To test the hypothesis that a gene (or genes) in the "MODY1 region" of the long arm of chromosome 20 contributes to the development of NIDDM, we conducted linkage studies in 29 extended Caucasian families in which many members were affected with NIDDM. A total of 498 individuals, including 159 NIDDM patients with an average age at diagnosis of 47 years, were genotyped for eight highly polymorphic microsatellite markers spanning a 31-cM region on chromosome 20q12-13.1. Using affected sib-pair analysis, we obtained evidence suggesting linkage between NIDDM and markers D20S119, D20S178, and D20S197 (allele sharing identical-by-descent [IBD], 0.56 for all three; P = 0.005, P = 0.009, and P = 0.004, respectively). Multipoint nonparametric linkage (NPL) analysis also showed evidence for linkage of NIDDM with the same three markers. The evidence for linkage was much stronger (allele sharing IBD by affected sibpairs, 0.64 [P < 0.0001]; maximum NPL score, 3.3 [P = 0.009]) in the 14 families whose average age at diagnosis of NIDDM was above the median (47 years) for all families. In these 14 families, one particular allele of the microsatellite D20S197 was transmitted from heterozygous parents to NIDDM offspring more frequently than expected (P < 0.01). This indicates that the marker allele and the disease allele are in linkage disequilibrium, implying that they are in close proximity. Consequently, the recently identified MODY1 gene (hepatocyte nuclear factor 4) is an unlikely candidate gene for NIDDM in our families, since it is located about 8 cM centromeric of D20S197. In conclusion, we have identified a new region on chromosome 20q that contains one or more NIDDM genes distinct from the recently identified MODY1 gene.
OBJECTIVE: To analyse the causes of multi-drug resistant tuberculosis (MDR-TB) and to evaluate the effects of chemotherapy with ofloxacin and other antituberculosis drugs. METHOD: 27 cases with MDR-TB were treated with the regimen of 3KPTHOX/PTHOX. Changes of sputum convesion, X-ray manifestations and side effects after the treatment were also evaluated. RESULTS: 67% of the MDR-TB patients were due to irregular treatment, and 18% were caused by improper treatment. The sputum culture conversion rate at end of treatment was 89%. The bacteriological relapse rate of converted cases during 2 year follow-up was 8%. CONCLUSIONS: MDR-TB might be prevented by a combination of health education and rational use of short-course chemotherapy. Ofloxacin and other second-line anti-tuberculosis drugs are effective and were tolerated in treating patients with MDR-TB.
The changes of brain water content and Evan blue(EB) were investigated on the model of infection brain edema induced by Pertussis Bacilli(PB) in 72 rats. Nimodipine was admenistred to show its effect on the PB induced brain edema. The experiment results showed that the water content and EB content in the PB group were significantly increased than those normal group and normal saline group at 30 min, 4 hours and 24 hours after the injection of PB (P < 0.05). Nimodípine could significantly reduce brain water content and EB content, so that brain edema was attenuated. It indicates that nimodipine showed advantage to treatment of PB brain edema.

To investigate whether the mast cells (MC) of the ileocecal junction (ICJ) is elevated in the irritable bowel syndrome (IBS) and the possible roles of the MC in IBS, the biopsies of ICJ were stained specifically by histochemistry for the MC in the IBS group (n = 20) and the normal group (n = 19). The structure relation between MC and nerves was studied through an electronic microscopy and an immunohistochemical method demonstrating neuronspecific enolase. The results demonstrated that the number of the MC of ICJ was significantly elevated in the IBS (P = 0.019) and that mast cells were close to nerves which were often unmyelinated nerves in lamina propria. The results indicate that the MC of ICJ may be responsible for the pathophysiology of the IBS. We conclude that the MC of ICJ may be a mediator between the gut and the nervous system in the IBS, and that the mast cell stabilizer or the antagonists of the mast cell products may have potential treatment effects on the IBS.
To understand the roles of autoantibodies to beta 2 adrenergic receptors in the pathogenesis of asthma, we investigated the positive chronotropic action of beta 2-selective adrenergic agonist, clenbuterol, on cultured neonatal rat cardiomyocytes. Thereafter, we detected the autoantibodies to beta 2-adrenergic receptors in the sera from patients with asthma as it could inhibit the positive chronotropic action of clenbuterol. In the sera of all patients with asthma (16 cases) there were autoantibodies to beta 2-adrenergic receptors, but not in the normal controls (20 cases). Further study showed that the inhibitory autoantibodies were IgG type. This experiment suggests that the autoantibodies to beta 2-adrenergic receptors may play an important role in the pathogenesis of asthma.
To investigate the role of enteroviral infection in the patho genesis of dilated cardiomyopathy (DCM), we used the nested reverse transcription polymerase chain reaction to detect enteroviral RNA in 21 myocardial specimens and 17 peripheral blood samples from patients with DCM and 21 patients (controls) with cerebral trauma. Enteroviral RNA was detected in 9 of 21 myocardial tissues and 6 of 17 blood samples, whereas all controls myocardial samples were negative. In addition, antibodies of coxsackievirus group B were detected in 4 of the 17 patients with DCM by neutralization test. The present study suggested that DCM might be associated with persistent enteroviral infection.
To treat the physiological dysfunction and psychic disturbances of the patients with penile torsion, a variety of surgery were used to eliminate its abnormality. A modificed of Nesbit procedure was used to correct the major malformation, several parallel ellipsoid patches which axis forms an angle of 45-degree with that of the penis were removed from the tunica albuginea on the convex side of the corpora cavernosa. Besides, stitches were necessary to suture the tunica albuginea to the opposite pubis. It was suitable plicate the tunica albuginea on the convex side of the corpora cavernosa for mild case. The penises of the patients had a normal appearance at both flexible and errect conditions during a 3-month follow-up. Penile torsion could be corrected satisfactorily by shortening the convex side of the corpora cavernosa with excision of ellipsoids or plication of the tunica albuginea, which angulates 45-degree with the penile axis.
On the rat acute lung injury model produced by intraperitoneal cavity injection of zymosan showed that the respiratory rate became slow at first time (1st day), subsequently became rapid. Pulmonary histology showed that there was an accumulation of neutrophils in pulmonary microvessels, neutrophils infiltration in interstitial tissue, interetital and olveolar edema, hyaline membrane, atelectasis. Positive expression of ET-1 in experimental rats was stronger than than contral rats. Image analysis showed that OD value was elevated, especially at the fist day. The quantity of ET-1 was corresponding to the degree of pathologic change of acute lung injury. It suggests that the ET-1 might play an important role in the mechanism of acute lung injury.
To assess the application of a modified thoracotomy approach in general thoracic operation. 311 general thoracic operations were performed using a modified small posterolateral thoracotomy surgical procedures included lobectomy plus lymph nodes dissection in 186, pneumonectomy in 23, esophagectomy for esophageal cancer in 23. The modified method of thoracotomy had following advantages: (1) reduce postoperative morbidity; (2) reduce bleeding and bank blood transfusion; (3) shorter operating time. This study suggested that the modified small posterolateral thoracotomy have some advantages than standard approach. It should be used in some selected thoracic operations.
OBJECTIVE: To evaluate the therapeutic effects of phacoemulsification and the foldable intraocular lens implantation. METHODS: The surgery was performed through a 3.2 mm incision on 72 eyes of 65 patients with senile, complicated, congenital and traumatic cataracts. RESULTS: Post-operatively, the visual acuities with spherical correction or without correction were 0.5 or better in 94.4% at one day, and 1.0 or better in 70.8%, 75.8%, and 76.5% at one week, one month and three months respectively. The mean post-operative astigmatism was 0.89 +/- 0.83 D at one week and 0.73 +/- 0.76 D at one month which was respectively less than that in the control group with a 6.5 mm incision (P < 0.001), but no statistical difference between them at three months (P > 0.05). CONCLUSION: The foldable intraocular lens implantation through a small sutureless incision could reduce early post-operative astigmatism and shorten the period of visual rehabilitation.
OBJECTIVES: To investigate the morphological changes of retinal microcirculation in streptozotocin (STZ)-diabetic rats and the contribution of leukocyte to diabetic retinopathy (DR). METHODS: 12 male Wistar rats were randomly divided into normal control group, 3 month group and 6 month group of STZ-diabetic rats. Each group was perfused with 4% paraformaldehyde and fixed. Rat retina digest preparations were observed by light microscope, and the number of pericyte was counted. RESULTS: The decrease of pericytes in STZ-diabetic rats was remarkable compared with that of normal control at 6 months. At 6 months, STZ-diabetic rat retinal digest preparations showed that some of the capillaries were blocked by leukocytes. CONCLUSIONS: After injection of STZ in rats for 6 months, early morphological changes of DR were observed, and leukocytes played a potential role in the pathogenesis of DR.

A human YAC genomic pool with more than 4,000 clones was constructed by means of "terminal modification" of YAC vector and genomic inserts, as well as optimization of relevant experimental parameters. The transformation rate is approximately 400-500 transformants per microgram DNA. Preliminary characterization of randomly sampled clones demonstrates that YAC-specific inserts, in the average size of 450 kb and ranging 150-750 kb, were detected from 80% of the clones. The vector ligation experiment indicates the efficient reduction of "coligation" by terminal modification.
To study the clinical value of minor incision for cholecystectomy, compared conventional open cholecystectomy (300 patients, group A), alparoscopic cholecytectomy (300 patients, group B), and minor-incision cholecystectomy (300 patients, group C) with regard to duration (day) of operation, amount of intraoperative bleeding, complications, time (day) of gastrointestinal function recovery, intravenous infusion, and expense and time (day) of hospitalization. The results showed that the operating time, amount of bleeding, duration of intravenous infusion, time of gastrointestinal function recovery and hospitalization in group A were significantly different from those in group B and C (P < 0.05), but the incidence in group A was lower than that in group B. The operating time, amount of intraoperative bleeding, and the time of gastrointestinal function recovery and intravenous infusion in group B were similar to those in group C, but there was a high rate of intraoperative and postoperative complication in group B (1.66%). There was no complication in group C (P < 0.05). Comparison of the effects among group A, B and C showed that it was superior in group C to those in group A and B.
One strain of the viruses was isolated from preipheral blood lymphocytes (PBL) of a renal transplant recipient. PBL isolated from blood samples were cocultured with the PHA actived cord blood lymphocytes (CBL). Two of twelve recipient's samples found cytopathic effect after 10 to 14 days. Examination of ultrathin-sections of the virus infected cells by electron microscope showed herpes-like virus particles. Detection of indirect immunofluorescences with McAbs against HHV-6 was positive in the infected cells.
