I-cell disease. A case report and review of the literature.

I-cell disease has been reported by many authors but the electron microscopic findings have been reported only rarely. The patient under study was a female infant with a normal delivery after 38 weeks' normal intrauterine life. She showed the physical findings characteristic of I-cell disease, and the diagnosis was made by the analyses of lysosomal enzymes. The child died at the age of 2 years and 3 months due to respiratory insufficiency. By electron microscopy, various-shaped membrane-bound vacuoles were observed in the cytoplasm of various cells such as hepatocytes, myocardial muscle cells, epithelial cells of the renal glomeruli, proximal renal tubular cells, fibroblasts, and chondrocytes. By histochemical analyses we found that these intracytoplasmic storage vacuoles contained glycosaminoglycan and proteoglycan.

Assignment of amides in the amino acid sequence of mammalian calmodulin by mass spectrometry.

The primary structure of mammalian calmodulin from various sources was analyzed by secondary ion mass spectrometry. The assignments of amide groups at residues 24, 60, 129 and 135, which had been controversial in the previous reports, were in perfect agreement with the amino acid sequence deduced from the cDNA sequence of calmodulin of other vertebrates. The result is against the post-translational amidation or deamidation process of this protein.

A single amino acid alteration in the initiation protein is responsible for the DNA overproduction phenotype of copy number mutants of plasmid R6K.

A novel type of high copy-number (cop) mutants of a mini-R6K plasmid were isolated. The mutations were mapped in the pir gene which encodes the pi initiation protein for plasmid R6K DNA replication. They resulted in an alteration by substitution of a single amino acid: threonine to isoleucine at the 108th position for the cop41, and proline to serine at the 113th position for the cop50, of the 305 amino acid pi protein. The cop41 mutation in the pi protein was found to be trans-dominant over the wild-type allele in the copy control of plasmid R6K. Moreover, it was shown that the altered pi protein was not overproduced in maxicells carrying this mutant plasmid and had a higher affinity to the repeated sequence which is present in the pir promoter region. Most likely the mutated pi protein also interacts more efficiently with the same repeated sequences, a target of pi, in the replication origin region and increases the frequency of the initiation event per cell division.

Cardiovascular profile of Bay K 8644, a presumed calcium channel activator, in the dog.

Bay k 8644, although belonging to dihydropyridines, has been reported to exert effects directionally opposite to those of dihydropyridine calcium channel blockers such as nifedipine. The present experiments were carried out to elucidate whether this is indeed true. All experiments were done on isolated, blood-perfused dog-heart preparations described below, and Bay k 8644 was administered intra-arterially. In sinoatrial (SA) node preparations Bay k 8644 increased sinus rate by about 37% of the basal value at the highest dose (10 micrograms). In atrioventricular (AV) node preparations Bay k 8644 decreased AV conduction time by accelerating AV nodal but not intraventricular conduction. However, the decrease in AV conduction time remained at about 10 ms (about 7.8% of the basal value) even at the highest dose (10 micrograms). In papillary muscle preparations driven at a fixed rate of 120 stimuli/min Bay k 8644 increased the force of contraction by about 100% of the basal value at the highest dose (10 micrograms). In spontaneously beating papillary muscle preparations Bay k 8644 failed to increase the rate of automaticity although it increased the force of contraction. In all preparations Bay k 8644 decreased (coronary) blood flow down to about 20% of the basal value at the highest dose (10 micrograms). The positive chronotropic and dromotropic effects of Bay k 8644 reached respective peaks nearly at the same time as its vasoconstrictor effect, whereas its positive inotropic effect reached a peak after the vasoconstrictor effect had nearly worn off.(ABSTRACT TRUNCATED AT 250 WORDS)

Nicardipine releases sustained coronary arterial constriction induced by acetylcholine in the rhesus monkey.

In five rhesus monkeys (Macaca mulatta) the vascular bed of the left anterior descending coronary artery (LAD) was perfused at constant pressure with autologous blood. Nicardipine (1-30 micrograms), a dihydropyridine calcium antagonist when injected into the LAD, increased LAD flow without producing significant changes in other cardiohemodynamic variables. With continuous infusions of acetylcholine into the LAD, the flow decreased by about 34% of the basal value. Under these conditions, intra-LAD nicardipine produced an increase in LAD flow almost to the same extent as in the control. The effects of nicardipine were qualitatively similar to those of other calcium antagonists observed in previous experiments and indicate that nicardipine would be useful as an antianginal drug.

Evaluation of phasic blood flow velocity in the great cardiac vein by a laser Doppler method.

In the present study, characteristics of the phasic flow pattern in the great cardiac vein and the mechanism of such pattern formation were investigated using a laser Doppler velocimeter with an optic fiber probe. The laser Doppler velocimeter allowed measurements of venous blood velocity under more physiological conditions than were possible with previous methods. Moreover, venous blood flow measurement in the great cardiac vein mirrors the effects of myocardial contraction on the venous flow more directly than does measurement in the coronary sinus. Thus, our method is considered very useful. Results obtained from the present study are as follows: 1) Measurement of the phasic flow in the great cardiac vein was made in 11 anesthetized dogs using our laser Doppler method. The blood velocity curve obtained in the great cardiac vein was always characterized by a prominent systolic flow wave (SFW). The mean value for the maximum velocities under control conditions in 11 cases was 40 +/- 13 cm/s. The blood velocity increased with the onset of left ventricular ejection and decreased gradually after the peak formation at mid- or late systole.--2) Besides the above SFW, one or two small wave components were frequently observed during the atrial contraction period and/or during the isovolumic contraction phase. On the waveform during the atrial contraction period, two cases showed forward flow, while one case showed reverse flow. The small reverse flow waves during the isovolumic contraction phase were found in seven cases.(ABSTRACT TRUNCATED AT 250 WORDS)

Computed tomography of a diploic epidermoid cyst of the skull.

The computed tomographic (CT) and other roentgenological characteristics of a skull diploic epidermoid cyst in a 14-year-old boy with Ito syndrome, a neurocutaneous syndrome, are presented. When this boy was 3 years 10 months of age, a plain roentgenogram of the skull showed an area of decreased density in the left parietal bone. The area has grown for the subsequent 10 years plus and a characteristic sclerotic margin has become evident. CT examination showed a cystic lesion in the diploe, the outer third of high density and the inner third low density. CT allowed prediction of the operative and histological findings. It was useful not only for the diagnosis but also in the pathogenesis.

Hypomelanosis of Ito associated with benign tumors and chromosomal abnormalities: a neurocutaneous syndrome.

A 14-year-old Japanese boy with hypomelanosis of Ito (HI) who has been followed up from the neonatal period is described. In addition to neurocutaneous symptoms consisting of intractable epilepsy, severe mental retardation, sensory neuropathy and cutaneous hypopigmentation, the following abnormalities were detected; a ventricular septal defect, a mature cystic teratoma in the posterior mediastinum, a diploic epidermoid cyst of the parietal bone and chromosomal abnormalities, i.e. mos, 45, XY, t(14q 21q)/46, XY, t(14q 21q)+mar. This disorder showed not only multisystem involvement but also tumor formation, which had not been reported previously. Because of this, it is better to use the term "Ito syndrome" instead of "HI" or "incontinentia pigmenti achromians (IPA)," which emphasize only the cutaneous symptoms.

Evaluation of the severity of hypoxanthine-guanine phosphoribosyltransferase deficiency using viable T cells.

Peripheral T cells from 3 Lesch-Nyhan patients, 3 normal subjects, and 3 brothers with hypoxanthine-guanine phosphoribosyltransferase (HGPRT) deficiency but without Lesch-Nyhan syndrome (so-called partial deficiency) have been analyzed. Although these brothers contained HGPRT activities neither in the hemolysates nor in the T cell extracts at levels detectable by the regular radioenzyme assay, the enzyme deficiency had not caused any typical neurological symptoms of the Lesch-Nyhan syndrome. Although the T cells from these brothers were at least 10-fold more resistant to 6-thioguanine than normal T cells, they were more than 30-fold less resistant than the T cells from 3 Lesch-Nyhan patients indicating that there is a clear difference in the severity of the enzyme deficiency between the brothers and the Lesch-Nyhan patients. These data indicate that the long-term T cell culture in the medium containing a purine analog whose toxicity depends on a salvaging enzyme is useful for evaluating the severity of the enzyme deficiency in viable cells.

Cytoplasmic estrogen receptors in carcinoma of the uterine cervix.

The concentration of cytoplasmic estrogen receptors (ER) in cancer of the uterine cervix was measured in 30 cases (28 squamous cell carcinomas and 2 adenocarcinomas). The mean ER concentration in squamous cell carcinoma was 19.3 +/- 26.0 fmol/mg cytosol protein; for pre- and postmenopausal women 6.82 +/- 9.86 and 28.6 +/- 30.1 fmol/mg protein, respectively, were found, the latter being significantly higher. When 'ER-positive' was defined as concentrations greater than 10 fmol/mg protein, 12 of the 28 cases (43%) were found to be ER-positive. There were no significant differences between the ER concentrations of clinical stage I and II squamous cell carcinomas (19.2 +/- 24.2 and 20.9 +/- 27.2 fmol/mg, respectively). ER were detectable in the cervical tissue from all of the control cases of myoma of the uterus. There were, however, no differences in the ER content between the proliferative and secretory phases, but the concentration in premenopausal women was significantly lower than that in postmenopausal women. In comparison with the controls, the mean ER level in both pre- and postmenopausal women with squamous cell carcinoma was significantly lower, due to the fact that ER were not detectable in 57% of these cases. Both cases of cervical adenocarcinoma were ER-positive.

Tissue and intracellular distribution of estradiol receptors in the uterine endometrium.

A study of the tissue and intracellular distribution of estradiol receptors was made of the uterine endometrium of the rat, using an immunofluorescence technique. It was found that nearly all cells of the uterine gland have a uniform distribution of receptors, which does not change throughout the sexual cycle from proestrus to estrus. In contrast, a dense pattern of distribution of estradiol receptors was seen in the endometrial stroma during proestrus, whereas a diffuse pattern was seen during estrus. Intracellularly, the transition from the cytoplasm to the nucleus of estradiol reacting to receptors was found to be positively correlated with the incubation temperature and the length of incubation.

Concurrent endometrial hyperplasia with carcinoma of the corpus uteri.

Using 117 uteri which were extirpated in cases of carcinoma of the corpus uteri, a study was made to investigate the relationship between the site of the cancer and the site of associated endometrial hyperplasia. It was found that 75% of the cases had carcinomas of the upper portion of the uterus primarily of the uterine fundus. About 50% of the cancers were associated with adenomatous or atypical hyperplasia, and these two types of hyperplasia were frequently found in the fundus or upper portion of the uterus. In contrast, there was no site of particular prevalence in cases with concurrent cystic hyperplasia. About 70% of the cases had adenomatous or atypical hyperplasia on one or both of the histological blocks adjacent to the cancer.

Immunohistochemical localization of carcinoembryonic antigen in adenocarcinoma of the endocervix and the endometrium.

Immunoperoxidase localization of carcinoembryonic antigen (CEA) was performed on tissue sections of adenocarcinoma of endocervical and endometrial origin to clarify the criteria for their differentiation. Twenty-two (96%) of 23 endocervical and 32 (70%) of 46 endometrial adenocarcinomas revealed positive CEA immunostain. The extent of staining did not seem to correlate with the degree of differentiation in either kind of cancer. Among the positive cases, CEA staining was located over whole cytoplasm in 18 (80%) of 22 positive endocervical adenocarcinomas and on apical surface in 24 (75%) of 32 positive endometrial adenocarcinomas. The pattern of CEA distribution in endocervical adenocarcinoma of endometrioid type was not similar to that observed for primary endometrial adenocarcinoma.
