Demonstration of hemoglobin in the kidney of battered children.

We examined specimens of renal tissue taken from 40 cadavers of children (18 battered and 22 non-battered children) by an immuno-peroxidase technique using anti-human hemoglobin (Hb) or anti-human myoglobin (Mb) antiserum. Hb was detected in the proximal tubular cells of the kidney of six battered children who had sustained severe or moderate external injuries, whereas Mb was not found. In contrast, Mb was detected and Hb was not found in the renal tubules of one child who had died from a traffic accident. In other cases including 13 cases of children who died from a sudden bout of physical violence or some other impulsive act and 4 cases of children who had died from malnutrition, neither Hb nor Mb was found in the kidney. Our findings appear to indicate that large quantities of Hb were released from the multiple lesions of the battered children, entered the circulation and were deposited in the kidneys. It thus seems that the immunohistochemical detection of Hb in the kidney may be useful for evidencing the extensive antemortem bruises which are often found in battered children.

[Traumatic intracerebral hemorrhage developing in the apparent course].

The victim, 52 year old man, was thrust down and hit his left occiput against the concrete floor. He was hospitalised and his comatose state continued to the death. On admission, blood pressure was 212/110 mmHg and the computed tomography scan of the head showed only an extensive right subdural hematoma. But the intracerebral hemorrhages in the right frontal, temporal and parietal lobes were recognized 10.5 hours after the trauma. A subdural hematoma was evacuated by operation on the second hospital day. The intracerebellar hemorrhage also appeared 16 hours after the trauma. Blood pressure fluctuated between 160/80 and 200/110 mmHg. The photo of CT scan at 38.5 hours after the trauma showed little subdural hematoma and new intracerebral hemorrhage located in the left temporal lobe. On the third hospital day, he was equipped with a respirator and blood pressure was between 132/84 and 242/100 mmHg. The reaction of the pupils to light disappeared on the 8th hospital day. Blood pressure gradually decreased on the 9th and 10th hospital days and he died on the 11th day. Autopsy revealed a bruise in the left occiput, a linear fracture in the frontal and left parietal bones and a small amount of subdural hematoma on the surface of the right cerebral hemisphere. Cortical contusions were found in the right frontal, the both temporal and the left parietal lobes. Intracerebral hemorrhages were found in the right frontal, the both temporal and the right parietal lobes. Intracerebellar hemorrhage was also found. Cardiac hypertrophy and atherosclerosis of the aorta were recognized. We thought that small hemorrhages which were not clearly detectable by CT scan immediately after injury may have developed into massive intracerebral and intracerebellar hemorrhages due to high blood pressure after a hospitalization.

Phenotyping of erythrocyte acid phosphatase and esterase D by high field strength isoelectric focusing on cellulose acetate membrane.

Phenotyping of erythrocyte acid phosphatase (EAP) and esterase D (ESD) by cellulose acetate membrane isoelectric focusing (CAM-IEF) under a nonequilibrium condition is described. In an attempt to improve the method of CAM-IEF, we shortened the electrode distance to provide a higher field strength at a given (low) voltage. Various carrier ampholytes for EAP typing and various chemical separators for ESD typing were also tested. Good separations were obtained after 30 min IEF for EAP typing and 25 min for ESD typing. When applied to blood stains and stored for various periods at room temperature, the stains up to 8 months old could still be phenotyped for EAP and those up to 4 weeks old for ESD. CAM-IEF is suitable for routine forensic work of EAP and ESD phenotyping.

[Diagnosis and significance of forensic neuropathological findings].

Among the diseases of the nervous system, Alzheimer-type dementia, head injuries, brain death and fat embolism were discussed on the pathological diagnosis and medico-legal significance. 1. Alzheimer-type dementia 1) There were marked differences in microscopic findings of ATD group and control group of elderly subjects whose intellects have been maintained. The main histological features of ATD were presence of numerous senile plaques and neurofibrillary changes, and both of them were thought to be helpful diagnostic markers. 2) Double stains by GFAP stain and Congo red stain were performed. By the former stain, typical senile plaques, encircled by reactive astrocytes and penetrated interiorly by their processes, were demonstrated. By the latter stain, amyloid core was well detected. By LCA (leucote common antigen) stain, microglias were thronging about amyloid core of senile plaques. By thioflavin S stain, amyloid core and amyloid angiopathy were well detected and some neurofibrillary tangles showed staining properties of amyloid. 3) Using the autopsy criteria for the diagnosis of Alzheimer's disease reported by Khachaturian, senile plaques were counted on microscopic magnification x200 field. Numerous plaques satisfying thoroughly the criteria were detected. 2. Head injuries Old intracranial injuries with long clinical courses were discussed in the aspects of forensic medicine. It can be often understood from postmortem examinations that alterations of personalities, abnormalities of behaviors, and disturbances of visual fields or other senses resulted from the old lesions of their respective territories and pathways in each brain. In forensic medicine, such observations and discussions are very important to clarify the relations between the old intracranial injuries and causes of death. 3. Brain death In cases with the duration of brain death extending over several days, the breakdown of processes of their cerebral astrocytes were shown by GFAP stain. 4. Fat embolism In 62 autopsied cases of fat embolism, fractured bones and survival times after injuries were summarized in tables. In cases of systemic fat embolism, the gross and microscopic findings of brain and spinal cord were discussed.

[Injuries caused by automobile dragging: a report of three cases].

Three cases of victims who were dragged a long distance by automobiles are reported. Injuries caused by such dragging are discussed. [Case 1] A 50-year-old man was found dead under an automobile after having been dragged for a distance of 530 m. Deep abrasions were found in the right temporal region, both scapular regions, the buttocks, and the heels. There were excoriations toward upper side in the right hypochondriac region along the rib arch, and multiple rib fractures with and without severe hemorrhaging. Alcohol levels were 3.54 mg/ml in the femoral venous blood, and 4.54 mg/ml in the urine. It seemed that the victim was drunk and had been lying on his back and that the automobile's underbody had caught him in his right hypochondriac region and had compressed the thoraco-abdominal regions. [Case 2] A 50-year-old drunken man, who had been lying on his back on the side of a road, was dragged by a car for a distance of 2 km. The victim showed marked and deep abrasions of the back, the buttocks, the lateral surface of both ankles, and on the ulnar surface of the left forearm. In the left forearm, the ulna and soft tissues had been rubbed flat and were blackened. The alcohol levels in the femoral venous blood and in the urine were 3.31 and 4.19 mg/ml, respectively. [Case 3] An 83-year-old woman was found dead on the side of a road. In the right temporal region, the skull had been rubbed flat and the right cerebral hemisphere was exposed and flattened. Severe abrasions were observed on both shoulders, the buttocks, and on the back surfaces of the lower extremities. Her sandals and a broken cane were found on the road 2 km from the scene. The offender has not yet been arrested. Injuries caused by the road surface in traffic accidents usually take the form of excoriations or abrasions, and the three victims of this report showed severe and characteristic injuries. Excoriations or abrasions develop into abrasive injuries, defects of the skin or soft tissue, and to the exposure of the internal organs from continuous, prolonged dragging. Some of these accumulated injuries might have been caused by frictional heat.

Forensic neuropathological and immunohistochemical studies on Alzheimer-type dementia (AD/SDAT).

Alzheimer-type dementia (ATD) was discussed not only from the aspects of forensic neuropathology by using various techniques for general stains and silver-impregnation, but also immunohistochemically by using antiserum against glial fibrillary acidic protein which is a specific marker for astrocytes. There were marked differences in microscopic findings of ATD group and control group of elderly subjects whose intellectual have been maintained. The main histological features of ATD were presence of numerous senile plaques and neurofibrillary changes, and both of them were thought to be helpful diagnosis markers. Double stains by glial fibrillary acidic protein (GFAP) stain using avidin-biotin peroxidase complex (ABC) method and Congo red stain were performed. By the former stain, typical senile plaques, encircled by reactive astrocytes and penetrated interior by their processes, were demonstrated. By the latter stain, amyloid core was well detected. Another method which is particularly sensitive to amyloid, such as amyloid core or amyloid angiopathy, is the thioflavin S stain. Immunohistochemical stains for glial fibrillary acidic protein demonstrated an intense participation within the plaques by processes of reactive astrocytes. Using the autopsy criteria for the diagnosis of Alzheimer's disease reported by Khachaturian, senile plaques were counted on microscopic magnification x200 field. Numerous plaques satisfying thoroughly the criteria were detected. The report by Blessed et al. that there is a highly significant correlation between mean plaque counts in sections of cerebral cortex of elderly subjects and degrees of dementia is very helpful for forensic pathologists and is highly evaluated.

[Statistical studies on suicides in Shiga Prefecture during the 15-year period, 1974 to 1988].

The authors examined 3,335 cases of suicides among the unnatural deaths in Shiga Prefecture during the 15-year period, 1974 to 1988. The results obtained were as follows: The number of suicides in a year amounted to 200 during the period from 1974 to 1982, and increased to 280 in 1983 at maximum. The cases of suicide were more in male than in female. The incidence of suicide was higher in March to July and the peak of incidence was observed in April. The highest frequency of suicides was in 40-49 year age group in male, and 70-79 year group in female. Among various procedures of suicide, hanging was most frequent during the past 15 years in both sexes, and the following procedure was carbon monoxide poisoning in male and drowning in female. As to the occupation, housewives and person without a regular occupation constituted 53.1% of the total suicidal cases, and products makers and laborers were found in 11.1%. With regarded to the causative factors, "suffering from illness" was the most frequent factor, and followed by "insanity or mental disorder", "problems of economic distress", "disagreement with family" and others.

[Hypertrophic cardiomyopathy--examination findings, family histories and results of close examinations of a family in two autopsied cases].

A 55-year-old woman, who died immediately after getting an intramuscular injection of lincomycin for treatment of a cold, and a 14-year-old boy, who died suddenly while practicing soccer, were autopsied respectively and their disease was diagnosed as hypertrophic cardiomyopathy by characteristic pathological findings. Each family history was investigated. Especially, the latter case showed clearly a family line of hypertrophic cardiomyopathy. At the request of the family, his elder brother and mother were examined closely at the University Hospital. Their illness was diagnosed accurately as hypertrophic cardiomyopathy from their findings of electrocardiograms, echocardiograms and endomyocardial biopsies. They are now taking medications and guidances of a doctor as to physical activity and way of life as outpatients at the University Hospital.

[Aortoesophageal fistula by swallowed foreign body--a case report and review of the literature].

A case of aortoesophageal fistula is presented. The etiology, symptoms, and diagnosis of aortoesophageal fistula are reviewed. A 22-year-old nurse died just after massive hematemesis. The autopsy finding revealed massive hemorrhage in the gastrointestinal tract and conspicuously pale organs. The fistula between the thoracic descending aorta and esophagus was observed. Microscopically, acute inflammation and blood clots containing bacterial colonies were found around the fistulous tract. Eight days before her death, she felt retrosternal sticking pain just after eating a baked slice of sea bream. She consulted a family doctor, but no foreign body was discovered in the esophagus with radiography and endoscopy. As a dull pain and slight fever did not disappear and continued intermittently, she often consulted the doctor. A small hemorrhage in the left wall of esophagus was noted 2 days before her death with endoscopy. She was hospitalized over the night and the next day she was discharged and get home. After taking a nap, she felt a syncopal attack in the afternoon. A tarry stool was noticed in the evening. The next morning, she died suddenly in exsanguination. The clinical feature of aortoesophageal fistula is uniform and has been described as Chiari's triad, i.e., the chest pain, the symptom-free interval and the signal hemorrhage, and the fatal hemorrhage. The present case showed the typical symptom of aortoesophageal fistula by swallowed foreign body, though fish bone or any foreign body was not discovered with examinations. Approximately 100 cases of aortoesophageal fistula by esophageal foreign body have been reported since 1818. The present case is the second one reported in Japan as caused by foreign body.

Oxygen exchange between the Fe(IV) = O heme and bulk water for the A2 isozyme of horseradish peroxidase.

Resonance Raman spectra were observed for compound II of horseradish peroxidase A2, and the Fe(IV) = O stretching Raman line was identified at 775 cm-1. This Raman line shifted to 741 cm-1 upon a change of solvent from H2(16)O to H2(18)O, indicating occurrence of the oxygen exchange between the Fe(IV) = O heme and bulk water. The oxygen exchange took place only at the acidic side of the heme-linked ionization with pKa = 6.9.

Resonance Raman evidence for oxygen exchange between the FeIV = O heme and bulk water during enzymic catalysis of horseradish peroxidase and its relation with the heme-linked ionization.

Raman spectroscopic studies of compound II of horseradish peroxidase show that the oxygen atom in the FeIV = O group of the heme is rapidly exchanged in H2O at pH 7.0 but not in an alkaline solution (pH 11.0). This conclusion is based on studies of shift in the FeIV = O stretching mode of compound II in H2(18)O; further studies show that the FeIV = O heme is hydrogen-bonded to an amino acid residue of the protein in neutral solutions but not in the alkaline solution. Deprotonation of this residue takes place with the midpoint pH at 8.8 and accordingly corresponds to the so-called heme-linked ionization. It is concluded that this hydrogen-bonded proton plays an important part in the oxygen exchange mechanism. From this it seems clear that this hydrogen-bonded proton has an essential role in the acid/base catalysis of this enzyme and that alkaline deactivation of this enzyme can be attributed to the lack of a hydrogen-bonded proton at high pH.
