Noninvasive quantification of regional myocardial blood flow and ammonia extraction fraction using nitrogen-13 ammonia and positron emission tomography.

This report describes the theoretical basis and a method to quantitate regional myocardial blood flow (RMBF) and ammonia extraction fraction (E) in man, noninvasively, with N-13 ammonia and positron emission tomography (PET). Two patients with hypertrophic cardiomyopathy, whose left ventricular (LV) walls were markedly thick, were employed in this study to avoid partial volume effects and cross contamination between LV walls and blood pool. RMBF and E were calculated from time-activity curves of myocardial tissue and left atrium derived from serial 6-second PET images of the heart. The time-activity curve of left atrium was used as an arterial input function. The results were RMBF = 67 +/- 4 ml/min/100 g, E = 80 +/- 13% and 65 +/- 10 ml/min/100 g, 81 +/- 16% for each patient. The validity of the present method was discussed.

Kinetics of [11C]N,N-dimethylphenylethylamine in mice and humans: potential for measurement of brain MAO-B activity.

Carbon-11-labeled N,N-dimethylphenylethylamine ([11C]DMPEA) was synthesized by the reaction of N-methylphenylethylamine with [11C]methyl iodide. This newly synthesized radiotracer was developed for the purpose of in vivo measurement of monoamine oxidase-B activity in the brain using a metabolic trapping method. Initially, biodistribution was investigated in mice. The rapid and high uptake of 11C radioactivity in the brain was observed following intravenous injection of [11C]DMPEA, the peak of which was reached at 1 min, followed by a decrease at 1-5 min and slowly thereafter. The kinetics of [11C]DMPEA in the human brain were determined using positron emission tomography (PET) and showed that 11C radioactivity increased gradually over 60 min following initial rapid uptake of 11C radioactivity, with basal ganglia and thalamus showing high accumulation.

[Problems related to tracer concentration and wall thickness: pitfalls in positron CT diagnosis].

Positron emission computed tomography (PET) is regarded an excellent technique for quantitative measurements. However, its accuracy is related to the spatial resolution of the system. The relation between myocardial wall thicknesses as measured by X-ray CT or MRI and the radioactivity as measured using PET was studied in 37 patients. 1. In patients with transmural infarction, the infarcted myocardium was imaged as a region of low radioactivity. However, the myocardium usually exhibited wall thinning, so that partial volume effects must be taken into account in evaluating the radioactivity. 2. In the infarcted regions, the regions of the low radioactivity tended to be larger than those of wall thinning. 3. There were cases with the regional low radioactivity without wall thinning in myocardial infarction and in hypertrophic cardiomyopathy. Because patients with myocardial infarction frequently had regional wall thinning, it seems necessary to correct partial volume effects for the infarcted regions which differ from the normal. It was concluded that, to estimate regional myocardial blood flow or metabolism using PET, it is necessary to supplement another morphological diagnostic method to evaluate myocardial wall thickness.

[Dynamic function studies with positron computed tomography: measurements of regional myocardial blood flow in hypertrophic cardiomyopathy].

A major attribute of positron computed tomography (PCT) is its ability to dynamically measure physiologic processes of the heart. However, quantitative data acquisition and analysis are limited by three major problems: bidirectional activity-cross contamination between the myocardium and the blood, the partial volume effect, and the lower temporal resolution of PCT devices. In the present study, we performed serial six-second PCT scans (fast dynamic study) using 13N-ammonia, and evaluated measurements of regional myocardial blood flow. Six patients with hypertrophic cardiomyopathy (HCM) and five normal subjects were observed. 13N ammonia (4.2-13.0 mCi) was intravenously injected as a bolus, and serial six-second PCT scans were initiated at the times of the tracer injections. The results were as follows: 1. Initial passage of the tracer through the right and left sides of the heart were observed with serial six-second images. 2. In patients with HCM, Myocardial 13N activity was determined by assigning a region of interest (ROI) over the myocardium of greater than 25 mm wall thickness as measured by contrast enhanced CT. In these ROIs, corrections for the partial volume effect and spillover of radioactivity from the blood were not necessary. Arterial 13N activity was determined noninvasively by assigning a ROI over the left atrium. 3. The temporal relationships between the arterial input function and myocardial tracer concentrations were demonstrated. Arterial 13N activity was greatest in the second or third frame, and it rapidly declined thereafter. Myocardial 13N activity increased rapidly, reaching a plateau from the sixth to the ninth frame.(ABSTRACT TRUNCATED AT 250 WORDS)

Automatic radiologic reporting system using speech recognition.

A radiograph report is usually made from an oral dictation by a radiologist, which is then typed. Typing Japanese is rather inconvenient and consumes many hours. In this paper we introduce a computer-assisted reporting system for radiologic images using speech recognition. The hardware of the reporting system consists of a speech recognizer DP-200(NEC) and a personal computer PC-8801 or PC-9801. The DP-200 has the capability of storing 500 different words spoken by a radiologist. At present, three application programs have been designed. These are for the interpretation of a liver scintigram, a bone scintigram and a chest radiograph. Data entry is done by the radiologist at a CRT display terminal in a conversational manner with predefined and predetermined branching. The time required to make a normal report using the liver or bone scintigram system was within one minute. The reporting time was several minutes in the case of an abnormality report. It is suggested that the system is useful for making an imaging report, for constructing the data base for the interpretation of medical images and for the picture archiving and communication system.

Fast dynamic study in cardiac positron CT using 13N-ammonia in man.

Fast dynamic studies with positron computed tomography (PCT) of the heart have been relatively few because of the technical limitations of prevalently used PCT devices. In the present study, we used a high-sensitivity, whole-body multislice PCT device (POSITOLOGICA-II) and performed serial 6-s PCT scans after the intravenous bolus injection of 13N-ammonia in 15 cardiac patients and 5 normal subjects. On the first image (0-6 s), 13N activity was primarily in the right atrium and ventricle. On the third image (12-18 s), it was primarily in the left atrium and ventricle. These blood-pool images permit evaluation of size and configuration of ventricles and atria in cardiac patients and normal subjects. Clearance of 13N activity in the blood pools and lungs occurred primarily during the 1st min. Thereafter, the myocardial images were delineated. In patients with heart failure, delayed clearance of the tracer from the blood pools and lungs was observed. The results indicate that initial passage of the tracer through the heart can be observed with the use of fast dynamic PCT.

Statistical properties of molecular tree construction methods under the neutral mutation model.

The statistical properties of three molecular tree construction methods--the unweighted pair-group arithmetic average clustering (UPG), Farris, and modified Farris methods--are examined under the neutral mutation model of evolution. The methods are compared for accuracy in construction of the topology and estimation of the branch lengths, using statistics of these two aspects. The distribution of the statistic concerning topological construction is shown to be as important as its mean and variance for the comparison. Of the three methods, the UPG method constructs the tree topology with the least variation. The modified Farris method, however, gives the best performance when the two aspects are considered simultaneously. It is also shown that a topology based on two genes is much more accurate than that based on one gene. There is a tendency to accept published molecular trees, but uncritical acceptance may lead one to spurious conclusions. It should always be kept in mind that a tree is a statistical result that is affected strongly by the stochastic error of nucleotide substitution and the error intrinsic to the tree construction method itself.

Visualization of specific binding sites of benzodiazepine in human brain.

Using 11C-labeled Ro15-1788 and positron emission tomography, studies of benzodiazepine binding sites in the human brain were performed on four normal volunteers. Rapid and high accumulation of 11C activity was observed in the brain after i.v. injection of [11C]Ro15-1788, the maximum of which was within 12 min. Initial distribution of 11C activity in the brain was similar to the distribution of the normal cerebral blood flow. Ten minutes after injection, however, a high uptake of 11C activity was observed in the cerebral cortex and moderate uptake was seen in the cerebellar cortex, the basal ganglia, and the thalamus. The accumulation of 11C activity was low in the brain stem. This distribution of 11C activity was approximately parallel to the known distribution of benzodiazepine receptors. Saturation experiments were performed on four volunteers with oral administration of 0.3-1.8 mg/kg of cold Ro15-1788 prior to injection. Initial distribution of 11C activity following injection peaked within 2 min and then the accumulation of 11C activity decreased rapidly and remarkably throughout the brain. The results indicated that [11C] Ro15-1788 associates and dissociates to specific and nonspecific binding sites rapidly and has a high ratio of specific receptor binding to nonspecific binding in vivo. Carbon-11 Ro15-1788 is a suitable radioligand for the study of benzodiazepine receptors in vivo in humans.

[Clinical evaluation of imipenem/cilastatin sodium in children].

Nine pediatric patients with moderate or severe bacterial infections (3 septicemia or bacteremia, 2 pyelitis, 2 tonsillitis, 1 pneumonia and 1 pyothorax) in hospital were treated with MK-0787/MK-0791. The drug was administered intravenously by 30 or 60 minutes drip infusion in 3 or 4 divided doses totalling 24 mg/24 mg-70.2 mg/70.2 mg per kg/day. Clinical effectiveness were excellent in 4 cases, good in 2 cases, poor in 2 cases and not evaluated in 1 case. The overall efficacy rate was 75%. A slight decrease of WBC was observed in 1 case. It was concluded that MK-0787/MK-0791 was a useful antibiotic for the treatment of infections in pediatric practices. Pharmacokinetics of intravenously administered MK-0787/MK-0791 was studied in 2 other cases. The MK-0787/MK-0791 was administered intravenously by 30 or 60 minutes drip infusion to 2 cases at a dose of 10 mg/10 mg/kg. The mean half-life (T1/2) of MK-0787 was 1.03 hours and MK-0791, 0.69 hour. The mean urinary recovery rate of MK-0787 within 6.5-7 hours after administration was 62.5% and MK-0791, 76.7%.
