[Capsular pseudobulbar mutism in a patient of lacunar state].

A patient of mutism with pseudobulbar palsy and frontal lobe syndrome resulting from lacunar state was reported. The patient, a 64-year-old man, was admitted to Gifu University Hospital because of a decrease in spontaneous activity, lack of volition and anorexia. The CT scan, performed on July 29, 1987, demonstrated lacune in the right internal capsule (IC), periventricular lucency especially around the anterior horn of lateral ventricles, and ventricular dilatation. He was transferred to a medical ward because of repeated aspiration pneumonia. Neurological examination revealed mutism, pseudobulbar palsy, and frontal lobe signs. The CT scan, performed on March 30 1988, demonstrated the newly developed lacune in the left IC. The MRI also showed two coinciding lacunes, one in the genu of the right IC and the other in the anterior limb of the left IC. The SPECT with 123I iodoamphetamine showed decreased blood supply predominantly to the frontal lobes. A mechanism by which the mutism occurs is discussed from two points of view, pseudobulbar palsy and frontal lobe syndrome. He developed initially frontal lobe syndrome in which paucity of spontaneous speech was noted. The CT scan at that time demonstrated lacune in the right IC. About eight months later when he became mute, the CT scan showed lacunes in bilateral ICs without any other low density areas in frontal language areas such as Broca's area, subcortical area and supplementary motor area. As the MRI showed that the right lacune was in the genu but the left lacune was in the anterior limb of IC, the left cortico-bulbar tract was thought to be not directly involved. The SPECT showed decreased blood supply predominantly to the frontal lobes. Although dysphagia improved, mutism did not improve at all. Therefore it is postulated that both pseudobulbar palsy and frontal lobe dysfunction might play a role in producing the mutism of this patient.

[Prognostic study of ovarian clear cell carcinoma].

Prognostic studies were performed on 17 cases with clear cell carcinoma of the ovary. The results are follows: 1) The average age was 45.9 years and ranging from 35 to 57 years. 2) Regarding the FIGO clinical stage, all of 5 patients with stage I and 3 of 5 with stage II survived over 2 years, while all of patients with stage III and 2 with stage IV were dead within 2 years. 3) Of 9 patients who had complete resection for primary operation, 8 (89%) survived over 2 years. All of 8 patients who had incomplete resection were dead within 2 years. 4) All cases had been received various chemotherapy; CQ + 5-FU (n = 5), CPM + ADM + CDDP + 5-FU (n = 7), 5-FU + CPM + MMC (n = 2), CDDP (n = 1), CDDP + MMC + 5-FU (n = 1), MCNU + VCR + 5-FU (n = 1), but none of them improved the prognosis of this histological type. 5) With or without administration of CDDP, there was no significant difference in the prognosis. These results suggest that early diagnosis of disease, curative surgical resection and selection of effective chemotherapeutic agents are important things to improve prognosis of clear cell carcinoma of the ovary.

Antibody to poly(ADP-ribose) is an indicator of obstetric complications in pregnant patients with systemic lupus erythematosus.

Anti-poly(ADP-ribose) antibodies were measured by an enzyme-linked immunosorbent assay (ELISA) in 6 pregnant women with systemic lupus erythematosus (SLE), 11 normal pregnant women and 6 randomly selected female SLE patients. Four pregnant SLE patients who had either an abortion after 13 weeks of gestation or a premature delivery showed very high titers of anti-poly(ADP-ribose) antibodies in week 8 of pregnancy. However, the titers of anti-poly(ADP-ribose) antibodies of all the normal pregnant women were similar to those of non-pregnant female SLE patients, being slightly higher than those of normal non-pregnant women. The isotype of anti-poly(ADP-ribose) antibodies in pregnant SLE patients was IgG, which did not crossreact with either DNA or cardiolipin. The number of pregnant SLE patients tested was small, but the coincidence of abortion with high titers of anti-poly (ADP-ribose) antibody was very close. Therefore, a high titer of anti-poly(ADP-ribose) antibody in pregnant SLE patients seems useful as an indicator of abortion or fetal distress.

Serum levels and biochemical characteristics of human ovarian carcinoma-associated antigen defined by murine monoclonal antibody, CF511.

The murine monoclonal antibody (Mab) against human common epithelial ovarian carcinoma, CF511, was generated by immunising mice with human fetal tissue extract from early first trimester, followed by booster injection of an ovarian cancer cell line. Mab CF511 recognised the 600 kDa sialylated glycoprotein as different from previously known tumour associated-marker antigens. Distribution of the Mab CF511-recognised antigen (CF511 antigen) in various tissues and sera was investigated. In immunohistochemical analysis, Mab CF511 reacted strongly with tumour cells of ovarian serous, clear cell, endometrioid and undifferentiated carcinoma and partially with those of mucinous carcinoma. Mab CF511 also reacted with breast carcinoma as well as lung carcinoma. In normal tissues, Mab CF511 cross-reacted with only five tissues, namely lung, breast, thyroid gland, fallopian tube and uterus. Serum levels of CF511 antigen were tested by ELISA inhibition using Mab CF511. This assay showed the circulating CF511 antigen levels to be elevated in 25 of 36 sera from patients with various clinical stages of common epithelial ovarian carcinoma compared to three of 47 and three of 111 sera from patients with other benign gynaecological diseases, including ovarian cysts, uterine fibroids with or without endometriosis and normal healthy subjects, respectively. For the relation between antigen levels and clinical stages of common epithelial ovarian carcinoma, greater than 34.0% ELISA inhibition was detected in 100% of patients with advanced stages (FIGO III and IV) compared with in 35.3% with early stages (FIGO I and II) patients. While patients with breast carcinoma (100%) and lung carcinoma (100%) also had elevated circulating CF511 antigen levels, patients with hepatoma, colorectal carcinoma and gastric carcinoma had no detectable elevation of antigen. Although the test showed a high degree of specificity, the detection of an elevated CF511 antigen level would not be so helpful in distinguishing patients with ovarian carcinoma from those with either breast carcinoma or lung carcinoma. These data suggest that CF511 antigen is a useful new ovarian tumour marker for diagnosis and management of the disease.

An adult case of psychogenic alopecia universalis.

A 39-year-old male with psychogenic alopecia universalis is reported to be a rare adult case. There are two characteristic features which are the reactive depressive state and hair loss on the head, eyebrows, axilla and genital organ. Discussions on the psychogenesis of these findings are as follows: The first is that the psychic escape from social life may be represented as the hair loss. And the second is that the depressive and anxious states as a result of his suffering from the initial alopecia areata which brought him changes in the physical image exaggerate the skin findings which lead to alopecia universalis.

Quantitative estimation of compliance of human systemic veins by occlusion plethysmography with radionuclide--methodology and the effect of nitroglycerin.

Volume-pressure relationship and compliance of human systemic veins were estimated quantitatively and noninvasively using radionuclide. The effect of nitroglycerin (NTG) on these parameters was examined. Plethysmography with radionuclide (RN) was performed using the occlusion method on the forearm in 56 patients with various cardiac diseases after RN angiocardiography with 99mTc-RBC. The RN counts-venous pressure curve was constructed from (1) the changes in radioactivity from region of interest on the forearm that were considered to reflect the changes in the blood volume of the forearm, and (2) the changes in the pressure of the forearm vein (fv) due to venous occlusion. The specific compliance of the forearm veins (Csp.fv; (1/V).(delta V/delta P] was obtained graphically from this curve at each patient's venous pressure (Pv). Csp.fv was 0.044 +/- 0.012 mmHg-1 in class I (mean +/- SD; n = 13), 0.033 +/- 0.007 mmHg-1 in class II (n = 30), and 0.019 +/- 0.007 mmHg-1 in class III (n = 13), of the previous NYHA classification of work tolerance. There were significant differences in Csp.fv among the three classes. The systemic venous blood volume (Vsv) was determined by subtracting the central blood volume, measured by RN-angiocardiography, from total blood volume, measured by the indicator dilution method utilizing 131I-human serum albumin. Systemic venous compliance (Csv) was calculated from Csv = Csp.fv.Vsv. The Csv was 127.2 +/- 24.8 ml.mmHg-1 (mean +/- SD) in class I, 101.1 +/- 24.1 ml.mmHg-1 in class II and 62.2 +/- 28.1 ml.mmHg-1 in class III. There were significant differences in Csv among the three classes. The class I Csv value was calculated to be 127.2 +/- 24.8 ml.mmHg-1 and the Csv/body weight was calculated to be 2.3 +/- 0.7 ml.mmHg-1.kg-1 of body weight, which was very close to the values for Csv reported previously by other investigators in animal experiments. The administration of NTG caused the RN counts - venous pressure curve to become steeper and shift upward. It increased Csv significantly in all cases.

An improved sensitive assay method for the heterogeneity of alpha-foetoprotein: possible application for early differential diagnosis.

A combination of lectin-affinity electrophoresis and antibody-affinity blotting was used for the qualitative determination of molecular species of alpha-foetoprotein. Concanavalin A and erythrophytohaemagglutinin were used as lectins. This method was able to quickly discriminate between the molecular species of alpha-foetoprotein of recurrent ovarian yolk sac tumour and those of non-malignant liver diseases and cord sera at term, using only 3 microliters of sera containing 200 micrograms/l of alpha-foetoprotein. The results indicate that this assay method might be useful for the differential diagnosis of recurrent yolk sac tumour from drug- or blood transfusion-induced liver diseases and for the monitoring of the serum alpha-foetoprotein level of patients with yolk sac tumour producing less than 200 micrograms/l of alpha-foetoprotein.

Adenosine triphosphatase in the uterus and duodenum of chicken hens during eggshell formation.

Plasma calcium concentration and uterine and duodenal adenosine triphosphatase (ATPase) activities were determined during shell formation for high (H) and low (L) shell strength lines of hens selected from the last of four consecutive generations. The H and L lines were divided into three groups according to shell formation at 0, 15, and 22 h following oviposition. Plasma total calcium was determined from blood samples collected from the common carotid artery. Activity of ATPase was determined in uterine and duodenal mucosa. Shell strength, shell weight, percentage of shell per egg and shell thickness of the H line hens significantly exceeded those of the L line. During shell formation, no significant fluctuation in plasma calcium levels was observed within a line, but overall mean plasma calcium concentrations were higher in the H line than L line. Uterine ATPase activity increased with time after oviposition in both lines, with that of the H line being greater. Duodenal ATPase activity of H line hens remained fairly constant throughout the period, but this value showed fluctuations in the L line hens. It thus appears that laying hens with high and low shell strength may vary in their ability to use calcium for shell formation.

[The analysis of the difference in anti-cancer drug sensitivity of 3 clones separated from bladder cancer cell line].

One of the major problems with cancer chemotherapy is the development of drug resistance during treatment. Two mechanisms are considered as the cause of drug resistance, natural and acquired. It is now considered that cancers can be composed of multiple clonal subpopulations of cancer cells. In this study, we separated three clones (C1, C3 and C8) from NBT-2 (human bladder cancer cell line) by limiting dilution. We examined the growth rate and the transplantability to nude mice and performed chromosomal analysis of three clones. The doubling time of C1 is 22 hours, and those of C3 and C8 were 25 and 36 hours, respectively. Each clone was transplantable to nude mice, but we could not find out any histological difference among them. The chromosome numbers of C1 was 66, and those of C3 and C8 were 68 and 63, respectively. We could also find out karyotypic difference among them. We could therefore consider that these three clones had different biological features and studied the difference in drug sensitivity among these three clones and the parent cell line. Cells (1 x 10(4)/well) were incubated in microplates with ten different chemotherapeutic agents for 72 hours. Then 3H-thymidine (1 microCi/ml) was added to each. After 24 hours, cells were harvested and the uptake of 3H thymidine was counted with a liquid scintillation counter. According to the reaction pattern, these chemotherapeutic agents were divided into three groups. 1. The radio isotope uptake of three clones and parent cell line was proportionally inhibited by increasing the drug concentration (carboplatin, (glycolato-o, o-) diammine platinum (II), ifosfamide).(ABSTRACT TRUNCATED AT 250 WORDS)

[Production of monoclonal antibody to renal cell carcinoma].

We report a novel way of obtaining a monoclonal antibody to renal cell carcinoma (RCC). BALB/c mice were immunized with RCC cells (ACHN, ATCC CRL1611) and hyperimmunized spleen cells were fused with Sp/2 murine myeloma cell line by PEG 2000. Many hybridoma supernatants were screened by enzyme linked immunosorbent assay (ELISA). After the cloning by limiting dilution, we established a hybridoma reactive to RCCs and named it A25. It belonged to the IgG1 subclass of immunoglobulins. According to our results using ELISA, 4 of 5 RCC cell lines were reactive to A25, while the remaining 23 non RCC cell lines did not react. The supernatant from A25 was used as a primary antibody preparation for avidin-biotin complex immuno peroxidase staining of multiple cases of RCC, normal tissue, and other tumors. This antibody reacted with 3 of 3 grade 1 RCC, 10 of 11 grade 2 RCC and 0 of 3 grade 3 RCC. Proximal tubules of the kidney shared this antigen. However, cross reactivity of this antibody was observed to pyloric glands of the stomach and adenocarcinoma of the colon. The epitope of A25 seemed to originate from normal kidney tubules. Low grade tumors preserved this epitope well, but this character of the original tissue seemed to disappear as tumor grade increased.

Prostaglandin A and J: antitumor and antiviral prostaglandins.

Prostaglandin(PG)s of the A and J series are categorized as antitumor, antiviral PGs. They have a reactive alpha, beta-unsaturated carbonyl in the cyclopentenone ring, which could be the active moiety. They are actively incorporated into cells and in part transferred to the nuclei, and lead to cell cycle arrest at G1 or cell death depending on the dose.

[Prognostic factors of common epithelial ovarian cancer treated by surgery and cisplatin based combination chemotherapy].

Retrospective analysis of prognostic factors in 171 patients who had common epithelial ovarian cancer (WHO) and treated by surgeries and cisplatin based combination chemotherapies were performed by survival assay and multivariate analysis. In FIGO stage 3, the estimated parameter values were in following order: residual tumour age grade performance status histological type. On analysing Stage patients, histological grading and histological typing had an effect on prognosis. Patients with grade 2 or tumours had a worse prognosis than did those with grade 1 tumours, and patients with clear-cell carcinoma or undifferentiated adenocarcinoma showed a poor prognosis.

Detection of a novel antigen (1G12) on the surface of peripheral blood mononuclear cells from cancer patients by a sensitive radioimmunoassay. A possible marker for the early diagnosis of cancer.

A murine monoclonal antibody designated 1G12 has been produced by immunizing mice with A549 human lung adenocarcinoma. Using radiolabeled monoclonal antibody 1G12, a sensitive radioimmunoassay to detect 1G12 antigen has been developed. The antigen was detected on the surface of peripheral blood mononuclear cells (PBMC). A study was performed to examine the frequency of distribution of this antigen on the surface of PBMC from healthy donors and patients with cancer. An antigen level of above 30 units per 1 x 10(6) PBMC was considered positive. None of 41 healthy donors (0%) and 16 of 96 patients (16.7%) with benign diseases of the lung, ovary, and uterus were positive. In contrast, 27 of 41 patients (65.8%) with lung cancer, 14 of 18 patients (77.8%) with ovarian cancer, and 16 of 27 patients (59.2%) with uterine cervical cancer had elevated levels of 1G12 antigen. When patients were grouped by stages of cancer, PBMC from patients in relatively early stages (stages I and II) also gave positive results, i.e., 9 of 17 patients (59.2%) with lung cancer, 6 of 10 patients (60%) with ovarian cancer, and 11 of 22 patients (50%) with uterine cervical cancer of these stages were positive. These results suggest that the detection of 1G12 antigen on PBMC of patients may be useful for early diagnosis of cancers.

[Response of cardiac patients to dynamic exercise: analysis with "systolic" pressure-volume area].

In the present study, we constructed a left ventricular pressure-volume loop from catheterization data and RN-angiocardiography. By connecting the point of origin of the plane, not the point of Vd, with the end-systolic point of the loop, we calculated a "systolic" pressure-volume area (PVA), elastic potential energy (EPE), and stroke work (SW). A cardiac output (CO)-pulmonary artery wedge (PAW) pressure plot was constructed at rest and during exercise, using a bicycle ergometer, to evaluate cardiac pump function. End-systolic volume (ESV) was obtained from the left ventricular P-V loop, and the response of ESV to exercise was investigated. Thirty-five patients with various cardiac diseases were categorized in two groups according to the delta CO/delta PAW obtained from the shift of the CO-PAW plot during exercise; group I (n = 28) with the properly functioning hearts having the ratio greater than 0.12 l/min/mmHg and group II (n = 7) with the poorly functioning hearts having the ratio less than or equal to 0.12 l/min/mmHg. Group I was subdivided further into two subsets according to the changing pattern of ESV during exercise; group I-A with decreased or unchanged ESV (n = 20) and group I-B with increased ESV in which end-diastolic volume (EDV) was increased during exercise (n = 8). During exercise, PVA and SW were unchanged but SW/PVA increased in group I-A, suggesting improvement of external mechanical efficiency. SW/PVA was unchanged in group I-B, despite the increase in PVA and SW. This suggested that external mechanical work increased as a result of increased cardiac oxygen consumption. In group II, PVA increased, SW was unchanged and SW/PVA decreased, which could be explained by the mechanism that external mechanical work during exercise decreased as compared with that at rest. It was suggested that a different mechanism may have been responsible for the production of external mechanical work among patients in group I-A and in group I-B with properly functioning hearts judged from the CO-PAW plot. Therefore, it seems useful to calculate PVA, SW and SW/PVA during exercise using the left ventricular pressure-volume loop for evaluating cardiac function.

[The evaluation of CA54/61 in the diagnosis of ovarian cancer--with special reference to the cooperative results of 5 institutes].

The clinical usefulness of a newly developed one-step enzyme-immunoassay, MKS-15, in the diagnosis of ovarian cancer was evaluated with 1,563 serum samples collected at five institutes. In this assay, two monoclonal antibodies, MA54 and MA61, which both recognize the carbohydrate chain in a high molecular weight mucin-type glycoprotein, were used as the immobilized antibody on beads and peroxidase-labeled antibody, respectively. When the cut-off value was set at 20 IU/ml (mean + 3SD), 56% of the sera from ovarian cancer patients were positive, and the positive rates for serous, mucinous, mesonephroid, endometrioid, undifferentiated, and metastatic cancers were 63%, 55%, 52%, 38%, 55%, and 62%, respectively. In sera from patients with benign ovarian tumors, only 4% of the cases were positive, but 7% of benign and low potential malignant cases of mucinous cystadenoma were positive, indicating that CA54/61 cannot distinguish benign mucinous cystadenoma from malignant ones. Since the positive rate for CA125 is rather low in mucinous cyst-adenocarcinoma, CA54/61 may be of clinical value.
