Morphological characteristics of dermal diabetic microangiopathy.
In an attempt to characterize dermal diabetic microangiopathy, dermal microvessels were qualitatively and quantitatively assessed in 26 healthy control subjects and 47 non-insulin-dependent diabetic patients. Basement membrane thickness (BMT) was significantly greater and the homogeneous BM more frequent in diabetic than in control microvessels. BMT did not change with duration of diabetes but significantly decreased with age and onset age of diabetes, suggesting either a different process of BM thickening with respect to age in diabetic dermal microvessels or the implication of genetic influence on the pathogenesis of dermal diabetic microangiopathy. Endothelial cell hyperplasia was not noted in dermal diabetic microvessels. In view of the fact that the hyperplasia has been reported in other tissues and is closely associated with diabetic microangiopathy, the nature of dermal microvessels or endothelial cells or the milieu around dermal microvessels might be different from other organs.

Morphological study of recombinant human transforming growth factor beta 1-induced intramembranous ossification in neonatal rat parietal bone.
Recombinant human transforming growth factor beta 1 (rhu TGF beta 1) was injected singly or repeatedly for 3-12 days into the periosteum of the right side parietal bone of neonatal rats under the period of bone growth, and the time course of histological changes of the bone was observed by light and electron microscopy and by enzyme histochemistry. The repeated injections of rhu TGF beta 1 at 200 ng/day increased the thickness of the bone tissue on the treated side, which was about twice the nontreated side value after 12-day injections. On the dermal side, preosteoblasts in the periosteum increased in an early stage of treatment, and thereafter, differentiation into osteoblasts, increase of bone matrix, bone marrow cavity formation, and increase of osteoclasts within the bone marrow cavities were observed. Activation of osteoblasts on the dura mater side was also seen. The single injection of rhu TGF beta 1 at 200 ng resulted only in increased osteoprogenitor cell layers and bone matrix formation in an early stage, and the thickness of the osteoprogenitor cell layers and bone tissue at 12 days after single injection was comparable to the values on the nontreated side. At 1 microgram, however, the osteoblasts were activated, and the osteoprogenitor cell layers and bone matrix formation were markedly increased. At 12 days, the bone tissue thickness on the treated side was about twice the nontreated side value, as in the repeated treatment groups.(ABSTRACT TRUNCATED AT 250 WORDS)
Three dimensional imaging of tracheobronchial system using spiral CT.
The purpose of this study was to evaluate the clinical efficacy of 3D reconstruction of the tracheobronchial system using spiral CT. A total of 25 patients with tracheobronchial abnormalities, stenosis (n = 21) and fistula with esophagus (n = 4), underwent a single breathhold spiral CT (5 mm collision, 5 mm)/s increment). With respect to localization, extent and degree of stenosis and size of fistula were compared with findings at bronchoscopy. The CT location and extent of stenoses were consistent with bronchoscopic findings in all 21 patients. The diameter and shape of the lesions were not evaluated in five patients with severe stenoses. In patients with fistula, 3D CT image demonstrates the location and size of fistula in all four patients. Spiral CT serves to demonstrate accurate and useful 3D reconstruction images for planning and monitoring therapy.
Estimation of coronary flow reserve with the use of dynamic planar and SPECT images of Tc-99m tetrofosmin.
BACKGROUND: We developed a noninvasive method to examine coronary flow reserve with technetium 99m tetrofosmin based on the microsphere model. According to the microsphere model, myocardial blood flow (MBF) can be calculated by MBF = q / integral C(t)dt, where q is myocardial activity and C(t) is tracer concentration in blood. Because the ratio of integral C(t)dt at stress to rest is equal to the ratio of the first transit count in the pulmonary artery (PA) and attenuation factors were canceled out, we calculated the increase ratio of MBF (MBF(IR)). METHODS AND RESULTS: After injection of dipyridamole, tetrofosmin was injected as a bolus and serial dynamic planar images were obtained to measure the first transit count in PA (PAC). Myocardial single photon emission computed tomography was performed to measure the regional myocardial count (RMC). MBF(IR) was calculated as [(RMCs x PACr)/(RMCr x PACs) - 1] x 100, where r and s denote resting and stress conditions, respectively. In contrast, the increase in the myocardial uptake ratio (MUR(IR)) was defined as (RMCs x SCr/RMCr x SCs - 1) x 100, where SC is syringe count of tracer. The results were as follows: (1) The mean MBF of healthy subjects was 46.9% +/- 22.8%. (2) MBF(IR) of the infarcted region and ischemic region was significantly decreased (8.3% +/- 12.2% and 11.2% +/- 11.9%, respectively; P <.001). (3) MUR(IR) was significantly lower than MBF(IR) (14.1% +/- 21.2%; P <.001). (4) MBF(IR) decreased according to the heart rate at rest (r = 0.47; P <.05). CONCLUSIONS: MBF(IR) is a potential parameter with which to evaluate coronary flow reserve when the changes of arterial input function during stress are considered.
Laparoscopic cholecystectomy in the elderly: analysis of pre-operative risk factors and postoperative complications.
A retrospective study was conducted of two groups of patients over (group 1, n = 57) and under (group 2, n = 655) the age of 70 years who underwent laparoscopic cholecystectomy (LC). The pre-operative physical status and systemic complications, operation time, postoperative complications, postoperative hospital stay and other clinical features of the two groups were compared. The incidence of pre-operative complications in group 1 was significantly higher than that in group 2 (P < 0.05). Postoperatively no severe complication was found in any patient. Group 1 showed significantly prolonged operation time and postoperative hospital stay compared with group 2 (P < 0.05). The difference between the groups in the intra-operative treatment time and postoperative treatment is attributed to the greater prevalence of common bile duct stone in group 1 as there was little difference between the groups in the postoperative recovery after exclusion of these patients. No pulmonary complications, which are associated with LC, were observed; the postprocedure pain was slight and the period of bedrest was short. If complications associated with pneumoperitoneum can be prevented, this surgery is an excellent measure to improve the quality of life of even elderly patients with cholecystolithiasis.
Role of transforming growth factor-beta 1 in glomerulonephritis.
The role of transforming growth factor-beta 1 (TGF-beta 1) in human glomerulonephritis is still poorly understood. The relationship between expression of TGF-beta 1 protein or TGF-beta 1 mRNA and tissue damage was investigated using the enzyme-antibody and in situ hybridization method in frozen slices of renal tissue from 30 patients: 25 with glomerulonephritis (18 with immunoglobulin A nephropathy; two with focal glomerulosclerosis, one with membranous nephropathy, one with crescentic glomerulonephritis and three with lupus nephritis) and five control patients with minimal change disease. The expression of TGF-beta 1 mRNA was high in sclerotic and proliferative glomeruli, as well as in the tubular epithelial cells within tubulointerstitial lesions. There was significant correlation between the severity of tissue damage in immunoglobulin A nephropathy and the presence of TGF-beta 1 protein and TGF-beta 1 mRNA. These findings suggest that TGF-beta 1 is involved in glomerulosclerosis and the development of tubulointerstitial lesions, indicating its potential importance in the progression and aggravation of immunoglobulin A nephropathy.
Role of transforming growth factor-beta 1 and -beta 2 in ddY mouse nephropathy.
We investigated the glomerular distribution of transforming growth factor-beta (TGF-beta 1 and TGF-beta 2) protein and the expression of its mRNA, and related factors, in ddY mice, aged 5-60 weeks, before and after the onset of nephropathy, TGF-beta 1 protein expression was observed from the age of 20 weeks onwards, peaking at 50 weeks, and then declining. Expression of TGF-beta 2 protein gradually increased from 5 to 60 weeks. TGF-beta 1 and TGF-beta 2 mRNA were both detected from 5 to 60 weeks. The mesangial matrix expansion index (MMEI) was significantly higher in mice with nephropathy than in those without nephropathy, as was the expression of TGF-beta 1 and TGF-beta 2 proteins (P < 0.05). TGF-beta 2 was significantly positively correlated with the MMEI (P < 0.05). Infiltration of CD68-positive monocytes/macrophages gradually increased until 60 weeks, and was significantly correlated with the expression of TGF-beta 1 (P < 0.05) and TGF-beta 2 (P < 0.01). These findings indicate that TGF-beta 1 and TGF-beta 2 were overexpressed in ddY mice with overt nephropathy compared with pre-nephropathic mice. TGF-beta 2 may be an important mediator of mesangial matrix expansion in ddY mouse nephropathy.
Role of tenascin in human immunoglobulin A nephropathy.
Tenascin is a component of the extracellular matrix that responds rapidly to inflammation or injury. The activity index and chronicity index of immunoglobulin A nephropathy are mainly used to decide whether or not steroid therapy is indicated, but are sometimes difficult to evaluate histologically. We investigated whether tenascin staining of the glomeruli was an indicator of the activity or chronicity indices in patients with immunoglobulin A nephropathy. Tenascin staining was evaluated immunohistochemically in 38 renal specimens, including 32 from patients with immunoglobulin A nephropathy and six from control kidneys, and the extent of staining was scored. Tenascin staining was correlated with the chronicity index (r = 0.643, P < 0.0003) but not with the activity index.
Atrioventricular nodal re-entrant tachycardia with two functionally discrete fast pathways.
We present a case with two forms of atrioventricular nodal re-entrant tachycardia (AVNRT) that revealed similar H-A-V sequences, but could be differentiated only by their retrograde atrial activation sequences. Both tachycardias were induced following anterograde slow pathway conduction, suggesting the slow pathway as the anterograde limb of the re-entry circuit. The earliest atrial activation site of one form was in the same region of the bundle of His as that of the common type of AVNRT, while that of the other form was the ostium of the coronary sinus. Properly timed extra-stimuli delivered from the atrium or ventricle during the latter tachycardia penetrated through the fast pathway without resetting the tachycardia cycle length. These rare phenomena suggest the existence of two functionally discrete fast pathways, of which the alternative pathway alters to become the more predominant retrograde limb according to time and circumstances.
Dose-dependent effect of an oral adsorbent, AST-120, in patients with early chronic renal failure.
We evaluated the dose dependence of an oral adsorbent, AST-120, in 31 patients with early chronic renal failure (baseline serum creatinine: 1.2-3.0 mg/dl). Twenty-three patients were given AST-120 and eight patients were not. AST-120 was administered at three different maintenance doses, < 3.0 g, 3.0 g and 6.0 g/day, according to patients' ability to tolerate treatment. The treatment period was 12 months. The slope of the reciprocal of serum-creatinine concentration versus time was calculated to assess the progression of renal failure. This slope became significantly less steep after AST-120 treatment at 6.0 g/day, but did not change significantly at the other doses. These findings suggest that 6.0 g/day of AST-120 may delay the initiation of dialysis in patients with early chronic renal failure.
Reiter's syndrome associated with HLA-B51: a case report.
A 32-year-old Japanese man developed polyarthritis with mild fever and conjunctivitis. Clinical assessment indicated non-specific arthritis, aseptic pyuria induced by infection with Chlamydia, and conjunctivitis. He was diagnosed with reactive arthritis (Reiter's syndrome). Serotyping of human leucocyte antigen (HLA) class I and II revealed positivity for B51(5), A2, A33(19), B44(12), Cw1, DR4 and DR6, but B27 was negative. He was treated with a combination of doxycycline, oral prednisolone, diclofenac sodium and salazosulphapyridine. Fever and arthralgia improved and he became negative for anti-Chlamydia immunoglobulin (Ig) A and IgG antibodies. HLA-B51 may be involved in the pathogenesis of Reiter's syndrome in this Japanese patient.
