Report of the consensus conference on diagnostic criteria of ALI/ARDS at high altitudes in Western China.

China is a mountainous country. The Qing-Zang plateau, Yun-Gui plateau, and Yellow Land plateau, which are known as the world's ridge, are located in the west of China. The highland area over 3,000 m in height occupies one-sixth of the land area of China and half the highland area of the world. About 60-80 million people live in the regions where the elevation ranges from 1,500 m to 4,000 m. In the latter half of the last century, the influence of complex environmental factors on the human body, such as low oxygen pressure, cold climate, strong radiation, high wind speed, and super-evaporation, were studied in an extensive and profound way and formed an important field of altitude medicine. For a long time, many researchers have carried out investigations related to the systemic inflammatory response syndrome (SIRS), acute respiratory distress syndrome (ARDS), and multiple organ dysfunction syndrome (MODS) and its pathophysiological mechanism initiated by cytokines and mediators, regarding which many problems are still unclear. However, systematic research into the mechanism from SIRS to ARDS to MODS in the highlands remains blank. The diagnostic criteria of ALI/ARDS in the highlands are quite different from that in the plain, and thus was a central topic for discussion at this meeting.

Expression of vascular endothelial growth factor and its role in oncogenesis of human gastric carcinoma.

AIM: To establish the role of vascular endothelial growth factor (VEGF) in the oncogenesis of human gastric carcinoma more directly. METHODS: The expression of VEGF and its receptor kinase-domain insert containing receptor (KDR) in human gastric cancer tissue were observed by immunohistochemical staining. VEGF levels were manipulated in human gastric cancer cell using eukaryotic expression constructs designed to express the complete VEGF(165) complimentary DNA in either the sense or antisense orientation. The biological changes of the cells were observed in which VEGF was up-regulated or down-regulated. RESULTS: VEGF-positive rate was 50%, and VEGF was mainly localized in the cytoplasm and membrane of the tumor cells, while KDR was mainly located in the membrane of vascular endothelial cells in gastric cancer tissues and peri-cancerous tissue. In 2 cases of 50 specimens, the gastric cancer cells expressed KDR, localized in both the cytoplasm and membrane. Introduction of VEGF(165) antisense into human gastric cancer cells (SGC-7901, immunofluorescence intensity, 31.6%)) resulted in a significant reduction in VEGF-specific messenger RNA and total and cell surface VEGF protein (immunofluorescence intensity, 8.9%) (P<0.05). Conversely, stable integration of VEGF(165) in the sense orientation resulted in an increase in cellular and cell surface VEGF (immunofluorescence intensity, 75.4%) (P<0.05). Lowered VEGF levels were associated with a marked decrease in the growth of nude mouse xenografted tumor (at 33 days postimplantation, tumor volume: 345.40 +/- 136.31 mm3)(P<0.05 vs control SGC-7901 group: 1534.40 +/- 362.88 mm3), whereas up-regulation of VEGF resulted in increased xenografted tumor size (at 33 days postimplantation, tumor volume: 2350.50 +/- 637.70 mm3) (P<0.05 vs control SGC-7901 group). CONCLUSION: This study provides direct evidence that VEGF plays an important role in the oncogenesis of human gastric cancer.

[Application of differential wavelet in detecting QRS complexes of ECG signal].

An algorithm based on wavelet transforms (WT'S) has been developed for detecting QRS complexes of ECG signal. In this paper, we use Mallat algorithm to calculate dyadic Marr wavelet transforms, analyse the relationship between singularity of the signal (the peak of R wave) and local maximal value of WT's from the view of equivalent wavelet filter, discuss the performance distinction between quadratic differential wavelet and single differential wavelet for singularity detection. Some other tactics are also applied to improve the anti-interference performance of the algorithm and to increase the correct detection rate of QRS complexes. Verified by the MIT-BIH Arrhythmia Database, the correct detection rate of QRS complexes is 99.8%.

Identification of subdominant cytotoxic T lymphocyte epitopes encoded by autologous HIV type 1 sequences, using dendritic cell stimulation and computer-driven algorithm.

Conventional analysis of the cytotoxic T lymphocyte (CTL) response to HIV-1 may underestimate the true breadth of CTL epitopes recognized. This underestimation could be due to several reasons, including (1) the use of laboratory-adapted stains of HIV or consensus sequences, which would lead to the identification of only highly conserved epitopes, (2) the use of EBV-transformed B cells (B-LCLs) and vaccinia virus constructs in standard assays that may obscure low level CTL responses due to high EBV or vaccinia reactivity, and (3) relatively insensitive assays wherein PBMCs instead of professional APCs are used to stimulate CTL responses. To address these problems, we first identified an immunodominant HLA-B7-restricted CTL epitope, by standard cloning methods, in a long-term nonprogressor (LTNP). To determine whether the patient had CTLs specific for autologous viral sequences other than the dominant epitope, proviral DNA was cloned and sequenced. A matrix-based epitope algorithm (EpiMatrix) was used to identify the top 2% of peptides from the viral sequences with the highest likelihood of binding to HLA-B7. These 55 peptides were synthesized and tested for HLA-B7 binding in a T2/B7 cell line; 10 peptides were able to stabilize HLA-B7 on the cell surface. By using peptide-pulsed autologous dendritic cells as a more sensitive method of CTL stimulation, we found three additional subdominant CTL epitopes.

[Prediction and evaluation of heterosis of beef cattle and their application].

The genetic structure and genetic variation of eight beef cattle cross parents populations were analyzed by six microsatellite loci, and heterosis of beef cattle was predicted. On the basis of microsatellite analysis, the effect of 18 cross combinations was estimated by the method of individual animal model BLUP. A new method of molecular quantitative genetics that select best of all cross combination was submitted. The results showed that the combinations with Hereford Limousine and Charolais as paternal parent are better than others in Fengning and Longhua regions; the combinations with Limousine Angus and Hereford as paternal parent are better than others in Zanhuang regions; the combinations with Hereford Limousine and Piemontese as paternal parent are better than others in Funing regions. Effect of three breeds cross is better than two breeds.

[Clinical study on modified sijunzi decoction in treating intestinal metaplasia of gastric mucosa].

OBJECTIVE: To investigate the effects of Modified Sijunzi decoction (MSJZD) in treating gastric pre-cancerous lesion. METHODS: Two hundreds and two patients with chronic gastritis and intestinal metaplasia (IM) of gastric mucosa diagnosed by gastroscopy and pathological examination of biopsy were divided into 2 groups, 117 cases of the treatment group were treated with MSJZD and 85 cases of the control group were treated with Weimeisu. For both groups, re-examination was taken after a 3-month treatment. RESULTS: The cure rate and total effective rate of IM in the treatment group were 55.6% and 87.2% respectively, while in the control group were 11.8% and 55.3% respectively (P < 0.01). CONCLUSIONS: The intestinal metaplasia process of gastric mucosa is reversible and MSJZD has good effect on it. This study provided a new way of pre-cancerous lesion treatment and gastric cancer prevention.

A baculovirus vector derived from immediately early gene promoter of Autographa californica nuclear polyhedrosis virus.

A transfer vector was constructed in which the neomycin resistance (neo) gene was under the control of a copy of Autographa californica nuclear polyhedrosis virus (AcMNPV) IE1 gene promoter at the p10 locus. After cotransfection of Spodoptera frugiperda (Sf9) cells with the transfer vector and infectious AcMNPV DNA, the recombinant virus-containing neo gene was selected by serial passage of the mixed progenies from cotransfection. This was done at low MOI in the presence of G418 in growth medium and was followed by limited dilution. RNA dot hybridization showed that the neo gene was transcribed from immediately early phase to very late phase, in infected Sf9 cells. These results demonstrate that a new baculovirus vector system had been constructed in infected cells. Furthermore, a new method for selection of positive recombinant baculovirus had been developed.

Nucleotide sequence of a 1446 base pair SalI fragment and structure of a novel early gene of Leucania separata nuclear polyhedrosis virus.

A 1446 bp SalI fragment of LsNPV was sequenced by the silver staining method, and two large open reading frames (ORFs, ORF1 and ORF2) were found, both contain typical characteristics of the 5' regulatory elements of baculovirus early genes. ORF1 is 345 bp long with the capacity to encode a putative protein of 114 amino acid residues with MW about 13 kDa and was designated p13 gene, ORF2 comprises 248 bp from the 3' end of the fragment. In the untranslated region (UTR) of ORF1, a 33 bp mini cistron (ORF3), a core recognition sequence (CGTCG) for many bHLHzip transcription factors and a late promoter sequence TTAAG are present. In the UTR of ORF2, two host transcription factor binding elements (CACGTG and GATA motif) and two CGT motifs were found. Some regular leucine zipper-like structures, designated leucine trans-conformation structure and LVT repeat, were found near the N-terminus and the middle of p13 protein. The leucine trans-conformation structure that is near the N-terminus consists of 4 leucines and 7 other amino acids between every two leucines, and every leucine is located at a conformation shift point of the predicted secondary structure of the p13 protein. In LVT repeat, L-6aa-V-6aa-T-6aa is repeated once. The functions of those structures remain unclear, and the two ORFs, not found in the genome of Autographa californica nuclear polyhedrosis virus, are possibly two new genes.

[Motilin and gastrin in human milk].

The concentrations of motilin and gastrin were determined in the blood and milk of 45 women 3-5 days postpartum, and in the blood of 20 healthy non-pregnant women as control. Plasma motilin concentration (443.05 +/- 140.79 ng/L) and serum gastrin level (301.32 +/- 100.98 ng/L) were significantly higher in postpartum women than those in the control (366.12 +/- 96.23 ng/L and 76.13 +/- 13.11 ng/L, respectively) (P < 0.01). The concentrations of both motilin and gastrin in the milk were approximately one half of those in the blood and they were not present in the boiled milk. The results indicated that both hormones in the human milk may be important for development and maturation of gastro-intestinal function in neonates, especially for immature babies.

[Changes of plasma motilin concentration in pregnancy and early postpartum period].

Plasma motilin concentration were determined by radioimmunoaction from 180 women during pregnancy and early postpartum period as compared with 20 healthy non-pregnant women. The results showed that mean plasma motilin concentration (384.40 +/- 110.30 ng/L) was higher in the first trimester of pregnancy than that of healthy non-pregnant women (366.12 +/- 96.23 ng/L), however, this difference did not reach statistical significance (P > 0.05). The mean plasma motilin concentration (323.90 +/- 125.10 ng/L) was lower in the second trimester of pregnancy than in the first trimester of pregnancy (P < 0.05), while the mean plasma motilin concentration in the third trimester of pregnancy (121.04 +/- 27.00 ng/L) was significantly lower than in second (P < 0.01) and the mean plasma motilin concentration in 3-5 d after delivery (443.05 +/- 140.79 ng/L) reached an even higher value (P < 0.01). Our results suggests that pregnancy appears to have a profound inhibitory effect on plasma motilin and this may in part be responsible for the gastrointestinal hypomotility during pregnancy.

[A study on gastric mucosa damage, antigen location, and virus dissociation in patients with epidemic hemorrhagic fever].

Gastric mucosa damage was investigated with techniques of gastroscopy, gastric biopsy, antigen location with immunohistochemical enzyme staining and virus dissociation in 135 patients with epidemic hemorrhagic fever (EHF). We found that the major pathologic finding in gastric mucosa is hemorrhage of squamous shape. EHF virus antigen exists in epithelial cytoplasm of the superficial layer and lamina propria of gastric mucosa and the virus is dissociated from gastric mucosa. The virus is identified as of field-rat serotype with electronmicroscopy and monoclonal antibody techniques. These results suggest that it is necessary to start antivirus therapy early and to give prophylactic measures to prevent gastric mucosa damage and upper digestive tract bleeding.

[Effect of different preparation crafts on the content of baicalin in shuanghuanglian (SHL) injection].

The content of baicalin is stable when baicalin is added quantitatively in preparation. The time of extraction of the effective components of SHL with alcohol and the concentration of alcohol have little effect on the content of baicalin. The components can be extracted once, and the content of alcohol is preferably 85%. The pH level has greater influence on the content of baicalin, and should be controlled between 7.0 to 8.0. 0.2% activated carbon is suitable in preparation.

Identification and quantitation of HIV-1 in the liver of patients with AIDS.

OBJECTIVE: To detect and quantify HIV-1 in the liver in vivo. DESIGN: Fourteen liver biopsy samples and corresponding blood lymphocytes and monocytes from patients with AIDS were studied for HIV-1 using quantitative polymerase chain reaction (PCR). In addition, expression of HIV-1 antigen and messenger (m) RNA in 10 autopsy liver specimens was examined by immunohistochemistry and in situ hybridization. RESULTS: The amount of HIV-1 DNA in nine liver samples ranged from 850 to 27,000 copies per 10(6) cells, with mean and median values of 8150 and 3500 copies per 10(6) cells, respectively. Five other samples had no detectable HIV-1 DNA by PCR. Intracellular expression of HIV-1 antigen and mRNA was also detected in both Kupffer cells and hepatocytes by in situ studies. CONCLUSION: These findings strongly indicate that HIV-1 could replicate in the liver of a majority of patients with AIDS.

Virions of primary human immunodeficiency virus type 1 isolates resistant to soluble CD4 (sCD4) neutralization differ in sCD4 binding and glycoprotein gp120 retention from sCD4-sensitive isolates.

Primary isolates of human immunodeficiency virus type 1 (HIV-1) are much less sensitive to neutralization by soluble CD4 (sCD4) and sCD4-immunoglobulin (Ig) chimeras (CD4-IgG) than are HIV-1 strains adapted to growth in cell culture. We demonstrated that there are significant reductions (10- to 30-fold) in the binding of sCD4 and CD4-IgG to intact virions of five primary isolates compared with sCD4-sensitive, cell culture-adapted isolates RF and IIIB. However, soluble envelope glycoproteins (gp120) derived from the primary isolate virions, directly by detergent solubilization or indirectly by recombinant DNA technology, differed in affinity from RF and IIIB gp120 by only one- to threefold. The reduced binding of sCD4 to these primary isolate virions must therefore be a consequence of the tertiary or quaternary structure of the envelope glycoproteins in their native, oligomeric form on the viral surface. In addition, the rate and extent of sCD4-induced gp120 shedding from these primary isolates was lower than that from RF. We suggest that reduced sCD4 binding and increased gp120 retention together account for the relative resistance of these primary isolates to neutralization by sCD4 and CD4-IgG and that virions of different HIV-1 isolates vary both in the mechanism of sCD4 binding and in subsequent conformational changes in their envelope glycoproteins.

[Small bowel transit time measured with native lactulose breath hydrogen test].

Native lactulose was used in breath hydrogen test (BHT) to detect small bowel transit time (SBTT) in normal persons (34 cases) and patients with functional diarrhea (24 cases) and functional constipation (12 cases). The mean SBTT was 80.6 +/- 28.3 min, 58.4 +/- 42.2 min and 104.2 +/- 21.0 min respectively. When the mean SBTT in the patient groups was compared with that in the normal group, there was statistically significant difference (P less than 0.05 and 0.01 respectively). The results showed that native lactulose produces hydrogen successfully (94.3%). Few side-effects were found, if a dose of 10g was used.

Augmentation of various immune reactivities of tumor-bearing hosts with an extract of Cordyceps sinensis.

In order to enhance general reactivity of immune system in the tumor-bearing host, we employed extract of Cordyceps sinensis (CSE) as a biological response modifier. Cordyceps sinensis is an interesting material produced by a kind of mushroom parasitic to larval moths and was used to hasten recovery from exhaustion in ancient China. In this experiment, C57BL/6 mice implanted subcutaneously with syngeneic EL-4 lymphoma cells were employed as the host. Oral administration of the extract leads to a reduction of tumor size and prolongation of the host survival time. As judged by plaque-forming cells against T-dependent (sheep erythrocytes) and T-independent (bacterial lipopolysaccharide) antigens, CSE showed to augment the antibody responses. As for the activities of peritoneal macrophages, chemotaxis was dramatically depressed within a few days after EL-4 transplantation up to the end of life, but treatment with CSE at -14, -7, -4, +4, +7 and +10 days after the tumor transplantation augmented the activity about four times stronger than that of control. Phagocytic activity of macrophages was also decreased in tumor-bearing mice treated with cyclophosphamide (100 mg/kg) 3 and 5 days after tumor transplantation. But administration of CSE restored the activity to more than the normal level. The overall efficacy of CSE was tested with protective activity against systemic infection by Salmonella enteritides. The tumor-bearing mice receiving this medicine lived significantly longer than any other groups without CSE.

Sulcus topography of the parietal opercular region: an anatomic and MR study.

The study describes the sulcal and gyral topography, variability, and left-right asymmetry of the parietal operculum. Eighty postmortem hemispheres as well as sagittal magnetic resonance images from 20 health volunteers (40 hemispheres) were evaluated. Four different types of parietal opercular sulcus topography were recognized. Most frequently, and conforming with the anatomic "textbook pattern", the inferior postcentral sulcus (POCS) is the sulcus anterior to the posterior ascending ramus (PAR) of the Sylvian fissure (type 1). Variations were the following: lack of a PAR (type 2), interposition of an intermediate opercular sulcus and gyrus between PAR and POCS (type 3), and direct transition of PAR into POCS with subsequent lack of a classical supramarginal gyrus (type 4). Inconstancy of the sulcal standard arrangement was especially pronounced among left hemispheres, where the patterns differed from type 1 in one third of the cases. Types 2 and 3 were significantly more frequent in left hemispheres, whereas type 4 occurred significantly more frequently in right hemispheres. Upon intraindividual left-right comparison, a remarkable 38% of the brains showed gross asymmetry of the parietal opercular sulcus patterns, characterized by a left type 2 or 3 and/or a right type 4; another 5% exhibited a reverse type of asymmetry. The findings supplement previous data on gross variability and left-right asymmetry of the posterior Sylvian fissure and its lower bank. They indicate that the Sylvian fissure is an unreliable landmark with respect to inferior parietal structures especially in left hemispheres. Individual mapping of perisylvian topography may contribute to studies on structural-functional relationship.

Total surface of temporoparietal intrasylvian cortex: diverging left-right asymmetries.

Measurements of the total convoluted surface area of cortex buried in the temporoparietal transition of the Sylvian fissure were performed using "three-dimensional" magnetic resonance morphometry in 10 randomly selected human cadaver brains. The measurements comprised the planum temporale, which covers the superior temporal gyrus posterior to Heschl's first transverse gyrus, and the caudally adjacent walls of the descending and ascending rami of the Sylvian fissure. Also, the total convoluted surface of the planum temporale was compared with its superficially exposed surface as determined by photographical planimetry after brain section, so that the index of cortical folding of the planum temporale could be estimated. The following results were obtained: Cortical folding of the planum temporale did not differ significantly between left and right hemispheres (p greater than .6). The total and the superficially exposed surface of the planum temporale showed significant leftward asymmetry (p less than or equal to .0078). In contrast, the total surface of cortex buried in the caudal segments of the Sylvian fissure posterior to the planum exhibited significant rightward asymmetry (p less than or equal to .016). The combined cortical surface area buried in the whole posterior Sylvian fissure caudal to the first transverse gyrus did not show significant left-right asymmetry (p greater than 0.3). The data suggest divergent lateral asymmetries in the posterior intrasylvian region with excesses of superior temporal cortex in left hemispheres and of supramarginal cortex in right hemispheres.
