Adenocarcinoma of the bladder 29 years after ileocystoplasty.

We report a case of adenocarcinoma found at the bladder site 29 years after ileocystoplasty. The patient was a 43-year-old woman who had undergone a closed-loop type of ileocystoplasty for a small tuberculous contracted bladder when she was 13 years old. The tumor that arose in the bladder site was colonic type adenocarcinoma. Four cases of neoplasm occurring in the ileal portion following ileocystoplasty have been reported in the English and Japanese literature, but no case arising originally in the bladder site has been reported.

[Surgical treatment of lung cancer with chest wall invasion].

From 1955 through 1987, 64 cases underwent operation at Mie University Hospital for carcinoma of the lung invading the chest wall (p-T3). According to the classification by Mishina et al, the extent of tumor invasion of the chest wall was p3a-b in 63%, p3c in 18%, and p3d in 19%. Histologically, the tumors were epidermoid carcinoma in 565, adenocarcinoma in 31%, large cell carcinoma in 11%, and small cell carcinoma in 2%. The post surgical staging of N factor was N0 in 39%, N1 in 33%, and n2 in 28%. Extrapleural resection was performed in 27 cases and extended resection (en block resection of chest wall and lung) was performed in 30 cases with an operative mortality of 0%. The actuarial three-year survival rate (Kaplan-Meier method) for patients with p3d was 18.2% but three-year survival for patients with p3a-b and p3c was more than 30%. In spite of p3a-b, however, four-year survival for patients without extended resection was decreased to 11.6%. Four-year survival of patients with adenocarcinoma, epidermoid carcinoma, and large cell carcinoma was 5.9%, 28.4% and 75% respectively, lymphatic metastases reduced survival, with a three-year survival rate of 52.8% for patients with N0 disease and 8.35 for those with N2 disease. Among patients with extended resection, four-year survival for patients 60 years of age of less was 50.8%, greater than the 13.3% four-year survival for the patients more than 61 years of age. We conclude that long-term survival can be influenced by the extent of tumor invasion. In factor, histologic type, and the patient's age, and that extended resection and adjuvant therapy should be applied for treatment of lung cancer with chest wall invasion.

[A case report of Rastelli operation in 2-month-old infant with truncus arteriosus (type II)].

A two-month-old girl with heart failure from truncus arteriosus (Collett & Edwards Type II) underwent a total correction by the Rastelli procedure using 12-mm-diameter Hancock valved conduit. The orifice of pulmonary arteries was closed from inside without detachment of the pulmonary artery from truncus. The distal anastomosis of the conduit was made to left pulmonary artery. Primary sternal closure was difficult and delayed closure was performed using splint with a resin plate. The skin was primarily closed using bilateral advancement myocutaneous flaps. Complete closure of the sternum was made on the 11th postoperative day. A rotation flap of the right abdominal rectal muscle was used to cover the partially necrotic skin over the sternum. The patient had persistent respiratory and cardiac problems, but was discharged 14 months after surgery.

Cyclosporin A and anti-Ia antibody cause a maturation defect of CD4+8- cells in organ-cultured fetal thymus.

The effects of anti-Ia antibodies and cyclosporin A (CsA) on phenotypic differentiation of murine thymocytes were assessed in organ-cultured fetal thymuses. Both agents specifically abrogated the generation of CD4+8- thymocytes. Immunohistochemical studies revealed that Ia antigen on the organ-cultured thymic epithelial cells did not disappear with the addition of the agents, although anti-Ia antibody was proved to bind to Ia antigen during the culture. On the other hand, CsA neither changed the expression of Ia nor bound to it. As CsA is known to block the signalling cascade initiated by perturbation of T-cell receptor (TcR), it is suggested that both the Ia expression in the thymus and the signalling via receptors on thymocytes, the signals presumably generated by TcR binding to class II MHC molecules, might be necessary for phenotypic differentiation of class II MHC-restricted T cells (CD4+8- cells), but not for class I MHC-restricted T cells (CD4-8+).

[Anesthesia for aorto-coronary bypass graft surgery in children with Kawasaki disease].

We investigated anesthetic management of aorto-coronary bypass graft surgery (CABG) in the children who have coronary disease due to acute febrile mucocutaneous lymph node syndrome (Kawasaki disease). A retrospective observation was made on anesthetic management of 17 patients with Kawasaki disease. They were 13 boys and 4 girls and their ages ranged from 5 to 13 years. They underwent CABG during the past 10 years (1976-1986) in Tokyo Women's Medical College Daini Hospital. Induction of anesthesia was carried out with halothane-nitrous oxide-oxygen (GOF) in 16 patients and morphine-nitrous oxide-oxygen (GOM) in one patient. Anesthesia was maintained with GOF in 11 patients, and in 6 patients with GOF and narcotics. High-dose fentanyl was not used for anesthetic induction in children with Kawasaki disease. Pulse rate increased significantly after endotracheal intubation. Blood pressure increased after endotracheal intubation in 5 patients and after sternotomy in one patient. Hypotension developed in 2 patients. Coronary spasm did not occur during the all procedures. Control of blood pressure during CABG in children was easier than in adult patients. But the control of pulse rate offers a problem in anesthetic management of the children with Kawasaki disease.

[Genesis of systolic anterior motion (SAM) of the mitral valve in patients with aortic regurgitation].

This study was performed to evaluate the incidence and genesis of systolic anterior motion (SAM) of the mitral valve apparatus in patients with aortic regurgitation (AR). The study population consisted of 44 patients with non-rheumatic AR, without significant aortic stenosis or mitral regurgitation. The presence and location of SAM in the short-axis view were determined by M-mode echocardiography guided by two-dimensional echocardiography. The extent and direction of the regurgitant jet were decided by pulsed or two-dimensional Doppler echocardiography. SAM was observed in 21 (48%) of the 44 patients, and it was more frequently observed in patients with an etiology of aortic valve prolapse or annuloaortic ectasia than in those of other etiology (10/14 vs 10/30; p less than 0.05). Twenty-eight patients whose regurgitant jet was directed posteriorly and impinged on the mitral valve apparatus had a significantly higher incidence of SAM than did the other 16 patients (18/28 vs 3/16; p less than 0.01). In eight of 10 patients in whom the direction of the regurgitant jet could be precisely observed by two-dimensional Doppler echocardiography. SAM was observed at the place where a regurgitant jet was directed along the anterior mitral valve in the short-axis view. M-mode measurements (LVDd, LVDs, %FS, LVDd-LVDs) of the patients with SAM had greater values than those of patients without SAM.(ABSTRACT TRUNCATED AT 250 WORDS)

Determination of erythrocyte porphyrins by reversed-phase high-performance liquid chromatography using capsule-type silica gels coated with silicone polymer.

A simple, rapid and reliable method is described for determining erythrocyte porphyrins using a new type of reversed-phase silica whose surface is coated with silicone polymer. After a single-step extraction of blood with N,N-dimethylformamide, zinc protoporphyrin IX and protoporphyrin IX were separately quantified in a single run in 5 min. The column life was so long that no alteration in elution profile or retention time was apparent after 1500 injections of samples. The method described would be useful for the screening of lead exposure or iron-deficiency anaemia, and also for the diagnosis of erythropoietic protoporphyria.

Quantitative analysis of the distribution of serotonin-immunoreactive cell bodies in the mouse brain.

The distribution of serotonin neurons in the mouse brain was studied using a peroxidase-antiperoxidase (PAP) immunohistochemical method to obtain quantitative data on serotonin-containing cell bodies. The average total number of perikarya of serotonin neurons was about 26,000, of which about 17,000 cells (66.9%) were situated in the raphe system. The nucleus raphe dorsalis was the main region where a large number of serotonin neurons were present, and it contained about 9000 cells. Other than the raphe system, serotonin-immunoreactive perikarya were seen in the following regions: the reticular formation (18.9%), the lemniscus medialis (8.5%), the nucleus interpeduncularis (1.5%), the subpyramidal region (4.0%) and the periaqueductal gray (0.2%).

Combined factor VII and protein C deficiency found in a patient with peripheral pulmonary artery stenosis accompanied by progressive pulmonary hypertension and hemoptysis.

A congenital deficiency of factor VII and protein C was found in a 21-year-old female suffering from recurrent and progressive attacks of dyspnea and hemoptysis over the last four years. She has been followed in our Department since the age of 17 under a diagnosis of peripheral pulmonary artery stenosis and pulmonary hypertension as confirmed by cardiac catheterization and angiography. Prolonged prothrombin time repeatedly examined during this time period prompted us to perform detailed coagulation studies. We found that factor VII and protein C were both half normal in activity as well as in antigen. Three other members of her immediate family were also found to be affected with this combined deficiency. Since the genes encoding factor VII and protein C are located in different chromosomes, the 13th and the second chromosomes, respectively, expression of the combined hereditary deficiency is a random and very rare association on the basis of frequencies of 1:50,000 for factor VII and 1:16,000 for protein C deficiencies.

Interaction of Penicillium notatum phospholipase B with divalent cations.

The interaction between Penicillium notatum phospholipase B and divalent cations such as Ca2+ and Mg2+ was studied. When the purified enzyme, present at concentrations of submicrogram to microgram per ml, was incubated with submillimolar to millimolar concentrations of CaCl2 or MgCl2, the enzymatic activity was remarkably decreased (to no more than 30% of original activity, when the enzyme was incubated with 2 mM CaCl2 for 15 min). The inhibitory effect of divalent cations was reversible, since dialysis against a metal chelator, such as EDTA or EGTA, substantially restored the enzymatic activity. Atomic absorption analysis showed the purified enzyme molecule to be present in a complex with Ca2+ at a ratio approaching 1:1, and this Ca2+ binding was shown to be extremely tight, since repeated dialyses of the enzyme molecules against EDTA or EGTA could remove the divalent cations only in a gradual manner. During this process, the enzyme activity increased also gradually. The remnant fraction of tightly bound Ca2+ was released from the enzyme molecule after the denaturation of the enzyme by treatment with guanidine hydrochloride, and the apoenzyme recovered its substantial activity after removal of the denaturing agent by dialysis. On the other hand, the content of Mg2+ in the purified enzyme molecule was lower than that of Ca2+, and the association of Mg2+ with the enzyme was much weaker in comparison to that of Ca2+. Atomic absorption analysis of the enzyme exposed to exogenous Ca2+ showed a fast removal, by dialysis, of unbound and weakly bound divalent cation, followed by a gradual removal of endogenous Ca2+ and a concomitant increase of enzymatic activity, which are similar to data obtained for the purified enzyme. Results shown in this report suggest some regulatory roles of divalent cations, especially of Ca2+, in the enzymatic function of P. notatum phospholipse B.

Characterization of proteoglycans synthesized by rat thyroid cells in culture and their response to thyroid-stimulating hormone.

A Fisher rat thyroid cell line was maintained in culture and the cells were labeled with [3H]glucosamine, [35S]sulfate, and [35S]cysteine to examine the synthesis of proteoglycans. 3H and 35S radioactivity from these precursors were incorporated into both chondroitin sulfate (CS) and heparan sulfate (HS) proteoglycans. CS proteoglycans were almost exclusively secreted into the medium while HS proteoglycans remained mainly associated with the cell layer. Single chain glycosaminoglycans released by papain digestion or alkaline borohydride treatment of either the CS or HS proteoglycans had average molecular weights of approximately 30,000 on Sepharose CL-6B chromatography. Both CS and HS proteoglycans were relatively small and contained only one or two glycosaminoglycans chains. 3H and 35S incorporation into both CS and HS proteoglycans were increased by thyroid-stimulating hormone (TSH) in a dose-dependent manner, which is in part explained by an adenylate cyclase-dependent mechanism as indicated by a similar effect in response to dibutyryl cAMP. TSH enhanced the incorporation of 35S into CS from [35S]cysteine about 1.5-fold and that from [35S]sulfate about 2-fold. This result demonstrated that the increased 35S incorporation from the [35S]sulfate precursor reflects an actual increase in sulfate incorporation and is not simply a result from an apparent increase in specific activity of the phosphoadenosine phosphosulfate donor. Analysis of disaccharides from chondroitinase digests revealed that the proportion of non-sulfated, 4-sulfated, and 6-sulfated disaccharides was not altered appreciably by TSH. These results, together with the disproportionate increase in 3H incorporation into CS from [3H]glucosamine, indicated that TSH increased the specific activity of the 3H label as well. Chase experiments revealed that CS proteoglycans were rapidly (t1/2 = 15 min) secreted into the medium and that the degradation of cell-associated proteoglycans was enhanced by TSH.

Increased cytosol aminopeptidase and lactate dehydrogenase in serum originating from lymphocytes in measles and rubella infection.

We determined cytosol aminopeptidase (c-AP; EC 3.4.11.1) and lactate dehydrogenase (LDH) levels in serum; these enzymes are known to originate from lymphocytes in patients with measles and rubella. In patients with measles (n = 19), both enzyme levels increased markedly with the onset of rash: mean (+/- SD) c-AP was 269.7 +/- 103.5 U/L and LDH was 1149.5 +/- 255.2 U/L. In patients with rubella, activities of both enzymes increased mildly: c-AP (n = 18) was 81.6 +/- 24.4 U/L and LDH (n = 13) was 674.0 +/- 168.8 U/L. Increased c-AP and LDH levels in patients with measles and rubella presumably originate from the destruction of infected, activated lymphocytes, especially T lymphocytes.

Asbestos fibers in human pulmonary and extrapulmonary tissues.

The association between asbestos fibers in human lung tissues and those in other extrapulmonary organs was studied by analysing three autopsied cases, one of whom had a high pulmonary asbestos burden, one an intermediate burden, and one a low burden. The amount, types and sizes of asbestos fibers in the tissues of lung, liver, spleen, pancreas, kidney, and gastrointestinal tract were identified and measured by transmission electron microscopy equipped with an energy dispersive X-ray analyser. The following results were obtained: 1) The concentrations of asbestos fibers in extrapulmonary organs tended to increase with that in lung tissues. 2) The types of fibers identified in lungs were approximately consistent with those in other organs. 3) A relatively large range of fiber length was observed in both lungs and other organs. The findings extend and support previous light microscopic studies comparing asbestos bodies in various organs, and may imply that persons with high levels of asbestos in their lungs are also likely to have the same types and high levels of asbestos in their other tissues.

Pharmacodynamics of chlorzoxazone in rats.

Pharmacological effect-time profiles of a centrally acting muscle relaxant, chlorzoxazone, were evaluated by both rotarod and crossed extensor reflex (CER) methods. Drug response was measured by percent change in the remaining time on the rod for the former method and in intensity of isometric contraction of the muscle for the latter method. The drug response was quantitated using the logarithmic-logistic function and then biophase levels were calculated. The results indicated that the maximum response was observed 10 min after drug administration for both methods at every dose studied. However, the values of ED50 indicated that the CER method was more sensitive than the other. Pharmacodynamic analysis offered the results that the dose-normalized biophase levels for each dose obtained from the CER method were coincident with each other and that the logarithmic-logistic function was proven to be of use for the pharmacodynamic modeling which could simply assess the bioavailability. On the other hand, all dose-normalized values obtained from the rotarod method did not scatter around the same line. This evidence indicated a violation of assumption for the pharmacodynamic analysis. From the above data, the CER method proved to be of use for assessing the drug response based on the dose-independent kinetics with high sensitivity.

Diagnosis of neuroblastoma metastasis in bone marrow with a panel of monoclonal antibodies.

Neuroblastoma, along with rhabdomyosarcoma, Ewing's sarcoma, and acute lymphoblastic leukemia/lymphoma, is one of the small, round-cell tumors of childhood. All of these malignancies show a propensity to metastasize to bone marrow. Occasionally when the clinical picture is unclear and the tumor is particularly anaplastic, it can be difficult to arrive at a diagnosis by conventional histological and biochemical procedures. In the present study, a panel of nine monoclonal antibodies was used to undertake a detailed analysis of seven bone marrows contaminated with tumor cells: six cases of stage IV neuroblastoma, and one case of stage IV-S neuroblastoma. The antibody profiles obtained were compared with those deduced from the studies of over 20 marrows from patients with acute lymphoblastic leukemia. A comparison of these data with those obtained from the studies of rhabdomyosarcoma and Ewing's sarcoma cell lines and tissues suggests that when high levels of tumor cells are present in the marrow, it is possible to obtain a confident diagnosis of either neuroblastoma or acute lymphoblastic leukemia. In addition, the immunocytological identification of neuroblasts in bone marrow enables accurate staging without histological examination.

Surfactant replacement therapy in neonatal respiratory distress syndrome. A multi-centre, randomized clinical trial: comparison of high- versus low-dose of surfactant TA.

We conducted a prospective, randomized, controlled trial comparing the efficacy of two doses of a reconstituted bovine surfactant (Surfactant TA) in premature infants requiring mechanical ventilation shortly after birth for respiratory distress syndrome. Forty-six infants weighting 1000-1499 g were randomized into two groups: a low-dose group (23 infants given a single dose of 60 mg surfactant lipid/kg) and a high-dose group (23 infants given a single dose of 120 mg/kg). The mean (SD) age at which surfactant was given was 5.5 (+/- 1.2) h in the low-dose group and 6.0 (+/- 1.5) h in the high dose group. Both treatments improved oxygenation (increased arterial-alveolar PO2 ratio) with decreased mean airway pressure, the high-dose surfactant having a more beneficial effect in prolonging the response. Infants in the high-dose group had significantly less (P less than 0.05) incidence of both intraventricular haemorrhage and bronchopulmonary dysplasia. This prospective trial documents that a greater benefit can be obtained by increasing the dose of surfactant (120 mg/kg) beyond 60 mg/kg treatment of premature infants with severe respiratory distress syndrome (RDS).
