[Multilocular cystic renal cell carcinoma: a report of 2 cases].

We report 2 cases of renal cell carcinoma which rarely occur as multilocular cystic masses. By means of echography, both cases were discovered as abnormal shadows occupying the middle portion of the left kidney. Using computerized tomography and selected renal arteriography, we tried to obtain an accurate diagnosis, but few specific malignant findings were gained. Finally, we made the histopathological diagnosis. In addition, we concluded by careful pathological examination that they were multilocular cystic growths. It was difficult to differentiate them preoperatively from multilocular cystic nephroma. Both patients are still alive without tumor recurrence.

[Transitional cell carcinoma of the ureter with an inverted growth pattern which was accompanied with bladder tumor: a case report].

The patient was a 75-year-old man who complained of macrohematuria which had set in from April 12, 1988. Under the clinical diagnosis of bladder tumor, total cystectomy with ureterosigmoidostomy was performed on June 25. During his surgery, we discovered a polypoid lesion of the right ureter close to the common iliac artery. The tumor was peanut-sized and had a stalk suggesting benign nature, so we resected it. Postoperative pathological examination revealed the tumor was transitional cell carcinoma (grade 2) with an inverted growth pattern. Though we could find no signs of tumor recurrence, the presence of tumor's residuum could not be ruled out. We performed nephroureterectomy of the right side on December 19. Follow-up on July 1, 1989 this patient revealed no tumor recurrence.

[Unilateral hydronephrosis with hypertension due to ureteral endometriosis].

A 35-year-old female patient was hospitalized for a headache. She was referred to our department for the evaluation of right hydronephrosis noted on the excretory urogram which was performed as part of a hypertensive diagnostic study. Endocrine examination revealed renal hypertension. Excretory urogram and antegrade pyelography showed obstruction of the lower part of the right ureter. On surgical exploration, the lower part of the right ureter was surrounded by brown tissue. Complete hysterectomy and ureterovesiconeostomy were performed. Histologically, the brown tissue around the ureter was diagnosed as endometriosis. One year after the operation, excretory urogram showed normal urinary tract and the blood pressure was 130/80 mmHg. Endometriosis is a common gynecological disease but ureteral endometriosis is relatively rare. Review of the Japanese literature disclosed 16 previous cases of ureteral endometriosis and we report the 17th case with a review of the literature.

[Mass-screening for breast cancer using C-mode display ultrasonography].

In order to elevate the detection rate of breast cancer in mass-screening, we examined whether the C-mode display ultrasonography is useful for detection of breast cancer. 1) Diagnostic sensitivity of C-mode ultrasonography was 88.2%, specificity was 66.7% and predictive value was 58.8%, respectively. 2) The detection rate of C0mode ultrasonography was 100% for tumors more than 5.0cm, 87.5% for 2.0-5.0cm, 74.2% for less than 2.0cm. Especially, the diagnostic value of lesser than 1.0cm breast cancer was high, 66.7%. 3) In findings of C-mode ultrasonography, images characteristic of carcinoma can be obtained regarding their unclear margin (70.6%), heterogenous (88.2%) and hypoechoic (70.6%) internal echoes and malignant halo (61.8%). Our present data suggest that C-mode ultrasonography is very useful for the mass screening for breast cancer.

NADH- and NADPH-linked aquacobalamin reductases occur in both mitochondrial and microsomal membranes of rat liver.

To elucidate the mechanism for the synthesis of the coenzyme forms of cobalamin in mammals, rat liver aquacobalamin reductase was partially characterized. Rat liver contained both NADH- and NADPH-linked aquacobalamin reductases. The NADH-linked enzyme was distributed in the mitochondria (approx. 40%) and microsomes (60%), identical to the distribution of the NADPH-linked enzyme. The two mitochondrial NADH- and NADPH-linked enzymes were located inside of the outer membrane.

TSK-Toyopearl gels for the preparative separation of sterol carrier protein2 from rat liver.

The application of TSK-Toyopearl gels to the preparative separation of a basic and low-molecular-weight protein, sterol carrier protein2 (SCP2), was studied. SCP2 was purified from 105,000 g supernatant of rat liver (S105) by ion-exchange chromatography on CM-Toyopearl 650M and gel permeation on Toyopearl HW60S. Separation of S105 by CM-Toyopearl 650M was carried out at a high flow-rate in the presence of 10% (v/v) glycerol, a stabilizer of the protein. Toyopearl HW60S showed a significant ion-exchange effect on the elution of SCP2. Using an elution buffer of ionic strength of 45 mM, a highly efficient purification of SCP2 on HW60S was achieved. SCP2 was purified approximately 5000-fold to apparent homogeneity with an overall yield of 69%.

Cardioprotective effects of amlodipine in the ischemic-reperfused heart.

Amlodipine is a dihydropyridine derivative belonging to the group of pharmacologic calcium entry blocking agents and is characterized as having a slow onset and relatively long duration of action with minimal effects on cardiac electrophysiology and myocardial contractility. The protective effect of amlodipine was studied in isolated blood-perfused feline hearts made globally ischemic for 60 minutes followed by reperfusion for 60 minutes. Ischemic-induced alterations of left ventricular developed pressure and complicance were monitored. In 11 control and 7 drug-treated hearts, amlodipine produced significant decreases in myocardial oxygen consumption (6.2 +/- 0.4 to 4.4 +/- 0.4 ml oxygen/min/100 g) and coronary vascular resistance, as assessed by changes in perfusion pressure (120 +/- 1 to 100 +/- 4 mm Hg). Amlodipine administered before the onset of global ischemia decreased the development of ischemic contracture as reflected by a progressive increase in resting left ventricular diastolic pressure. The return of contractile function, 60 minutes afer reperfusion, improved significantly in the amlodipine-treated group compared with controls, and there was better maintenance of the tissue concentration of Na+, Ca2+ and K+. A canine model of regional myocardial ischemia (90 minutes) followed by 6 hours of reperfusion was used to assess the cardioprotective effects of amlodipine, 150 micrograms/kg, administered 15 minutes before reperfusion. Infarct size, expressed as a percentage of the area at risk, was smaller in the amlodipine-treated group (n = 10) than in the control group (n = 10) (34.5 +/- 3.8% vs 45.9 +/- 2.8%, p = 0.027). Risk region size did not differ between groups and both groups were comparable with respect to the hemodynamic parameters of heart rate, blood pressure and rate-pressure product. Amlodipine prevented the gradual reduction in coronary blood flow observed in the control group. It is concluded that amlodipine reduces myocardial ischemic injury by mechanism(s) that may involve a reduction in myocardial oxygen demand as well as by positively influencing transmembrane Ca2+ fluxes during ischemia and reperfusion.

Effect of combination of 5-fluorouracil, epirubicin and mitomycin C (FEM) on advanced gastric carcinoma.

Fifteen patients with advanced gastric carcinoma were treated with the combination of 5-Fluorouracil (5-FU), Epirubicin (EPI) and Mitomycin C (MMC). Twelve of 15 patients were evaluable. Three patients showed partial response (25%) and 4 showed no change (33%) according to the criteria of Japan Society for Cancer Therapy. Mean survival time for responders, nonresponders and all evaluable patients were 14. 3, 3.6 and 6.3 months, respectively. Two patients with partial response lived over 18 months. Major toxicities included myelosuppression, nausea and vomiting, and alopecia. These were mild in general and reversible by discontinuance. No cardiotoxicity developed. It was concluded that this combination therapy is beneficial in patients with advanced gastric carcinoma because it showed measurable clinical effectiveness with less toxicity than FAM therapy (a combination of 5-FU, Doxorubicin and MMC).

Arachidonic acid metabolites by cytochrome P-450 dependent monooxygenase pathway in bovine adrenal fasciculata cells.

[1-14C]Arachidonic acid was incubated with microsomes of bovine adrenal fasciculata cells in the presence of 1 mM NADPH for 30 min at 37 degrees C. The metabolites were separated and purified by reverse phase high performance liquid chromatography, and identified by gas chromatography-mass spectrometry. Identified metabolites were four dihydroxyeicosatrienoic acids (DHTs) (5,6-, 8,9-, 11,12-, 14,15-DHTs), 20-hydroxyeicosatetraenoic acid and eicosatetradioic acid. The formation of these metabolites was dependent on NADPH and inhibited by SKF-525A. 14,15-DHT was also formed by isolated bovine adrenal fasciculata cells. These results indicate that cytochrome P-450 dependent arachidonate monooxygenase pathway may exist in bovine adrenal fasciculata cells. Addition of the chemically synthesized epoxyeicosatrienoic acids (EETs) to isolated bovine adrenal fasciculata cells stimulated cortisol production. Among four regioisomeric EETs, 14,15-EET was most potent and stimulated steroidogenesis in a dose-related manner over a range of 0.5 to 5.0 microM.

Lower fat deposition and energy utilization of growing rats fed diets containing sorbose.

1. Growing rats were fed diets containing graded levels (0, 100, 200 and 300 g/kg diet) of sorbose for 6 weeks. Protein, fat and energy deposition were determined by carcass analysis. 2. The values for growth, serum insulin level, digestible energy (DE), metabolizable energy (ME) and fat and energy deposition declined with the increment of dietary sorbose. 3. The efficiency of protein utilization (protein retained/protein consumed) was hardly affected by dietary sorbose. The DE and ME of sorbose per se was calculated as 14.09 and 12.35 kJ/g respectively. The efficiency of energy utilization (energy retained/ME intake) decreased with the increase of dietary sorbose, although sorbose had an ME. 4. The relative weights of gastro-intestinal tract and liver were positively associated with dietary sorbose level, although the reverse was true for the amount of stomach content, being heavier with higher dietary sorbose. 5. It is suggested that dietary sorbose, as a sweetener as well as a bulky agent, seems to be a suitable sugar for the obese and diabetic with special reference to lower body fat and energy deposition without reducing protein utilization.
