[Individualized chemotherapy of adult acute nonlymphocytic leukemia].

The objective of the current study is to clarify the concept and to demonstrate the encouraging results of individualized chemotherapy for adult acute nonlymphocytic leukemia. Settlement of target point for remission induction and individual adjustment of the treatment schedules according to response to therapy is fundamental for the treatment strategy. Since 1982, 37 consecutive adults with newly diagnosed acute nonlymphocytic leukemia were treated with individualized chemotherapy using intensive DCMP sequential multi-step therapy. Eighty-four percent of patients entered CR. Actuarial remission duration at greater than 4 years was 63.4 +/- 10.2% (95% confidence interval).

[Combination chemotherapy of multiple myeloma].

Seventy patients with progressive multiple myeloma received combination chemotherapy with cyclophosphamide and prednisolone (CP), BCNU, cyclophosphamide, procarbazine and prednisolone (BCPP), and MCNU, cyclophosphamide, melphalan and prednisolone (MCMP) as first line treatment. Total remission rate in patients treated with CP, BCPP, and MCMP was 76.2%, 86.1%, and 91%, and complete response rate 26.1%, 33.3% and 63.7%, respectively, 5-yr survival in the patients treated with CP and BCPP regimen was 51.9 +/- 11.1%, 39.7 +/- 8.9%, respectively, however, the difference was not significant, 1-yr survival in the patients treated with MCMP was 91 +/- 8.7%. It was postulated that long-term survival or cure can only be anticipated if the treatments giving high CR rates was developed. The study, though preliminary, supports the notion that MCMP therapy should be used as primary standard treatment for patients with multiple myeloma.

[Multi-agent combination chemotherapy for adult acute lymphoblastic leukemia--individualized chemotherapy].

Eighteen adults with acute lymphoblastic leukemia (ALL) including 14 previously untreated and 4 relapsed cases were treated with individualized and intensified induction and post-remission therapy. CNS prophylaxis was performed with i. th. methotrexate alone and started early during induction. All patients achieved complete remission. In a median follow up of 65 weeks, 2 yr-disease free survival (DFS) rate was 73.5 +/- 13.0% in previously untreated patients. This treatment program has resulted in markedly improved remission rate and DFS in adults with ALL.

The establishment of a clinical diagnostic method of the visual evoked potentials (VEPs) in the cat: the effects of recording electrode positions, stimulus intensity and the level of anesthesia.

The establishment of characteristic wave pattern of visual evoked potentials (VEPs) in the cat was attempted in order to be aid for clinical veterinary practitioners with the evaluation of visual dysfunction. 1) The position where the largest response of the VEPs was detected was close to the midline of the occipital area in normal cats. 2) The VEPs consisted of three components with latencies within 100 msec after flash stimulus. 3) Flash stimuli of higher illumination produced VEPs of shorter latencies and increased amplitude. Intensity of more than 0.6 J was necessary to obtain stable VEP patterns. 4) The latencies of VEPs hardly changed with time course under pentobarbital anesthesia, although they showed a fluctuant variation.

[The curative treatment of adult acute nonlymphocytic leukemia].

Based on bone marrow findings and bone marrow stem cell kinetics and response to treatment, we have developed individualization of intensive induction and postremission chemotherapy for adult acute nonlymphocytic leukemia (ANLL). Thirty-four consecutive adults with ANLL were treated with an intensified induction regimen and a high dose sequential postremission therapy consisting of daunomycin, Ara-C, 6-MP and prednisolone (DCMP). The first course of remission induction was continued till achievement of a complete marrow aplasia which resulted in a decrease of leukocyte count less than 0.6-0.8 X 10(9)/L, a decrease of marrow nucleated cell count to less than 8 X 10(9)/L, and a decrease of marrow leukemic cell to less than 5%. Postremission therapy consisted of 4 courses of DCMP and a course of high-dose Ara-C. The first postremission course was initiated within 2-3 weeks subsequent to the last induction course. Twenty-eight of 34 patients (82.4%) achieved complete remission. The 4 year disease free survival rate was 64.4 +/- 14.0%. The results convinced us that individualized intensive induction and postremission therapy of adult ANLL given at the time of minimal residual leukemic disease in early remission might be sufficiently effective to produce long-term DFS to be considered potential cured.

Recent trends in the treatment and prognosis of adult leukemia with characteristics of patients in Japan: transition during the fifteen years from 1971 to 1985.

Patients with acute (2,569) and chronic (957) leukemia diagnosed at 19 institutes took part in the study on the "Multidisciplinary Treatment of Leukemia" between 1971 and 1985 and were investigated retrospectively. By dividing the 15 years into three five-year periods, we were able to compare patient ratios in the different periods. The proportions of acute to chronic leukemia cases showed no obvious change; however, the proportions of cases diagnosed as acute lymphocytic leukemia in acute leukemia showed a significant increase. The main chemotherapeutic drugs used during the three time periods were cytarabine or its analogues, the anthracyclines, 6-mercaputopurine and prednisolone, against acute myelogenous leukemia, and the vinca alkaloids, prednisolone and the anthracyclines against acute lymphocytic leukemia. The rate of complete remission from acute myelogenous leukemia made marked progress, from 45.1% during 1971-1975 to 62.3% during 1981-1985, but that of acute lymphocytic leukemia showed no significant progress, being 65% during 1971-1975 and 69.7% during 1981-1985. The durations of remission, however, and the survival times for patients with acute lymphocytic leukemia, as well as for those with acute myelogenous leukemia, became significantly longer over the three periods. Median survival times from chronic myelocytic leukemia were 37-40 mo in all three periods, showing no progress. There was a better prognosis in cases of chronic myelocytic leukemia with, than without, Philadelphia chromosome. Except for a low incidence of chronic lymphocytic leukemia in Japan, adult leukemia patients' characteristics and prognoses seem to be almost the same in Japan as in the U.S.A. and Europe.

[Phase I study and early phase II of SM-108 (4-carbamoylimidazolium-5-olate) in lung cancer. SM-108 Study Group].

Phase I study with a new oral purine antagonist, SM-108, was conducted in a total of 73 cases in a 5-day consecutive schedule with the dosage ranging from 20-2,500 mg/m2 at II institutes. The incidences of subjective and objective side effects were 20.5% (15/73) and 31.5% (23/73), respectively, however the correlation between these effects and their dosages was unclear. MTD and DLF values were not determined. SM-108 levels in serum reached maximum in 2-4 hours after oral administration of SM-108, and exhibited a dose-response relationship up to the dosage of 2,000 mg/m2/day. Forty to 60% of the administered drug was excreted into urine in 24 hrs; thus, the main excretory pathway was considered to be renal. An early phase II study was undertaken in patients with lung cancer in 5 cancer centers and university hospitals. Each patient had received 400 mg/m2/day (in two divided doses) or 600 mg/m2/day (in three divided doses) for more than 2 weeks. Only one case (adenocarcinoma) showed a minor response out of 27 cases but not reached partial response according to our criteria. In the 400 mg/m2/day group and the 600 m/m2/day group the incidences of subjective side effects (mainly GI tract disturbance) were 33.3% (4/12) and 40.0% (6/15), while objective side effects (hematological changes) were 16.7% (2/12) and 26.7% (4/15) respectively. In conclusion, we could not determine the dose limiting factor and maximum tolerated dose from our phase I clinical study and early phase II study for lung cancer.

[Phase II study of SM-108 (4-carbamoylimidazolium-5-olate) in hematological malignancies].

A total of 165 patients were entered into this study and 140 were evaluate for effects and 165 for toxicities. Of 39 patients with chronic myelogenous leukemia (CML) 21 achieved complete remission (CR), 6 achieved partial remission (PR) with a response rate of 69.2%. In MDS, of 11 patients with chronic myelomonocytic leukemia (CMMoL), one good partial response and 4 partial response were observed (CR + PR:45.5%); of 14 patients with RAEB, one complete response, 4 partial response (CR + PR: 35.7%); of 11 patients with RAEB in T, 3 partial response were observed (response rate: 27.3%). Of 13 patients with polycythemia vera, 4 excellent effect and 7 moderate effect (84.6%) were observed. Seven of 30 patients with acute myelocytic leukemia achieved partial response (23.3%). Mean dosages of SM-108 until remission were 400-500 mg/m2/day on CMMoL, RAEB in MDS, polycythemia vera and CML, and 600-800 mg/m2/day on RAEB in T and AML. In the analysis of adverse effects of SM-108, a subjective side effects including mainly gastrointestinal toxicities were observed in 38 cases (23.0%) of the patients : 26 patients (15.8%) showed objective side effects including liver dysfunction, but these symptoms were transient and not serious. Our study indicates that SM-108 is useful agent against MDS, especially CMMoL, RAEB, RAEB in T, polycythemia vera and CML.

[Combination chemotherapy of multiple myeloma--BCNU.cyclophosphamide.procarbazine.prednisolone and MCNU.cyclophosphamide.procarbazine.prednisolone therapy].

Forty-four patients with progressive symptomatic multiple myeloma were treated with a protocol combining cyclophosphamide (10 mg/kg weekly), procarbazine (2 mg/kg daily), prednisolone (20 mg daily), and either BCNU (1 mg/kg weekly) (BCPP protocol) or MCNU (0.8-1 mg/kg) (MCPP protocol). Of these, 34 patients were treated with the former, and 10 patients with the latter. With BCPP protocol, 12 achieved a complete response, 11 evidenced a 75% response, and 7 displayed a 50% response. With MCPP protocol, on the other hand 2 achieved a complete response, 6 showed a 75% response, and 1 exhibited a 50% response. The median tumor halving time in both groups was 77 days and 57 days respectively. The 5-yr disease-free survival of patients treated with BCPP protocol was 62.0 +/- 10.8%. In MCPP group, 8 of 10 patients are alive with more than 49-87 weeks survival. Although myelotoxicity was moderate, other toxicities were moderate. Toxicity requiring dose modification and discontinuation of the scheduled therapy was observed.

[Neutropenic enterocolitis in adults with acute leukemia].

Neutropenic enterocolitis, also known as typhlitis or ileocecal syndrome, in leukemic patients undergoing chemotherapy has a high reported mortality. A recent increase in the incidence of neutropenic enterocolitis is associated with aggressive chemotherapy for acute leukemia. In this report, we report the incidence, diagnosis, and treatment of neutropenic enterocolitis during low-dose DCMP and high-dose DCMP regimen for 95 adults with acute nonlymphocytic leukemia, and review the literature pertaining appropriate medical and surgical management, and method of prevention. Finally we propose the favorable results of gut sterilization for the treatment and prevention of the disease.

[Bestatin treatment of patients with myelodysplastic syndromes].

Seven patients with myelodysplastic syndromes (M-DS) were treated with 30 mg Bestatin daily. Six of the patients (85.7%) responded. Clonogenic bone marrow cell culture studies in the patients have demonstrated intrinsic hemopoietic stem cell and progenitor cell abnormalities. After Bestatin treatment, these abnormalities involved in differentiation and maturation of hematopoietic progenitors were markedly improved and resulted in the improvement of hematological findings in responders. Addition to the culture system of Bestatin at a concentration of 0.01 microgram/ml and 1 microgram/ml enhanced CD4 positive T cell colony formation without enhancing CFU-GM and CFU-GEMM. These results suggest that the effects on hematopoiesis of Bestatin may be mediated by T cells, not by direct action of the drug.

[Treatment of myelodysplastic syndrome using SM-108 in relation to stem cell kinetics].

Eighteen patients in a high-blast group of myelodysplastic syndrome were treated with low-dose SM-108 (4-carbamoylimidazolium 5-olate). Seven of the 18 patients (38.9%) achieved a partial response, and 4 (22.2%) achieved complete response. Treatment was associated with significant, but transient myelotoxicity. Changes in CFU-GEMM clonal growth were examined in bone marrow from patients during the treatment.

Immunotherapy with bestatin for acute nonlymphocytic leukemia in adults.

In a cooperative randomized control study of immunotherapy with bestatin in combination with chemotherapy in adults with acute nonlymphocytic leukemia (ANLL), 101 patients (48 in the bestatin group and 53 in the control group) out of 115 patients registered were evaluated as eligible. The bestatin group achieved a statistically significant prolongation of survival compared with the control group in overall ANLL and acute myelogenous leukemia. In the analysis of patient age, the bestatin group achieved a statistically significant prolongation of both the remission duration and survival in patients aged 50 to 65 years, while the differences were not significant in the 15 to 49 age group. The bestatin group tended to achieve a higher rate of reinduction of remission in patients who had recurrence of leukemia. Side effects developed in only 5 (9.6%) of 52 patients treated with bestatin. None of these side effects were particularly serious in nature. It is concluded that bestatin is useful for prolongation of survival of adult patients with ANLL, making for a longer remission duration especially in elderly patients and with few side effects.

The treatment of chronic myelogenous leukemia in blastic crisis with the chemotherapy incorporating vindesine-prednisolone.

Twenty-three patients with chronic myelogenous leukemia (CML) in terminal transformation were treated with regimen incorporating vindesine and prednisolone with or without cytosine arabinoside. Twenty-one patients in blastic crisis (BC) were classified into subgroups on the blast cell morphology. Of these 21 patients, 5 (23.8%) had lymphoid morphology, 12 (57.2%) had myeloid morphology, and 4 (19.0%) had monocytoid morphology. All of five patients with lymphoid morphology and ten of twelve patients with myeloid morphology achieved complete remission (83.3%), while none of four patients with monocytoid morphology achieved complete remission. Two patients in accelerated phase also attained complete remission. Our results indicate that the regimens incorporating vindesine and prednisolone with or without cytosine arabinoside should be the treatment of choice in CML-BC both lymphoid and myeloid morphology and accelerated phase of CML as well.
