Extensor tendon involvement in Smith's and Galeazzi's fractures.

Extensor tendon involvement in three Smith's and two Galeazzi's fractures are reported. In two cases of previous Smith's fracture, the extensor indicis proprius tendon had been entrapped and ruptured in one case, and in the other the extensor digitorum communis tendon of the index was pierced by the fragment. Both showed a severe tenodesis effect on the index finger and thumb. In a new case of Smith's fracture, long extensor tendons of the fingers and thumb had been trapped beneath a dorsally displaced third fragment making closed reduction impossible. In the two cases of Galeazzi's fracture; in one the extensor carpi ulnaris tendon was trapped between the dorsally displaced ulnar head and the avulsed styloid process and in the other, the extensor digiti minimi tendon was caught beneath the radial border of the dorsally dislocated ulnar head.

In vitro inhibition of human renin by statine-containing tripeptide renin inhibitor (ES-1005).

Dipeptide and tripeptide derivatives containing a statine residue were synthesized as human renin inhibitors. ES-305, bis[(1-naphthyl)methyl]acetyl-histidyl-statine-2(S)-methylbutylami de, was found to be a highly potent human renin inhibitor that is species-specific and enzyme-specific. The replacement of the methylbutylamide of ES-305 with the leucyl-lysinol (ES-1005) showed similar high potency against human renin (Ki value of 2.4 x 10(-9) M) and monkey renin (Ki value of 7.9 x 10(-9) M) as ES-305. ES-1005 competitively inhibited human renin. The compound was about one order of magnitude less potent against pig, dog, and rabbit renins. It had moderate inhibitory potencies against cathepsin D and pepsin (IC50 of cathepsin D and pepsin of 1.6 x 10(-5) and 8.0 x 10(-6) M, respectively). ES-1005, a newly synthesized tripeptide derivative containing statine, is a highly potent inhibitor of not only primate renin but also a wide variety of nonprimate renins.

Present status and perspective of percutaneous transluminal coronary angioplasty from the viewpoint of short and long term results: comparative study of the results of PTCA and CABG procedure.

Balloon technology has advanced to incorporate the skills of angioplasters while efforts to enhance their skills continue. These factors have contributed to the expansion of percutaneous transluminal coronary angioplasty (PTCA) indications. We have analyzed the comparison of short and long term results between PTCA and CABG revascularization procedures. In the development of revascularization procedures, one graft surgery has significantly declined in use since 1983 (2 years after the start of PTCA) while there has been an increase of multi graft surgery (more than 2 grafts). On the other hand, PTCA has showed a linear increase since 1982 and reached 160 cases in 1985. The growth of complex angioplasty other than PTCA for single discrete lesions is parallel to that of PTCA and has been used in 44% of overall cases. The growth curve of angioplasty crossed over that of revascularization surgery in 1983. PTCA was successful in 246 patients out of 300 overall cases representing 88% success rate and in 340 lesions representing an 81% success. For CABG the patency rate was 89.5% which means a out of 638 grafts were successful. PTCA was conducted in 137 cases with multiple lesion. That data could be interpreted as mean patient success of 120/137, lesion success was 196/265 with a success rate of 88% and 73%, respectively. Primary results in 284 multi CABG cases were good with a patency rate of 91% (487 patent grafts out of 536 anastomoses). However, in-hospital deaths were 3.5% higher (10 cases with CABG group). The effective dilatation of high-grade organic lesion was found to be closely related to the improvement of clinical symptoms and a marked decrease in incidence of ergonovine induced spasms at the angioplasty site in patients with vasospastic angina (VSA). Thus PTCA can be accepted as an alternative therapy to CABG in VSA. In the long term follow up, work load response parameters such as exercise time, % predicted HR, PRP, Mets and the modified treadmill exercise scores improved significantly after the successful PTCA.

Transfer of latissimus dorsi to replace a paralysed anterior deltoid. A new technique using an inverted pedicled graft.

A new method of replacement for the paralysed anterior deltoid is described. The latissimus dorsi with its neurovascular pedicle is freed and rotated, and then placed over the anterior part of the paralysed muscle. The lever arm of the transposed muscle is as long as that of the deltoid, and the muscle volume is enough to restore the natural contour of the shoulder. We report operations on 10 patients with shoulder palsy, eight caused by brachial plexus injury and two by resection of a neurilemmoma of the plexus. In six cases, active flexion to over 90 degrees was achieved. At least one of the rotator cuff muscles or the long head of biceps should have some active function if good results are to be obtained.

Systemic and coronary hemodynamic effects of beta-adrenoceptor blocking agents in coronary artery disease.

Systemic and coronary hemodynamic effects of acebutolol (10 mg i.v.), a cardioselective beta-adrenoceptor blocking agent were investigated in 11 patients with coronary artery disease and significant arterial obstructive lesions. Efficacy was assessed by simultaneous left and right heart catheterization and with an inlaying Webster thermodilution catheter in the coronary sinus. The data were compared with data from 7 other patients who received 2 mg i.v. of propranolol, a non-cardioselective beta-blocker. With acebutolol, (1) the heart rate was reduced significantly (p less than 0.001), (2) no significant changes were observed in the LVSP, LVEDP, mean PWP, LVmax dp/dt/p, LV negative dp/dt/p, CI, SWI and SPI, (3) CSF and MVO2 decreased significantly (p less than 0.01) 5 min after injection and (4) the CVR showed a significant elevation (p less than 0.05) after 5 min. With propranolol, (1) the heart rate decreased significantly (p less than 0.05), (2) there were no significant changes in LVSP and LVEDP, (3) the mean PWP increased significantly (p less than 0.05), (4) the LVmax dp/dt/p, CI and SWI decreased significantly (p less than 0.05), (5) the CSF and MVO2 decreased markedly (p less than 0.01) and (6) the CVR increased markedly (p less than 0.01). As compared to the effects of 2 mg i.v. of propranolol, those produced by acebutolol (10 mg i.v.) were characterized by a predominant negative chronotropic action with minimal negative inotropic action, combined with a reduction in CSF and MVO2. The findings suggest that the efficacy of acebutolol in pump failure caused by myocardial ischemia during effort angina is mediated by improvement of the myocardial oxygen demand-supply imbalance.

A rapid method for the detection of papillomavirus in warts: the frequency of virus detection in various types of warts.

A rapid method was devised for the detection of virus particles in wart specimens. The upper layer of warts was cut perpendicularly to the surface, and the freshly cut surface was lightly touched to an electron microscope grid. The grid was then stained with a small drop of phosphotungstate and observed electron microscopically. On the specimen grid thus prepared, papillomavirus particles were easily discriminated from tissue debris. Papillomavirus particles were detected in 71% of verrucae plantares, 78% of verrucae palmares, 50% of verrucae vulgares and 75% of condylomata acuminata by the present method.

Statine-containing dipeptide and tripeptide inhibitors of human renin.

Dipeptide and tripeptide derivatives containing a statine residue were synthesized as inhibitors of human renin. ES-305, bis[(1-naphthyl)methyl]acetyl(BNMA)-histidyl-statine 2(S)-methylbutylamide was found to be a highly potent inhibitor of human renin with a Ki value of 1.7 X 10(-9) M. Dipeptide derivatives with the BNMA group at the N-terminal (BNMA-Val-Sta-isoleucinol [ES-313], BNMA-Leu-Sta-isoleucinol [ES-316], and BNMA-Nle-Sta-isoleucinol [ES-317]) had potencies against human renin that were similar to the potency of ES-305. All these dipeptide derivatives competitively inhibited human renin. The inhibitors were also potent against monkey renin but were less effective against renins from pig, goat, dog, rabbit, and rat. ES-305 had little effect on cathepsin D and pepsin at the concentration of 10(-5) M. The other derivatives showed detectable inhibition of cathepsin D (IC50, 10(-6) - 10(-7) M) and pepsin (10(-5) - 10(-6) M). All the compounds had little or no effect on trypsin, chymotrypsin, angiotensin converting enzyme, and urinary kallikrein at the concentration of 10(-5) M. Our results indicate that ES-305 is a highly potent and specific inhibitor of human renin. This compound is superior to other, previously described statine-containing renin inhibitors with respect to molecular size and enzyme specificity.

Endomyocardial biopsy findings in pediatric patients with post myocarditic state.

Endomyocardial biopsy was performed on eight children 5 to 12 years old, who were in post myocarditic state. They were evaluated within 2 to 25 months (mean lyr and 1 m) after the onset of the symptoms. Two of the patients developed heart failure and six patients developed other cardiac manifestations such as syncope, palpitation or ECG abnormalities at onset. Definite elevation of viral antibody titer was observed in four patients. Radionuclide angiography was also performed in all eight patients. An abnormal perfusion area was observed in six patients as a focal hypoperfusion area by T1-201 myocardial imaging. Ejection fraction was examined by Tc-99m-HSA gated equilibrium ventriculography. LVEF was reduced in 3 patients and RVEF was reduced in 2 patients. Judging from the histopathological findings, these patients were divided into three categories: chronic or smoldering myocarditis (3 patients); healing or healed myocarditis (4 patients); and post myocarditic hypertrophy (1 patient). Measurement of left ventricular function was obtained by cardiac catheterization and left ventriculography (LVG), which revealed some abnormal findings such as increased left ventricular mass index, increased left ventricular end diastolic volume index (LVEDVI) and reduced left ventricular ejection fraction (LVEF). Therefore, endomyocardial biopsy findings of pediatric patients in a post myocarditic state reveal certain histopathological abnormalities even in the long-term follow-up period in the absence of cardiac dysfunction.

[Evaluation of the visualized left atrium by 201Tl myocardial scintigraphy].

201Tl myocardial scintigraphy was evaluated in patients with 18 mitral stenosis cases. Left atrium was to be seen in ANT (16.7%), LAO (16.7%), MLAO (16.7%) and L-LAT (11.1%), respectively. Furthermore, 201Tl uptake ratio of left atrium, right ventricle, pulmonary area tended significantly to increase in visible group compared with invisible group. PCW pressure was correlated with left atrial Tl uptake (r = 0.51, p less than 0.005), and it indicated some correlation between left atrial pressure and 201Tl uptake.

Trial of isolation of a virus from sera of patients with Kawasaki disease.

We tried to isolate pathogenic viruses from specimens of patients with Kawasaki disease. Blood clots and sera, spinal fluid, throat swabs, stool and urine from 24 patients with Kawasaki disease were studied. The specimens were inoculated into HEL cells and Vero cells. The cells were observed for one month, but no cytopathic effect (CPE) occurred. Blind passage was performed, but no degeneration of the second series of cells was observed. Fluorescent antibody indirect methods to determine viral antigens in cells inoculated with patients' specimens was also negative.

Clinical application of NMR-CT for idiopathic cardiomyopathy.

The accuracy of a newly developed nuclear magnetic resonance-computed tomography (NMR-CT) technique in diagnosing idiopathic cardiomyopathy was assessed and compared with other procedures such as echocardiography, coronary angiography, left ventriculography, myocardial biopsy, and electrocardiography. In case 1, the NMR-CT clearly revealed thickening of the lateral ventricular free wall and ventricular septum, which strongly suggested hypertrophic cardiomyopathy. Catheterization showed a pressure gradient of 54 mm Hg and this patient was diagnosed as having hypertrophic obstructive cardiomyopathy. In case 2, the NMR-CT showed dilatation of the ventricular cavity indicative of dilated cardiomyopathy. This was confirmed by echocardiography, which revealed the enlarged cavity of the ventricle and poor movement. In case 3, the patient had marked hypertension; the cardiac silhouette was enlarged, but the NMR-CT revealed that the ventricular free wall and septum were of normal thickness. This study shows that NMR-CT is of value in the differential diagnosis of idiopathic cardiomyopathy.

Gigantism associated with McCune-Albright's syndrome.

The case of a 16 year-old boy with McCune-Albright's syndrome which is rarely accompanied by gigantism was studied endocrinologically. The stimulation of growth hormone (GH) release by hypoglycemia, the decline of elevated GH by hyperglycemia and a little lower somatostatin like immunoreactivity (SLI) may support abnormalities of hypothalamic function, but the existence of pituitary microadenoma cannot be ruled out because of the paradoxical suppression of GH release by oral administration of bromocriptine (CB-154) and L-DOPA and the stimulation of GH release by intravenous administration of TRH.

Radionuclide imaging for assessment of myocarditis and postmyocarditic state in infant and children. Thallium-201 myocardial imaging and technetium-99m-HSA gated equilibrium ventriculography.

T1-201 myocardial imaging and Tc-99m-HSA gated equilibrium ventriculography were performed in 1 infant and 11 children between 2 months old and 12 years old with myocarditis. The time of first evaluation was between 1 week and 13 months after the onset. Their clinical manifestations were congestive heart failure in 2 patients, pericardial effusion in 2 patients, syncope in 1 patient, convulsions in 1 patient and palpitations in 7 patients. Significant elevation of virus antibody titer was demonstrated in 6 patients. A perfusion defect was observed with T1-201 imaging in 9 of 12 patients with myocarditis. A right ventricular image was observed in 5 patients. A reduced LVEF was present in 4 patients and a reduced RVEF was found in 2 patients. Cardiac catheterization was performed in 8 patients and an endomyocardial biopsy was done in 7. The myocardial specimens revealed postmyocarditic pathological changes upon microscopic evaluation. No coronary artery obstructive or stenotic lesions were observed in these patients. T1-201 myocardial imaging is a useful non-invasive technique for evaluating the cardiac performance and myocardial damage in acute myocarditis and the postmyocarditic state. We conclude that T1-201 uptake of myocardium is dependent upon both regional coronary perfusion and the activity of myocardial cells in patients with myocarditis and in the postmyocarditic state.

Study of left ventricular function and myocardial viability in patients with left ventricular aneurysm developed after myocardial infarction. A comparative study of medical and surgical therapy.

We evaluated the treatment of left ventricular aneurysm (LVA) caused by myocardial infarction in 44 patients showing cineangiographical features of left ventricular aneurysm. Of the 44 patients, 28 were treated non-surgically (N-S) and 16 were treated surgically (S). Combined aortocoronary bypass graft (ACBG) with aneurysmectomy was performed on 10 patients. Clinical symptoms in LVA patients were angina (34%), congestive heart failure (31.8%), arrhythmia (29.5%), mitral regurgitation (9%), embolism (4%) and septal perforation (2.3%). Distribution of coronary arterial lesions were single vessel (isolated LAD) 29.5% and multiple vessel 59%. Parameters of LV performance measured at baseline in all LVA patients were: CI 3.05 +/- 0.64 L/min/m2, LVEDP 19.0 +/- 3.5 mmHg, LVEDV 200.6 +/- 25.9 ml, diast. wall stress 50.7 +/- 16.8 g/cm2, EF 0.46 +/- 0.15, LV dp/dt/p 17.8 +/- 2.1 S-1, SWI 61 +/- 24 gm/m2. LV performance after surgery showed clear decreases in LVEDP, LVEDV and wall stress (p less than 0.05, p less than 0.02 and p less than 0.02, respectively). In contrast, EF, LV dp/dt/p and SWI increased significantly (p less than 0.02, p less than 0.1 and p less than 0.01, respectively). Comparison of the results of restudy with first catheterization data in the N-S group showed decreases of EF, contractility index and LV dp/dt/p, each reaching p less than 0.1. Residual myocardial motion 1 year after the first cineangiographic study showed a significant decrease (-12.8 +/- 26.7%) in the N-S group, whereas in the S group it significantly increased to (+60.4 +/- 52.7%). A significant difference in coefficient of variation between N-S and S groups was found. Thus, it can be concluded that aneurysmectomy or concomitant myocardial revascularization with aneurysmectomy improves left ventricular diastolic performance and increases residual myocardial viability.

Hypoplastic left coronary artery. In association with occlusive intimal thickening of a coronary artery with ectopic ostium and with atresia of the left coronary ostium.

Two cases of hypoplastic coronary artery (HCA) are presented. Case 1, a 13 year old girl, died suddenly during a long distance race. She had HLCA with marked intimal thickening and an ectopic left coronary ostium above the commisure between the non-coronary and left coronary cusp at post mortem examination. The right coronary artery (RCA) was enlarged and also supplied parts of the area normally supplied by the left coronary artery (LCA). Pathological findings revealed a normal RCA and an extremely hypoplastic LCA with occlusive proliferation of the intima and a myocardial infarction of the left ventricle. Case 2, a 6 year old girl, had a history of effort angina. Selective coronary angiography was performed which failed to demonstrate the orifice of the LCA by aortography. However, the hypoplastic LCA was visualized by RCA angiography as a consequence of anomalous collaterals from the atrioventricular branch of the RCA. We postulate that HCA results from various conditions, including stenosis of the coronary artery orifice, an aberrant course between the pulmonary artery and aorta and ectopic positioning of the coronary artery ostium. In addition, HCA may also be associated with occlusive coronary artery abnormalities.
