Involvement of protein kinase C in muscarinic agonist-induced contractions of guinea pig ileal longitudinal muscle.

1. Carbachol (10(-5) M) caused an initial transient contraction (phasic contraction) and a subsequent late contraction (tonic contraction) in the guinea pig ileum. The phasic contraction was markedly suppressed by calmodulin antagonist W-7, but not by protein kinase C inhibitor H-7. 2. The tonic contraction was suppressed by H-7, but not by W-7. 3. The carbachol-induced phasic contraction in the absence of extracellular Ca2+ was suppressed by both W-7 and H-7. 4. Carbachol (10(-5) M) stimulated the formation of inositol phosphates (IPs) in the guinea pig ileum. The carbachol-induced IPs formation was dependent on the extracellular Ca2+ concentration.

Surgical reconstruction of the ocular surface in advanced ocular cicatricial pemphigoid and Stevens-Johnson syndrome.

PURPOSE: Ocular cicatricial pemphigoid and Stevens-Johnson syndrome often cause ocular damage and blindness not amenable to surgical correction. We present a new surgical technique for reconstructing affected eyes. METHODS: Fourteen eyes of 11 patients with cicatricial keratoconjunctivitis (seven patients with cicatricial pemphigoid and four with Stevens-Johnson syndrome; average age +/- S.D., 55.5 +/- 25.4 years) were treated with a combination of allograft limbal transplantation, amniotic membrane transplantation, and tarsorrhaphy, followed every 15 minutes by artificial tears derived from the patient's blood serum. Eight eyes required concomitant penetrating or lamellar keratoplasty because of corneal opacity. RESULTS: With a mean follow-up of 143 days (range, 10 to 608 days), we achieved successful ocular surface reconstruction in 12 eyes, with minimal recurrence of symblepharon. Failure occurred in two eyes (one each in 9- and 10-year-old boys) that developed corneal infiltration and vascularization. CONCLUSIONS: A combination of allograft limbal transplantation, amniotic membrane transplantation, and tarsorrhaphy, followed by the use of serum-derived tears, can reconstruct the ocular surface in most cases. Although in this study the follow-up period was short and relatively few patients were studied, this approach appears to offer an alternative to keratoprosthesis for treating severe cicatricial keratoconjunctivitis with dry eye.

Reductions in bone strength after fluoride treatment are not reflected in tissue-level acoustic measurements.

Acoustic velocity measurements are used to estimate tissue-level bone strength after fluoride therapy for osteoporosis. However, acoustic measurements provide information about elasticity, not strength, and bone elasticity does not necessarily correlate with bone strength at a tissue level. The current study was undertaken to evaluate the effects of fluoride treatment on tissue-level acoustic velocities, and to determine the relationship between acoustic velocity and bone strength measured in the femur, femoral neck, and spine. Young adult rabbits were treated with either 0 or 100 parts per million of fluoride in their drinking water for six months. After treatment, the bones were harvested for measurement of tissue fluoride, bone strength, and acoustic properties. Acoustic velocities were measured in the femoral midshaft using an acoustic microscope with a 50 MHz transducer. Both longitudinal and transverse velocities were measured. After the initial acoustic measurements the bone specimens were treated to remove either the organic matrix or mineral, and the acoustic measurements were repeated. Fluoride treatment increased bone fluoride levels 7-8 fold and reduced all biomechanical parameters. Most notably the fracture force of the femoral neck was reduced by 25% (p < 0.005), and the fracture stress of the L-5 vertebra was reduced by 19% (p < 0.05). Fluoride treatment had no significant effect on any of the measured acoustic velocities. The elastic anisotropy of the bone was decreased by demineralization (p < 0.0001) and increased by removal of the organic matrix (p < 0.0001), but unaffected by fluoride treatment. Acoustic measurements were not correlated with bone strength in the femoral neck or femoral midshaft. There was a positive correlation between the longitudinal velocity measured in the femur and the vertebral fracture stress, but this was the only positive association between acoustic velocities and strength measurements. These data cast doubt on the utility of high frequency (>2 MHz) acoustic measurements for evaluating the efficacy of fluoride therapy, especially in the hip.

Cloning and characterization of a melanin biosynthetic THR1 reductase gene essential for appressorial penetration of Colletotrichum lagenarium.

Melanin biosynthesis of Colletotrichum lagenarium is essential for appressorial penetration of the host plant. A melanin deficient mutant 9141 (Thr-) has a defect in the conversion of 1,3,8-trihydroxynaphthalene to vermelone in the melanin biosynthetic pathway. The mutant formed nonmelanized appressoria and had little infectivity on cucumber leaves. A cosmid clone pCR1 was selected from a cosmid library of wild-type C. lagenarium by means of a heterologous probe BRM2, one of the clustered genes involved in melanin biosynthesis of Alternaria alternata. pCR1 transformed the Thr- mutant 9141 to wild-type phenotype. A DNA fragment (THR1) homologous to BRM2 was subcloned from pCR1 and the nucleotide sequence determined. THR1 contains one open reading frame that encodes a protein of 282 amino acids. A transformant resulting from gene disruption showed a light brown phenotype different from the dark brown phenotype of the wild-type 104-T. The transformant formed nonmelanized appressoria and had little infectivity. The THR1 amino acid sequence contains a region highly similar to the Ver1 gene involved in the conversion of versicolorin A to sterigmatocystin in aflatoxin biosynthesis by Aspergillus parasiticus and to the T4HN reductase gene involved in the conversion of 1,3,6,8-tetrahydroxynaphthalene to scytalone and 1,3,8-trihydroxynaphthalene to vermelone in melanin biosynthesis by Magnaporthe grisea. Expression of the THR1 gene during spore germination of C. lagenarium was detected by RNA blotting. We propose that the C. lagenarium THR1 gene encodes a reductase involved in conversion of 1,3,8-trihydroxynaphthalene to vermelone.

Evaluation with MIB1 antibody of proliferative activity in ovarian clear cell adenocarcinoma.

The purpose of this study was to determine whether proliferative activity is a prognostic factor in ovarian clear cell adenocarcinoma. The paraffin-embedded sections from eight cases of ovarian clear cell adenocarcinoma were immunostained with monoclonal antibody MIB1 using microwave oven heating for antigen retrieval. The difference in survival (Kaplan-Meier) between patients with high and low MIB1 expression was estimated with the generalized Wilcoxon test. MIB1 positivity ranged from 1.80 to 27.12% with a mean of 11.43%. A cutoff point of 10% dichotomized patients into two groups. The survival of high (> or = 10%) expressing tumors was worse than that of low (< 10%) expressing tumors (p < 0.05). MIB1 immunostaining may have useful prognostic value in ovarian clear cell adenocarcinoma.

Apoptosis of colon cancers assessed by in situ DNA nick end-labeling method.

Apoptosis in colon cancers was investigated using in situ DNA nick end-labeling. Seventy-six colon cancer cases were divided according to histologically defined malignant grading, degree of invasion and other pathologic indicators. The apoptotic labeling index (ALI) was highest in cases with invasion limited to the submucosa, the difference as compared to other stages being statistically significant, while no correlation was noted between ALI and histological type. ALI significantly decreased with lymph node involvement of cancers. In contrast, there appeared to be no link with vessel invasion. ALI was significantly higher in lesions smaller than 2 cm in diameter. These results indicate that apoptosis in colon cancers is more frequent at a relatively early stage when the lesions are small, and suggest that the apoptotic phenomenon may play some role in regulating the size and progression of such tumors.

Different expression of Bcl-2 protein in gastric adenomas and carcinomas.

In order to cast light on the possible role of bcl-2 protein (Bcl-2) expression in gastric tumorigenesis, 33 cases of gastric adenomas and carcinomas originating from the same stomachs were immunohistochemically investigated for Bcl-2 protein (Bcl-2) expression, accumulation of p53 protein and cell proliferation as determined by the Ki-67 labeling index (LI). Bcl-2 expression was detected in 24/33 (72.7%) adenomas and in 6/33 (18.2%) carcinomas, the difference being statistically significant (P = 0.0001). Only 4 of 33 (12.1%) cases exhibited expression in both adenoma and carcinoma lesions in the same stomachs. Immunoreactivity was decreased in areas of cellular and structural atypia in adenoma lesions (P < 0.008), and appeared to be positively linked to the tumor progression and the degree of differentation in carcinomas, although it did not reach statistical significance. Accumulation of p53 protein was rare in the adenomas but was found in 15/33 (45.5%) of carcinoma lesions, with a significant dissociation from Bcl-2 immunoreactivity. No apparent relation between Ki-67 LI and either adenoma grading or carcinoma typing was noted, although average Ki-67 LI of the highest labeling areas in carcinomas was statistically higher than in adenomas (P = 0.0001). These results indicate that the regulation of Bcl-2 expression may differ between gastric adenomas and carcinomas, may be correlated with tumor differentiative features. In addition, p53 accumulation may play an important role in the onset of malignancy.

Responses of periodontal nerve terminals to experimentally induced occlusal trauma in rat molars: an immunohistochemical study using PGP 9.5 antibody.

The response of periodontal nerves to experimentally induced occlusal trauma in rat molars was assessed by immunohistochemistry for protein gene product 9.5 (PGP 9.5) at light and electron microscopic levels, and by computerized image analysis. The occlusal surface on the left upper first molar of 8-wk-old male Wistar rats was raised approximately 1 mm under ether anaesthesia. The rats were perfusion-fixed on d 1, 2, 3, 4, and 7 after bite-raising and then decalcified for 2-3 wk. Frozen sagittal cryostat sections were stained by the avidin-biotin complex method. By the second day after bite-raising many Ruffini endings were swollen and their outline unclear at the light microscopic level. Transmission electron microscopy disclosed PGP 9.5 reaction products within Ruffini endings that had unusually long cytoplasmic projections extending through enlarged slits of the Schwann sheaths and also diffuse extracellular PGP 9.5-immunoreactivity near the Ruffini endings. From d 2 to 4, thin nerve fibres on the pressure side of the periodontal ligament were orientated irregularly and had a prominent beaded appearance. An increase in beaded nerve terminals occurred at d 2-4 post elevation, and decreased later. These results suggest that occlusal trauma indices specific changes in the distribution and shape of nerve terminals in the periodontal ligament.

Cloning and structural analysis of the melanin biosynthesis gene SCD1 encoding scytalone dehydratase in Colletotrichum lagenarium.

Appressoria of the phytopathogenic fungus Colletotrichum lagenarium contain melanin, which has been implicated as an important factor in the penetration of host plants. A cDNA clone containing the melanin biosynthetic gene encoding scytalone dehydratase (SCD1) from C. lagenarium was identified by hybridization with a heterologous cDNA probe from Magnaporthe grisea. The cDNA clone was used to identify a cosmid containing SCD1 in a genomic library of C. lagenarium, and the nucleotide sequence was determined for both the cDNA and genomic clones. The SCD1 gene contained one open reading frame composed of 188 codons and two deduced introns of 57 and 67 nucleotides. The deduced amino acid sequence of the N-terminal region of SCD1 showed high similarity to the amino acid sequence of scytalone dehydratase from Cochliobolus miyabeanus. A plasmid containing the SCD1 gene transformed the melanin-deficient mutant 9201Y (Scd-) to the wild phenotype but did not complement the conditional scytalone dehydratase-deficient mutant C. lagenarium 8015. Genomic DNA analysis indicated that the SCD1 gene is a single locus in C. lagenarium. Transcripts of the SCD1 gene were detected 2 h after the start of conidial germination.

Correlation of apoptosis with tumour cell differentiation, progression, and HPV infection in cervical carcinoma.

AIMS: To clarify the significance of apoptosis in the progression of uterine cervical neoplasias, including cervical intraepithelial neoplasia (CIN), microinvasive carcinoma (MIC), and invasive squamous cell carcinoma (ISCC) categories, in relation to cell proliferation and human papilloma virus (HPV) infection. METHODS: Forty six cases of CIN I/II, 75 of CIN III, 16 of MIC, and 44 of ISCC were examined using formalin fixed and paraffin wax embedded samples. The TdT mediated dUTP-biotin nick end labelling (TUNEL) method for detection of apoptotic cells was performed along with Ki-67 immunohistochemistry. Presence of HPV-DNA was confirmed by PCR-RFLP assay. RESULTS: Apoptotic labelling indices, calculated after counting positive nuclei among at least 2000 nuclei, showed significant positive correlation with histological malignant grading in CIN and tumour cell invasion into stroma. In contrast, similar Ki-67 labelling index values were found in CIN, MIC, and ISCC. Although HPV-DNA was detected in 35/46 CIN I/II (76.1%), 53/74CIN III (71.6%), 9/16 MIC (56.3%), and 36/44 ISCC (81.8%), there was no apparent relation with the apoptotic labelling indices. CONCLUSIONS: Apoptosis in cervical neoplasias may be closely related to tumour cell differentiation and progression. It also seems unlikely that HPV itself is directly related to pathways regulating apoptosis.

Increased bone formation in rat tibiae after a single short period of dynamic loading in vivo.

Based on our quantum concept for mechanically adaptive bone formation, we hypothesized that a single bout of loading would increase bone formation at the endosteal surface in rat tibiae, with a maximal response 4-8 days after loading and a stimulus-response relationship for load magnitude. Bending loads were applied to right tibiae of rats at 31, 43, 53, or 65 N for a single bout of 36 or 360 cycles; bone formation was assessed 1-4, 5-8, or 9-12 days after loading. A single loading episode increased lamellar bone formation rate (BFR) in all groups (P<0.05) and was maximal 5-8 days after loading. A distinct dose-response relationship was not evident among all load magnitudes or for duration, but 65 N was significantly more osteogenic than loads of 31-53 N (P<0.05), consistent with a threshold response to loading. There was also evidence for a significant increase in BFR (P<0.05) and double-labeled surface (P<0.01) within 4 days of loading, suggesting that bone-lining cells were activated directly by the stimulus. Thus subtle changes in BFR may occur by modulating the activity of surface cells, but large modeling drifts and anabolic responses require recruitment and differentiation of osteoprogenitor cells near the bone surface.

Nitric oxide inhibitor L-NAME suppresses mechanically induced bone formation in rats.

We hypothesized that nitric oxide may act as an intermediary in the transduction of mechanical loading of bone into a bone formation response. In the present study, 48 rats were divided into the following three treatment groups: control, treatment with N omega-nitro-L-arginine methyl ester (L-NAME; an inhibitor of nitric oxide synthase), and treatment with D-NAME (the less active enantiomer of L-NAME). The rats were subdivided into groups subjected to four-point bending or sham loading of their right tibiae. Bone formation was measured at the midshaft of the loaded and nonloaded (left) tibiae of each rat using histomorphometric methods. The application of four-point bending, but not sham loading, resulted in new bone formation. Treatment with L-NAME reduced the rate of mechanically induced bone formation by 66% compared with the control group. Bone formation rates in nonloaded or sham-loaded limbs were not affected by L-NAME treatment. The results suggest that nitric oxide may play a role in the transduction of a mechanical stimulus into a biological response in bone.

Dendritic cells: a novel cellular component of the rat incisor enamel organ appearing in the late stages of enamel maturation.

Immunocompetent cells in the enamel organ of rat incisors were examined immunohistochemically using OX6, ED1, and ED2 monoclonal antibodies known to recognize the Class II MHC molecules, a monocyte-macrophage lineage, and residential macrophages, respectively. The OX6 immunopositive cells (MHC cells) were located exclusively in the enamel maturation zone. MHC cells increased in number in the incisal direction and occasionally extended cytoplasmic processes deep into the ameloblast layer. Migration of MHC cells in the ameloblast layer were also encountered. MHC cells lacked phagolysosomes and could be distinguished from typical macrophages. ED2 immunopositive cells were not seen in the enamel organ. ED1 positive cells displayed identical localization to MHC cells except that some appeared in the transitional zone. MHC cells could not be seen in the enamel organ of rat molar tooth germs. Our data confirmed the presence of a large population of "dendritic" immunocompetent cells in the enamel organ of rat incisors and characterized the ultrastructural features of these cells. Biological significance of the immunocompetent cells in the enamel organ during amelogenesis needs to be clarified.

Fulminant pneumonia and sepsis due to Aeromonas hydrophila in an alcohol abuser.

A 69-year-old alcoholic man with pneumonia and sepsis due to Aeromonas hydrophila is presented. He died of suffocation by a copious amount of hemoptysis six hours after his first symptoms of abdominal pain, diarrhea and dyspnea. Aeromonas hydrophila was isolated from blood and bronchial secretion. A fulminant form of pneumonia could develop in patients with predisposing underlying conditions such as alcoholism with chronic hepatitis and diabetes mellitus. Aeromonas hydrophila pneumonia may be characterized by hemoptysis and rapid clinical deterioration with a high mortality rate.

Is Baker's cyst a risk factor for pulmonary embolism?

We encountered a 73-year-old man with acute pulmonary embolism (PE) and Baker's cyst. Venography revealed that the right popliteal vein was compressed by Baker's cyst and deep venous thrombosis (DVT) had developed. DVT associated with Baker's cyst is rather common and these two conditions are thought to be causally related. Baker's cyst is the most frequent mass lesion in the popliteal region. We suggest that Baker's cyst is a risk factor for PE as well as surgery and trauma.

A case of hypertensive-diabetic cardiomyopathy demonstrating left ventricular wall motion abnormality.

We report a case of hypertensive-diabetic cardiomyopathy demonstrating left ventricular regional wall motion abnormality, with a normal coronary artery documented on coronary arteriography. Dipyridamole-infusion 201Tl scintigraphy demonstrated transient perfusion defects in the infero-posterior wall of the left ventricle, where reduced wall motion was demonstrated on contrast left ventriculography. Myocardial SPECT (single photon emission tomography) imaging with [123I] beta-methyliodophenylpentadecanoic acid (BMIPP) and 201Tl demonstrated reduced [123I]BMIPP uptake compared with 201Tl uptake in the infero-posterior wall of left ventricle. These results suggest that the impairment of myocardial free fatty acid metabolism is an etiologic or contributory factor for regional wall motion abnormality, together with small-vessel coronary artery disease, in this patient.
