[Diurnal variation and responses of corticoids to dexamethasone and ACTH before and after adrenal surgery in primary aldosteronism].

We studied diurnal variation and the responses of plasma corticoids to dexamethasone and ACTH before and after adrenal surgery in 11 patients with primary aldosteronism. Diurnal variation of plasma corticoids was examined in all cases. Before adrenal surgery, plasma aldosterone (Ald) was higher at all times, deoxycorticosterone (DOC) was high value at 5:00 and then normal value, 11-deoxycortisol (S) was high value at 5:00 and 9:00, thereafter normal value, while corticosterone (B) and cortisol (F) were almost normal value at all times. The circadian rhythm was observed in these corticoids. Dexamethasone (2 mg/day) was administered to 8 patients for 10 days. Plasma Ald and DOC were significantly suppressed only at 5:00, while B, S and F almost suppressed at all times. After dexa suppression, the circadian rhythm of Ald still observed, while the diurnal variation of the other corticoids was even. ACTH (1 microgram) was injected intravenously to 9 patients. Responses of DOC, B and Ald were higher, and of S and F were almost normal. Diurnal variation of plasma corticoids was observed after 1 month of adrenal surgery. Ald was lower, the other corticoids were normal values. ACTH (1 microgram) was injected to the same case before surgery. Responses of these corticoids to ACTH were slightly lower. Contents of Ald in adenoma tissues were higher than those in adjacent and normal adrenal tissues, and the contents of the other corticoids in adenoma tissue were almost normal value. As these results showed, before surgery these corticoids were secreted from adenoma without ACTH, although these were responsible to ACTH, and the presence of regulative factor of circadian rhythm except ACTH was suggested. After surgery, not only the responses to ACTH of mineralocorticoids but these of glucocorticoids decreased.

Electron microscopic study on the epiglottic mucosa in human fetuses.

The differentiation of the epiglottic mucosa in human fetuses has so far been studied only by light microscopy. So, we studied the epiglottic mucosa in human fetuses (at gestational ages from the 7th to 21st week) by scanning (SEM) and transmission electron microscopies (TEM), and could disclose more detailed features of cellular differentiation of this mucosa. These results and the review of the literatures lead to the following conclusions: 1. Primary cilia appeared in the epiglottic mucosa around the 7th week of gestation. 2. Ciliated cells and the stratified squamous epithelium of the lingual surface appeared at the similar period as reported by others, but our study revealed that the squamous cells are immature even at the 21st week. 3. Glycogen granules in non-ciliated cells decreased paralleling the differentiation of the cells, suggesting that they provide a source of energy necessary for the cell differentiation.

A case of recurrent malignant pheochromocytoma complicated by watery diarrhea, hypokalemia, achlorhydria syndrome.

A patient is reported who had undergone right adrenalectomy for pheochromocytoma and 15 yr later developed a recurrence in the same site complicated by the watery diarrhea, hypokalemia, achlorhydria syndrome. This tumor was histologically defined as a composite malignant pheochromocytoma-ganglioneuroblastoma (well differentiated type). Vasoactive intestinal polypeptide and catecholamine concentrations were elevated in both plasma and the tumor. The tumor somatostatin content also was high. The tumor was immunohistochemically determined to contain both vasoactive intestinal polypeptide and somatostatin.

Histopathology of otitis media with effusion. An electron microscopic study of human temporal bones.

Ultrastructural studies of the middle ear mucosa appear to be of significant value in better understanding the pathology of otitis media with effusion (OME). Our present study was undertaken in order to take advantage of the use of electron microscopy in investigating all areas of the middle ear mucosa. Tissues studied were obtained from the fresh postmortem temporal bones of three patients with OME and terminal head and neck malignancies. In the mucoid type of effusion (cases 1 and 2), goblet cells were seen to proliferate and secretory activity was greatly enhanced. In contrast, there was no evidence of secretory cell proliferation in the serous type of effusion. It was noteworthy that accumulated fluid was not homogeneous in the same ear, as exemplified by case 1, in which both mucoid and serous effusions were present. This occurrence was possibly the result of topographic diversity involving the secretory activity of the middle ear.

Pseudomonas labyrinthitis.

Pseudomonas aeruginosa is the gram-negative bacterial rod which is often isolated from chronic aural discharge. This microorganism may also cause necrotizing infection of the external auditory canal in certain patients with impaired host-defense mechanisms. Involvement of the inner ear by this microbe is extremely rare. In this communication, we report a case of pseudomonas labyrinthitis which resulted from traumatic middle ear injury. Infection produced massive granulations and extensive bone destruction of the otic capsule. This case shows that while P.aeruginosa is usually an avirulent opportunistic pathogen, it may also cause a highly destructive labyrinthitis if the inner ear is entered.

The morphology of bone mineral as revealed by small-angle X-ray scattering.

Diffuse small-angle X-ray scattering of oriented bone from bovine femur and canine femur was described in terms of an ideal isotropic two-dimensional two-phase system which consists of mineral phase and organic phase. The microstructure of powdered, randomly-oriented bone from bovine femur was found to be affected by grinding. An analysis of small-angle scattering from oriented bone showed that bone mineral was to a large extent in the form of needle-like particles with a 50-60 A the diameter, and the dimension of organic phase transverse to the longitudinal axis of long bone was in the range 45-55 A. The intersect distribution function was directly calculated from the scattering intensities, and the results strongly suggested the presence of needle-like mineral with sharp edges.

Effect of salt on photocycle and ion-pumping of halorhodopsin and third rhodopsinlike pigment of Halobacterium halobium.

The cytoplasmic membranes of Halobacterium halobium contain at least three retinal pigments: bacteriorhodopsin (bR), halorhodopsin (hR), and a third rhodopsinlike pigment (tR). The amplitudes of the phototransient in the photolysis of hR and tR were measured in various salt solutions. Halogen ion (except fluoride) was required to retain the photocycle of hR. Parallels between the amplitude of the phototransient of hR and the magnitude of the photo-induced tetraphenylphosphonium (TPP+) uptake suggests that hR is a light-driven halogen pump, which supports the hypothesis by Schobert and Lanyi (J. Biol. Chem., 1982, 257:10306-10313). The order of effectiveness of halogen was Br- greater than Cl- greater than I-. On the other hand, no specific ion was required to retain the photocycle of tR, and tR was concluded to be nonelectrogenic.
