Abnormality of osmotic fragility and morphological disorder of bovine erythrocytes infected with Theileria sergenti.

The osmotic fragility and the surface structure of erythrocytes obtained from 3 calves infected with Theileria sergenti and from 3 phlebotomized ones were compared. As the parasitemia progressed, the osmotic fragility of the erythrocytes significantly increased in the infected calves. Particularly the hemolysis ratio in the isotonic area (21.5-94.1%) obviously increased. On the other hand, the percentage of parasitized cells in the erythrocytes did not show so much high values (16.1-21.3%). Similar phenomenon was found in each different percentage of erythrocytes suspension which was separated from density gradient centrifugation. No significant difference in the serum osmotic pressure between the infected calves and the phlebotomized calves was found. By scanning microscopy, the erythrocytes of infected calves, which were collected at the crisis period of parasitemia, were almost completely deformed and showed echinocyte form. Moreover, the appearance ratio of echinocyte form in the erythrocytes population was superior to the percentage of parasitized erythrocytes. Similar membranous alterations were also observed in the erythrocytes of grazing cattle in the crisis period of the theileriosis. It was proven that abnormality of osmotic fragility and morphological disorders of erythrocytes occurred not only in parasitized erythrocytes but also in non-parasitized ones in T. sergenti parasitemia.

[Effects of choosing a task on interpersonal feeling and appreciation].

From the viewpoints of relative deprivation and social comparison jealousy, this article reports on a study that investigated the effects of choice on subsequent appropriate feeling and appreciation for another person who is superior to subjects in the same or different field. A 2 x 2 between-groups design was used to determine the effects of choice (present or absent) and field (same or different). Individual differences were assessed by using three scales: Self-Esteem, Self-Monitoring, and Social Desirability. Forty-one male subjects participated in a task and received negative feedback, and then evaluated a superior on both pencil and paper and bogus-pipeline (BPL) measures. Most importantly a significant Choice x SE interaction was obtained on BPL: It means that through experiencing choice the high Self-Esteem subjects can maintain appropriate feeling and appreciation even on BPL although some self-presentation is inevitable.

[Study on the genesis of posturally induced crackles from hemodynamic data--in patients with ischemic heart disease having normal respiratory function].

The presence of fine crackles is suggestive of heart failure in patients without pulmonary disease. We have been interested in the clinical observation that fine crackles are frequently detected when posture was changed from sitting to supine positions or in patients going from sitting position to supine position with passive legs elevation in patients without obvious evidence of heart failure. We named these crackles, "the posturally induced crackles (PIC)". We have already reported that PIC was frequently detected in patients with ischemic heart disease. The present study was performed to estimate the mechanism of the genesis of PIC and to clarify its significance. Seventy-three patients with ischemia heart disease were included in this study. Pulmonary sounds were auscultated in sitting and supine positions and during passive elevation of both legs in a supine position. Patients were divided into 3 groups according to the presence or absence of fine crackles, i.e., those in whom fine crackles were not detected in either position (PIC (-)), those in whom fine crackles were detected in a supine position or during passive elevation of both legs, but not in a sitting position (PIC (+)), and those in whom fine crackles were detected even in a sitting position (Persistent crackles). We measured various hemodynamic parameters (cardiac index, RA pressure, PA pressure and PAW) and parameters of pulmonary circulation (pulmonary blood volume, pulmonary "venous" compliance) in these 3 groups and comparisons were made between them.(ABSTRACT TRUNCATED AT 250 WORDS)

[Response of cardiac patients to dynamic exercise: analysis with "systolic" pressure-volume area].

In the present study, we constructed a left ventricular pressure-volume loop from catheterization data and RN-angiocardiography. By connecting the point of origin of the plane, not the point of Vd, with the end-systolic point of the loop, we calculated a "systolic" pressure-volume area (PVA), elastic potential energy (EPE), and stroke work (SW). A cardiac output (CO)-pulmonary artery wedge (PAW) pressure plot was constructed at rest and during exercise, using a bicycle ergometer, to evaluate cardiac pump function. End-systolic volume (ESV) was obtained from the left ventricular P-V loop, and the response of ESV to exercise was investigated. Thirty-five patients with various cardiac diseases were categorized in two groups according to the delta CO/delta PAW obtained from the shift of the CO-PAW plot during exercise; group I (n = 28) with the properly functioning hearts having the ratio greater than 0.12 l/min/mmHg and group II (n = 7) with the poorly functioning hearts having the ratio less than or equal to 0.12 l/min/mmHg. Group I was subdivided further into two subsets according to the changing pattern of ESV during exercise; group I-A with decreased or unchanged ESV (n = 20) and group I-B with increased ESV in which end-diastolic volume (EDV) was increased during exercise (n = 8). During exercise, PVA and SW were unchanged but SW/PVA increased in group I-A, suggesting improvement of external mechanical efficiency. SW/PVA was unchanged in group I-B, despite the increase in PVA and SW. This suggested that external mechanical work increased as a result of increased cardiac oxygen consumption. In group II, PVA increased, SW was unchanged and SW/PVA decreased, which could be explained by the mechanism that external mechanical work during exercise decreased as compared with that at rest. It was suggested that a different mechanism may have been responsible for the production of external mechanical work among patients in group I-A and in group I-B with properly functioning hearts judged from the CO-PAW plot. Therefore, it seems useful to calculate PVA, SW and SW/PVA during exercise using the left ventricular pressure-volume loop for evaluating cardiac function.

[Direct arrhythmia surgery in ventricular tachycardia: an experience with 18 consecutive patients].

18 patients with ventricular tachycardia (VT) underwent direct arrhythmia surgery between 1984 and 1988. There were 8 patients with ischemic VT, and 10 with nonischemic VT. Operative technique consisted of ablative procedures of the arrhythmogenic area determined by pre- and intraoperative mapping. Induced VT was usually unstable and transient during operation, so that instantaneous multi-point mapping was necessary in almost cases. For VT originated in the left ventricle or interventricular septum, the earliest excitation point determined by the epicardial mapping did not always predict the endocardial arrhythmogenic focus. Pre- and/or intraoperative endocardial mapping was important in this regard. Cryocoagulation (-150 degrees C, 120 sec) was mainly used as an ablative procedure; for ischemic VT, endocardial resection was added, and in nonischemic VT originated in the right ventricular outflow tract, transmural resection was combined with the cryoablation. In performing surgery for nonischemic VT, care must have been taken to make transmural cryocoagulation because the arrhythmogenic focus could exist intramurally. There were no operative deaths. In one patient with nonischemic VT, reoperation was required. After a mean follow-up of 17 month, all the patients are free from sustained VT.

[Reliability of the lipid emulsion test for reticuloendothelial system].

The reticuloendothelial system is important as one of the self defense mechanisms. We performed two investigations to confirm the reliability of the lipid emulsion test for determining the reticuloendothelial system function. 1) The lipid emulsion serum clearance was contrasted with the counts of lipid emulsions phagocytosed by Kupffer cells in liver. 2) Lipid emulsion clearance was contrasted with the hepatic uptake rate of 99mTc-phytate. The result demonstrated a positive correlation among lipid emulsion clearance, the counts of lipid emulsions phagocytosed by Kupffer cells and the hepatic uptake rate of 99mTc-phytate. These results confirm that the lipid emulsion test is sufficient to reflect Reticuloendothelial system function.

[Studies on intraoperative cardiac mapping].

Precise determination of the arrhythmogenic area by the intraoperative cardiac mapping procedure is a prerequisite to successful surgical therapy for tachyarrhythmias. Because of the limitation of time during operation, mapping should be performed precisely and quickly. For this purpose we have developed and used an on-line portable minicomputer system (modified HPM-6500; Fukuda Denshi Co., Ltd.). A ventricular epicardial excitation sequence map is acquired by measuring the conduction time difference for 51 predetermined points on the whole ventricular surface. The measured data are instantaneously digitized and computed to be displayed as an isochornous map in real time basis. Similar program is available for atrial excitation map. With this system it takes only about ten minutes to get an epicardial isochornous map. Recently a multi-channel signal-processing unit has been incorporated in the HPM-6500. With the use of a sock electrode or a card electrode, a map can be obtained on a few cardiac contractions. Multi-point simultaneous mapping has been proved to be especially important to get a VT isochornous map, for induced VTs during operation are usually unstable and transient.

Increased tubulin messenger RNA in the frog retina after optic nerve transection.

The optic nerve of the bullfrog (Rana catesbeiana) was transected and the regeneration process was investigated. The nerve regeneration was confirmed by observing the extension of the axon and by examining the recovery of axonal transport. Following optic nerve transection, the rapid and transient increase of messenger RNA of alpha-tubulin gene was observed in the retina, but no specific change of actin messenger RNA was observed.

Immunotherapy of patients with terminal-stage malignant tumors with an immunopotentiator OK-432--a comparison of SU-PS test-responding and -nonresponding patients.

Terminal malignant tumor cases were treated with immunopotentiator OK-432. The absolute numbers of neutrophils and lymphocytes and different lymphocyte subsets were determined, and SU-PS and PPD skin tests were also performed to monitor the immunologic status of each patient. The SU-PS test changed to positive in one-third of the patients 2-6 weeks after the start of therapy. The SU-PS responding patients showed an increase in lymphocytes and Leu II+ cells, and an elevation of the OK T4/T8 ratio 2 weeks later. In all patients from the responding group, the (OKIal+ - Bl+) cells decreased in the peripheral blood immediately after the positive change in the SU-PS test. In the SU-PS nonresponding group, the OKT4+ cells tended to decline with time, while the OKT8+ cells increased. That is, the OKT4/T8 ratio remained low throughout the test period. In the SU-PS responding group, OK-432 therapy prolonged the survival time.

Preparation of monoclonal antibodies to Theileria sergenti and their reactivity to antigens in experimentally infected cattle.

Two distinct monoclonal antibodies (3-H and 11-D) were produced against Theileria sergenti. These two new products, together with monoclonal antibody 1-G obtained in a previous study, were used to detect the parasites in experimentally infected cattle. During the first period of dexamethasone treatment, which was carried out to increase parasitemia in the infected cattle, the number of erythrocytes detected by 3-H, 11-D and 1-D increased in two experimentally infected calves. During the second period of dexamethasone treatment, the number of infected erythrocytes detected by 3-H and 11-D were similarly increased, but the number of infected erythrocytes detected by 1-G did not increase and infected erythrocytes in one calf were not detected by 1-G.

Facioscapulohumeral dystrophy associated with sensorineural hearing loss, tortuosity of retinal arterioles, and an early onset and rapid progression of respiratory failure.

Two sibling cases of facioscapulohumeral dystrophy (FSHD) are described. One was characterized by sensorineural hearing loss, marked tortuosity of retinal arterioles, an early onset and progression of severe restrictive-type pulmonary dysfunction, and cor pulmonale. The other had a mild course of FSHD without involvement of any other organ than muscles at the time of diagnosis. Recently, a new nosological entity of FSHD, with sensorineural hearing loss and tortuosity of retinal arterioles, was advocated. Our cases, especially the first case, seem to belong to this newly recognized entity of FSHD. Moreover, it is noteworthy that our first case exhibited rapid aggravation of severe restrictive-type respiratory failure and cor pulmonale, leading to death, which was never seen in any other reported cases.

The heterogeneous distribution of argininosuccinate synthetase in the liver of type II citrullinemic patients. Its specificity and possible clinical implications.

The authors analyzed the heterogeneous distribution of hepatic argininosuccinate synthetase of type II citrullinemia in reference to its specificity and clinical implications. The low content of the enzyme in the liver of type II citrullinemic patients is associated with two kinds of the enzyme distribution that can be visualized by means of an immunohistochemical method (Saheki and colleagues. Biomed Res 1983;4:235-238). Among the 25 cases of type II citrullinemia examined, 11 exhibited homogeneous distribution of the enzyme, as in the control livers. On the other hand, 14 presented the clustered distribution, in which the hepatocytes stained positively with antisera to argininosuccinate synthetase formed a cluster among the poorly stained cells. No clustered distribution of the enzyme was present in the liver of control patients either with or without liver diseases. No clustered distribution of arginase and aldolase B was observed even in the liver of type II citrillinemic patients. These results suggest that clustered distribution is specific to argininosuccinate synthetase in the liver of type II citrullinemic patients. From considerations concerning the heterogeneous distribution of the enzyme and certain clinical parameters as well, the authors suggest that the clustered type in type II citrullinemia has a less favorable prognosis with regard to fatality.
