T cell repertoire selection in T cell receptor transgenic mice.

T cell receptor (TCR) gene segments begin to rearrange in CD4-8-thymic lymphoblasts. In severe combined immune deficiency (scid) mice the development of T cells is arrested at this early stage as the scid thymus does not contain any CD4+ or CD8+ lymphocytes. This block in T cell development can be overcome by introducing productively rearranged TCR genes into the scid strain which results in the formation of CD4+8+ lymphocytes. While this early differentiation step requires TCR's of any specificity, later developmental stages depend on the specificity of the TCR: in scid mice, a transgenic TCR restricted by Db class I major histocompatibility complex (MHC) antigens allows the formation of CD4-8+ but not CD4+8- lymphocytes in Db positive but not Db negative animals. Thus, a TCR-MHC interaction in the absence of nominal antigen is required for the generation of mature T cells, and this interaction determines the CD4/CD8 phenotype. If both nominal antigen and presenting MHC antigen are present developing T cells are deleted at an immature CD4+8+ stage preventing the formation of more mature and functional autoaggressive T cell progeny. These experiments indicate that the immune system first learns about self by positive and negative selection of self recognizing lymphocytes from a continuously turning over pool of lymphocytes without requirement for idiotypic network interactions.

[Electron microscopic observations of blood vessels in ependymoma].

The findings of an ultrastructural study of the blood vessels and their surrounding structures in ependymoma are demonstrated. Six surgically removed cases of ependymoma are investigated. The lumen of the blood vessels, especially in vascular rosettes is often wider than those of normal brain capillaries. The endothelial cells are structurally varied, but sometimes attenuated in many areas. Also, they often have several fenestrations. Moreover, they have increased pinocytotic vesicles and filaments, and contain occasional Weibel-Palade bodies, multivesicular bodies and other cytoplasmic organelles. The perivascular space is often wide, especially in intraspinal ependymoma, and filled with multiple layers of basal laminae permeated by electron, dense fluids. There are also collagenous fibers and Luse bodies. Basal lamina covering tumor cell surfaces is more distinct than other basal laminae. The tumor cells facing the blood vessel interestingly show structures similar to those of astrocytic vascular feet. The fenestration in the endothelial cells probably has an important role in the increased vascular permeability of ependymoma.

[Growing skull fracture with rapid growth: a case report].

A case of growing skull fracture with rapid growth is presented. A 4-month-old male fell and struck the right side of his head. Skull X-ray just after injury showed a right parietotemporal linear skull fracture with the maximum width of 4 mm. The width of the fracture line increased to 9 mm 7 hours after injury. Serial CT scan demonstrated growth of contusional hematoma just beneath of the fracture. Six days later, a pulsatile mass was palpated beneath the scalp, although neurological state was normal. The width of the fracture line was demonstrated to increase up to 11 mm on X-ray at that time. CT scan revealed subgaleal cerebral herniation surrounded by brain edema. He was operated 11 days after injury. It is thought that the contusional hematoma and following brain edema played an important role in the genesis of rapid growth of the skull fracture.

[Evaluation of tumor marker CA15-3 in breast cancer].

CA15-3, a new tumor marker for breast cancer, was determined in various malignant diseases including breast cancer and various benign diseases, and its clinical significance and usefulness were studied. In 18 normal individuals, the value of CA15-3 was 8.9 +/- 3.3 U/ml (mean +/- SD). In primary breast cancer, the positivity was 20% for Stage I, 0% for Stages II and III and 100% for Stage IV. Of 17 cases of recurrent breast cancer, 13 (77%) were shown to be positive. The therapeutic effect and the value of CA15-3 were well correlated with each other. As for other malignant tumors, positive cases were observed in 50% of recurrent cancer of the stomach and in 14% of malignant tumors of the biliary system. All of these cases were terminal-stage cancers. The CEA value determined simultaneously showed a good correlation, r = 0.87 (p less than 0.01) with CA15-3 in malignant tumors other than breast cancer. In breast cancer, however, the correlation between the two was low, r = 0.18. These results suggest that CA15-3 is not necessarily useful in the diagnosis of primary breast cancer, but is useful as an indicator of the effect of therapy for recurrent breast cancer and for the prediction of recurrence.

[A case of multiple septic intracranial aneurysms--successful treatment with surgical and conservative therapy in the same case].

Here reported is a case of multiple septic intracranial aneurysms which were successfully treated with surgical and conservative therapy. A 44-year-old man was admitted to our hospital because of headache, vomiting and visual disturbance. He had had a fever and had been under treatment for a respiratory tract infection during the preceding 3 months. Physical examination on admission revealed pansystolic heart murmur over the cardiac apex. Neurological examination revealed neck stiffness, papilledema and right homonymous hemianopsia. Laboratory data showed the presence of inflammatory process. A CT scan showed a high density area in the left occipital region, and vertebral angiography showed a saccular aneurysm on a distal branch of the left occipitotemporal artery. Fourteen days after admission, the operation of clipping the neck of the aneurysmal artery was performed and the hematoma evacuated to lower the increased intracranial pressure. He had been well after the operation until 3 weeks later when a follow-up angiography showed a new unruptured aneurysm on a distal branch of the right middle cerebral artery with a relapse of the infection. Then, he was treated with appropriate antibiotics and antifibrinolytic agents. A repeated angiography 1 month later showed resolution of the aneurysm. The mechanism of resolution of septic aneurysm and its treatment are discussed.

[Evaluation of CDDP administration into the body cavity].

CDDP (100 mg bolus) was administered into the body cavity for cancerous pleural effusion and ascites, and its effectiveness and pharmacokinetics were studied. The cases treated consisted of 7 of gastric cancer, 1 of pancreas cancer, 2 of lung cancer and 2 of breast cancer. A decrease in body cavity fluid was observed in all cases. The cases in which treatment was effective were broken down into CR 2 cases, PR 8 cases and NC 2 cases, the efficacy rate being 83%. This procedure produced distinctly fewer side effects than intravenous administration. Disturbance of renal function and G.I. symptoms were negligible, but anemia was often found. The free-CDDP levels in the body cavity fluid remained of over 1.0 microgram/ml after 12 hours and those in plasma remained after 1 hour. Intracavitary administration thus seems an excellent method that enables a drug to act on cancer cells directly at high concentrations over many hours.

Molecular characterization of a meiotic recombinational hotspot enhancing homologous equal crossing-over.

We have cloned and sequenced a meiotic recombinational hotspot between the A beta 3 and A beta 2 genes in the major histocompatibility complex (MHC) of the mouse. This recombinational hotspot in the Mus musculus castaneus cas3 haplotype was previously localized to a region of 9.5 kb of DNA in which five independent crossing-over events occurred at the unusually high frequency of 0.6%. Aside from cas3, the hotspot appears to be absent in many other MHC haplotypes. We have now confined the five recombinational breakpoints to a stretch of 3.5 kb of DNA. From the nucleotide sequence around the recombinational breakpoints, determined in the parental cas3 and b haplotypes as well as for two recombinant haplotypes, we show that the two recombinant haplotypes were generated by homologous equal crossing-over and place the breakpoints within two non-overlapping stretches of 10 and 36 bp, respectively. Comparison of the DNA sequences of the hotspot-positive cas3 and the hotspot-negative b haplotypes reveals a number of differences, in particular, a CAGA-repeat sequence which is present in CAS3 in six, but only four copies in C57BL/6 DNA. This repeat sequence is reminiscent of one in a previously characterized hotspot in the E beta gene.

Shark (Prionace glauca) elastoidin: characterization of its collagen as [alpha 1(E)]3 homotrimers.

A new type of collagen was isolated from elastoidin of great blue shark by limited pepsin digestion. The collagen alpha chain of elastoidin, designated alpha 1(E), was very similar in electrophoretic and chromatographic behavior and amino acid composition to shark skin alpha 1(I) chain, but they were genetically-distinct on the basis of CNBr-peptide maps. The collagen molecule of elastoidin was shown to be an [alpha 1(E)]3 homotrimer.

Hemorrhagic infarction after vasospasm due to ruptured cerebral aneurysm.

Hemorrhagic infarction after vasospasm is a rare condition in patients with aneurysmal subarachnoid hemorrhage (SAH). Induced hypertensive therapy is used for patients with vasospasm, but this treatment has a risk of inducing hemorrhagic infarction. A total of 221 patients whose first computed tomographic (CT) scans were examined within 2 weeks after SAH were investigated for this study. There was symptomatic vasospasm in 99 (45%), cerebral infarction in 37 (17%), and hemorrhagic infarction in 13 (6%). Hemorrhagic infarction usually occurred 20 to 30 days after aneurysmal rupture; this period corresponds with the remission stage of the vasospasm. On CT scans, the hemorrhagic infarction was revealed as a leaky hemorrhage in a low density area in 11 cases, and a massive hemorrhage with mass effect was seen in 2 cases. These findings suggest that hemorrhagic infarction after vasospasm may sometimes be fatal. Cerebral blood flow autoregulation in patients with vasospasm was normal or of a hypertensive type during the remission stage of vasospasm, when hemorrhagic infarction usually appeared. This finding shows that induced hypertension therapy is ineffective during this stage; it should be stopped by this stage because it is ineffective and also may aggravate hemorrhagic infarction.

Alpha heavy chain disease lacking secretory alpha chain, with cobblestone appearance of the small intestine and duodenal ulcer demonstrated by endoscopy.

Ultrastructural and immunohistochemical studies of the small intestine are described in a Japanese patient with alpha heavy chain disease who had a history of colonic ulcers. Endoscopic examinations revealed multiple gastric erosion, duodenal ulcer, and a thickened, cobblestone-like pattern composed of small nodules in the duodenum and jejunum, which was similar in appearance to Crohn's disease. An electron microscopic study showed that the numerous, infiltrated cells in the jejunal lamina propria were matured plasma cells with atypical structure of the organelles. These plasma cells had alpha heavy chain protein devoid of light chain. Although secretory component was demonstrated normally in the epithelial cells by immunofluorescent methods, no association of this component with alpha heavy chain protein could be observed in any of the plasma and epithelial cells of this case. These facts suggest the absence of secretory alpha chain or secretory IgA, and a deficiency of the mucosal secretory immune system in this patient.

[Clinical study of serum and tissue concentrations of FT-207 and 5-FU in patients with cancer of the large intestine following preoperative application of FT-207 suppositories].

1.5 g/day of FT-207 suppository was administered for two weeks prior to operation to twenty patients with large bowel carcinoma of familiar poliposis coli. The level of FT-207 and 5-FU in serum, lymph node, tumor and normal colonic mucosa were determined by bioassay. The levels of FT-207 in the rectum, sigmoid colon and tumor were higher than that in serum. The levels of 5-FU in the rectum, sigmoid colon, tumor and lymph node were also higher than that in serum. The levels of FT-207 and 5-FU in carcinoma of the rectum was higher than those in serum. Compared with the level in normal rectal mucosa, only the level of 5-FU in rectal carcinoma was higher. From the histological point of view, the highest levels of FT-207 and 5-FU was observed in well-differentiated adenocarcinoma. There was no side effect experienced in our series, therefore, FT-207 suppository seems to be one of the safe promising preoperative chemotherapies.

[Effect of clonidine on plasma insulin level in mice (author's transl)].

The influence of adrenergic agents, epinephrine and clonidine, on plasma immunoreactive insulin and plasma glucose concentrations was studied in mice. Subcutaneous injection of epinephrine in fasted mice did not alter the plasma glucose concentration (PG), while plasma immunoreactive insulin concentration (IRI) tended to increase gradually. Intravenous injection of glucose markedly increased IRI. Glucose-induced IRI increase was inhibited by a subcutaneous injection of epinephrine in spite of high elevation of PG. This inhibition of glucose-induced IRI increase by epinephrine was reversed after treatment with an alpha-adrenergic blocking agent, phentolamine. Propranolol, an beta-adrenergic blocking agent suppressed IRI to a greater extent as compared with IRI induced by simultaneous injection of glucose and epinephrine. These results indicate that beta-adrenoceptor stimulating action accelerates the insulin release induced by glucose while alpha-adrenoceptor stimulating action inhibits it. Subcutaneous injection of clonidine in fasted mice slightly decreased IRI and increased PG. The response of PG to clonidine was dose-dependent. Glucose-induced IRI increase was inhibited by an intravenous injection of clonidine, and PG was elevated under the same conditions. The inhibition of glucose-induced IRI increase by clonidine was reversed when phentolamine was given, and under these conditions, PG showed no change. Propranolol treatment did not result in a recovery of the inhibition of glucose-induced IRI increase by clonidine. When compared with the results of epinephrine treatment, it may be concluded that clonidine shows alpha-adrenoceptor stimulating action in the secretion of insulin from beta-cells of the endocrine pancreas.
