One-step fractionation of neutral and acidic glycosphingolipids by high-performance liquid chromatography.

A procedure for a simultaneous separation of ganglioside components and neutral glycolipid components by high-performance liquid chromatography was described. One column packed with DEAE-derivatized controlled-pore glass (DEAE-CPG) was serially connected to two columns of underivatized, controlled-pore glass (CPG). A mixture of gangliosides and neutral glycolipids were loaded on DEAE-CPG and eluted with a mixture of chloroform-methanol-water, with increasing methanol and water (the first-phase gradient elution), followed by elution with increasing concentrations lithium acetate from 0.015 to 0.1 M in a mixture of chloroform-methanol-water (the second-phase gradient elution). Neutral glycolipids, mono- to hexaglycosylceramides , were separated within 80 min of the first-phase gradient elution, and mono- to tetrasialosylgangliosides were separated during the second-phase gradient elution within 60 min. The method has been applied to the determination of glycolipids isolated from rat tissues, and the procedure was found to be highly reproducible.

Juxtasellar mycotic abscess.

A rare case of juxtasellar mycotic abscess with suprasellar extension is reported. The patient had double vision, only a mild abducens palsy, and no sign of inflammation, history of nasal sinusitis, or diabetes mellitus. Although a histologic examination showed massive fungal mycelia with thin capsules and no tumor cells, the neuroradiologic findings were like those in a clival chordoma or epidermoid tumor.

Precocious puberty due to postmeningitic hydrocephalus.

A case of precocious puberty due to postmeningitic hydrocephalus was presented. Concentrations of serum gonadotropins and estradiol were found to be elevated, and the response of LH to LHRH was also high, similar to that in the late pubertal stage. CT scanning demonstrated marked dilatation of the third ventricle. It was considered that this was one of the factors which caused the precocious puberty.

Glycosphingolipids in clonal variants of rat fibrosarcoma cells with different transplantability.

Neutral glycolipids and gangliosides of four clonal variants of rat fibrosarcoma AS-653 cells with different transplantability were analyzed. A highly malignant clone A had a much lower quantity of GM3 ganglioside and a much higher quantity of lactosylceramide as compared to clones Z and G, which showed low transplantability and less malignancy, and a variant clone P, which showed tumorigenicity only in ascites form. A similar correlation was found with the quantity of an unidentified slow-migrating neutral glycolipid in various clones. This glycolipid was present in trace amount in the original highly malignant clone A, increased moderately in clones G and Z, and increased greatly in clone P, which showed no subcutaneous transplantability. The results of these studies suggest that a blockage of synthesis of GM3 ganglioside and a long chain neutral glycolipid occurred with enhanced malignancy, and an enhanced synthesis of GM3 ganglioside and the long chain neutral glycolipid is associated with a decrease in transplantability.

A case of pituitary adenoma with possible simultaneous secretion of thyrotropin and follicle-stimulating hormone.

A TSH- and FSH-screening pituitary adenoma was demonstrated in a 23-yr-old man who presented with bitemporal hemianopsia, but without clinical or laboratory evidences of hyper- or hypothyroidism or hypogonadism. Basal TSH (29-45 microunits/ml) and FSH (44-63 mIU/ml) were moderately elevated. GH and ACTH secretion were impaired, and basal PRL and testosterone were normal. TRH and LRH elicited a significant increase in TSH and FSH, respectively. The plasma glycoprotein alpha-subunit concentration was normal, and its response to simultaneous administration of TRH and LRH was low. Plasma TSH was not suppressed completely by exogenous T3. After operation and radiotherapy, elevated TSH and FSH as well as failure of T3 to suppress TSH were corrected. No significant changes in thyroid hormone concentration in the blood were detected in spite of the reduction of TSH. TSH and FSH concentrations in the tumor extract were lower than those in normal pituitaries. Histologically, the tumor was very vascular and consisted mostly of a single type of cell. The tumor cells were positively stained for FSH beta-subunit by immunohistochemical study, but for none of other pituitary hormones or subunits examined. Electron microscopic examination revealed relatively abundant single type secretory granules of high electron density. It is possible that the tumor simultaneously secreted TSH and FSH, the former possibly being immunologically active, but biologically inactive.

Pharmacokinetic and biopharmaceutical studies on azelastine hydrochloride in beagle dogs by quantitative selected-ion monitoring.

A quantitative selected-ion monitoring method for the determination of plasma concentrations of the new potent antiallergic agent, 4-(p-chlorobenzyl)-2-(hexahydro-1-methyl-1H-azepin-4-yl)-1-(2H)-phthalazinone hydrochloride (azelastine hydrochloride, A 5610), using N-methyl deuterium labelled azelastine as the internal standard, has been developed. This specific method has a lower limit of sensitivity of about 0.5 ng/ml plasma. The utility of this method was demonstrated by pharmacokinetic studies in the beagle dog after a single oral 20 mg dose as aqueous solution or as sugar-coated tablets, and after a single 10 mg i.v. dose. Plasma concentration-time curves following i.v. dose were satisfactorily described by a bi-exponential equation, and the mean values of half-lives (t 1/2) for the distribution and elimination phases were 17.5 +/- 5.0 min and 160 +/- 14 min, respectively. From the comparison of the areas under the curves after the p.o. dose of aqueous solution and tablet to that after i.v. dose, the absolute availabilities were estimated to be 16.6 +/- 2.5% and 19.6 +/- 0.4%, respectively. These low availabilities could be explained on the basis of a first-pass effect. From the analysis according to the Loo-Riegelman method, the absorption rate was revealed to be described by a first-order process. The absorption half-lives were 48.1 +/- 10.2 min for the solution and 58.4 +/- 21.8 min for the tablet. The lag times before onset of absorption were 18.2 +/- 2.1 min and 40.9 +/- 10.8 min, respectively. This seemed to be due to the difference in gastric emptying time or disintegration time of the tablets in the gastro-intestinal tract.

The effect of food on the absorption of alpha-tocopheryl nicotinate in beagle dogs and human volunteers.

The effect of food on absorption of d,1-alpha-tocopheryl nicotinate was studied in cross-over studies with eight beagle dogs and four human volunteers. When 100 mg of alpha-tocopheryl nicotinate in gelatin capsule was administered p.o. to beagle dogs in both a fasted state and after eating, the plasma peak level (Cmax) and area under the curve (AUC) of this ester in nonfasted state was five-fold higher than in fasted state, although the peak time (tmax) did not markedly differ between both conditions. Cmax and AUC after oral administration of 600 mg of alpha-tocopheryl nicotinate in human volunteers in nonfasted state was 32-fold and 29-fold higher, respectively, than during fasted state, without delay in tmax. The plasma levels of this ester following ingestion of 200 mg in fasted state were not detectable. A comparison of the 600-mg with 200-mg dose level during nonfasted state showed the former to be 1.8-fold higher in Cmax and 2.3-fold in AUC.

[Computed tomography in metastatic brain tumors and meningeal carcinomatosis (author's transl)].

The computed tomogrpahy has been proved to be highly reliable in diagnosis of CNS diseases. The findings in 26 cases of metastatic brain tumors, and 2 cases of meningeal carcinomatosis performed with Hitachi CT-H scanner are analized. Metastatic tumors are seen on plain CT as mainly isodense nodules (60%). But, on enhanced CT, they increase in their density (97%). Larger nodules of metastatic tumors (over 2 cm in diameter) are tend to contain less dense central area, so called central cavitation. The direction of the expansion and/or extension of the perifocal low density is characteristic to the location of the tumor. In cases of meningeal carcinomatosis, diffuse periventricular higher density can be recognized enhanced CT.

[Surgical treatment of intracranial hematoma in an infant with hemophilia B (author's transl)].

Surgical experience of intracerebral hematoma in an 8-month-old boy with severe hemophilia B, which was not diagnosed preoperatively, was presented. Carotid angiography and removal of hematoma by craniotomy were carried out quite safely under fresh-blood-transfusion. After several months ventriculoperitoneal shunt was placed, because of progressing hydrocephalus and multiple porencephaly, this time, under the cover of the factor IX complex. The clinical features of the intracranial hemorrhage in hemophilia were discussed, with analysis of 52 operation cases. Whenever we encounter a patient with intracranial hemorrhage, especially in infancy, hemorrhagic diseases, as hemophilia, must always be borne in mind. Although intracranial hemorrhage is still the most fatal complication of hemophilia, the surgical risk has been greatly diminished by advanced replacement therapy. We stress the need of immediate diagnosis and positive surgical treatment with adequate replacement therapy, if indicated.

[Traumatic thrombosis of the internal carotid artery: Report of three cases due to head injury with basal skull fracture (author's transl)].

Three cases of traumatic thrombosis of the internal carotid artery due to head injury with basal skull fracture were reported. The mechanism of thrombosis, the symptomatologic characteristics and the prognosis were discussed with reference to the data in the literature. Four mechanisms by which the condition may be produced include; injury to the intrapetrous or cevernous portion of the carotid artery during basal skull fracture, injury to the point of emergence of the carotid artery from the cavernous sinus as a result of shearing strains suffered at the time of injury, a direct blow to the neck or trauma to the paratonsillar area by a foreign object carried in the mouth, and stretching of the carotid artery by heperextension and lateral flexion of the neck. Secondary symptoms due to thrombosis of the carotid artery after an interval develop most commonly from twelve to twenty four hours. The prognosis in these patients is mostly poor. Therefore, angiography should be performed immediately on any patient suspected of having this lesion and treatment should be considered.

[Cerebellar hemangioblastoma with multiple aneurysms].

We have reported a case of cerebellar hemangioblastoma in a 50 year-old Japanese female in which bilateral kidney and ovarian cysts, adrenal adnomatus hyperplasia, and erythrocytosis were observed. Futhermore, an aneurysm not initially observed was detected by cerebral angiography done 23 months after tumor extirpation. We have evaluated the relationship between cerebellar hemangioblastoma and concurrent multiple aneurysms with reference to reported cases.
