Autoantibodies against oxidized low-density lipoprotein (LDL) and carotid atherosclerosis in patients with rheumatoid arthritis.

OBJECTIVES: To examine the relationship between autoantibodies against oxidized low-density lipoprotein (oxLDL-Abs) and the progression of carotid atherosclerosis in patients with rheumatoid arthritis (RA). METHODS: Fifty RA patients without evidence of risk factors for atherosclerosis (RA group) and 30 healthy volunteers (normal group) were investigated. The mean intima-media thickness of the common carotid artery (mean CCA-IMT) was measured by high-resolution B-mode ultrasonography. The titer of IgG oxLDL-Abs was measured by enzyme-linked immunosorbent assay. The relationships among mean CCA-IMT, IgG oxLDL-Ab titer and patient factors such as body mass index, systolic blood pressure, diastolic blood pressure, C-reactive protein (CRP), erythrocyte sedimentation rate (ESR), and serum lipid levels were examined. RESULTS: Mean CCA-IMT, CRP, ESR and titer of IgG oxLDL-Abs were significantly higher in the RA group than in the normal group. Although mean CCA-IMT showed a positive correlation only with age in multivariate analysis, IgG oxLDL-Ab titers in the RA group were positively associated with mean CCA-IMT and independently with age and sex by multiple regression analysis. CONCLUSIONS: IgG oxLDL-Abs appear to be associated with the degree of carotid atherosclerosis in patients with RA, and are independent of traditional risk factors for atherosclerotic diseases. These results suggest a possible link between autoimmune mechanisms and accelerated atherosclerosis in RA.

Immunogenicity and involvement in the host's effector mechanisms of the oncogene--induced transformation--associated cell surface antigens.

The clonal expression of oncogene-induced transformation-associated cell surface antigens (TAAs) of a WFB rat fetus fibroblast was analyzed by developing mAbs to TAAs. We identified four TAAs. Two out of four TAAs seemed to be involved in the cytotoxicity by NK cells. One is an hsp-like molecule. These TAAs may play an important role in the antitumor surveillance mechanism by the host immune system. By clarifying the molecular characteristics, it would be possible to apply the genes encoding these molecules in the immunotherapy of cancer.

Cytologic diagnosis of Molluscum contagiosum of the mons pubis. Report of two cases.

1) Two cases of molluscum contagiosum diagnosed by cytologic examination were reported. 2) Cytologic findings of molluscum contagiosum are described. 3) The usefulness of cytologic examination in detection of unsuspected and/or suspected molluscum contagiosum is emphasized. 4) One molluscum contagiosum case showing coexistence of fungal organisms which seemed to be pityrosporum ovale is reported.

Experimental study on chemonucleolysis. With special reference to the change of intradiscal pressure.

The purpose of this study is to investigate mechanical change in the intervertebral disc after chemonucleolysis. Thirteen monkeys were used. After transperitoneal approach to the lumbar spine, 400 units of Discase was injected into each disc. The animals were killed 1 day; 1, 2, 3, 4, 6, and 12 weeks; and 1 year after chymopapain injection. Compression load was applied to the segment composed of a disc and adjacent vertebral bodies. Intradiscal pressure was measured simultaneously. A linear relationship was noticed between compression load and intradiscal pressure. In chymopapain-injected discs, the pre-existing pressure decreased or disappeared, and the increase of pressure for a given load was lower than that of the control. Our biomechanical study suggested that the decrease of intradiscal pressure as a possible mechanism of symptom relief in chemonucleolysis.

Clinicopathological characteristics and surgical procedures for carcinoma of the papilla of Vater.

BACKGROUND/AIMS: In the present study, clinicopathological characteristics of carcinoma of the papilla of Vater and suitable operative procedures for treatment were investigated. MATERIAL AND METHODS: Sixty cases (40 male and 20 female) of resected carcinoma of the papilla of Vater were studied clinicopathologically. RESULTS: Among the patients, a polypoid type without ulceration and an ulcer-formation type, with regard to the gross appearance, were found in 37 cases and 23 cases, respectively. Submucosal invasion, invasion to the sphincter Oddi, pancreatic parenchymal invasion and lymph node involvement were found more frequently in the ulcer-formation type than in the polypoid type without ulceration. Postoperative survival curves revealed that the prognosis was poor when either lymph node involvement, invasion to pancreatic parenchyma or ulcer formation was found. Local resection was performed in seven cases, in all of whom carcinoma was found histologically in the cut surface of the tumor. This is easily explained by the fact that mucosal spread or interstitial infiltration was frequently found even in cases with carcinoma at a relatively early stage. CONCLUSIONS: Local resection for carcinoma of the papilla of Vater is inadequate as a curative resection, and pylorus-preserving pancreatoduodenectomy should be the treatment of choice for carcinoma of the papilla of Vater. However, since there were no differences between the postoperative survival curves of patients who underwent pancreatoduodenectomy or local resection, local resection may still be suitable for patients with other major diseases or a poor condition.

Comparison of the effects of various anticholinergic drugs on human isolated urinary bladder.

We investigated the effects of various anticholinergic drugs (atropine, oxybutynin, terodiline and propiverine) on the contractions induced by acetylcholine, KCl, CaCl2, and electrical field stimulation, in human isolated urinary bladder smooth muscles using the muscle bath technique. Urinary bladders were obtained from 20 patients who underwent total cystectomy due to malignant bladder tumor. The detrusor preparations were taken from the intact part of the dome of the bladder. Acetylcholine caused a concentration-dependent contraction in human detrusor preparations. Atropine (10(-9)-10(-6) M), oxybutynin (10(-8)-10(-5) M), terodiline (10(-7)-10(-5) M) and propiverine (10(-7)-10(-5) M) caused parallel shifts to the right of the concentration-response curves to acetylcholine. The rank order of pA2 values was: atropine > oxybutynin > terodiline = propiverine. Atropine did not suppress the maximum contraction to acetylcholine, while the other drugs significantly suppressed the maximum contractions at the higher concentrations. Each drug caused a concentration-dependent inhibition of the KCl (80 mM)- and CaCl2 (5 mM)-induced contractions; the maximum inhibitions of terodiline and propiverine were significantly greater than those of oxybutynin and atropine. Each drug caused a concentration-dependent inhibition of the contraction induced by electrical field stimulation; the maximum inhibitions of terodiline and propiverine were significantly greater than those of oxybutynin and atropine. The results suggest that the drugs have both anticholinergic and calcium antagonistic effects. Furthermore, it also appears that part of the human bladder contraction, which was significantly inhibited by terodiline and propiverine, is an atropine-resistant component.
