A new alpha chain variant Hb Tonosho [alpha 110(G17)Ala----Thr]: subunit dissociation during cation exchange chromatography for Hb A1c assay.

A new alpha chain variant, alpha 110(G17)Ala----Thr, was detected because of subunit dissociation during the determination of the Hb A1c by automated cation exchange high performance liquid chromatography. The abnormal hemoglobin overlapped the cathodic edge of the band of Hb A in isoelectrofocusing. It was slightly unstable in the isopropanol test and had a slightly increased oxygen affinity. The abnormal alpha chain eluted slightly faster than the normal alpha chain in reversed phase high performance liquid chromatography. The amino acid substitution was determined by purification of S-alkylated alpha T-12,13 tryptic peptide, chymotryptic digestion, and sequencing of an octapeptide alpha 110-117. The abnormal alpha chain comprised about 14% of the total alpha chain. A biosynthetic study did not suggest selective loss of the abnormal chain in reticulocytes.

Morphology of the transplantable colon carcinoma derived from the spontaneous colon carcinoma of WF-Osaka rat strain.

Two lines of the transplantable colon carcinoma were established. They were from the spontaneous colon carcinoma of WF-Osaka rat strain which had been set up in our laboratory. Transplant procedure was carried out mainly by the intraperitoneal implantation, and, in particular, the treating with a specified way of HIBITANE for disinfection against colonic flora at the primary transplant was quite effective. The growth speed of the transplanted colon carcinoma was slow at first and gradually increased its speed. Histology of the transplanted tumor altered gradually from the glandular pattern at first to the medullary pattern in the late stage of the transplant generation. Each tumor node in the late stage of transplant generation was composed of numerous small nodules separated by thin stromal tissue. Squamous metaplasia and central necrosis were seen in the center of the small nodules. Two lines of the transplantable colon carcinoma were named C1 and C2 and the former is at now the 101th generation and the latter is at the 107th generation.

Surgical-orthodontic correction of open bite accompanied with Class III malocclusion.

In a patient with a long face, skeletal open bite and excessive mandibular body length, it was found that the sagittal split ramus osteotomy to close the open bite was unstable, when the lower jaw alone was a moved surgically to correct the open bite and progenie. There was a slight relapse of the open bite and lingual compensation of the upper incisors, which may have been due to tissue rebound during the retention period.

[A case of an operation for lung cancer after PTCA].

A 64-year-old lung cancer patient combined with angina pectoris was admitted to our hospital. Chest roentgenogram showed a coin lesion in the left upper lobe. Broncho-fiberscopic examination proved adenocarcinoma. Coronary angiography revealed 90% stenosis of the left circumflex artery. Percutaneous transluminal coronary angioplasty (PTCA) was done and resulted in success. Eight days after PTCA, left upper lobectomy for lung cancer was undergone. Postoperative course was uneventful. Compared with coronary artery bypass grafting, PTCA was very useful for the patient having malignant disease combined with angina pectoris because of little surgical stress and no delay of operation.

The DJH complex remains active in recombination to VH segments after the loss of mu-chain expression in mu-positive pre-B cells.

AT11-2 is an Abelson virus-transformed B precursor cell line which is capable of differentiating Ig- from mu+ cells via functional recombination of VH segments to preexisting DJH complexes. We describe here that after a mu+ subclone (VDJ+/DJ) generated from Ig- AT11-2 (DJ/DJ) cells by in vitro functional VH to DJH recombination subsequently lost mu-chain expression either by the recombination of a pseudo VH segment to the VHDJH+ allele or by the deletion of VHDJH+ allele, a novel productive joining of VH segments to the preexisting DJH complex occurred. These results indicated that VH to VHDJH rearrangement was not suppressed in mu-chain producing cells and that the DJH complexes still remained active in the recombination to VH segments after the loss of mu-chain expression. Our results may also suggest that VH to DJH rearrangement, but not VH to VHDJH rearrangement, is suppressed in mu-chain producing cells to maintain allelic exclusion. Our cell differentiation system should continue to be valuable for elucidating the mechanism of suppression and associated implications regarding allelic exclusion.

[Neoadjuvant chemoradiotherapy in locally invasive transitional cell carcinoma of the urinary bladder: early results of PVB and CAP therapy].

A total of 22 patients with locally invasive transitional cell carcinoma of the urinary bladder were treated with neoadjuvant cis-diamminedichloroplatinum (CDDP), vincristine, peplomycin (PVB) or cyclophosphamide, doxorubicin, CDDP (CAP) combined with radiation therapy in our institutes between June, 1982 and May, 1988. Twelve patients were entered into the PVB regimen and the remaining 10 patients into the CAP regimen. In the PVB treated group, clinical response was obtained in 2 complete response (CR) and 6 partial response (PR), 8 out of 12 patients (66.7%). Downstaging was noted in 8 out of 12 patients (66.7%). In the CAP treated group, a clinical response was obtained in 1 of CR and in 2 of PR out of the 9 patients with evaluable lesions (33.3%). Downstaging was noted in 6 out of 9 patients (66.7%). There were no significant side effects in either the PVB or CAP treated groups, and these neoadjuvant therapies were well-tolerated. These results indicated that neoadjuvant PVB or CAP combined with radiation therapy would be useful in the management of invasive bladder cancer.

A study on colon cancer-prone rats of WF-Osaka strain.

We have established a colon cancer-prone substrain in WF strain rats strictly bred by sister x brother mating for more than 20 years. Colon carcinomas were located only in the ascending colon with no remote metastases. Each incidence of colon carcinoma varied from 30 to 40% in the respective investigation. There was no apparent sex difference. Approximately 9% of colon carcinomas were associated with gastric carcinoma in the prepyloric region and they died within four months of age due to malnutrition and intestinal bleeding. There were a few cases of carcinomas of the terminal ileum and the rectum. All of these carcinomas from three different portions showed histologically well differentiated tubular adenocarcinoma. It was found that about 40% of colon carcinomas showed spontaneous regression in the period from four to twelve months old. We have also succeeded in establishing two lines of the transplantable colon carcinoma (C1 and C2) and the transplantable gastric carcinoma (S1 and S3) from those of spontaneous colon carcinomas and gastric carcinomas. Then recipient female rats inoculated intraperitoneally with these transplantable carcinomas newly developed adenocarcinomas of the corpus uteri, which had never been found in the rats of this strain. In addition, the transplantable tumor line of adenocarcinoma of the corpus uteri was also established (U2). When transplanted these tumors intraperitoneally (S1, S3, C1, C2 and U2), male and female recipient rats extremely increased in the incidence of carcinomas of the stomach and the colon. As far as female recipient rats were concerned, a large number of carcinomas of the corpus uteri were also found regardless of the derivation of tumors. We believe that the established colon cancer-prone rat strain (WF-Osaka) as well as those of transplantable tumor lines will open a further research fields and will be available as an animal model of colon cancer for human beings.

Morphology on spontaneous regression of the autochthonous colon carcinoma in WF Osaka rat strain.

In order to study the spontaneous regression of the colon cancers of WF Osaka strain rats, laparotomies were carried on 701 rats in sequence at the age of four months. Among them, 260 cases were found having colon cancers at the laparotomy. 107 out of them showed spontaneous regression of the colon cancer in the ascending colon. Most of the cases which spontaneously regressed were rather the early stage (stage 1 and 2) of the colon cancer, though 26 cases of advanced cancer also showed spontaneous regression. Grossly the lesions of the spontaneous regression were those of thickened, elongated ascending colon, with occasional dilation. Some cases showed the cystic formation at the portion of former lesions. There was a case which showed normal appearance of the ascending colon with regional lymph node metastasis measuring 1.5 x 1.5 centimeters. Histological appearance was varied showing reconstruction of the muscular layer and mucularis mucosae. Mucosal epithelium was almost normal looking, but glandular arrangement showed cystic dilation. The ectopic existence of glandular composition was seen in the restored lesion in the muscular layer. Macrophage infiltration was indispensably main histological reaction with central necrosis of formerly existed cancer cells, which was considered to be cellular immunity against cancer cells, or exogenous substances, such as viruses in cancer cells.

Morphology on the transplantable gastric carcinoma in WF Osaka rat strain.

We have established four lines of transplantable gastric carcinoma in WF Osaka rats. They were derived from adenocarcinomas of the glandular stomach, which were developed along with the lesser curvature, measuring less than 1.0 square centimeter. Morphology of these four lines of the transplantable tumor showed essentially an identical figure in their respective generations. Histology in the early generation of transplanted tumors showed well differentiated tubular adenocarcinoma similar to the primary lesion of the stomach. Later the histological appearance of these four lines of the transplanted tumor altered from glandular structure to keratinizing squamous cell metaplasia of cancer cells. The lines of S4 and S5 transplantable tumor were recently established, but the lines of S1 and S3 transplantable tumor have been transplanted as far as more than the 60th generation, and changed their histological appearance to scirrhous carcinoma in the line of S1 and medullary carcinoma in the line of S3. The growth speed of S1 line became slow in the later generations compared to the former ones, and that of S3 line became extremely rapid and recipient rats died with cachexia within two weeks. Effectual activities of tumor enhancement for the gastric and the colon cancer, previously reported elsewhere, recently decreased, and gastric carcinoma was rarely induced by these two lines. Lately established S4 and S5 transplantable tumor lines showed vigorous flourish of inducing gastric and colon cancers.

[Primary cancer of the vesical diverticulum in a patient with spinal cord injury: report of a case].

A case of primary cancer of the vesical diverticulum in a patient with spinal cord injury is reported. A 78-year-old man with L1 incomplete paraplegia, who complained of asymptomatic gross hematuria, was referred to our hospital on September 2, 1986. Urethrocystoscopy showed papillary tumor in the diverticulum and three small stones. Histopathological findings of the biopsied specimen of the tumor revealed transitional cell carcinoma grade 3. Partial resection of the bladder involving diverticulum with right ureterovesiconeostomy and right pelvic lymphadenectomy was performed on October 17, 1986. Histopathological findings revealed transitional cell carcinoma grade 3 with submucosal invasion into the muscular layer (pT3bINF beta, ly2, V (+]. Two courses of combination chemotherapy with cyclophosphamide, adriamycin and cisplatin were given as a postoperative adjuvant chemotherapy, however he died of pneumonia on January 2, 1987. There have been reported 11 cases of a vesical neoplasm in patients with spinal cord injury in Japan.

[A clinical survey of renal pelvic and ureteral tumors associated with bladder tumor].

A clinical survey was performed on 80 cases of renal pelvic and ureteral transitional cell carcinomas we treated between January, 1963 and December, 1986. The cases included 30 of renal pelvic tumors, 17 of ureteral tumors, 3 of renal pelvic and ureteral tumors, 7 of renal pelvic and ureteral and bladder tumors, 16 of ureteral and bladder tumors and 7 of renal pelvic or ureteral tumors after treatment for bladder tumors. There were 37 cases of bladder tumors: 7 cases with preceding bladder tumors, 23 cases of synchronous bladder tumors, and 13 cases of subsequent bladder tumors. The 5-year survival for all cases was 60.2%. The 5-year survival for 43 cases unrelated with bladder tumors was 80.5% and that for 37 cases of bladder tumor was 41.6%. Therefore, there was a significant difference between these 2 groups (p less than 0.005). The 5-year survival for 50 cases without synchronous bladder tumors at first diagnosis was significantly higher than that for 23 cases with synchronous bladder tumors (p less than 0.001). Subsequent bladder tumors occurred after 2 to 48 months (mean 10 months) of the initial treatment for renal pelvic and ureteral tumors. Six of the 7 cases of preceeding bladder tumors were superficial tumors of pTa and pT1 and 3 cases had vesicoureteral reflux.

[Two cases of psychotic state following normal-dose benzodiazepine withdrawal].

We report two cases of severe withdrawal symptoms after abrupt discontinuation of a long-term normal-dose benzodiazepines (BZD) administration. Case 1, a 61-year-old man, suffered from delirium on the 7th day after abrupt discontinuation of nitrazepam, 10 mg/day. Case 2, a 49-year-old woman, suffered from auditory hallucination on the 4th day and visual cognitive disorder on the 5th day after abrupt discontinuation of nitrazepam, 5 mg/day, and triazolam, 0.5 mg/day. A withdrawal syndrome after discontinuation of normal-dose BZD is uncommon, and a psychotic withdrawal reaction is even more uncommon. We show how a continuous administration of BZD for a period of longer than 6 months and the presence of severe insomnia are risk factors predictive of a psychotic reaction. We also explain the predictive method used to determine the onset time of such a severe state. In the case of a psychotic state, we recommend intravenous diazepam injection. To prevent withdrawal reaction, we also recommend a gradual reduction after administration of normal-dose BZD.

Changes in EEG associated with sleep-awake behavior in young adult versus aged adult Fischer-344 rats.

Age-related spontaneous cortical and hippocampal EEG changes associated with level of arousal in rats were investigated. The EEG of rats with chronically implanted electrodes were recorded using bipolar leads and simultaneously analyzed with a computer to obtain the power spectra. In the awake stage, EEGs in the aged rats consisted predominantly of slow waves when compared with those in the young rats. The cortical EEG activity of the aged rats exhibited specific irregular burst waves which consisted of two spectral peaks at 8-9 Hz and 15-16 Hz during the drowsy period. The relative powers of the cortical and hippocampal waves in the aged rats differed from those of the young rats during the slow-wave sleep stage. These findings suggest that the slowing of the EEG in the rats during the awake stage may be related to decreased brain activity associated with aging, and that the irregular burst waves in the cortical EEG in the aged rats appear to correlate with the changes observed in age-related human sleep patterns.
