[Towards cloning the gene responsible for myotonic dystrophy].

For the purpose of cloning the gene of myotonic dystrophy (DM) using the technique of reverse genetics, we have introduced new methods such as microdissection, a YAC library and a Not I linking library and cloned many DNA fragments derived from the region of 19q13.2. Then we have assigned these to chromosome 19 by linkage map (CEPH families and linkage disequilibrium) and physical map (PFGE and in situ hybridization). Here we have described these methods.

Randomized trial of combination antibiotic therapy in patients with hematological disorders. Hanshin Study Group of Hematopoietic Disorders and Infection.

A 3 arm prospective randomized trial was designed to compare the effectiveness of aztreonam (AZT) + clindamycin (CLDM), amikacin (AMK) + AZT and CLDM + AMK as antibiotic therapy for fever in patients with hematological disorders. Between July 1987 and June 1988, one hundred and sixty of the 199 febrile episodes entered in this study were evaluated for response. Underlying diseases were hematological malignancies in 88.8% of the subjects, and there was no statistically significant differences in incidences of the diseases among the 3 groups. Efficacy rates were 64.2% in the AZT + CLDM group, 52.8% in the AMK + AZT group and 35.2% in the CLDM + AMK group. Efficacy rates in those with neutrophil counts was less than 500/microliters before starting the antibiotics were 53.3%, 43.8% and 25.0%, respectively, in the 3 groups. The combination of AZT + CLDM was found to be the most effective even in the treatment of infections associated with febrile neutropenic patients.

[Bone marrow transplantation for MDS].

Five patients with myelodysplastic syndrome (RA: 4 cases, RAEB in T: 1 case) were treated with myeloablative immunosuppressive therapy followed by bone marrow transplantation (BMT). Median age was 20-y-o (11-31-y-o). All patients were prepared with cyclophosphamide and total body irradiation. Engraftment was documented in all patients. One patients (case 4, 31-y-o female) died of brain hemorrhage due to the thrombocytopenia refractory to platelet transfusion because of anti-platelet antibody in 34 days after BMT. A patient with RAEB in T was also died of respiratory failure from interstitial pneumonia on Day 173. One patient (case 1, 22-y-o, female) progressively became granulocytopenic and thrombocytopenic status after BMT. She suffered from life-threatening infection and then received a second bone marrow cell infusion from the same donor without any preparative conditioning. These results suggest that BMT could be the treatment of choice for MDS, especially for the patients with RA who have poor prognostic factors including life-threatening cytopenia and or cytogenetical abnormalities.

[Mass-screening for breast cancer using C-mode display ultrasonography].

In order to elevate the detection rate of breast cancer in mass-screening, we examined whether the C-mode display ultrasonography is useful for detection of breast cancer. 1) Diagnostic sensitivity of C-mode ultrasonography was 88.2%, specificity was 66.7% and predictive value was 58.8%, respectively. 2) The detection rate of C0mode ultrasonography was 100% for tumors more than 5.0cm, 87.5% for 2.0-5.0cm, 74.2% for less than 2.0cm. Especially, the diagnostic value of lesser than 1.0cm breast cancer was high, 66.7%. 3) In findings of C-mode ultrasonography, images characteristic of carcinoma can be obtained regarding their unclear margin (70.6%), heterogenous (88.2%) and hypoechoic (70.6%) internal echoes and malignant halo (61.8%). Our present data suggest that C-mode ultrasonography is very useful for the mass screening for breast cancer.

[Mixed blast crisis with the cytogenetic evidence of three clonal evolutions].

A 46-year-old man was diagnosed as having chronic myelogenous leukemia (CML) in chronic phase in Dec. 1985. In Dec. 1987, anemia and leukocytopenia progressed, and the percentage of blast cells increased in the bone marrow. The blast cells were lymphoblastoid and positive for TdT. It was treated as a lymphoid crisis with vincristine and prednisolone, and complete remission was achieved. However, the blasts (11%) were observed in the bone marrow in Mar. 1988, and the chromosomal analysis revealed 46, XY, t (2q-; 11q+), t (9q+; 22q-) in 13 out of 20 cells. In June, the percentage of the blasts increased again, but chromosomal analysis showed a different karyotype, 46, XY, t(2p-; 11p+), t(9q+; 22q-) which was observed in 9 out of 10 cells. Then, myeloblastoid cells increased rapidly in spite of the chemotherapy in Dec. 1988. The chromosomal analysis showed 46, XY, 2p-, 7q-, 9q+, 11p+, 22q- in all analyzed cells. The rearrangement of the bcr gene could be detected by the Southern blotting. The blasts were positive for CD7, CD11, CD13, CD33, CD36, CD41 and CD42, suggesting that the blasts had the surface phenotypes of both myeloid and megakaryocytoid-lineage. This is a case with the mixed blast crisis that changed from the lymphoid to the myelo-megakaryocytoid in nature, in which three clonal evolutions were observed during the clinical course.

Preclinical detection in Japanese families with myotonic dystrophy using polymorphic DNA markers.

Myotonic dystrophy (DM) is a genetic disease inherited by an autosomal dominant trait and characterized by multi-organ disorders. Although its biochemical basis has been unknown, the DM locus is closely linked to D19S19 and APOC2 on the long arm of chromosome 19 both in Japanese and Caucasian populations. Linkage studies of Japanese DM families using these polymorphic DNA markers detected two asymptomatic gene carriers in two unrelated families.

Blood flow redistribution during spontaneous wheel walk of the rat.

Changes in regional blood flow and arterial pressure in the rat during spontaneous walk in a wheel were observed. An electromagnetic flow probe was implanted around the carotid, superior mesenteric, or renal artery, or the terminal aorta and a catheter for pressure measurement was inserted into the terminal aorta or the common carotid artery. The wheel had a diameter of 35 cm and rotated passively as the rat walked. When hindquarter (terminal aortic) flow increased markedly during wheel walk, carotid flow decreased, superior mesenteric flow decreased or remained unchanged, and renal flow did not change. Arterial pressure remained almost unchanged and heart rate increased an average of about 10%. Semiquantitative considerations indicated that arterial pressure was maintained in the face of the profuse increase in hindquarter flow during wheel walk by an increase in cardiac output rather than shifts of blood flow from other regions.

[Study on follow-up of patients with biopsy-proven breast disease].

Eight hundred forty-one patients with biopsy-proven benign breast disease were followed annually in prospective manner for 1-10 years to analyse breast cancer development at aspect of histologic features, age at entry and mammographic patterns by Wolfe. No association was found between patients who developed cancer and those who did not with respect to the initial histologic features. The rate of breast cancer development in patients in thirties was the highest. The breast cancer development was found most often in less than three years follow-up period. The breast parenchymal patterns at entry related to cancer development. In 10 cancer patients, 8 patients showed DY pattern and 2 showed P2 patterns. The present results demonstrate that the patients with biopsy-proven benign breast disease in thirties, in less than 3 years follow up period and have parenchymal patterns of P2 or DY should be followed carefully.

[Multivariate analysis of acute leukemia].

The outcome of chemotherapy in patients of 150 cases of acute leukemia was investigated. Patients were divided into two groups, that is group I: 100 cases of acute leukemia treated between 1980-1984, group II: 50 cases of acute leukemia between 1985-1986. Complete remission was achieved in 66% of group I and in 82% of group II. Using multivariate analysis, the initial levels of FDP and WBC count and age were found to be important factors to induce complete remission. Female sex was suggestive of a longer remission duration and survival by discrimination analysis.

[Blind-ending bifid ureter: 3 case reports].

Blind-ending bifid ureter is one of the most rare anomalies of the upper urinary tract. Three cases of blind-ending bifid ureter are reported. Case 1: A 53-year-old man was admitted with right lumbal colic pain. Kidney-ureter-bladder X-ray revealed the right ureteral stone and drip intravenous pyelography (DIP) revealed the left blind-ending bifid ureter. Case 2: A 61-year-old woman admitted with the complaint of asymptomatic microhematuria. DIP revealed extension of the right middle ureter and the left incomplete duplication of ureter. Retrograde pyelography revealed the right blind-ending bifid ureter. Case 3: A 57-year-old woman was admitted with asymptomatic microhematuria. DIP revealed the left blind-ending bifid ureter. We collected 68 cases of blind-ending bifid ureter reported in Japan including our own according to the definition of Culp.

[Clinical evaluation of monotherapy with ceftazidime for severe infections complicating hematological disorders. Hyogo Cooperative Study Group of Infectious Diseases Complicating Hematological Disorders].

Clinical effects of the monotherapy with ceftazidime (CAZ) were evaluated in patients with severe infections associated with febrile granulocytopenia in hematological disorders in 10 institutions. CAZ (4-6g/day) was administered intravenously by drip infusion divided into 2 to 4 doses. 83% of the underlying diseases were hematological malignancies. Infections mainly consisted of documented sepsis (10%), presumed sepsis (60%). Overall efficacy rate was 65%, and that of septic patients was 83.3%. Adverse reactions were minimal, and this study revealed safety of CAZ.

Characterization of antigens of Paragonimus miyazakii by ELISA.

The characterization of adult worm and excretory-secretory (ES) antigens of P. miyazakii was performed with a sandwich enzyme-linked immunosorbent assay (ELISA) IgG and Fab' fragments were prepared from P. miyazakii-infected rat sera. The former was purified by affinity column chromatography. The latter was prepared from IgG2c by passage through a Protein A affinity column, then conjugated to horseradish peroxidase. Cross-reactivity of the ELISA with Fasciola hepatica. Fischoederius elongatus. Taenia saginata. Schistosoma mansoni and S. Japonicum was negligible. With Paragonimus westermani, the same genus as P. miyazakii, cross-reactivity was below 30%. The molecular weight of both antigens was comparable (36,000) as determined by Sephadox G-100 gel filtration. Isoelectric points in both antigens had many peaks in a pH range of 3.9-4.7. In adult worm, pI of the major peaks were 4.6 and 4.7 and minor part of peaks had the common isoelectric points with four ES peaks in a pH range of 4.1-4.6. Antigenicity per protein concentration of both antigens was examined by the ELISA. The ES antigen was about five times as active antigenically as adult worm antigen. These results suggest that ES is one of the major antigens inducing antibody synthesis in rats infected with P. miyazakii, and that other antigens may be present within the adult worm.

Nucleotide sequence of mouse EndoA cytokeratin cDNA reveals polypeptide characteristics of the type-II keratin subfamily.

EndoA cytokeratin (EndoA) belongs to a family of intermediate filaments (IFs) and is coordinately expressed with EndoB cytokeratin during early mouse embryogenesis. We have isolated and sequenced a cDNA from a library constructed from mRNA of parietal yolk sac-like cells, PYS-2, which are derived from mouse teratocarcinoma. Sequence analysis reveals that EndoA is composed of 490 amino acids, its Mr is 54,362, and it contains a central alpha-helical coiled-coil structure flanked by non-alpha-helical domains. The amino acid sequence of EndoA is highly homologous with human cytokeratin No. 8 (93%) and with bovine cytokeratin No. 8 (91%) not only in the central domain, but also in its tail portion, which is less conserved among other intermediate filaments. A comparison with the other cytokeratin proteins characterizes this polypeptide as a non-epidermal type of cytokeratin of the basic (type-II) subfamily. The C-terminal sequence of EndoA is identical to that of human and bovine cytokeratin No. 8 and also highly conserved in other intermediate filaments (desmin, vimentin, glial fibrillary acidic protein and EndoB). It suggests that these may be involved in interaction with some cell component(s), or in more general roles to form IFs. The N-terminal head region is rich in Ser residues including possible phosphorylation sites.

Effect of the dietary alpha-linolenate/linoleate balance on lipid compositions and learning ability of rats. II. Discrimination process, extinction process, and glycolipid compositions.

Donryu strain rats through two generations were fed semi-purified diets supplemented with safflower seed oil (rich in linoleic acid) or with perilla seed oil (rich in alpha-linolenic acid), or a conventional laboratory chow (normal control diet). Brightness-discrimination learning ability was determined to be the highest in the perilla oil-fed group, followed by the normal group, and then by the safflower group, extending our earlier observation in a different strain of rat that alpha-linolenic acid is a factor in maintaining high learning ability (Yamamoto, N., M. Saitoh, A. Moriuchi, M. Nomura, and H. Okuyama. 1987. J. Lipid Res. 28: 144-151). After the brightness-discrimination learning test was administered, extinction of learning was measured. The time required for extinction was significantly longer in the safflower group than in either the perilla group or the normal diet group. Thus, the dietary alpha-linolenate/linoleate balance affected both the learning and the extinction of learning. The glycolipids of the cerebrum, cerebellum, and olfactory lobe were analyzed. Although the fatty acid compositions of the sulfatide and gangliosides were significantly different in the three parts of the brain, relatively little difference was observed in the fatty acids of glycolipids between the safflower group and the perilla group, suggesting that gross changes in brain glycolipids are not responsible for the differences in learning abilities between these dietary groups.

Physico-chemical properties and stabilities of the highly potent calcium antagonist benidipine hydrochloride.

(+/-)-(R*)-2,6-Dimethyl-4-(m-nitrophenyl)-1,4-dihydropyridine-3, 5-dicarboxylic acid (R*)-1-benzyl-3-piperidinyl ester, methyl ester hydrochloride (benidipine hydrochloride, KW-3049), a highly potent calcium antagonist, was examined to clarify its physico-chemical properties, i.e. melting point, spectra (UV, IR, NMR, MS), X-ray diffraction pattern, thermal analysis, solubilities, pKá, partition coefficient and chromatography (TLC, HPLC). The stabilities of KW-3049 were studied under various conditions by HPLC. KW-3049 in solid state was very stable to heat and moisture, and fairly stable to light.

[Experience with extracorporeal shock-wave lithotripsy of upper urinary tract stones].

The first 121 cases of upper urinary tract stone treated by extracorporeal shock wave lithotripsy (ESWL) at our clinic were analyzed. Three months after ESWL, 78% of the patients became stone free, and 17% of the patients had residual stone fragments which were less than 5 mm in diameter and considered passable. The success rate was therefore about 94.6%. No severe side effects were observed, although temporary elevations in serum creatinine phosphokinase, lactate dehydrogenase, glutamic-oxaloacetic transaminase, aldolase and total bilirubin occurred. These results indicated that ESWL was a safe and effective procedure for upper urinary tract stones.
