Myotonic dystrophy: fragility of band 3 membrane protein upon resealing of erythrocyte ghosts and the normality of ATPase activity, sialic acid content, and spectrin extractability of ghosts.

Since a number of recent reports (2,14) have shown that myotonic dystrophy (MyD) might be an inherited disorder of cell membranes, we studied erythrocytes of 10 patients with MyD to investigate the membrane abnormality. In the osmotical aspect, the relative hemolysis of erythrocytes in patients with MyD in a solution of 75 mM NaCl displayed almost half the value of the normal level. Our experiments using electrophoresis demonstrated that a main polypeptide of membrane, band 3 (by the nomenclature of Fairbanks et al. (15], in patients with MyD was very fragile in the process of erythrocyte ghosts resealing in the isotonic medium. No significant differences were observed in membrane-bound Ca2+-ATPase activities, sialic acid contents, and spectrin extractability assessed by SDS-polyacrylamide gel electrophoresis between normal and MyD. It is suggested that abnormality of MyD erythrocytes may be localized to the surface membrane, especially around band 3.

How to investigate cholestasis: utility of ultrasound as the first imaging study.

In modern gastroenterology, ultrasonography, particularly the real-time modality, is one of the most important diagnostic procedures, and the value of ultrasound in the diagnosis of cholestasis, and obstructive jaundice in particular, is indisputable. Although sector-type real-time ultrasonography has certain merits, for a routine examination, the linear scan is much more practical. It should be the first imaging investigation in patients with jaundice, along with the routine hematologic and biochemical tests. In obstructive jaundice, these studies are followed by ERC or PTC, and a definitive diagnosis is possible in the large majority of patients. Such a diagnosis very rarely requires correction at surgery.

Studies on the structure of a novel peptide antibiotic, K-582.

A novel peptide antibiotic, K-582, which exhibited significant growth inhibition of Candida, viruses and ascites tumor in mice, was found in the culture medium of a strain of Metarhizium anisopliae by Kondo et al. (J. Antibiotics 33, 535-542 (1980) ]. K-582 consisted of two components, designated K-582 A and K-582 B. Threonine, tyrosine, ornithine, and an unusual amino acid were common in both peptides, but lysine was an extra component of K-582 A. The unusual amino acid was identified to be threo-gamma-hydroxy-L-arginine (OHArg) by means of mass, nuclear magnetic resonance and infrared spectrometries of the derivatives and the related compounds. The threonine and the arginine were assigned to be L-configuration, and the ornithine and the tyrosine to be D-configuration in both K-582 A and K-582 B, and the lysine to be L-configuration by comparison of their optical rotatory dispersion spectra with those of standard amino acids. The elucidation of primary structure revealed that they were closely related heptapeptides with the following sequence: K-582 A:H-Arg-OHArg-Orn-Thr-Orn-Lys-Tyr-OH; K-582 B:H-Arg-OHArg-Orn-Thr-Orn-OHArg-Tyr-OH, and had the identical sequence in terms of the configuration of their constituents, namely L-L-D-L-D-L-D.

Ultrasonic anatomy of the biliary system.

Real-time ultrasound has vastly improved the diagnosis of the diseases involving the tubular structures of the liver and biliary tract. Puncture of the bile duct and gallbladder can now be carried out under ultrasound visual guidance. An understanding of ultrasound anatomy is essential for taking full advantage of ultrasound in diagnosis.
