[Hyperbilirubinemia in neonates associated with total parenteral nutrition].

Clinical studies on the pathogenesis of hyperbilirubinemia (HB) in neonates during total parenteral nutrition (TPN) were performed. From 1971 to 1982, 77 neonates underwent TPN for more than 2 weeks in our institutions. Forty-four of them (57.2%) developed HB during the first one month of life. Twenty eight of 32 patients (87.5%) with dominant infection developed hB, while only 16 of 45 (35.6%) without infection developed HB. Patients with any kind of obstructive intestinal diseases evoked HB more commonly than patients with other diseases. In addition, HB occurred more often in the patients receiving more than 110 kcal/kg/day than those receiving less than that. Gestational ages, birth weights, the duration of TPN and fasting periods had no significant influence on the occurrence of HB.

A new immuno quantitative method by latex agglutination--application for the determination of serum C-reactive protein (CRP) and its clinical significance.

We developed a new method for the determination of CRP by latex immunoassay which measures the increase in optical density as a result of latex agglutination due to antigen-antibody reaction. This latex agglutination photometric immunoassay can be used for quantitative analysis of various biological substances. We have established the best conditions for CRP determination for the instrument employed here (LA system) which allows rapid and accurate to low concentration measurement. The measurement range of the system is from 0.01-3 mg/l (non-diluted method), which when compared to the lower detection limit of RIA, 3 micrograms/l, it has the same order of magnitude. It is more sensitive than radial immunodiffusion which has the lower detection limit of 2 mg/l and has the similar value to that of radio-electroimmunodiffusion, 10 micrograms/l. Furthermore, the usual method used in the clinical laboratories, the capillary microprecipitation method, has the lower detection limit of approximately 10 mg/l. The method presented here is adequately sensitive to measure the low concentration of CRP among healthy individuals which has not been possible except by using RIA. The normal value derived from 106 healthy individuals by this method is found to be 0.2 +/- 0.2 mg/l (mean +/- 2SD).

A novel deletion in delta beta-thalassemia found in Japan.

High molecular weight DNA from a Japanese individual homozygous for delta beta-thalassemia was analyzed by the blot hybridization technique of Southern. Results indicated a large deletion of the non-alpha-globin gene cluster, starting in the vicinity of 3' to the A gamma-globin gene and extending through the 3' side of the beta-globin gene. Persistent expression of the gamma-globin gene in adult life has been supposed to be caused by loss of a region located about 3-4 kb 5' to the delta-globin gene from comparison of the extents of deletions in several different forms of delta beta-thalassemia and HPFH (hereditary persistence of fetal hemoglobin). But the novel deletion found in the present case of delta beta-thalassemia suggests that the above putative regulatory region does not have this effect on expression of the gamma-globin gene. Some explanations of expression of fetal type globin genes in this delta beta-thalassemia are discussed.

A correlative radioimmunoassay and immunohistochemical study of LHRH-induced LH depletion in the anterior pituitary of male and female neonatal rats in vitro.

After an exposure of 24 h to synthetic LHRH (100 ng/ml) in vitro, the anterior pituitaries of 4-day-old rats show a notable loss of immunoreactive material in most LH cells in males, but not in females. When radioimmunoassayed without incubation, the pituitary LH content of 4-day-old female rats is 2.8 times higher than that of males of the same age. LHRH treatment stimulates a higher rate of LH discharge in females than in males, but if LH release is expressed as a percentage of the initial pituitary LH content, there is no apparent difference. In both sexes, more than 70% of the initially stored LH is discharged into the medium after 24 h of LHRH stimulation. In males, this discharge produces a pronounced depletion, but in females, the pituitary still contains 78.2% of the initial LH content despite the large amount of hormone released. From these results, it is concluded that in newborn rats the LH synthetic rate in females is higher than that in males. This high synthetic activity, together with the large store of LH, may explain why prolonged LHRH treatment fails to cause LH depletion in females. At 4 days of age LHRH had no stimulatory effect on pituitary synthesis of LH in either sex.

Effects of hyperphenylalaninemia in the fetal stage on the postnatal development of fetal rat brain.

The effect of exposure at different prenatal stages to maternal hyperphenylalaninemia (HyPhe) on the somatic and neurological development of fetuses in rats was studied, with special respect to the change of relevant enzyme activities in the brain. While evident somatic damage was found only in the fetuses exposed to maternal HyPhe at a last stage of gestation, distinct mental retardation seemingly due to some irreversible damage to the brain was observed in all the treated fetuses regardless of the timing of exposure, and a significantly reduced activity of 2', 3'-cyclic nucleotide 3'-phosphohydrolase (CNPase), a marker enzyme of myelin, was confirmed in the mantle region of the brain.

Structure of the chromosomal gene for human serum prealbumin.

The human prealbumin gene has been cloned and its complete nucleotide sequence determined. The gene has a size of about 6.9 kb and is composed of four exons and three introns. Two Alu family sequences having opposing polarity were found in introns. In the 5'-flanking region, we found two overlapping sequences which have extensive homology to the glucocorticoid-responsive element. Three sequences identical with the enhancer core sequence were identified in introns and the 3'-flanking region. Unusual tandem repeats of a sequence, TTTTG, were also found in the 5'-flanking region and introns.

Potentiating effects of caffeine on the cardiovascular teratogenicity of ephedrine in chick embryos.

Ephedrine was administered to 3-day chick embryos (Hamburger-Hamilton developmental stage 19) together with caffeine at doses where each agent alone caused minimal embryotoxicity. Embryos were examined for malformations on day 14 of incubation. The teratogenicity of ephedrine in the chick cardiovascular system was significantly potentiated by caffeine at a dose as low as 0.5 mumol (0.1 mg/egg; 2 mg/kg egg).
