[Clinical efficacy of Z-100 for the treatment of leukopenia caused by radiotherapy--a multi-center double-blind comparative study with inosine].

The clinical usefulness and the optimal dose of Z-100 for the treatment of leukopenia caused by radiotherapy for malignant tumors was evaluated in a double-blind study comparing 20 micrograms and 30 micrograms injections of Z-100 and inosine (Z-20, Z-30 and IN group, respectively). The effective rate according to the evaluation by the attending physicians was 67.4%, 79.1% and 48.9% in the Z-30, Z-20 and IN groups, respectively, with a significantly higher efficacy rate for the Z-20 group than for the IN group. The frequency of adverse reactions was 14.3%, 3.9% and 5.6% in the Z-30, Z-20 and IN groups, respectively, with the highest rate in the Z-30 group, but there was no significant difference among the three groups. The useful rate based on total evaluation considering the effective rate and the safety profile in each group was 60.8%, 77.3% and 46.8% in the Z-30, Z-20 and IN groups, respectively, with the highest rate in the Z-20 group showing significant difference from that in the IN group. The above data confirmed the clinical usefulness of Z-100 and suggested that the optimal dose was 20 micrograms twice weekly for Z-100.

[Clinical efficacy of Z-100 for the treatment of leukopenia caused by radiotherapy--a multi-center double-blind comparative study with L-cysteine].

The clinical usefulness of Z-100 (Z group) for the treatment of leukopenia caused by radiotherapy for malignant tumors was evaluated by comparing it with that of L-cysteine (L group) by the double-blind method. For 178 complete cases, there were no significant differences in the time-course of the WBC during radiotherapy between the two group. However, for cases with a large field of irradiation (15 X 15 cm = 225 cm2), the mean WBC count in the L group decreased more than that in the Z group. The Z group showed comparable or superior efficacy for the treatment of leukopenia in the L group although there were no significant differences. Adverse reactions were observed in 5.66% and 2.94% in the Z and L groups, respectively, without any significant difference between the groups. The utility rate, useful or better, was 73.9% and 69.2% in the Z and L groups, respectively, showing a slightly higher rate in the Z group than in the L group, although the difference between the two groups was not significant. These results confirmed that Z-100 has comparable or superior efficacy to L-cysteine for the treatment of leukopenia caused by large field radiotherapy for malignant tumors.

Evaluation of antifungal volatile compounds on the basis of the elongation rate of a single hypha.

A novel method is proposed for the evaluation of the activity of an antifungal agent administered as a gas. This system is composed of a batch-flow type reaction vessel, a gas flow system, and a microscopic observation system. The agar plate was prepared on the ceiling of the reaction vessel, and the mycelium of a fungus (Aspergillus niger or Rhizoctonia solani) was inoculated onto it. After preincubation at 25 degrees C for 24 h, the reaction vessel was connected to the gas flow system. An appropriate hypha was selected, and its elongation rate was measured. Then a sample holder containing an antifungal compound was inserted into the reaction vessel from the side hole to saturate the atmosphere inside with its vapor. The retardation or inhibition of the hypha elongation was observed on a television monitor and recorded on a video tape recorder. The antifungal compound was then removed, and the reaction vessel was flushed with air. If the hypha lived, it began to elongate again. By this method, antifungal activity of seven odor compounds could be evaluated quantitatively within several hours.

[An early clinical study of Z-100 in leukopenia caused by radiotherapy].

The clinical efficacy of Z-100 for the treatment of leukopenia caused by radiotherapy for malignant tumors was evaluated in an early clinical study using three doses, 2, 10 and 20 micrograms. According to the evaluation by investigators and time-course of WBC, the highest effective rate was achieved by 20 micrograms. Adverse reactions occurred in all groups, but there were no serious or clinically relevant reactions. These results suggested that Z-100 is clinically useful for the treatment of leukopenia caused by radiotherapy.

[Bronchiectasis treated with negative extra-thoracic pressure ventilation].

A 25-year old man with 5-year history of bronchiectasis was admitted to the ICU complaining of severe shortness of breath. He had a respiratory rate of 40 to 50 breath/min. On 0.5 l/min of oxygen with nasal cannula, arterial blood pH was 7.39, Paco2 52.3 mmHg, Pao2 45.0 mmHg. Then, on 1 l/min of oxygen, Pao2 was unchanged, but Paco2 increased to 58 mmHg. As his consciousness was so clear, we applied to him the negative extra-thoracic pressure ventilator which was designed by the authors. Negative extra-thoracic pressure ventilation (NETPV) was maintained at a IMV rate of 30 breath/min, peak negative extra-thoracic pressure of -20 to -30 cmH2O, and an inspiratory/expiratory ratio of 1:2. During NETPV, his respiratory rate and oxygen consumption were decreased and Pao2 was increased compared with his spontaneous breathing. He made a recovery from dyspnea, especially, he was able to take a deep breath. When NETPV was applied to him, pulmonary artery and arterial catheterizations revealed that central venous pressure was slightly decreased, cardiac index unchanged or slightly decreased, heart rate, systemic blood pressure, and pulmonary arterial pressure unchanged compared with spontaneous breathing. The patient was able to read books and maintained communication in his voice with his family and the medical staff. After 3 days of the treatment with NETPV, a marked improvement was noted and the patient was discharged from the ICU. NETPV has the benefits as follows. First, it is very easy for both a patient and a doctor to assist his breathing because an endotracheal intubation is not necessary.(ABSTRACT TRUNCATED AT 250 WORDS)

Tardive dyskinesia with inflated neurons of the cerebellar dentate nucleus. Case reports and morphometric study.

Four autopsied cases of tardive dyskinesia manifesting oral hyperkinesia revealed markedly inflated neurons in the cerebellar dentate nucleus (DN), which had not been described previously. The inflation of the neurons was proved to be statistically significant (P less than 0.01) by morphometric study. The nuclei were usually situated in the central portion of the cytoplasm. This inflated change was different from both central chromatolysis and grumose degeneration of the DN, typically observed in progressive supranuclear palsy and dentatorubropallidolysian atrophy, and seemed to be easy to miss without careful observation, since neuronal loss and gliosis were very mild in the DN. Among a few autopsied cases of tardive dyskinesia reported previously, degeneration of the DN was described in only two. It is believed, however, that the inflated neurons of the DN may not be so rare and may be related to the occurrence of some involuntary hyperkinesia, especially oral hyperkinesia following some neurotoxic disorders and/or neuroleptic medications.
