Potential role of heparin in prevention of liver metastasis from colon cancer.

The case of a 59 year-old man with sigmoid colon cancer and synchronous liver metastases is described in this report. Sigmoid colectomy and partial hepatectomy were performed, and hepatic arterial cannulation was done for prevention of hepatic recurrence. Heparin was injected to prevent catheter-related clots, and no anticancer drugs were used. He did well without signs of recurrence for 5 years after the initial operation. After we stopped the heparin administration, recurrence was detected in the liver. The patient underwent repeat hepatectomy, and he is now doing well without recurrence 2 years after the second operation. The clinical course of this case suggests that heparin may prevent liver metastasis of colorectal cancer.

Sickle-tip sheath for percutaneous implantation of hepatic arterial catheter with subclavian arterial access.

We used a new type of sheath-introducer to facilitate the percutaneous procedure of hepatic arterial catheter implantation with left subclavian arterial access. Insertion into the descending aorta was easy without any additional catheters because the sheath could be manipulated to the aortic arch. After hepatic arterial catheterization, the sheath was slid safely away by fitting a 5 F catheter to the end of the indwelling catheter. The procedures of sheath insertion and removal were successful in all 29 cases, without any complications. The sickle-tip sheath is a useful optional device for trans-subclavian catheterization to the hepatic artery.

Postoperative complications of repeat hepatectomy for liver metastasis from colorectal carcinoma.

BACKGROUND/AIMS: When a repeat hepatectomy is possible, it is the most effective treatment modality for recurrent colorectal liver metastasis. The aim of this study was to evaluate the surgical risks of repeat hepatectomy for liver metastasis from colorectal carcinoma. METHODOLOGY: Between 1986 and 1996, 60 patients with hepatic metastasis from colorectal carcinoma underwent surgery in the Department of Surgery I, Oita Medical University. Ten of them underwent a repeat hepatectomy. The cases of these 10 patients were studied retrospectively; in particular, postoperative complications and intraoperative blood loss were compared between the initial and second operation. RESULTS: During the second surgery, recurrence was detected adjacent to the hepatic stump in 9 of the 10 patients. During the initial surgery, 6 underwent non-anatomic resections, and 4 had anatomic resection, including 1 extended lobectomy, 1 lobectomy, and 2 segmentectomies. For the second surgery, 3 had anatomic resections, including 2 lobectomies, and 1 segmentectomy, and 7 underwent non-anatomic resections. There were no mortalities during the initial or second operation. There was no morbidity following the initial surgeries and 7 postoperative complications (intraabdominal abscess, 4 cases; biloma, 3 cases) following the second surgeries. Mean blood loss during the second operation (1044 mL) was significantly greater than during the initial operation (561 mL). CONCLUSIONS: The present results show that repeat hepatectomy for recurrent liver metastasis from colorectal carcinoma resulted in significantly greater intraoperative blood loss and postoperative complications than those of the initial surgeries. The blood loss and complications in the second operation, the one for the recurrence, were directly associated with the fact that the recurrence was so close to the hepatic stump. Since the resection line in the second surgery was adjacent to the hepatic hilus, resection of the lesion caused much more injury to the main bile duct and main portal vein than that caused by the.

Transvenous instrumental dilatation for obstruction of the hepatic portion of the inferior vena cava.

In this report, we shall describe a new nonoperative method of treatment for membranous obstruction of the hepatic protion of the inferior vena cava. A specially designed instrument similar to a Tubbs' dilator was inserted through the peripheral vein and advanced to the portion of obstruction along the previously placed Fogarty catheter under fluoroscopic control. The fracture of obstruction was successfully performed in two patients.

Antigenic variation of influenza viruses isolated from the recent epidemics.

In the winters of 1975-76 and 1976-77 there were fairly heavy epidemics of influenza in Japan, and on those occasions considerable discussions were held on the efficacy of influenza vaccine in connection with the variability of antigenic structure of the influenza virus. The type of the virus which prevailed in the winter of 1975-76 belonged to type A, but detailed antigenic analysis revealed that there had been two main antigenic types somewhat but definitely distinguishable from each other, the one is the well-known A/Victoria/75 type while the other is A/Tokyo/75 type. This epidemic seems to have started with the latter type, but soon the two types of virus prevailed hand in hand until the late part of the epidemic when the type A/Victoria/75 became overwhelming. The virus for the vaccine was however A/Tokyo/6/73 for that season, which is antigenically considerably different from both of these two strains. In the beginning of 1976, namely at the end of the epidemic season of type A, a few scattered epidemics due to type B influenza were reported from some parts of Japan. The result of antigenic analysis found that the virus belonged to B/Hong Kong/72 type, though showing some degree of antigenic schift. The same type of virus caused the epidemic of the winter 1976-77, which was considerably wide-spread and developed unexpectedly large number of cases. There were active discussion on the effectiveness of vaccine, of which B type was B/Gifu/1/73, recovered from the fairly large epidemic in 1973, and belonging to B/Hong Kong/72.

Non-reversed saphenous vein grafting: an improved technique or aortocoronary bypass surgery.

Prevention of graft failure produced by intervening valves in reversed segments of saphenous veins has been achieved by a new method of aortocoronary bypass surgery employing non-reversed, valve-free segments of vein. The large cephalic end of the greater saphenous vein segments is anastomosed to the aorta. The somewhat smaller, caudal end of the venous segment is anastomosed to the coronary artery, thereby reducing veno-coronary size discrepancy. Graft stenosis and thrombosis caused by venous valves is avoided.

Surgical experience with coronary arterial sequelae of Kawasaki disease in children.

The coronary arterial sequelae due to Kawasaki disease have been treated surgically in five children ranged from 6 to 9 years old. The procedures were aortocoronary bypass surgery in all cases, coronary artery aneurysmectomy in one case and left ventricular aneurysmectomy in one case. The early results of operated cases have been good with the graft patency of 86% confirmed by angiography one month after surgery. The characteristic features of coronary artery damage in patients with Kawasaki disease are coronary artery aneurysms, which manifest wall irregularity, thrombus, calcification and stenosis. The significant stenosis of coronary artery could often be observed at the inlet or outlet of the aneurysm in major coronary artery branches. Although the early results of aortocoronary bypass surgery in the children with Kawasaki disease have been good, long-term follow up is mandatory to investigate the status of autogenous saphenous vein grafted in children.

Reconstruction of plantar heel defects with free gracilis musculocutaneous flap.

Soft tissue defects of heel region are difficult to repair. The flaps taken from the nonweightbearing area of the sole produce good results. However, these flaps are limited in width and cannot always cover the large defects after excision of malignant tumors. The authors experienced two cases of plantar heel reconstruction with free gracilis musculocutaneous flap after wide excision of malignant skin tumors, and successfully covered the defects. The patients have been free of ulcer for 3 and 1.8 years respectively, after flap transfer. Free gracilis musculocutaneous flap should be added to the techniques for plantar heel reconstruction after wide excision of malignant skin tumors.

Spontaneous degeneration of testicular germ cells in congenitally athymic nude mice of four strains: a light microscopical observation.

Many hormonal studies have demonstrated that the presence of thymic factors affects testicular function. However, morphological studies on the testis in the absence of thymic factors are very limited. In the present study, seminiferous tubules of nude mice of four strains were light microscopically observed in order to investigate the effect of congenital athymia on germinal cell components. Testes obtained from congenitally athymic nude mice of CD1, BALB/c-nu/nu, LASAT, and KSN strains were fixed in Bouin's solution, dehydrated, and embedded in plastic. The samples were cut and their sections were stained with hematoxylin and eosin. The results showed that degeneration of testicular germ cells was focally observed in all four strains of athymic mice. The degenerating morphology was characterized by karyopyknosis and karyolysis of immature germ cells, multinuclear giant cell formation, and severe depletion of epithelial components with remaining Sertoli cells and/or some spermatogonia. KSN nude mice showed the most severe degenerative changes among the four strains of nude mice examined. Their significant degenerative changes first appeared at 2 weeks of age. In contrast, CD1 nude mice showed very slight degenerative features even at 20 weeks of age. BALB/c-nu/nu and LASAT nude mice had more degenerated epithelia than did CD1 nude mice; however, their significant degenerative changes were apparent at 20 but not at 10 weeks of age. Therefore, the severity and the onset of spontaneous degeneration and depletion of testicular germ cells in congenitally athymic mice were different among the four strains examined.

MR imaging in the evaluation of the therapeutic effect of B-RTO for gastric varices.

BACKGROUNDS/AIMS: Balloon-occluded retrograde transvenous obliteration (B-RTO) has become a new treatment option for gastric varices. In the present study, mid-term follow-up data after B-RTO were presented, and the role of magnetic resonance imaging (MRI) and MR-portography in the assessment of therapeutic effect was evaluated. METHODOLOGY: Twelve patients with gastric varices were treated with up to three sessions of B-RTO. The patients were followed up with MRI, MR-portography, and endoscopy for a mean of 12.3 months. RESULTS: In ten patients, one (n = 8) or two (n = 2) sessions of B-RTO were effective to produce immediate (< 2 weeks) variceal obliteration on MRI and MR-portography. Endoscopic confirmation of variceal eradication was obtained within three (n = 9) or six (n = 1) months after B-RTO in these patients. The remaining two patients who underwent three sessions of B-RTO showed only a significant reduction in variceal size immediately after B-RTO, but variceal obliteration was observed within three months with subsequent variceal eradication. There were no signs of exacerbation of gastric varices on MRI and endoscopy in any patient during the follow-up period. CONCLUSIONS: The results suggest that B-RTO is effective for the treatment of gastric varices. MRI and MR-portography may provide accurate assessment of therapeutic effect.

Patterns of efferent lymphatics of the mouse testis.

Previous studies in the rat demonstrated that lymph vessels arise from various sites in the testis and then anastomose to form the main lymph trunk toward the testicular artery. Although mice are as commonly used in the laboratory as the rat, precise knowledge concerning their testicular lymphatics has not been established. In the present study, the patterns of testicular lymphatic drainage in the mouse were examined using local injection of India ink into the testicular parenchyma. The results demonstrated that, besides the lymph trunk surrounding the testicular artery, a lymphatic drainage route running along the ductus deferens is also present.

Three-dimensional power Doppler sonography of tumor vascularity.

PURPOSE: To assess the clinical value of three-dimensional (3D) power Doppler imaging of intratumoral blood flow. MATERIALS AND METHODS: Two hepatocellular carcinomas (HCC), one renal cell carcinoma, one hepatic focal nodular hyperplasia (FNH), and one iliac bone metastasis of HCC were used. Images were collected by power mode using the GE-Yokogawa LOGIQ 700. Power Doppler images were recorded on S-VHS videotape and fed into a workstation. Flow information was then extracted, and 3D images were constructed by surface rendering. RESULTS: Preparation of 3D images could be accomplished in one minute after feeding flow information into the workstation. Intratumoral vascular structure could be easily evaluated. This 3D imaging was useful for the differential diagnosis of hepatic tumors, especially HCC and FNH, which were confirmed in 3D imaging to have distinctly different intratumoral vascular structures. CONCLUSION: 3D imaging of intratumoral blood flow was possible by 3D processing of the power Doppler images. 3D power Doppler imaging is expected to be helpful for the differential diagnosis of tumors.
