[A case of ruptured aneurysm of the profunda femoris artery treated successfully by transcatheter embolization].

A 55-year-old man with ruptured aneurysm of the profunda femoris artery was treated successfully by transcatheter embolization. The selective angiography of profunda femoris artery following embolization disclosed that the aneurysm was occluded completely on both its distal and proximal sites, and blood flow was preserved just proximal to the aneurysm. We concluded that this new, less invasive method of transcatheter embolization in a patient with aneurysm of the profunda femoris artery may be a treatment of choice.

Amphetamine-induced myoglobinuric acute renal failure.

A 36-year-old man was admitted because of sopor and dark urine after intravenous amphetamine injection. He subsequently developed myoglobinuria and acute renal failure. Serum myoglobin value was remarkably elevated to 83,000 ng/ml, and urine myoglobin was 400,000 ng/ml. Renal biopsy revealed tubular degeneration and tubular obstruction with myoglobin casts by immunofluorescence examination. Amphetamine-induced rhabdomyolysis was suspected to cause myoglobinuric acute renal failure.

Giant serpentine aneurysm in a long-term hemodialysis patient.

A long-term hemodialysis patient with a giant intracranial vascular channel, which has been called a giant serpentine aneurysm, is presented. A 50-year-old man with an eight-year history of hemodialysis treatment was admitted because of headache, nausea and double vision. Computed tomographic scans and nuclear magnetic resonance revealed intracranial abnormal shadow. The left vertebral arteriography showed that the distal portion of the left vertebral artery was dilated to 17 mm in diameter. The basilar artery showed a large tortuous vascular channel and globular aneurysms over 25 mm in diameter. This giant serpentine aneurysm is a rather rare disease. To our knowledge, it has not been reported as a complication in a hemodialysis patient, although fourteen cases have been reported in the literature. In our case, several conditions such as long-term hypertension, hyperlipidemia, hypercalcemia, atherosclerosis and abnormal blood flow due to arteriovenous fistula for hemodialysis treatment might be considered to play a role in the formation of the giant serpentine aneurysms.

Behçet's disease associated with aortic and mitral regurgitation: report of two cases.

Two rare cases of complicating valvular lesions in Behçet's disease are reported. Case 1: A 45-year-old man with combined aortic and mitral regurgitation had perforation of both valves. Six months after aortic valve replacement (AVR) and mitral valvuloplasty, an aortic paravalvular leak developed again, and was treated by surgery using an artificial graft with a prosthetic aortic valve. Case 2: A 40-year-old man with congestive heart failure and inflammatory signs had aortic and mitral regurgitation. Although cardiac medications for heart failure and treatment with methylprednisolone were started, he eventually had successful aortic valve replacement. Valvular disease complicating Behçet's disease is rare; only 15 cases, including our two, have so far been reported. Its rarity is partly because Behçet's disease predominantly involves the venous system, not the arterial system.

Malignant mesenchymoma of the bladder.

A case of malignant mesenchymoma of the bladder, containing fibroleiomyomatous, myxomatous, osteoid, and cartilaginous components is presented. The primary pedunculated tumor measuring 18 X 10 X 9 cm and weighing 934 g, arose from the anterior wall of the bladder and was treated by partial cystectomy. Six months after surgery, cystoscopy revealed a recurrent tumor situated at the dome of the bladder. The patient received surgery, chemotherapy, and irradiation for recurrent tumor, but died of the disease 21 months after the first operation. Autopsy revealed extensive local recurrence with wide-spread metastases of malignant mesenchymoma of the bladder and an incidental finding of coexisting clear cell carcinoma in the right kidney (pTl, NO, MO). There have been no previous reports of association of malignant mesenchymoma of the urinary bladder and renal cell carcinoma.

Nuclear magnetic resonance imaging of a benign cystic teratoma in the retroperitoneum.

A retroperitoneal teratoma arising from the vicinity of the upper pole of the right kidney in an 18-year-old woman is described. The plain anteroposterior abdominal radiographs showed irregular calcific shadows with tooth-like calcifications within the right upper quadrant of the abdomen. The findings of a primary cystic teratoma in the retroperitoneum examined by computerized tomography and magnetic resonance imaging of the abdomen are presented. Sagittal and coronal magnetic resonance imaging scans were useful to determine the extent of the mass, and to distinguish the mass from adjacent structures and organs.
