[Serum prolactin levels following abortion in normal and molar pregnancy].

The changes in serum specific hCG and prolactin levels were studied in 20 women after the evacuation of normal pregnancy at 5-10 weeks (group A) and 10 patients after the evacuation of molar pregnancy of 9-16 weeks (group B). The elimination of circulating hCG was faster in group A than in group B, and that in women after term delivery(group C)was faster than in both groups A and B. The half-life of the serum hCG level was 44.4 hrs in group A, 42.7hrs in group B, and 38.9hrs in group C, respectively. The mean level of serum prolactin in group A was 27.4ng/ml at 0 day, 64.5ng/ml at 4-7 days, and 31.9ng/ml at 13-15 days after evacuation, respectively. The surge was statistically significant. Though no regular tendency was found in the prolactin levels in group B, the levels were relatively higher than in group A and nonpregnant women. No correlation was found between hCG and prolactin levels in either group A or group B. The administration of drugs, such as methotrexate, did not affect the prolactin levels. On BBT, the resumption of ovulation tended to occur when the prolactin level, rather than the hCG level, returned to its normal range.

Characterization of a new fetal hemoglobin variant, Hb F Izumi A gamma 6Glu replaced by Gly, by molecular secondary ion mass spectrometry.

Molecular secondary ion mass spectrometry has characterized the structure of a new fetal hemoglobin variant, Hb F Izumi, without separation of peptides or amino acid analysis. First, the mass spectrum of a tryptic digest of the abnormal gamma globin revealed a decreased by 72 mass units in the molecular mass of peptide T-1,2, indicating the presence of a Glu leads to Gly substitution. Next, the analysis of the digest produced by the addition of staphylococcal protease, which specifically cleaves glutamyl peptide bonds, determined the site of substitution at 6th glutamic acid residue in peptide T-1,2 which contains two glutamic acid residues. Since this mass spectrometric approach provides digitalized data on peptide analysis, we call it 'digit printing'. The high sensitivity of this technique is especially promising for the analysis of molecular abnormality in various genetic disorders.

"Convulsoid responses" suggesting development of autonomous epileptogenicity.

To test a convenient and reliable neuronal index that suggests the development of autonomous epileptogenicity, we conducted in acute experiments on rabbits a comparative study of the changes in direct cortical responses (DCRs) in primary and mirror sites during electrically induced primary (PRID) and projected seizure discharges (PROD). It has been assumed that the primary site shows autonomous epileptogenicity during the PRID, whereas the mirror site during the PROD does not. We observed that in the primary site, the dendritic potentials (field EPSPs) were remarkably suppressed in amplitude or disappeared during the PRID and that there was a loss of the after-positivity (field IPSPs); these DCRs recovered gradually after termination of the PRID. Instead of DCRs, "convulsoid responses" (a general term for responses similar to the individual spontaneous waves which occur in all types of seizures discharges) were usually elicited in the primary site during PRID. In the mirror site during PROD, the DCRs showed three different behaviors: they were either unaffected, increased, or suppressed. Convulsoid responses were never elicited. In a few cases, independent seizure discharges were induced in the mirror site. During the independent seizure discharges, the DCRs in the mirror site disappeared and convulsoid responses similar to the individual independent waves were usually elicited, as in the primary site during PRID. We conclude that the convulsoid response was the most reliable indicator of the development of autonomous epileptogenicity, and that suppression or disappearance of DCRs was the supplementary sign of that development. These neuronal indexes may be useful for identification of secondary epileptogenesis.

Pituitary-ovarian relationships in women approaching the menopause.

With a view to evaluating pituitary-ovarian relationships in women approaching the menopause, serial determinations of basal body temperature (BBT) and of serum levels of follicle-stimulating hormone (FSH), luteinizing hormone (LH) and oestradiol were performed in 5 pre-menopausal and 9 young women throughout a complete menstrual cycle. The hormone levels in the early follicular phase and on LH peak-days in the two groups were then compared. In the pre-menopausal group the age range was 43-45 yr and the length of the menstrual cycles investigated varied from 30-56 days, the BBT curves being biphasic. Two pre-menopausal women demonstrated concentrations of FSH and LH that were higher than those in the young women, even though their serum oestradiol levels in the early follicular phase remained within the upper and lower mean values observed in the young women. This finding confirmed that, in addition to reduced ovarian oestrogen secretion, another, as yet unknown, factor must exist, which can produce a rise in serum gonadotrophin levels in women approaching the menopause.

CT angiography in hepatocellular carcinoma.

In 32 cases of hepatocellular carcinoma (HCC) computed tomographic angiography (CTA) was performed to evaluate its diagnostic usefulness. The contrast material was infused into both the hepatic artery and superior mesenteric artery. Computed tomography was performed during the arterial and the portal phase. According to the CTA findings, HCC could be classified into four types; 28 of 32 cases (87.5%) showed the characteristic findings of HCC. It was also apparent that CT portography was quite efficient in detecting the existence of tumor, in particular the presence of small intrahepatic metastatic lesions and tumor infiltration into the portal vein. Computed tomographic angiography performed during both the arterial and portal phase is useful in evaluating HCC prior to hepatic lobectomy or arterial embolization therapy.

Endotoxin clearance by exchange blood transfusion in septic shock neonates.

The role of exchange transfusion with fresh whole blood in septic shock infants was evaluated. Endotoxin was positive in 8 of 10 infants and completely removed from 6 by exchange transfusion. All 6 infants with negative endotoxins after exchange transfusions survived, but 2 with positive reactions died despite the exchange transfusion. Staphylococcus aureus was cultured in the blood from 2 infants with negative reactions and one of them survived. These findings suggested that the clearance of endotoxin has an important role in exchange transfusion among septic shock neonates.

Cord serum growth hormone in the human fetus. Sex difference and a negative correlation with birth weight.

Cord venous growth hormone (GH) were measured by radioimmunoassay in 88 infants and 11 anencephalic infants. The mean GH levels decreased gradually from midgestation, preterm to term fetus. There was a significant (p less than 0.01) sex difference in the GH level of term fetuses. Anencephalic infants had low GH levels. There was a significant (p less than 0.01) negative correlation between the cord GH level and birth weight in the normal infant. These data suggest that there is a sex difference for fetal GH secretion and that GH is not so important for fetal growth, but the hypothalamus plays an essential role in the regulation of fetal GH secretion.

Copper-binding proteins in the liver and kidney from the patients with Menkes' kinky hair disease.

Copper-binding proteins in the tissues from the patients with Menkes' kinky hair disease were examined by gel filtration on a Sephadex G-75 column. In the kidney, major part of copper was found to bind to low molecular weight protein, which corresponded chromatographically to metallothionein. This copper-binding protein contained a large amount of copper and a small amount of zinc. Cu: Zn ratio of this protein was different from that of metallothionein found in the fetal liver. In the liver of the same patient, however, there was no increase of copper bound to this protein.

Therapy for cervical cancer detected by mass screening.

Over a 19 year period since 1962, 972 cases of cervical cancer have been detected by mass screening in Miyagi Prefecture, a detection rate of 0.11%. Among these cases, 551 (56.7%) were cases of carcinoma in situ. In contrast, cervical cancers detected by means of private screening gave a detection rate of 0.39% (889 cases), among which there were 611 cases of invasive carcinoma (68.7%). Among the 421 cases of invasive carcinoma detected by mass screening, 352 were Stage I, 52 were Stage II, 14 were Stage III and 1 was Stage IV. Moreover, 178 of the 352 Stage I cases were Stage Ia. The 5 year survival rate for the cervical cancer patients detected by mass screening was 97.8% for Stage 0, 92.9% for Stage I, 75% for Stage II, 45.5% for Stage III and 0% for Stage IV. The 5-year survival rate for invasive carcinoma was 88.7%. Twenty-five deaths due to recurrence of the cancer were found, but all such cases were invasive carcinomas. There were no mortalities due to recurrence of carcinoma in situ among such cases detected by mass screening.

The electric fence for preventing invasion of Trimeresurus flavoviridis, Habu, the venomous snake.

For the purpose of preventing invasion of Trimeresurus flavoviridis, Habu, the venomous snake, the effect of an electric fence made of hollow tile blocks or polyethylene net was examined in 3 villages on Amamioshima and Tokunoshima islands in the southernmost area of Japan. The electric pulses were supplied by a pulse maker at 0.75 sec. intervals with 480 mA, 0.13 m coulombs at a load of 500 ohms and estimated peak of 8 to 10 kV. The anode from a pulse maker was connected to a stainless steel plate or a metal string woven in a vinyl rope on the fence and the cathode was grounded. The power to the pulse maker was generated by silicon solar cells, stored in a car battery and regulated by an electric eye switch to activate it at night. The necessary height of the fence to prevent Habu from slithering over the fence was experimentally determined to be 60 cm. In 3 villages, 6.75 ha were enclosed with 1000 m fence, 13.7 ha with 2100 m and 23.8 ha with 2500 m, respectively. The effect of the electric fence was remarkable as determined by the annual monitoring of the number of snakes sighted by people and the captivity rate in box traps inside the fence. The number of snakes found annually in Tean Village was 10 before construction and 5, 4, 5 after construction; 119 before construction 55, 20, 20 after construction at Tete; and 109 and 82 before construction and 15 and 11 after construction at Todoroki. The snakes were observed to aggregate outside the fence. The net barrier could be constructed at any topographical conditions with less expense. The effect was further pronounced when snakes were removed by box trap and other methods from inside the fenced area.
