Effect of hepatic artery embolization on the adenylate energy charge level and the redox state of the cirrhotic liver.

Hepatic energy charge ((ATP + 1/2ADP)/(ATP + ADP + AMP)) and arterial blood ketone body ratio (acetoacetate/3-hydroxybutyrate) were measured after hepatic artery embolization in carbon-tetrachloride-induced cirrhotic rats. In normal livers, energy charge slightly decreased at 3 h and returned to normal levels at 6 h. By contrast, in cirrhotic livers, energy charge was drastically decreased at 3 h after embolization and was not completely recovered at 6 h. Blood ketone body ratio, reflecting hepatic mitochondrial redox (NAD+/NADH) state, also decreased at 3 h after embolization in both groups, and its recovery was not observed in cirrhotic group at 6 h.

Percutaneous intrapelvic pressure registration in patients with ureterointestinal urinary diversion.

Intrapelvic pressure registration using percutaneous needle renal pelvic puncture in 4 patients with intestinal loop diversion was done to determine whether or not there is loop-ureteral reflux under physiologic condition in loop diversion without any urinary tract obstruction. There was no pressure elevation related to reflux on pressure recording. We concluded that loop-ureteral reflux in intestinal loop diversion does not occur without obstruction.

Short-term changes in blood ketone body ratios in the phase immediately after hepatic artery embolization: their clinical significance.

Changes in arterial and hepatic venous blood ketone bodies were investigated following transcatheter hepatic artery embolization (THAE) in patients with hepatocellular carcinoma. Acetoacetate/ beta-hydroxybutyrate ratio (ketone body ratio) in arterial blood was positively correlated with those of hepatic venous blood (r = 0.960, p less than 0.001), which reflects the mitochondrial redox potential in the embolized lobe. Nine cirrhotic patients were classified into three groups according to the changes in arterial blood ketone body ratio following THAE: Type A without decrease to below 0.7; Type B with a transient decrease to below 0.7, followed by its restoration within 5 hours; and Type C with decrease to below 0.7 without recovery within 5 hours. There were no serious complications in Type A and B patients. By contrast, severe sepsis and hepatic failure developed in Type C patients, possibly due to the extended embolization of both lobes. It is suggested that THAE can be successfully performed even in severely cirrhotic patients, as long as the embolized area is restricted to one lobe. In addition, changes in arterial blood ketone body ratios can give early information about the likely consequences of the THAE procedure just performed.

Changes in oxidative phosphorylation, adenylate energy charge, and respiratory components in chloramphenicol-treated regenerating rat liver.

To clarify the functional adaptability of mitochondria in the regenerating liver, the concentrations of respiratory components, hepatic energy charge levels, cytochrome oxidase activity, and phosphorylative activity were studied in mitochondria obtained from regenerating liver of rats treated with chloramphenicol (CAP). In the hepatectomized groups with CAP treatment, a dose-dependent decrease occurred in the concentrations of cytochrome a(+ a3), cytochrome b, cytochrome c + c1, flavoprotein, and pyridine nucleotide. Cytochrome oxidase activity and phosphorylative activity per milligram of mitochondrial protein also decreased dose dependently in CAP-treated hepatectomized groups, with a significant increase in these values per unit of cytochrome a(+ a3). Hepatic energy charge levels significantly decreased in the hepatectomized groups. However, no significant differences were seen among the hepatectomized groups. However, no significant differences were seen among the hepatectomized groups with or without CAP treatment. Our results suggest that hepatic energy charge is maintained at the same relative level by a compensatory enhancement in phosphorylative capacity associated with cytochrome oxidase activity per unit of cytochrome a(+ a3), in spite of a remarkable decrease in the concentrations of respiratory components.

Changes in coenzyme Q level in mitochondria of cirrhotic rat liver.

In the cirrhotic rat liver induced by carbon tetrachloride and phenobarbitone, the concentrations of mitochondrial Coenzyme Q were measured in comparison with other respiratory components. The concentration of cytochrome a(+a3) and Coenzyme Q significantly increased in the cirrhotic liver, without any changes in the ratio of Coenzyme Q to cytochrome a(+a3). It is suggested that such increase of Coenzyme Q plays an important role as one of the adaptive responses to compensate for the prolonged metabolic overload on the mitochondrial respiratory assembly. Also, from the findings that the concentrations of cytochrome a(+a3) in the mitochondria of cirrhotic liver increase concomitant with the severity of cirrhosis, it is suggested that the rise of Coenzyme Q levels may be one of the indicators for the decreased functional reserve capacity in liver cirrhosis.

Derangement of hepatic energy metabolism in lead-sensitized endotoxicosis.

Lead-sensitized endotoxicosis was investigated in rats in terms of hepatic energy metabolism. Lead acetate (Ld, 20 mg/kg BW) or endotoxin (Etx, 4 mg/kg BW) caused no deaths within 48 h. Ld plus Etx resulted in a lethality of 50 and 100% within 6 and 12 h respectively. Etx or Ld alone caused a slight but significant decrease in the hepatic tissue levels of total adenine nucleotides and/or ATP at 3 and 6 h after the application. The energy charge potential (ECP) remained normal. The ketone bodies acetoacetate and beta-hydroxybutyrate and the ratio AcAc/beta-OHB as well as the mitochondrial oxidative phosphorylation tended to increase; the hepatic tissue levels of pyruvate and lactate were increased after 3 h, indicative of an accelerated glycolysis. These alterations were no longer detectable after 6 h. In lead-sensitized endotoxemia (Ld + Etx), the total adenine nucleotides and ATP of the liver tissue decreased significantly to 84% (86%) and 71% (52%) of the controls within 3 and 6 h respectively, and the ECP had decreased from 0.865 to 0.684 at 6 h. The ketone bodies were increased, while the ratio AcAc/beta-OHB was significantly decreased at 6 h. The hepatic tissue lactate remained elevated. The mitochondrial activity was significantly reduced. A hyperglycemia (175 mg . dl-1) at 3 h changed into a hypoglycemia (50 mg . dl-1) at 6 h. It is suggested that Ld plus Etx causes a rapid impairment of hepatic mitochondria which leads to a drastic disturbance of the hepatic energy metabolism including hypoglycemia and contributes to an enhanced lethality in Ld-sensitized endotoxicosis.

[Evaluation of the effect of transcatheter arterial embolization as preoperative therapy of hepatocellular carcinoma].

From May 1982 till Nov. 1984, we performed 90 hepatic resection for primary or metastatic liver tumor. Preoperative transcatheter arterial embolization (TAE) was performed on 14 of them. All resected specimens of 14 cases were examined histologically. Effectiveness of TEA was estimated by the ratio of the necrotic area to the whole area of the tumor. Necrosis of the main tumor more than 90% was found in 10 of 14 cases, though necrosis of the daughter nodules was detected in only one of six cases. In hepatectomized patients, two-year-survival of the preoperative TAE cases had not been statistically improved compared with the non-TAE cases.

[The ureteral stent: is it useful?].

Double-J or double-pigtail ureteral stent was used in 7 cases, 6 cases for ureteral obstruction and 1 case for ureterocutaneous fistula in irradiated patients. In 5 cases ureteral stent failed to drain effectively, in 4 of these 5 cases due to stent obstruction by necrotic tissue which led to percutaneous nephrostomy in 3 cases. In 1 case the ureteral stent drained well and in another case effectiveness of ureteral stent was equivocal. We think that ureteral stent is neither safe nor effective for internal divertion except in short term use postoperatively.

Changes in the reticuloendothelial phagocytic function after partial hepatectomy.

The changes in the phagocytic function of the reticuloendothelial (RE) system after 67% hepatectomy in rats were studied by use of 51Cr-endotoxin as the phagocytable material. The humoral opsonic index was significantly increased to approximately 1.2 to 1.8 (control, 1.0 +/- 0.2, mean +/- SD) until the fourteenth postoperative day. In contrast, the phagocytic index was decreased to 0.068 +/- 0.016 (control, 0.103 +/- 0.015) on the first day, then returned to the normal level on the second day. In this early postoperative period, uptake rates of 51Cr-endotoxin in the liver were remarkably decreased to about 50% to 70% of control, whereas those in the spleen and lung were increased two- to threefold of control. From the third to the fourteenth day, the phagocytic index was significantly increased compared with the preoperative level. During this period, the uptake rates in the liver and spleen were within the normal range. These results suggest that the increases in the opsonic index of 67% hepatectomized rats represent the homeostatic response for maintaining or stimulating RE system phagocytic function, and that high or normal phagocytic index, concomitant with the increase in the opsonic index, implies an enhanced or compensatory stage, respectively. The decrease in the phagocytic index despite the high opsonic index is assumed to represent a compromised stage of the RE system.

[A case of recurrent yolk sac tumor of the testis in childhood: bulky retroperitoneal metastatic tumors cured by tumor resection and chemotherapy].

We carried out only high orchiectomy for a 1-year and 7-month-old boy with stage 1 yolk sac tumor of the testis, after 13-months a bulky retroperitoneal metastatic tumor was found. Following chemotherapy with CDDP, ACD, VBL, PLM and CTX two times after tumor resection, elevated serum AFP was normalized. He has been in continuous complete remission with no evidence of disease for 3 years and 2 months. Combination chemotherapy with CDDP has a dramatic effect on the yolk sac tumor of infantile testis. We believe that "watchful waiting" after high orchiectomy alone is the best modality for all cases of stage 1 yolk sac tumor of the infantile testis.

Changes in acetoacetate/beta-hydroxybutyrate ratio in arterial blood following hepatic artery embolization in man.

Acetoacetate/beta-hydroxybutyrate ratio in the hepatic venous blood was compared to the ratios in arterial blood and peripheral venous blood in hypoxic state following right hepatic artery embolization in 5 patients with liver cancer. Ketone body ratios in right hepatic venous blood were positively correlated with those in arterial blood (r = 0.960, p less than 0.001), but not with those in peripheral venous blood. The free NAD+/NADH ratio of the liver mitochondria, which is reflected by the ketone body ration in hepatic venous blood, can be evaluated by the ketone body ratio in the arterial blood.

Adenylate energy charge and cytochrome a (+a3) in the cirrhotic rat liver.

In the cirrhotic rat liver induced by phenobarbitone and carbon tetrachloride, adenylate energy charge, mitochondrial oxidative phosphorylation, and respiratory enzyme concentrations were studied along with serum albumin concentrations. Cytochrome a (+a3) concentrations of the liver increased with the severity of cirrhosis and were negatively correlated with the ATP-synthesizing ability per unit of cytochrome a (+a3). These changes were associated with the decrease in hepatic energy charge. Such decreased energy charge may be responsible for the decreased serum albumin level in the cirrhotic rat. It is suggested that such falls in hepatic energy charge may be one of the most important factors contributing to the decreased functional reserve of cirrhotic patients.

[Ureteroceles in adult women: report of two cases].

Two cases of ureteroceles are reported. Case 1: Transurethral prolapse of a simple ureterocele was seen in a 27-year-old woman. Bilateral vesicoureteral neostomy was carried out, and she has been living with no vesicoureteral reflux or complaints. Case 2: An ectopic ureterocele was seen in a 51-year-old woman. She had right complete double renal systems, neither of which was dilated. There were three stones in the ureterocele and several stones in the upper renal system. Ureterocelectomy and plasty of the ureteral orifices were performed, and a complete cure was achieved.
