[Statistical observation of crowns and bridges in Matsumoto Dental College].

A study was made of 11,425 crowns and 2,385 bridges which had been fabricated for 5,588 patients at the Prosthetic Clinic of Matsumoto Dental College during the fourteen-year period, from January 1973 to December 1986. Some of the results were as follows: 1. 753 patients, 1,585 crowns and 372 bridges in 1983 were the most numerous in all the survey years. 2. The rate of vital teeth of crowns has increased from 1984. 3. Of crowns, 22.1% and of bridge retainers, 22.3% were fabricated as porcelain fused to metal crown in 1985. 4. The rate of resin facing crowns of crowns and bridge retainers increased sharply in 1986. 5. Of crowns, 19.0% and of bridge retainers, 30.5% were fabricated as partial coverage crowns in 1986. 6. The rate of more than 5-unit bridges increased from 1982 to 1985.

Crowns

Occurrence of Gal beta (1----3) GalNAc-Ser/Thr in the linkage region of polygalactosamine containing fungal glycoprotein from Cordyceps ophioglossoides.

The nitrous acid deamination liberated the N-acetyl galactosamine containing glycoproteins from galactosminoglycan (CO-N) which has been isolated from Cordyceps ophioglossoides culture. Mild alkaline borohydride degradation of the purified glycoprotein released oligosaccharide alditols. The smallest oligosaccharide alditol was characterized to be Gal beta (1----3)-GalNAcol. This result indicated that polygalactosamine part attached to protein part via Gal beta (1----3) GalNAc-Ser/Thr as the linkage region. To our knowledge, this is the first report of Gal-GalNAc-Ser/Thr linkage from fungal glycoprotein.

[Clinical application of local hyperthermia combined with antineoplastic agents].

At present hyperthermia shows great promise when combined with other modalities. Local thermo-chemotherapy may also be a very attractive research field, but the combination of hyperthermia and anti-cancer drugs is still poorly understood. In this review, the background and the clinical reports on heat and chemotherapy which have been reported during the last year were analyzed. Finally, our clinical experiences with local hyperthermia combined with anti-cancer drugs for gastrointestinal cancer were also reported.

Adenocarcinoma

Structure and antitumor activity of an alkali-soluble polysaccharide from Cordyceps ophioglossoides.

A water-insoluble extracellular glucan (CO-1) was isolated from the precipitate formed on incubation of the culture filtrate of Cordyceps ophioglossoides at 37 degrees for 19 h. CO-1 was homogeneous as judged by h.p.l.c., electrophoresis, and ultracentrifugation, and the average molecular weight was determined by h.p.l.c. to be 632,000. The 1H- and 13C-n.m.r. and the i.r. spectra indicated that the glucosidic linkages in CO-1 were beta. From the results of methylation analysis, Smith degradation, n.m.r. studies, and enzymic hydrolysis, it was concluded that CO-1 is composed of a backbone of (1----3)-linked beta-D-glucopyranosyl residues with a beta-D-glucopyranosyl group attached to O-6 of every second D-glucopyranosyl residue of the backbone. CO-1 strongly inhibited the growth of Sarcoma 180 solid-type tumor. CO-1 polyalcohol, which was prepared by Smith degradation of CO-1, exhibited more-potent antitumor activity than CO-1. The absorption maximum of Congo Red shifted significantly in the presence of CO-1.

[Nutritional management after massive resection of the pancreas].

Eleven patients after total pancreatectomy and 68 patients after pancreatoduodenectomy were reviewed for evaluating the importance of nutritional management after massive resection of the pancreas. Nutritional supply by IVH resulted in maintaining and improving the hepatic function under obstructive jaundice in clinical and experimental studies. Administration of elemental diet after the operation shortened the period of IVH, and had a effect to spare the requirement of exogenous insulin. One of the most important problems after pancreatectomy with extensive dissection of the lymph nodes and the nerves around superior mesenteric artery was a malnutrition which appeared frequently after long term follow-up. The malnutrition should be treated by IVH as soon as possible for preventing further aggravation of digestive and absorptive function of the alimentary tract. Oral or nasogastric tube administration of elemental diet is useful for weaning the patient from IVH, and it serves to prevent subsequent development of malnutrition. In six totally pancreatectomized patients studied, postabsorptive plasma concentration of Arg., Lys., Thr., Ala., Gly., Ser. and Pro. were greatly elevated compared to normal value, however these abnormalities were normalized by 1 to 3 mg of glucagon administration. Glucagon administration resulted in no significant change in daily nitrogen balance.

Humans

Effectiveness and limitation of surgical treatment for chronic pancreatitis.

The effectiveness of various forms of therapy for chronic pancreatitis during the past fourteen years at our clinic was presented. Forty-nine patients underwent surgical treatment and twenty-six were treated medically, a total of seventy-five. The surgical procedures were classified as direct and indirect. Therapeutic results were better in the surgical group. Relief of abdominal pain was achieved in 92% of the direct and 94% of the indirect operative groups. Objective results, such as pancreatic function tests, were inconsistent. Direct operations were further evaluated by comparing the results of longitudinal pancreaticojejunostomy to other surgical procedures.
