Morphological changes of human mandibular bone during fetal periods.

Standardized photographs (lateral and basal view) of fetal mandibles (n = 80) were taken. Outline segments (from gnathion to condylion, on the coronoid process and from infradentale to gnathion in lateral view; and on the inferior margin in basal view) were analyzed into sine curves, according to Fourier series. Using the calculated Fourier coefficients, we statistically examined how mandibular shape changed with fetal growth. Materials were classified into four groups on the basis of the body length (BL): 100 < or = BL < 200 mm (BL1, n = 20); 200 < or = BL < 300 mm (BL2, n = 20); 300 < or = BL < 400 mm (BL3, n = 20); and 400 < or = BL < or = 500 mm (BL, 4 = 20). Statistical analyses were all done on these stature groups. On the outline from gnathion to condylion, the constant and the first term phase showed statistically greater values in BL4 than in the other groups, as assessed by t-test. The first term phase for the coronoid process significantly decreased with increments in stature in the canonical disciminant analysis. On the outline from infradentale to gnathion, although the cumulative contributions of the first two terms were constant (about 85%) in all four groups, the relative contributions with growing stature decreased in the first term and increased in the second term. The mean of the phase in the first term indicated slightly more than -90 degrees in BL1, and was less than -90 degrees in BL2 to BL4. The averages of the second term phases presented little change from BL1 to BL3, and were close to -100 degrees. From the canonical discriminant analysis, the constant was great and the first term amplitude was small in BL1 and BL4, and the constant was small and the first term amplitude was great in BL2 and BL3. Also, the second term amplitude in BL4 indicated a significantly bigger volume than in BL3. The constant for the inferior margin manifested statistically significant increments in BL2 to BL4, as assessed by t-test. We interpret the results as follows: 1. On the outline from gnathion to condylion, anteroinferior shift occurs in BL4, resulting in an acute gonial angle. The change of angle during the prenatal period precedes the postnatal findings that bring about a progressive decrease in the gonial angle. 2. Effects of rapid burnout type gypsum-bonded investment on performance of castings. Part 1. Surface aspects and fit of crowns.

Rapid burnout type gypsum-bonded investment materials were developed to shorten the time required for dental casting procedures. With these materials, molds can be prepared by rapid heating at 700 degrees C for 30 min from 30 min after the start of mixing. When the investment block was rapidly heated at 700 degrees C, no fractures were observed in the rapid burnout type investments with one exception, while a conventional cristobalite investment broke into pieces shortly after being placed in the furnace. Casting fins were sometimes induced only for the material which showed fracturing on rapid heating. No practical problems were found in the surface roughness of the castings. The 30 min-setting expansion was significantly different among the materials although there were no differences in thermal expansion, and the material showing greater 30 min-setting expansion was efficient to obtain better fit of the crown as in the conventional casting procedures.

[Clinical usefulness of serum assay of EIA-CYFRA 21-1 in lung cancer].

Enzyme immunoassay CYFRA 21-1, a novel lung tumor marker presented in serum (upper limit of normal values: 3.5 ng/ml) was measured in 84 patients with lung cancer and 141 patients with benign respiratory diseases. The positive rate for CYFRA 21-1 in primary lung cancer was 70.3% and the false positive rate in respiratory diseases was 24.1%. In respect to the histological types of lung cancer, CYFRA 21-1 was elevated in 85.0% of squamous cell carcinoma and in 64.8% of other cell types of primary lung cancer. Elevated CYFRA 21-1 levels were found in 40% of Stage I-II, in 80.0% of Stage IIIA-B, and in 73.5% of Stage IV. The overall diagnostic efficiency was 53.4% for CYFRA 21-1 and 44.5% for CEA, respectively. Receiver operating characteristic (ROC) curve analysis suggested that CYFRA 21-1 test has an advantage over CEA. From the results of this study, CYFRA 21-1 was more efficient than CEA as primary diagnostic marker in lung cancer.

Biocompatibility of a heparin-bonded membrane oxygenator (Carmeda MAXIMA) during the first 90 minutes of cardiopulmonary bypass: clinical comparison with the conventional system.

We clinically compared a heparin-bonded Carmeda MAXIMA membrane oxygenator to a nonheparin-bonded MAXIMA in 20 patients undergoing coronary artery bypass grafting or valve replacement. Reductions of fibrinogen, factor XII, and high molecular weight kininogen were greater in the MAXIMA group. Serum C3a and free hemoglobin were lower in the Carmeda group. The level of C4a, though remarkably lower than that of C3a, was higher in the Carmeda group then in the MAXIMA group. Both oxygenators performed well in terms of blood gas exchange. We conclude that the heparin-bonded Carmeda oxygenator offers superior biocompatibility during cardiopulmonary bypass.

Elevation of ceruloplasmin activity involved in changes of hepatic metal concentration in primary biliary cirrhosis.

Primary biliary cirrhosis (PBC) patients showed increased ceruloplasmin activity and Cu content in the serum as compared to the control group. Moreover, Cu, Fe, and Zn contents were measured in the livers of autopsied cases. The PBC patients showed increased Cu content and reduced Fe and Zn levels. In PBC cases, Cu transportation via bile is disturbed due to collapse of the interlobular bile ducts and consequently Cu accumulates in the liver. Thus, it is likely that serum ceruloplasmin activity increases to metabolize the accumulated hepatic Cu and that Cu content increases in the serum as a ceruloplasmin-bound form. Furthermore, reduction of Fe and Zn contents in the liver of the PBC patients indicated the possible relationship of ceruloplasmin to Fe and Zn metabolism as well as Cu metabolism.

[Nontraumatic femoral head necrosis: classification of bone scintigraphic findings and diagnostic value of SPECT following planar imaging].

This study was conducted to determine bone scintigraphic findings in nontraumatic femoral head avascular necrosis and diagnostic value of SPECT imaging following a conventional planar imaging. Forty-three femoral heads in twenty-six cases with idiopathic femoral head necrosis (n = 2), systemic lupus erythematosus (n = 22), aplastic anemia (n = 1), and renal transplantation (n = 1) were studied. The diagnosis for femoral head necrosis was based on magnetic resonance imaging as well as other diagnostic studies in all cases. Scintigraphic findings of planar and SPECT images were classified into six categories: normal (N); cold or decrease (C); partial increase with cold or decrease (PH+C); ring-like increase with a cold center (RH+C); partial increase (PH); diffuse and/or irregular increase (DH). Avascular necrosis was confirmed in twenty-four femoral heads, in which planar and SPECT images showed scintigraphic findings of N (n = 3, 2), C (n = 1, 3), PH+C (n = 2, 8), RH+C (n = 2, 3), PH (n = 9, 2), and DH (n = 7, 6), respectively. Femoral heads without avascular necrosis demonstrated planar and SPECT findings of N (n = 16, 12), C (n = 0, 6), and DH (n = 3, 1), respectively. When considering C, PH+C, and RH+C as diagnostic findings for avascular necrosis, sensitivities of planar and SPECT images were 21% and 58%, and specificities were 100% and 68%, respectively.(ABSTRACT TRUNCATED AT 250 WORDS)

Alteration of G protein levels in antigen-challenged guinea pigs.

To elucidate the mechanism(s) of altered responses of various receptors observed in asthma, we examined levels of G proteins in lung membranes isolated from guinea pigs at various stages of experimental asthma. We found that levels of Gi2 alpha and Gq alpha increase after multiple exposures to aerosolized antigen but not after a single exposure. Levels of Gi1 alpha, Gs alpha and G beta did not change under these experimental conditions. Tracheas from these animals exhibited hyperresponsive muscarinic bronchoconstriction and hyporesponsive beta adrenoceptor-mediated relaxation of endothelin-preconstricted tracheas as measured by an ex vivo isometric assay. No changes in numbers or affinities of membrane M3 muscarinic and beta-2 adrenergic receptors were detected as measured by binding of [3H]QNB and [125I]CYP, respectively. The impairment of beta adrenoceptor-mediated relaxation of tracheas isolated from animals treated by multiple challenges was reversed by the pretreatment of tracheas with pertussis toxin. On the other hand, the carbamylcholine-stimulated GTPase activity was higher in lung membranes isolated from animals in the multiple challenge group and was blocked by anti-Gq alpha protein antibody. Taken together, our present observations suggest that an underlying mechanism of altered receptor responses associated with asthma is in part due to increased expression of Gi2 alpha and Gq alpha proteins, leading to altered coupling of receptors to effector systems.

[A case of rupture of the ascending aorta owing to prosthetic graft dilation following replacement of the descending aorta].

A 55-year-old male was diagnosed as acute type A aortic dissection caused by retrograde propagation from a tear in the descending aorta. A replacement of the descending aorta was performed with a prosthetic graft (VASCUTEK). Sixteen days after the operation, the patient required an emergency operation due to a rupture in the ascending aorta. Transesophageal echo and contrast enhanced CT indicated the point of entry to be at the proximal anastomotic portion of the prosthetic graft and the prosthetic graft (30 mm in diameter) to be dilated to 45 mm. Intimal tear at anastomotic portion owing to graft dilation was suspected as the main factor causing rupture of the ascending aorta.

Antagonism by glibenclamide of the effect of morphine in hippocampal preparations.

We previously showed that morphine lowered the affinity of Ca2+ antagonist binding and subsequently enhanced field potentials in hippocampal preparations. In the present study, the effect of various K+ channel antagonists on these actions of morphine was studied. Higher Kd value of [3H]nitrendipine binding was obtained for membranes prepared from slices treated with morphine. Concomitant treatment of slices with morphine and tetramethylammonium (TMA) or glibenclamide attenuated the effect of morphine. Apamin and mast cell-degranulating (MCD) peptide were without effect on morphine-induced change in [3H]nitrendipine binding. In those experiments, no change in concentration of binding sites was observed. Glibenclamide reduced the morphine enhancement of field potentials. These results suggested the regulation of Ca2+ channels by morphine through K+ channel opening.

Involvement of protein kinase in environmental stress-induced activation of human multidrug resistance 1 (MDR1) gene promoter.

The human MDR1 gene can be induced in response to various environmental stimuli. To examine whether such stress-induced activation of the MDR1 gene can be modulated by protein kinase, we employed a stable human cancer KB cell line which contained the bacterial chloramphenicol acetyltransferase (CAT) gene directed by the MDR1 gene promoter. H-7, a protein kinase C inhibitor, at more than 40 microM inhibited activation of the MDR1 promoter that was induced by ethylmethane sulfonate, 5-fluorouracil or UV irradiation. DNA binding activity of nuclear factors recognizing the MDR1 promoter was augmented in KB cells treated with UV, but decreased in cells treated concomitantly with H-7. Okadaic acid alone was able to induce the promoter activation, and this induction was dependent on specific promoter sequences. Okadaic acid also enhanced the DNA binding activity of nuclear factors recognizing the MDR1 promoter. The phosphorylation of transacting factors may modulate the MDR1 gene promoter activity.

A novel disease with deficiency of mitochondrial very-long-chain acyl-CoA dehydrogenase.

Palmitoyl-CoA dehydrogenase activity in skin fibroblasts from seven patients with unidentified defects of fatty acid oxidation was measured in the presence and absence of antibodies against medium-chain, long-chain, and very-long-chain acyl-CoA dehydrogenases (VLCAD). Two of the patients, 4-5 month old boys, were found to have a novel disease, VLCAD deficiency, as judged from the results of very low palmitoyl-CoA dehydrogenase activity and the lack of immunoreactivity toward antibody raised to purified VLCAD.
