest infiltrative shadow disappeared rapidly after the start of treatment with oral prednisolone.
