Wrap-around cartilage flap for correction of unilateral cleft lip nose deformity.

A turnover contralateral cephalic alar cartilage flap is suggested for the correction of cleft nose alar deformity. The cartilage flap from the normal side wraps around the deficient medial crus and acting as a pulley reposition and augments the affected alar cartilage. The satisfactory and long-lasting results obtained in our small series seem to justify this approach for improving cleft nasal deformities, and we think it should be available in our surgical armamentarium.

Insulin-induced lipohypertrophy treated by liposuction.

Lipohypertrophy is one of the local side effects of repeated subcutaneous insulin injections. This case report demonstrates the successful cosmetic and therapeutic treatment of two lipohypertrophic masses in the lower abdomen of a young diabetic patient by suction-assisted lipectomy.

Closure of an open high below-knee guillotine amputation wound using a skin-stretching device.

A skin-stretching device (SSD) which is designed to harness the viscoelastic properties of the skin using incremental traction is described. It has proved to be of value in securing closure, within 72 h, of an open high below-knee guillotine stump wound which did not heal during a period of 2 months after amputation. The skin-stretching device provided healthy, durable and sensate skin for stump coverage. Hospitalization time was shortened and an early post-surgical prosthetic fitting and ambulation was achieved.

Traumatic self-inflicted bite with embedded tooth fragments in the lower lip.

An unusual case of traumatic self-inflicted bite of the lower lip in a 7-year-old girl is presented. As a result of the bite, due to a fall, four fragments of the right upper incisor tooth were embedded in the lower lip. The large tooth fragment appeared 18 days later, protruding through the skin of the lower lip. Radiographs confirmed the presence of these tooth pieces, which were removed under local anesthesia. Early diagnosis and surgical removal of the tooth fragments could prevent undesirable foreign-body reactions and scarring.

The use of CO2 laser for the treatment of xanthelasma palpebrarum.

CO2 laser has been used for the treatment of 22 patients with xanthelasma palpebrarum. This office procedure done under local anesthesia is fast and easy. The lipomatous yellow plaques are accurately photovaporized in a bloodless field, layer by layer, until the lesion is totally eradicated. The treated area heals by reepithelization, ostensibly with virtually no scar contraction. All our patients were satisfied with their results, rating them from good to excellent. In 5 patients some pigmentation changes were noted. During follow-up periods varying from 15 to 54 months, only 2 (9%) patients had recurrences, which were retreated successfully. We postulate that treatment of xanthelasma palpebrarum by CO2 laser should be seriously considered in most patients.

Dermatofibrosarcoma protuberans appearing during pregnancy.

Dermatofibrosarcoma protuberans is an uncommon skin tumor with a high potential for local recurrence if not adequately excised. There are only two reports in the literature describing dermatofibrosarcoma protuberans that enlarged considerably during pregnancy. On the other hand, the new development or enlargement of dermatofibromas in pregnancy is well documented. We present 2 additional patients in whom a dermatofibrosarcoma protuberans appeared and grew rapidly during pregnancy. Immunohistochemical studies were negative for estrogen and progesterone receptors. The possible pathophysiology and surgical management are presented and discussed.

Nipple-areola reconstruction with a custom-made silicone ectoprosthesis.

The use of a custom-made silicone ectoprosthesis of the nipple-areola complex is presented. It is suggested, after completion of breast mound reconstruction, for those women who cannot undergo or refuse to undergo any further surgical procedure, or else for the interim until the final reconstruction is performed. It is convenient to use and the aesthetic appearance is pleasing.

Necrotizing periorbital cellulitis following septorhinoplasty.

The complication of infection after a septorhinoplasty is extremely rare. An unusual occurrence of necrotizing periorbital cellulitis after a routine septorhinoplasty in a 30-year-old pediatrician is presented. It consisted of marked swelling and superficial necrosis of the left eyelids and high fever that started one day after surgery. beta Hemolytic streptococcus was identified as the causative pathogen. The patient responded well to systemic antibiotics and conservative local treatment. After three years of followup the cosmetic appearance is reasonable.

Postirradiation extensive cicatricial scalp alopecia treated by tissue expansion.

The scalp of a 20-year-old woman with extensive cicatricial alopecia caused by x-ray irradiation for tinea capitis has been reconstructed by the use of tissue expansion. Two stages were required to reconstruct the large defect. The hair-bearing posterior scalp has been expanded to cover the whole irradiated area, creating a new anterior hairline and a pleasing cosmetic result.

[Is Solcoderm suitable for treating malignant skin tumors?].

Solcoderm, a liquid preparation developed in the USSR, is recommended for topical treatment of benign and malignant skin tumors. We present 5 out of 25 patients treated in our department over the past 2 years who had been treated with Solcoderm for malignant skin tumors. The preparation appeared to have affected only the superficial layers of the tumor and had not penetrated in depth. Cellular remnants tended to spread far beyond the confines of the original tumor and seemed to become much more aggressive. To be effective, the technique of applying the drug would have to be much more complicated than originally specified. Both the functional and esthetic results are much worse than expected.

Chemical peeling: an alternative to rhytidoplasty.

Phenol chemical peeling is a well known ancillary procedure for facial rejuvenation. It may be used as an adjuvant to rhytidoplasty or may be an alternative in selected patients or in patients who refuse to undergo both procedures and prefer a single stage and scarless procedure. The results are very gratifying and long lasting.

Prophylactic antibiotics in aesthetic and reconstructive surgery.

The widespread use of antibacterial agents for prophylaxis has altered surgical practice markedly in the past 20 years and now represents one of the most frequent uses of antibiotics in hospitals, accounting for as many as half of all antibiotics prescribed. The present study was undertaken to determine the patterns of and reasons for antibacterial agent usage by today's practicing plastic surgeons in Israel. A survey of 78 Israeli plastic surgeons certified by the Israeli Association of Plastic Surgery was conducted. Questionnaires were mailed to all the certified plastic surgeons; 66 (84.6%) were completed and returned. The results indicate that prophylactic antibiotics are administered in a high percentage of aesthetic and reconstructive surgeries except for those in eyelids and nose. Most of the surgeons prefer to prescribe cephalosporins as their antibiotics of choice and the timing of administration is before or at the beginning of surgery. Placement of drains did not change the decision to use prophylaxis. Diabetes mellitus, steroid treatment, and chronic lung disease have a direct influence on the decision to use antibiotics; however, conditions such as ischemic heart disease, hypertension, and cigarette smoking do not influence their use. Personal experience was the main reason for prescribing antimicrobial agents. We conclude that although the infection rate in aesthetic and reconstructive operations is extremely low, most Israeli plastic surgeons still prefer to administer prophylactic antibiotics though no scientific hard data is available.

In favor of the subcutaneous forehead lift using the anterior hairline incision.

We present our experience with 980 women who had subcutaneous forehead lifts using the anterior hairline incision, during the years 1989-1996. The dissection is easy and fast; the forehead wrinkles are smoothed by the separation of the septa between the frontalis and the skin. The access to the corrugator and the procerus muscles is easy, and the adjustment of the brows to the desired location can be accurate. We use this approach for 90% of women who are eligible for upper face rejuvenation. We have obtained a 96% satisfaction rate; only 1.8% of our patients had minor and reversible complications. The scar, which is supposed to be the main disadvantage of this procedure, is almost unnoticeable, and none of our patients has permanently changed her hairstyle due to this operation. Now, during the peak of interest in forehead lifts with limited scars using the endoscope, is the time to highlight this time-worn, safe, reproducible, and effective approach.
