[Olfactory neuroblastoma: a histopathological ultrastructural and immunohistochemical study of 11 cases].

Eleven cases of olfactory neuroblastoma (ONB) were studied histopathologically, ultrastructurally and immunohistochemically. Light microscopical examination showed the tumor cell nests separated by fibrous connective tissue (present in 10/11 ONB), Homer-Wright rosette and/or Flexner rosette (9/11 ONB) and acidophilic fibrillary background (8/11 ONB). Ultrastructurally, there were neurosecretory granules (2/2 ONB) and neurofibrils (1/2 ONB) in cytoplasm of tumor cells. A panel of antibodies were used to characterize the immunohistochemical staining profile of ONB. The following results were obtained for the 11 neoplasms that were immunostained: NSE 11/11 (+), S-100 3/11 (+), Vimentin 4/11 (+), Keratin 1/11 (+) and NF, EMA, CEA, LCA all negative respectively. The significance of the morphological and immunohistochemical features of ONB in diagnosis and differential diagnosis were discussed.

[Pathological changes in the livers of rabbits infected with schistosome cercariae and treated with artemether or praziquantel in the early stage of infection].

When rabbits infected with Schistosoma japonicum cercariae for 7 d were treated ig with artemether (Art) 30 mg.kg-1, and repeated at 1-wk intervals (i.e. on d 14, 21, 28 after infection) for 4 times, dead worms were found in the liver sections, but no egg or egg granuloma was detected. In infected rabbits treated repeatedly with the above-mentioned dosage of Art at 2-wk intervals (i.e. on d 21, 35, 49) after the first dosing on d 7 after infection, few eggs and egg granulomas were seen occasionally. In another experiment, praziquantel (Pra) 40 mg.kg-1 was given ig to rabbits on d 21 after infection and the same dosage was administered repeatedly at 1- or 2-wk intervals for 2 times, the numbers of eggs and egg granulomas detected in the liver sections of these 2 groups were much less as compared with the control. In the latter group treated at 2-wk intervals, only degenerated eggs and egg granulomas were detected occasionally in the liver sections. The results indicate that if Art or Pra was given to rabbits in the early stage after infection, no apparent or only slight damage of the liver induced by the eggs was seen due to the fact that most or even all of the female worms were killed.

Beneficial effects of NTG on left ventricular volumes and systolic function in patients with myocardial infarction.

The effect of sublingual administration and intravenous drip of nitroglycerin (SLNTG and IVNTG) on left ventricular volumes (LVVs) were studied and systolic function was quantitatively assessed with two-dimensional echocardiography (2DE) in 28 patients with myocardial infarction (MI), including 13 cases without heart failure (MI-NHF) in one group and 15 with heart failure (MI-HF) in another. The results showed that after SLNTG and IVNTG in both groups the heart rate (HR) increased significantly (P < 0.05-0.001); the systolic blood pressure (SBP) decreased significantly (P < 0.05-0.001); the diastolic blood pressure (DBP) was not significantly changed (all P > 0.05); the left ventricular end-diastolic and end-systolic volumes (EDV and ESV) both significantly reduced [EDV: decreased by 5.8-11.6% (-13.2-19.0 ml), P < 0.05-0.001; ESV: decreased by 9.6-16.6% (-8.6-17.3 ml), P < 0.05-0.001)]; the left ventricular ejection fraction (LVEF) all significantly elevated (increased by 6.6%-9.4% (3.2%-3.4%), P < 0.05-0.001] except in MI-NHF group after IVNTG; the segmental EF of normal and hypokinetic segments also significantly increased (P < 0.05-0.001). The results suggest that NTG could beneficially reduce left ventricular EDV and ESV; and improve or strengthen its global and segmental systolic function in MI patients, particularly noticeable in patients with heart failure.

Diarrhoea in piglets and monkeys experimentally infected with Balantidium coli isolated from human faeces.

Ten piglets and four monkeys free from Balantidium were dosed with human faecal homogenate which contained 1.2 x 10(4)-4.8 x 10(4) B. coli cysts. The infection resulted in severe diarrhoea in piglets 1-6 and hydrocortisone-treated monkeys 1-2, moderate diarrhoea in piglets 7-10 and a subclinical infection in monkeys 3-4. In piglets 1-3 and monkeys 1-2, heavy infection of the intestinal mucosa extended from the terminal ileum to the rectum and the mucosa was severely damaged. In piglets 4-10, infection was heavy in the large intestine with moderate mucosal damage.

Cellular distribution of proteasome subunit Lmp7 mRNA and protein in human placentas.

Human leucocyte antigen (HLA) class I antigen expression is closely controlled in placental trophoblast cells, which interface directly with genetically disparate maternal blood and tissues during pregnancy. In this study, the possibility that LMP7, a proteasome component that may be required for processing of class I-associated peptides, might be lacking or refractory to cytokine induction in trophoblast cells that fail to display HLA class I antigens was investigated. Analysis of Lmp7 mRNA and protein in paraformaldehyde-fixed placentas by in situ hybridization and immunohistochemistry revealed that both HLA class I-positive and HLA class I-negative trophoblast cells contain Lmp7 gene products. Consistent with these results, northern blot hybridization studies showed that HLA class I-positive (JEG-3) and HLA null (Jar) trophoblast-derived cell lines contain Lmp7 mRNA. After 48 hr of exposure to HLA class I-modulating cytokines, Lmp7 mRNA levels in JEG-3 cells were markedly increased by two interferons (IFN-beta, IFN-gamma) and tumour necrosis factor (TNF) whereas at the same time point, Jar cell Lmp7 mRNA was modestly enhanced by IFN-gamma. Collectively, the findings indicate that expression of HLA class I antigens in trophoblast cells is unlikely to be restricted by lack of Lmp7 gene products and suggest that endogenous placental cytokines may have different influences on Lmp7 mRNA levels in phenotypically distinct trophoblast subpopulations.

Sampling strategies in a statistical approach to clinical classification.

This paper studies the sampling strategies for the Expert Network (EexNet), a statistical learning system used for patient record classification at the Mayo Clinic. The goal is to achieve high accuracy classification at an affordable computational cost in very large applications. The learning curves of ExpNet were observed with respect to the choice of training resources, the size, vocabulary coverage and category coverage of a training set, and the category distribution over training instances. A method combining advantages of different sampling strategies is proposed and evaluated using a large training corpus. As a result, Expert Network has achieved its nearly-optimal classification accuracy (measured by average precision) using a relatively small training set, with a fast real-time response which satisfies the needs of human-machine interaction.

The inhibitory effect of astragalus membranaceus on coxsackie B-3 virus RNA replication.

Using mice infected with coxsackie B-3 virus (CVB3) as a viral myocarditis model, we observed the inhibitory effect of Astragalus membranaceus (AM) on CVB3-RNA replication in myocardial tissue of mice by RNA-RNA in situ hybridization with negative-strand RNA probes labelled with 35S and quantitative imaging analysis of positive signals. The mechanism of its effect on CVB3-RNA replication has been investigated by detection of beta-interferon (beta-IFN) as well. Results showed that the copy numbers of CVB3-RNA as well as the histologic scores (necrosis) in myocardial tissues of infected-AM treated mice were significantly lower than those in infected and normal saline treated mice, suggesting that AM could inhibit the replication of CVB3-RNA, but its effect on CVB3-RNA replication had no correlation with induction of beta-IFN.

[Homology of the recombinant DNA of plasmid pDJH2 with the recombinant DNA probe of L. alstoni and analysis of its expression in Escherichia coli].

Dig-labeled recombinant DNA probe of L. alstoni which contains the entire structural OmpL1 gene was hybridized with the recombinant DNA of the plasmid named pDJH2 of the gene library of L. interrogans serovar lai strain 017. The result showed a high degree of homology among them; expression of recombinant DNA of pDJH2 was achieved by beta-D-galactosidase (IPTG) induction in E. coli. The molecular weight of this product is 68kd. Then they were treated with proteinase K and subjected to SDS-PAGE. The results showed it is a protein in nature. Using the specific monoclonal antibody E4B7D5 on immunoblotting and specific polyclonal antibody on dot-ELISA assay, we investigated the immune reaction and noticed that protein 68kd might be an antigen in character. E. coli which contains the recombinant plasmid pDJH2 were injected into BALB/c mice. Then the mice were challenged by leptospires of the strong virulence strain 017, but all the infected mice survived. In this paper, we first report the expression of recombinant DNA of L. interrogans serovar lai strain 017 in E. coli when injected with IPTG, and immunoprotection of BALB/c mice which were injected with the expression against the infection of L. interrogans serovar lai strain 017. pDJH2 may be the first recombinant for which the gene has been cloned and its expression product 68kd may be the immunoprotective antigens.

[Detection of human cytomegalovirus DNA in vascular plaques of atherosclerosis by in situ hybridization].

To examine the possible role of HCMV in the etiology of atherosclerosis, we searched for the presence of HCMV DNA in arterial vascular tissues by in situ hybridization. Biotin labelled HCMV DNA probe was used on specimens of arterial wall taken from patients with clinically significant atherosclerosis. Evidence for the presence of HCMV DNA was obtained in 13 specimens out of 32 atherosclerosis specimens. In contrast, only 1 specimen was positive in the control group (Total 15 specimens of normal arterial and venous wall), suggesting that there is a close relationship between human cytomegalovirus infection and atherosclerosis development. The results also showed that the positive hybridization signals were mainly observed in the nuclei of smooth muscular cells, rarely found in outer layer of arterial walls, cell membrane and cytoplasm. It was indicating that HCMV infection might be an etiological agent in the development of atherosclerosis.

[Effect of hyaluronic acid-stimulating factor on scar formation in wound healing process of deep partial thickness burns].

Hyaluronic acid-stimulating factor (HASF) was purified from the amniotic fluid and fetal serum of the gestational rabbit by means of affinity chromatography. After topical application of HASF on the deep partial thickness burn wounds, the level of hyaluronic acid (HA) in wound exudate was determined daily during the early period of healing. One month after wound healing, the local skin was harvested for collagen analysis. Histologic study was made to check the outline of collagen fibrils. The HASF was also used in rabbit fibroblast culture. The HA level contained in the supernatant was assayed. Result showed that HASF could elevate and maintain the HA level in the extracellular matrix of burn wound. In vitro, HASF was able to stimulate the fibroblast to produce hualuronic acid. The presence of HA in extracellular matrix then modulated collagen synthesis in the process of wound repair resulting in a higher content of type III collagen, thus bringing about a scarless healing of the deep patrial thickness burns.

[Clinical study on treatment of peptic ulcer with bushen kangkui decoction].

The clinical controlled study of Bushen Kangkui Decoction (BSKKD) on 150 peptic ulcer patients showed that it had a similar effect to injectio cimetidine, and had a lower rate of relapse. And the long-term therapeutic effect was better than that of injectio cimetidine. Simultaneous RIA showed that PGE2 of most treated patients was below normal level before the treatment, but it has increased in various degree after the treatment. The patients under the treatment of BSKKD had significant difference in PGE2 level (P < 0.05), while those treated with injectio cimetidine didn't have (P > 0.05). So the BSKKD has an effect on the increase of PGE2 level of the peptic ulcer patients.

[Studies on regulatory effects of acupuncture on mucosal secretory IgA in patients with allergic asthma].

We studied mucosal SIgA immunity of patients with allergic asthma treated by acupuncture. Our results showed that concentrations of SIgA and total IgA in saliva (P < 0.01, < 0.02) and in nasal secretions (P < 0.02, < 0.02), and levels of IgE in sera (P < 0.001) from patients with allergic asthma were significantly decreased after treatment with acupuncture. The results suggested that attacks of delayed and immediate allergic asthma could be effectively inhibited by acupuncture. The facts that there were no significant changes in IgA in supernatants of peripheral blood lymphocytes stimulated with PWM of patients with allergic asthma after treated by acupuncture were associated with regulation of negative feedback of T-lymphocyte expressing IgA-R and it released bound factor of IgA in production of polymeric IgA.

[A preliminary observation on the effects of inhaled nitric oxide on bronchial asthma].

Nitric oxide (NO) is a vasodilator as well as a bronchodilator. In order to estimate the therapeutic effects of NO on bronchial asthma, 10 patients in acute exacerbation stage were selected for this study. 40 ppm NO was inhaled for 20 minutes. The clinical stage and pulmonary function were evaluated instantly. The results showed that subjective symptoms were relieved and wheezing rales decreased to a different extent, FEV1 and PEF improved significantly (P < 0.05). It is suggested that low concentration NO inhalation can be used as a therapeutic measure for bronchial asthma.

[Experimental studies on the preventive effect of artemether against schistosomal infection].

In order to study the preventive effect of artemether (Art) against schistosomal infection, mice, rabbits and dogs infected with Schistosoma japonicum cercariae were treated intragastrically (ig) with Art on various days after infection. The preventive effect was assessed by total and female worm reduction rate, the number of the animals without female worm, measurement of some parameters relevant to the acute schistosomiasis, and histopathological observation on the liver. The results showed that in mice a promising effect was obtained when an initial dose of Art 300 mg/kg was given ig on d7 after infection with cercariae, followed by repeated dosing every wk for 4 times. In rabbits and dogs treated ig with Art according to above-mentioned appropriate regimen on d7 after infection at a dosage of 10 or 15 mg/kg given every 1 or 2 wk for 2-4 times, the total and female worm reduction rates were 96.8%-100%, and part of the animals were free from female worm. After preventive treatment of the rabbits with Art, the body temperature and eosinophil count were normal and no schistosome egg was found in the feces. When rabbits infected with 48-52 schistosome cercariae once every other day for 5 times were initially treated ig with Art 15 mg/kg on d7 after the first infection, and the same dosing was given repeatedly every 1 or 2 wk for 2-4 times, the total and female worm reduction rates were 97.6%-98.4%. Histopathological observation showed that the above-mentioned infected dogs and rabbits received preventive treatment with Art exhibited a promising protective effect on their livers. The protective effect of Art given every 1 wk on the liver was similar to that of the drug given every 2 wk. The results indicate that Art possesses an effect for prevention of schistosomiasis which could be helpful for reducing infection rate and intensity of infection, and controlling acute schistosomiasis.

[Analysis of human DNA fingerprints by using fluorescein labelled probe JH12.6].

A method of analysis of human DNA fingerprints has been established by using fluorescein (Fl-11-dUTP) labelled probe JH12.6. The clear and high distinctive patterns of human DNA fingerprints were obtained with this method. The DNA fingerprints of 78 unrelated individuals living in Yunnan province of China were detected, and their probability of chance association between random individuals was calculated as 7.4 x 10(-11), and their mean allelic frequency as 0.09. Comparing of DNA fingerprints with labelling probe JH12.6 by using between radioactive 32P and fluorescein showed that the method by using fluorescein labelled probe JH12.6 was simple, fast, safe and economical. It can totally replace the method by using radioactive 32P labelled probe JH12.6, and it can be broadly applied in forensic medicine and other fields.

An overview of statistical methods for the classification and retrieval of patient events.

Statistical methods that can support text retrieval are becoming an increasing focus of medical informatics activities. We overview our adaptation of existing knowledge sources to create pseudo-documents for concept based latent semantic indexing. Experience demonstrated this tack of limited practical value, since retrieval performance was invariably unsatisfactory. We discovered this was due in part to the introduction of a vocabulary gap between the queries and the cases we sought to retrieve. In part to address this problem, and to avail our large body of humanly coded text as a knowledge source, we developed a least squares fit alternative for the computer assisted indexing and retrieval of biomedical texts. This technique demonstrates equivalent or superior retrieval performance when compared to all other textual retrieval techniques. It does not depend upon elaborate knowledge bases, lexicons, or thesauri. It is a promising technique for classifying and retrieving the large volumes of clinical text.
