Different clinicopathologic findings in two histologic types of carcinoma of papilla of Vater.

The aim of this study was to investigate the differences between the clinicopathological findings in two histologic types of carcinoma of the papilla of Vater. We histologically classified carcinoma of the papilla into two types: 1) an intestinal type that resembles tubular adenocarcinoma of the stomach or colon, and 2) a pancreaticobiliary type that is characterized by papillary projections with scant fibrous cores. We examined 53 cases of resected carcinoma of the papilla. The intestinal-type carcinomas were similar to the intestinal mucosa in that they had lysozyme-containing, Paneth or argyrophil cells, as demonstrated by the immunohistochemically positive stainings for the anti-lysozyme antibody. Although both the sizes of the two types of carcinomas and the age distributions of cases with the two types of carcinoma were almost the same, the prognosis of the cases with the intestinal type was much better than that of the cases with the pancreaticobiliary type. Histological lymph node metastasis was found significantly more often in the pancreaticobiliary type. This result was supported by the fact that small carcinomas of the intestinal type showed little or no invasion into the surrounding interstitium, as opposed to the pancreaticobiliary type, which had a strong infiltrative tendency. The pathogenesis of carcinoma of the papilla of Vater should be further evaluated, taking into consideration the existence of these two histologic types.

Cerebral palsy of cystic periventricular leukomalacia in low-birth-weight infants.

We studied ultrasound findings and neurodevelopmental outcome of 24 infants weighing 2500 g or less with cystic periventricular leukomalacia. Fourteen infants had symmetrical cysts in the parietal or occipital region (group 1) and 10 had non-symmetrical cysts (group 2). Each infant was followed for more than 4 years of age (mean 5 years and 7 months). Twenty out of 24 (83.3%) children developed cerebral palsy. All of group 1 had cerebral palsy (8 diplegia and 6 ataxic diplegia), while 6 (60%) in group 2 developed cerebral palsy (4 diplegia and 2 hemiplegia). There was a significant difference in the incidence of cerebral palsy and motor ability between the two groups. The size and site of the cyst did not predict cerebral palsy. The presence of symmetrical cysts in the parietal or occipital region is a highly reliable neurosonographic finding for predicting cerebral palsy.

Importance of amino acid 101 within capsid protein VP1 for modulation of Theiler's virus-induced disease.

We constructed a Theiler's virus mutant designated DA3304, in which the amino acid at position 101 of VP1 was changed from a threonine to an alanine. Because of this single amino acid change, DA3304 could still produce a biphasic central nervous system disease similar to that produced by the wild-type DA virus. However, DA3304 was significantly attenuated in both the acute and the chronic phases and induced smaller demyelinating lesions than the wild-type DA virus. The data are most compatible with the attenuated phenotype in DA3304 being due to the change of binding efficiency between the virus and receptor resulting from the physical alteration at the mutation site.

Gender differences in quantitative EEG at rest and during photic stimulation in normal young adults.

In the present study quantitative EEG analysis was performed in 20 male and 20 female healthy adults in order to examine the gender differences in EEG activity at rest and during photic stimulation. The females generally showed a higher amplitude in the resting EEG than the males, with significant differences observed for the delta, theta, alpha 2 and beta bands at the limited electrode sites. The gender differences were more pronounced in EEG activity during photic stimulation, and the females had a higher EEG amplitude in the frequency band identical or harmonically related to the stimulus frequency. These findings provide further evidence that the gender differences exist in EEG activity in both stimulus and nonstimulus conditions.

Biosynthesis of erythromycin: origin of the methyl protons.

13C-Nuclear magnetic resonance (13C-NMR) spectroscopy has been used to locate deuterium atoms incorporated biosynthetically into erythromycin from [3-13CD3]- and [3-13C]sodium propionate and L-[13CD3]- and L-[13C]methionine in Saccharopolyspora erythraea JCM 4748. The alpha-shifts of deuterated methyl groups were clearly observed in broad-band deuterium and proton-decoupled 13C-NMR spectra. The seven propionate-derived methyl groups and the four methionine-derived methyl groups were shown to undergo no exchange of methyl protons during the biosynthesis of erythromycin.

Natural infection with attaching and effacing Escherichia coli (O15) in an adult cow.

A 19-month-old Holstein female manifested muco-hemorrhagic diarrhea. At necropsy, the colon contained a large amount of bloody material mixed with mucus and there was diffuse petechial hemorrhage on the mucosal surface. Histologic and electron microscopic examination of the colon revealed the characteristic lesion of attaching and effacing Escherichia coli (AEEC) infection. The organisms were associated with mucosal lesions and stained positively by an immunoperoxidase technique using antiserum to E. coli O group 15. Diarrhea caused by AEEC was recognized in this adult cow.

Telescience testbed experiments of intracellular recordings in the Xenopus oocyte.

The feasibility of intracellular recordings under constraints for experimental conditions in outer space were tested at a telescience testbed of the National Space Development Agency of Japan. We attempted to study the dose-response relationship of adenosine-induced potential changes in the Xenopus oocyte. The testbed simulated the distance from a ground control room to oocytes in an orbital laboratory; signals were delayed and compressed from one "station" to other. A microelectrode was inserted into the oocyte using remote control on the manipulator and on the xyz-planes of the platform with stereoscopic pictures observed through a head mounted display. When the transfer rate of the visual signals was decreased from 1.5 Mbps to 128 Kbps, insertion of the electrode became almost impossible because of reduced picture quality. Once the electrode was inserted, however, dose-dependent adenosine responses could be observed with little trouble.

Delayed appearance of the catalytic activity by immunization of a rabbit compared with the hapten binding.

Polyclonal antibodies catalyzing the hydrolysis of carbonate ester were generated by immunizing a rabbit with hapten(4-nitrophenyl phosphate II) conjugated to keyhole limpet hemocyanin. The hydrolytic activity of IgG purified from antisera exhibited plateu one month later than the simple hapten-binding. The affinity of IgG with substrate increased even after the hapten-binding reached plateu. These suggest a strategy to generate good polyclonal catalytic antibodies and the day to fuse spleen cell with myeloma cell to get good monoclonal catalytic antibodies.

[Renal tubular dysfunction in open pediatric heart surgery--preventive effect of haptoglobin].

Preventive effect of haptoglobin (Hb) for renal tubular dysfunction was studied in 16 pediatric patients undergoing open heart surgery under extracorporeal circulation (ECC). The patients were divided into two groups; Hp(+) group consisting of 7 patients administered haptoglobin (100 U/kg) in priming of ECC, and Hp(-) group of 9 patients not administered haptoglobin. As parameters of renal tubular function, urinary alpha 1-microglobulin-index (U-alpha 1m.I), urinary beta 2-microglobulin-index (U-beta 2m.I), urinary N-acetyl-beta-D-glucosaminidase-index (U-NAG.I), fractional clearance of alpha 1m (Fc-alpha 1m) and beta 2m (Fc-beta 2m), as well as Free-Hb, were measured just before start of ECC and then at periodical time after ECC to the 7th day. The results obtained are as follows. (1) Maximum Free-Hb in Hp(+) group was significantly less (p < 0.05) than that in Hp(-) group. (2) Every parameter of renal tubular function were shown a peak at the end of ECC. Maximum values of U-alpha 1m.I, U-NAG.I and Fc-alpha 1m in Hp(+) group were significantly less (p < 0.05) than those in Hp(-) group. Maximum U-beta 2m.I and Fc-beta 2m in Hp(+) group were less than those in Hp(-) group, although there was no significant difference. These results indicate that administration of haptoglobin suppresses the increase of Free-Hb and is useful for prevention of renal tubular dysfunction during ECC.

Allotransplantation of cryopreserved menisci in monkey: histological change.

The purpose of this study was to carry out a histological evaluation of cryopreserved menisci used as allografts in monkeys. Menisci were evaluated histologically at one, three, six and at twelve months after transplantation. In the deep layer of the allografts, the number of cells and the amount of the extracellular matrix had decreased at three to six months after transplantation. After twelve months, the evaluation revealed that both the cell counts and the extracellular matrix had increased returning to nearly normal levels. However, the colour of the matrix stained with Masson's trichrome had changed to red. This change may correspond with the occurrence that of young newly-formed collagen. Since the articular cartilage showed minimal degenerative change, it is concluded that meniscal transplantation is useful and should be considered for meniscal injuries.

[Left ventricular-right atrial shunt with bacterial and rheumatic endocarditis of tricuspid valve].

A 14-year-old boy who was operated on for left ventricular-right atrial shunt with bacterial and rheumatic endocarditis of the tricuspid valve (TV) is reported. The patient was pointed out perimembranous ventricular septal defect (VSD) at 4-month old and had complaint of occasional high fever for a half year. The echocardiography showed a vegetation at the TV. The vegetation did not disappear despite of administration of antibiotics for 2 weeks, and pulmonary embolism occurred. Then, the patient underwent resection of the vegetation and direct closure of VSD. The left ventricular-right atrial shunt was formed by the direct communication between VSD below TV annulus and the perforation of septal leaflet of TV adhered to the ventricular septum due to bacterial and rheumatic endocarditis. Earlier operation is needed for this type of case because antibiotics is not effective for the valvular disease.

Long-term results of trabeculotomy ab externo.

We retrospectively reviewed the records of patients followed for over 4 years after undergoing trabeculotomy to treat primary open-angle glaucoma (POAG) to evaluate the long-term effects of the procedure on intraocular pressure (IOP) control. IOP remained below 15 mm Hg, with no need for postoperative medication in 7 (13.2%) of 53 eyes. IOP was below 15 mm Hg in an additional eight eyes (15.1%) treated with topical medication. Topical medication was needed to maintain IOP between 16 to 20 mm Hg in 23 eyes (43.4%). Oral acetazolamide or additional surgery was needed in 15 (28.3%). A postoperative IOP below 20 mm Hg with or without topical medication was associated with a survival probability of 71.2%. Our results demonstrated that, although trabeculotomy largely failed to maintain an "ideal" postoperative IOP (below 15 mm Hg), it was not associated with severe operative or postoperative complications such as flat anterior chamber. Thus, the procedure probably should be the first choice not only in uncomplicated cases of congenital glaucoma and pseudoexfoliation glaucoma (as previously established), but also in cases of POAG in young patients with only slight damage to the optic disc or visual field and cases with forward movement of the iris-lens diaphragm.

[Quantitative analysis of EEG background activity in drug-naive schizophrenic patients].

In this study we performed quantitative EEG analyses in 12 drug-naive patients who fulfilled DSM-IIIR criteria for schizophrenia and in a group of 12 healthy subjects. The schizophrenic patients were found to have slower activity (delta and theta) than the controls. These differences were most marked in the frontal region for delta band and in the occipital region for theta band. The schizophrenic patients also had more beta 1 activity, particularly in the occipital leads. Conversely, they had less alpha 2 activity over all regions examined. Our findings indicate cerebral dysfunction in schizophrenia and are in line with the hypothesis of hypofrontality in this disorder.

[Differential diagnosis between osseous metastasis and degenerative joint disease of the vertebrae by bone SPECT: analysis by accumulation pattern].

To find whether or not bone SPECT can differentiate osseous metastasis from degenerative joint disease (DJD) of the vertebrae, 43 patients with increased vertebral uptake on bone scan, including 25 lesions with bone metastasis, 24 with DJD, and 4 with compression fractures due to osteoporosis, underwent bone planar scanning and SPECT. Increased accumulation in the vertebral lesions on bone SPECT transaxial images was classified into five accumulation patterns; mosaic, large hot, diffuse, peripheral and articular pattern. Mosaic, large hot and diffuse patterns were more frequently noted in patients with osseous metastasis (82%, 67% and 62%, respectively). On the other hand, 80% of the lesions with peripheral pattern and 70% of those with articular pattern were ascribed to DJD. In conclusion, bone SPECT provided much better anatomic information on the extent of 99mTc-MDP. Differential diagnosis between osseous metastasis and DJD of the vertebrae may be improved by bone SPECT.
