CNS changes in three cases of neuro-Behçet's disease were observed by computed tomography (CT), magnetic resonance (MR) and single photon emission computed tomography (SPECT). The present study illustrates the reversibility of lesions in the brain stem with MR, while CT failed to show any abnormal findings in the region of the disease. We conclude that MR is a quite useful method to detect lesions in the brain stem and to evaluate the effects of treatment in neuro-Behçet's disease. SPECT is also an important method for the evaluation of dementia recognized in neuro-Behçet's disease.
Two glucose clamp techniques were performed for 6 h starting 2 h before the initiation of feeding to investigate the effect of dietary propionate supplementation on insulin responsiveness to glucose and tissue responsiveness to insulin in relation to feeding in rams. The rams were fed alfalfa hay without (Cont diet) and with 10 mmol.kg BW-1.d-1 of calcium propionate (Prop diet) for 4 weeks in randomized order. With the hyperglycemic clamp, the ratio of plasma insulin increment to glucose infusion rate did not differ between the diets, but for the Prop diet the ratio was less during the pre-feeding period. With the hyperinsulinemic euglycemic clamp, the glucose infusion rate was lower (P < 0.05) for the Prop diet than the Cont diet, and increased (P < 0.05) after feeding. In rams supplemented propionate tissue responsiveness to insulin was reduced. Propionate supplementation may either impair glucose utilization in response to insulin infusion or enhance glucose production from propionate.

Fetal rat calvarium in an optimum (CMRL-1066) culture medium (with respect to amino acids. Tween 80, DNA precursors and hydrogen acceptors used in oxidative metabolism) produces new lamellar bone. The explanted osteoprogenitor cells survive, proliferate on living membrane bone surfaces and remodel membrane bone into lamellar bone. In the same culture medium, calvarial connective tissue and osteoprogenitor cells grow out of the membrane bone interstices onto a substratum consisting of bone matrix gelatin (BMG) and differentiate not into bone but into hyaline cartilage. Outgrowths from either bone or muscle onto a substratum of BMG prepared from bone autodigested in neutral buffer solutions, produce only fibrous connective tissue. In BGJ, a culture medium containing suboptimal ingredients for cell proliferation, calvarial bone produces neither lamellar bone nor new cartilage but is gradually resorbed and replaced by fibroblasts. In CMRL culture medium, outgrowths of mesenchymal-like cells from muscle onto a substratum of living membrane bone produce an epiphyseal plate-like columnar deposit of new hyaline cartilage. These observations suggest that the function of BMG is to evoke mesenchymal cell differentiation into prechondroblasts during the latent or migratory morphogenetic phase while the effect of the culture medium is to provide the bionutritional requirements for synthesis of hyaline cartilage matrix by chondrocytes during the patent phase of development.
Clinical effects of doxifluridine (group A, 600 mg/body/day; group B, 800 mg/body/day) combined with radiotherapy and immunotherapy were evaluated in patients with advanced cancer of the uterine cervix. Response rates were 84.2% (16/19 patients) in group A and 100% (18/18 patients) in group B, respectively (p=0.230). There was no significant difference in adverse reaction incidence between the methods but significantly higher grade adverse reaction were observed in group B than in group A (p=0.048). Time to progression (TTP) was longer in group B than in group A (p=0.081). The optimal 5'-DFUR dose was 800 mg/body (group B), by which higher grade adverse reactions were fully controlled and TTP was prolonged.
BACKGROUND: Small patient numbers in phase I trials may result in a safe but ineffective dose being recommended for phase II trials. A phase II dose escalation study may identify a dose that is both safe and effective. The Japanese phase I recommended dose of 60 mg/m2 of docetaxel (Taxotere) had been ineffective in phase II trials in ovarian carcinoma. PATIENTS AND METHODS: Patients previously treated with one platinum-based regimen for ovarian cancer received docetaxel (Taxotere) every 3 weeks. The first dose tested was 70 mg/m2. If none of the first 5 evaluable patients responded, the dose was increased. If at least one patient responded, 10 more patients were enrolled. Also, if fewer than 3 of these first 15 evaluable patients responded, the dose was increased. If at least 3 patients responded, another 15 patients were scheduled to be enrolled to confirm efficacy. Unacceptable toxicity in 4 of 5, or 10 of 15 patients would stop escalation. RESULTS: Dose escalation from 70 mg/m2 was not required because responses were noted with acceptable toxicity levels. Overall response in 25 evaluable patients treated at 70 mg/m2 was 24.0% (95% CI = 9.4-45.1%). CONCLUSION: Docetaxel 70 mg/m2 without premedication was identified as a safe and effective dose. Further testing of the phase II dose escalation design is worthwhile.
Bone morphogenetic activity is transmitted from the residue of a collagenase digest of bone matrix gelatin not only across cellulose acetate membranes but also through an interstitial fluid filled duplex diffusion chamber (a distance 300 + 2,000 mum). Collagenolysis enhances dissociation of the bone morphogenetic property of bone matrix and dissemination among mesenchymal cells proliferating in the host bed surrounding the diffusion chamber. The bone morphogenetic response is associated with secretion of interstitial fluid, enzymes, and fibrin as well as formation of new collagen fibrils beaded with coarse ruthenium red granules in the pores of the cellulose acetate membrane. Membranes with pore sizes too small to accommodate either new collagen fibrils or mesenchymal cell microvilli do not transmit the morphogenetic response.
Bone matrix gelatin labeled with radioactive glucose, cysteine, proline, tyrosine or tryptophan was enclosed in one chamber while an adjoined second chamber was left empty to become filled with interstitial fluid. The paired diffusion chambers were implanted into the anterior abdominal wall of allogeneic rats for 14 days. Release of the matrix gelatin (BMG) from the loaded chamber was followed by a bone formation on the outer membrane of the unloaded chamber. 3H-cysteine labeled components of BMG were disseminated into the interstitial fluids and throughout the host tissues 5 times more rapidly than 3H-proline labeled BMG. The relative rates of loss of radioactivity from the residue was cysteine greater than tryptophan greater than tyrosine greater than proline. Electron microscopic autoradiography revealed random dissemination of collagenous and non-collagenous derivatives of BMG. 3H-glucose distribution was coordinated with ruthenium red staining coarse granules and may have originated from BMG. A component of interstitial fluid staining positively with Coomassie brilliant blue and PAS methods was identified by SDS polyacrylamide gel electrophoresis of interstitial fluid aspirated from empty chambers attached to chambers loaded with bone matrix gelatin. The character of the bone morphogenetic property in bone gelatin has not yet been defined in physico-chemical terms but present evidence points to the view that it is associated with a protein molecule.
In a series of 33 bone graft operations, antigen-extracted, autolyzed, allogeneic (AAA) bone matrix gelatin was substituted for autologous bone. The period of follow-up was 2.0 to 3.5 years. AAA bone gelatin was resorbed more rapidly than whole bone. AAA bone gelatin was replaced by new bone in the same intervals of time as observed with autologous bone. In treatment of bone tumors with AA bone gelatin, the results of the operation depend upon the nature of the pathologic processes in the host bed. In normal host bed, tight contact between implant and recipient bone is essential for success. The overall results of a preliminary study of 33 cases of 91% successful.
