[Acute pulmonary thromboembolism after coronary artery bypass grafting].

The cases of acute pulmonary thromboembolism after coronary artery bypass surgery have been rarely reported in Japan. We, however, experienced 2 cases of them and lost them against intensive therapy. In Mitsui Memorial Hospital, 6 cases of pulmonary thromboembolism had been diagnosed in 1990 (0.074% in admission patients), four of the cases had occurred following surgical procedures. Its manifestations are deceivingly nonspecific and are difficult to distinguish perioperative myocardial infarction, pericardial tamponade, and pneumothorax, especially in postoperative states. We would like to emphasize that acute pulmonary thromboembolism is getting important problems also in Japan.

[A case of successfully removed posttraumatic high flow dural arteriovenous fistula in the posterior fossa].

A 49-year-old male patient was admitted to Ryukyu University Hospital complaining chiefly of progressive loss of mental activity for one year. He had a history of head trauma at the right retromastoid region when he was 24. Generalized convulsions developed three years later, and left exophthalmos, facial varix and impairment of visual acuity developed seven years later. Dural arteriovenous fistula of the posterior fossa was diagnosed at the age of 32, and feeding EC and tentorial arteries were successively ligated on the right several times without any effect. Angiography during this admission revealed tremendous collateral flows; a marked dilated tortuous occipital artery fed from the right vertebral artery, meningeal branches of VA and PICA, the marginal tentorial artery, and the posterior temporal artery from MCA, PCA were drained into the right transverse sinus. But transverse sinuses were occluded bilaterally, and venous outflows were directed to the superior sagittal sinus retrograde via the ascending cortical vein, Trolard veins, and sphenoparietal and cavernous sinuses. The final drainer was the superior ophthalmic vein on the left. Normal deep veins were not visible. In park bench position, the nidus was totally resected with a part of the transverse and thrombosed sigmoid sinus. Postoperative course was uneventful, and an angiogram showed complete disappearance of the AVF. Dural AVG in the posterior fossa with characteristics such as high flow, and which is rich in collaterals following palliative treatment indicates that total surgical resection should be undertaken.

[Surgical treatment of PDA in the elderly patient accompanied with aneurysm of the main pulmonary artery].

A case of the elder PDA with aneurysm of the main pulmonary artery is reported. The patient was a 59-year-old female who had been suffering from severe heart failure. The PDA was closed directly from inside of the pulmonary artery using balloon catheter under usual cardiopulmonary bypass with limited circulatory arrest, and the aneurysm was repaired by aneurysmorrhaphy. The postoperative course was uneventful and she was discharged after 34 postoperative days. The pathological finding of the aneurysm of the main pulmonary artery is a cystic mucoid degeneration like a finding of Marfan syndrome. The etiology of the aneurysm of the main pulmonary artery seems to be extended Ao-PA shunt besides somewhat congenital change of the pulmonary artery.

[The surgical treatment of aortic regurgitation with aneurysm of the ascending aorta].

The surgical treatment of the aortic regurgitation with aneurysm of ascending aorta has been considered by Wheat, Bentall and Cabrol as improved combined techniques. 24 cases of the aortic regurgitation with aneurysm of ascending aorta that had been operated in Mitsui Memorial Hospital from November 1975 to August 1990 were examined their late results with the classification of 3 types of operative techniques (aortic valve replacement alone, Bentall's procedure, and Cabrol's procedure). 5 cases of them with aortic valve replacement alone were elected by their aortic angiographic findings, were not significant differences in their late results in the other cases with Bentall's or Cabrol's procedures. So we considered that treatment of the ascending aortic aneurysm with aortic regurgitation would not be necessary to operate with the combined techniques, but their selection of the indication would be limited in the cases that were not widened in aortic ring diameter in their preoperative angiography, and not complicated the systemic diseases such as Marfan's syndrome.

[The perioperative management of dialysis for patients undergoing coronary artery bypass surgery with chronic renal failure].

Several methods of dialysis have been employed to maintain the perioperative water-electrolyte balance caused by the disorders with chronic renal failure. We have experienced 13 cases of coronary artery bypass surgery with chronic renal failure, and employed hemodialysis (HD) in 5 cases, hemodialysis with extracorporeal ultrafiltration method (HD + ECUM) in 5 cases, continuous ambulatory peritoneal dialysis (CAPD) in 3 cases for perioperative management. The perioperative changes of the circulatory blood volume and the fluid-balance, were assumed by positive reaction with varied over 60 mmHg in systolic blood pressure or demanded over double dose of catecholamines in each observed terms until the next day of the patients extubated. The cases with HD or HD + ECUM have changed the blood pressure more frequently (HD cases = 21%, HD + ECUM cases = 19%) than the cases with CAPD (CAPD cases = 3%). We conclude that the each methods are available to manage perioperative dialysis to undergo coronary artery bypass grafting should be employed with some techniques, CAPD will be the most favorable method to maintain the fluid balance stably for patients with severe compromised cardiac function.

[A simultaneous operation of CABG and bilateral femoropopliteal bypass: report of a successful case].

A 62-year-old man with both coronary artery disease and leg ischemia was successfully treated with a combined revascularization procedure. Coronary artery bypass grafting (CABG) was carried out using in situ left internal thoracic artery and right gastroepiploic artery grafts, and bilateral femoropopliteal bypass grafting were performed reversed saphenous vein grafts simultaneously. The patient recovered well and experienced neither angina nor intermittent claudication.

[Spontaneous aortic cusp rupture].

A 54-year-old man with sudden onset of aortic regurgitation due to aortic cusp rupture is presented. Urgent operation was undertaken and mitral valve replacement was performed with good post-operative course. Intraoperative examination revealed the detachment of right coronary cusp, prolapsing into left ventricle. There was no findings suggestive of bacterial endocarditis. Histological examination of excised valve revealed non-specific chronic inflammatory changes with focal area of myxomatous changes.

[Improvement of mediastinal shift after left pneumonectomy by injection of sulfur hexafluoride].

We encountered a 53-year-old patient with a carcinoid in the left main bronchus. His left lung volume was decreased and the mediastinum was markedly shifted to left. After sulfur hexafluoride (SF6) was injected into the postpneumonectomy pleural space, the mediastinum gradually moved to the center. As reported by many authors, SF6 is generally used after pneumonectomy to prevent mediastinal shift and overinflation of the remaining lung. However, in our patient, SF6 injection reversed mediastinal shift and improved the overinflated lung.

[Late results of TVSI for Ebstein's anomaly].

For 10 adult cases of Ebstein's anomaly, tricuspid valve supraannular imposition (TVSI) without excision of native tricuspid valve were performed and its late results (6 mo. to 11.5 y, mean 6.8 years) were investigated. All cases showed the severe low output syndrome after the surgery. But the postoperative recovery of the cardiac function was smooth and there was no early death nor A-V block. Both the A-V conduction and the RV subvalvular structures were preserved in TVSI and it contributed to the postoperative good contractility of RV. In two cases, the serious postoperative ventricular tachycardia or ventricular fibrillation were experienced, one case required open cardiac massage and 6 hours long assist circulation in the other. In late stage, one patient died 9 years after the TVSI from the failure of the bioprosthetic mitral valve which had been implanted simultaneously, and the other 9 survivors returned to the class I of NYHA classification. LVDd and DS obtained by echocardiography showed significant enlargement; 29 +/- 4.5 mm and 19 +/- 4.1 mm before operation to 42 +/- 3.1 mm and 29 +/- 5.8 mm in late stage respectively. EF showed the tendency of slight decrease from the preoperative supernormal value to the normal range. RVD showed significant decrease after the surgery (58 +/- 4.2 mm to 37 +/- 4.1 mm). Bioprosthetic valve failure in tricuspid position was not experienced. PAC or PVC were seen quite often and 8 of 9 cases received medication for arrhythmia even in late stage.(ABSTRACT TRUNCATED AT 250 WORDS)

[Simultaneous operations of coronary artery bypass grafting (CABG) using right gastroepiploic artery (rt. GEA) and cholecystectomy for silent gallstone detected preoperatively: a report of two cases].

We report two cases simultaneously operated on CABG using rt. GEA and cholecystectomy for silent gallstone detected by routine preoperative examinations. We prefer that two different procedures are performed simultaneously even if gallstone is asymptomatic, for about 50% of silent gallstone is said to become symptomatic within further five years in the literature and the pedicle of rt. GEA may be troublesome during cholecystectomy.

[A case experienced a difficulty in removing a catheter of IABP because of coagula formed in the balloon].

This is a report of a rare complication of intraaortic balloon pumping (IABP). A 71-year-old woman with ischemic heart disease and severe atherosclerosis had experienced a difficulty in removing the catheter of IABP because of coagula formed in the balloon and treated surgically by arteriotomy. The coagula was formed from blood entering into the balloon through a pin hole, which was most likely suspected to be made during insertion procedure or coronary angiography and percutaneous transluminal coronary angioplasty (PTCA) procedure done under IABP.

Antimicrobial peptide, tachyplesin I, isolated from hemocytes of the horseshoe crab (Tachypleus tridentatus). NMR determination of the beta-sheet structure.

The conformation of tachyplesin I, an antimicrobial cationic peptide of 17 residues found in the hemocyte debris of horseshoe crab, was investigated using two-dimensional NMR spectroscopy. The 1H NMR spectrum of tachyplesin I in aqueous solution could be completely assigned, and the secondary structure was substantiated by interpretation of the nuclear Overhauser effect, coupling constant, amide exchange rate, and temperature dependence of the amide chemical shift. Tachyplesin I takes on a fairly rigid conformation constrained by two disulfide bridges and adopts a conformation consisting of an anti-parallel beta-sheet (residues 3-8 and 11-16) connected by a beta-turn (residues 8-11). In this planar conformation, five bulky hydrophobic side groups are localized in one side of the plane and six cationic side groups are distributed at the "tail" part of the molecule (residues 1-5 and 14-17). This amphipathic structure of the molecule is presumed to be closely associated with the bactericidal activity.

Protective effect of recombinant tumor necrosis factor-alpha in murine salmonellosis.

The enhancement of resistance by i.p. administration of murine rTNF-alpha into mice against i.p. challenge with virulent Salmonella typhimurium was studied. Administration of TNF-alpha (5 x 10(4) U/mouse) into mice at 6 or 12 h before the challenge with S. typhimurium organisms enhanced the bactericidal capacity in the peritoneal cavities of the mice. The diminution of the infecting organism in the peritoneal cavities of the TNF-alpha-treated mice was not due to the systemic spread of the organism inasmuch as few organism were recovered from other areas such as the spleen and liver. The TNF-alpha treatment effected a slight increase of neutrophils in the peritoneal cavity, but did not effect an increase of macrophages, including Ia-bearing macrophages. The survival rate of mice infected with Salmonella was improved by the i.p. injection of TNF-alpha before infection. Co-administration of smaller doses of TNF-alpha (5 x 10(3) U) and murine rIFN-gamma (10(2) U) at 6 h before the challenge also effectively enhanced bactericidal activity and protectivity. The cooperative effect of TNF-alpha and IFN-gamma was only seen when these recombinant cytokines were injected together at the proper time before the challenge. Injection of rabbit anti-TNF-alpha serum abolished the effects of TNF-alpha and the cooperative effect of TNF-alpha and IFN-gamma. Furthermore, the serum could abolish the cooperative effect of IFN-gamma and LPS on bactericidal activity, suggesting participation of LPS-induced TNF-alpha in the cooperation.

Effect of murine recombinant interferon-gamma in the protection of mice against Salmonella.

The ability of recombinant murine (rMu) interferon (IFN)-gamma to activate anti-Salmonella-activity in normal mice and beige mutant (bg/bg) mice with Chediak-Higashi syndrome (CHS) was examined. Previous intraperitoneal (i.p.) injection of rMuIFN-gamma (10(4) U per mouse) significantly hindered the bacterial growth in the peritoneal cavities, spleens and livers of the mice after the i.p. infection with Salmonella enteritidis No. 11 strain. It was also effective on the beige mice that have phagocytic cells with a genetically impaired bactericidal function, suggesting that IFN-gamma activates the pathway irrelevant to the beige mutation. The effect was the maximum, when IFN-gamma was given 6 h before the challenge. The effect seemed to be due to the augmentation of bactericidal capacity rather than the prevention of systemic spread of bacteria. Recombinant human IFN-alphaA/D (10(2)-10(6) U per mouse), which produced effects identical to those of murine IFN-beta, did not show such a bactericidal effect. Bactericidal activity enhancement was also seen in mice that had been injected with a small amount of rMuIFN-gamma (10(2) U) and bacterial lipopolysaccharide (LPS) (10 ng) together at 6 h before the challenge, although the IFN-gamma or LPS alone at these doses produced very little if any effect. Bactericidal effect enhancement was seen in mice that had been injected with IFN-gamma at 6 h and LPS at 3 h before the challenge, while it could be hardly seen in mice injected with them in a reversed order.(ABSTRACT TRUNCATED AT 250 WORDS)

Thiazide-sensitive NaCl absorption in rat cortical collecting duct.

The mechanism of transepithelial NaCl transport was investigated in isolated perfused cortical collecting ducts from the kidneys of deoxycorticosterone-treated rats. In the presence of vasopressin, hydrochlorothiazide (0.1 mM) markedly reduced the net rate of Na absorption, Cl absorption, and active fluid absorption but did not significantly change the transepithelial voltage. Similarly, in the absence of vasopressin, hydrochlorothiazide decreased the rate of sodium absorption by 50% without affecting transepithelial voltage. Amiloride (30 microM) completely eliminated the lumen-negative voltage but decreased net sodium absorption only by approximately 50%. In the presence of amiloride, chloride absorption occurred against an electrochemical gradient for chloride, indicating that there was active chloride absorption. Bumetanide (0.1 mM) did not affect chloride absorption or spontaneous fluid absorption in the presence of vasopressin. The combination of amiloride and hydrochlorothiazide inhibited net sodium absorption by a greater extent than did either agent alone. These results demonstrate the presence of the following two parallel sodium transport pathways in cortical collecting ducts from mineralocorticoid-replete rats: 1) an electrogenic pathway blocked by amiloride, which presumably involves an apical sodium channel, and 2) a thiazide-inhibitable electroneutral pathway, which presumably utilizes apical Na-Cl cotransport and mediates secondary active transport of chloride.

Structure-activity relationships of dopamine- and norepinephrine-uptake inhibitors.

Quantitative structure-activity relationship (QSAR) analysis of 3-phenyl-1-indanamines, 1-amino-4-aryltetralins, and 6-phenylpyrrolo[2,1-a] isoquinolines has been performed for catecholamine-uptake inhibition activities. Similar equations were obtained for these series of congeners indicating a common tendency that the increase in hydrophobicity of the substituents on the primary phenyl ring (ring C) enhances the activity, and the important aromatic ring which interacts with the receptor is this ring C. It was also indicated that the effect of the introduction of the second N-methyl group differs depending on the series of congeners. These results were used to characterize a binding model for a pharmacophore, which comprised a phenyl ring and a basic nitrogen. This model defined the necessary three-dimensional features leading to the uptake inhibition, and degree of fitness with this model predicted the strength of the activity. Furthermore, it appeared likely that a substituent existing in a specific region of the inhibitor molecule causes a steric hindrance with the receptor site and reduces the activity.

Feasibility of intraoperative cytodiagnosis of lung cancer.

Of 354 thoracotomies, 114 cases involved intraoperative cytopathologic evaluation. The study included 86 men and 28 women, and 184 specimens were examined. Smears were taken from the lung lesion or lymph node and the chest wall or bronchial stump by imprint or needle aspiration. Intraoperative evaluations were compared with histologic findings. In 85 cases not diagnosed as malignant preoperatively, malignancy was confirmed in 97.6 percent of cases and histologic type in 71.8 percent. In the 28 cases diagnosed as malignant preoperatively, there was only one false-negative. The most important finding during thoracotomy is whether a lesion is malignant or not. The time required to obtain a pathologic diagnosis also is important. We are able to obtain the results of a cytologic diagnosis within 10 min. Cytologic diagnosis of malignancy during thoractomy may be a feasible and convenient method of diagnosis, especially where malignancy is suspected.
