Role of endogenous endothelin in the development of hypertension in rats.

To further elucidate the pathophysiologic role of endogenous endothelin (ET), we have studied the chronic effect of anti-ET gamma-globulin on the development of hypertension in spontaneously hypertensive rats (SHR) and stroke prone SHR (SHR-SP) for three weeks. In neither SHR nor SHR-SP did repetitive bolus injection of anti-ET gamma-globulin suppress the rise in blood pressure. The present data suggest that endogenous ET is not likely to play a key role in the development of hypertension, although ET may induce a local vasoconstriction at the injured vascular wall.

Tissue reactions after experimental luxation injuries in immature rat teeth.

Immature permanent teeth often develop complications after luxation injuries. The pathology and the degenerative processes of the complications, however, are not well understood. In this study, we histologically examined the long-term reactions of immature rat teeth after standardized luxation injuries delivered in our previously developed experimental model. The upper first molars of 3-week-old male Wistar rats weighing 50-60 g were pushed horizontally toward the palate to cause a constant amount of dislocation. The follow-up changes of the luxated molars were studied up to 56 days after the trauma. Soon after the trauma, the periodontium had irregular fibers, degenerated cells, hemorrhage and edema in various places. After 12 h, a cell free area appeared in the palatal periodontium. In the alveolar bone and on the surface of the root, resorption occurred one day and 5-7 days after injury respectively. Although the damage in the periodontium and alveolar bone was gradually repaired, the root resorption, especially in the palatal cervical region, continued throughout the experimental period. In the most cases, the dental pulp did not seem to be damaged immediately after the trauma. By the 3rd day after injury, however, displacement of odontoblasts into the dentinal tubules was observed in many teeth. Moreover, formation of irregular dentin was observed in some teeth after 7 days. These changes of the pulp tended to disappear in the later stages of the experiment.

The 'oral 1,25-dihydroxyvitamin D3 pulse therapy' in hemodialysis patients with severe secondary hyperparathyroidism.

Many hemodialysis patients are still suffering from secondary hyperparathyroidism although 1,25-dihydroxyvitamin D3 (1,25(OH)2D3) has been used to treat renal osteodystrophy for the last two decades. The main reason for its failure to correct the secondary hyperparathyroidism is that in patients, hypercalcemia occurs before adequate parathyroid hormone (PTH) suppression is obtained when a large daily dose of 1,25(OH)2D3 is started. In this study, the oral dose of 1,25(OH)2D3 (4.0 micrograms) was administered only twice a week at the end of hemodialysis ('oral 1,25(OH)2D3 pulse therapy'), in 19 patients with severe secondary hyperparathyroidism. Serum immunoreactive PTH started to decrease after 6 weeks of therapy, and the original level of 41.2 +/- 7.24 was reduced to 24.4 +/- 6.12 ng/ml by the end of the 6-month therapy (p less than 0.001). Serum alkaline phosphatase also was reduced by 64.4%. Three out of 19 patients suffered from hypercalcemia during the 4th month of therapy. Calcium supplement given to 6 other patients with severe secondary hyperparathyroidism did not lower serum PTH levels significantly after 6 weeks of therapy, although serum calcium levels increased and were sustained above 10 mg/dl for the last 5 weeks. These findings strongly suggest that the suppressive effect of the oral 1,25(OH)2D3 pulse therapy was attained by a direct action of 1,25(OH)2D3 on the parathyroid gland rather than by its ability to elevate serum calcium levels. In conclusion, the oral 1,25(OH)2D3 pulse therapy effectively lowered PTH levels in hemodialysis patients who cannot tolerate large daily doses of 1,25(OH)2D3.

Transitional cell carcinoma of the bladder or renal pelvis in children.

Transitional cell carcinoma (TCC) of the bladder or renal pelvis is rare in children. We report 2 children with TCC of the bladder and 1 with TCC of the renal pelvis. One of the two children with bladder carcinoma experienced frequent intravesical recurrences, which is in contrast to the usual clinical course of bladder carcinoma in children. In the 3rd child, renal pelvic carcinoma was found incidentally in a renal pelvis specimen removed during pyeloplasty.

Comparative studies of the colony-promoting activity of porcine kidney extract with several interleukins and colony-stimulating factors.

Porcine kidney extracts (PKE) possess colony-promoting activity (CPA) which stimulates primitive hematopoietic cells in the presence of granulocyte/macrophage colony-stimulating factor (GM-CSF), but PKE itself does not stimulate colony formation on murine bone marrow cells. We have compared the CPA of PKE with that of recombinant cytokines or CSFs such as interleukin-1 alpha (IL-1 alpha), IL-3, IL-6, granulocyte colony-stimulating factor (G-CSF), GM-CSF and macrophage colony-stimulating factor (CSF-1). All of these factors were less potent than PKE. Furthermore, the combinations of IL-1 alpha or PKE with G-CSF, GM-CSF, IL-3 or IL-6 were examined in the presence of one of these factors such as CSF. It is found that PKE acts synergistically with G-CSF, GM-CSF, IL-3 and IL-6, showing enhancement ratios of 10, 2.5, 4.2 and 30, respectively. The combination of IL-1 alpha resulted in poor colony formation in contrast with those of PKE, except for CSF-1. These results suggest that the CPA of the factor(s) in PKE differ from the cytokines and CSFs tested in this study, and is significantly affected by various types of CSF.

Separation and characterization of three positional isomers of dimaltosyl-cyclomaltoheptaose (dimaltosyl-beta-cyclodextrin).

A mixture of maltosylcyclomaltoheptaoses (maltosyl-beta-cyclodextrins, G2-beta CDs) was prepared from maltose and beta-cyclodextrin (beta CD) through the reverse action of Klebsiella pneumoniae pullulanase. Three positional isomers of dimaltosyl-beta CD in the mixture were separated by high-performance liquid chromatography on a reversed phase column and a graphitized carbon column. Their molecular weights were measured by fast-atom bombardment mass spectrometry, and the structures were established by methylation analysis, hydrolysis with glucoamylase to the known compounds, three positional isomers of diglucosyl-beta CD, and 13C-nuclear magnetic resonance spectroscopy.

Enhancement of murine erythropoiesis in vitro by a porcine kidney extract.

We have examined the effect of porcine kidney extract (PKE) on the growth of erythroid progenitor-derived colonies in a methylcellulose culture. The addition of PKE resulted in an enhancement of burst-forming-unit-erythroid (BFU-E)-derived colonies, and the enhancement of the colony was also observed in a low concentration of erythropoietin (Epo) and fetal calf serum (FCS). The activity of PKE on BFU-E was compared with the erythroid growth factors, such as recombinant murine interleukin-3 (IL-3), recombinant murine granulocyte/macrophage colony-stimulating factor (GM-CSF) and recombinant human granulocyte colony-stimulating factor (G-CSF) which are all well known to stimulate BFU-E growth. IL-3 showed a potent burst-promoting activity (BPA), but GM-CSF and G-CSF did not enhance BFU-E growth in our culture conditions. In the cultures supplemented with Epo, the rapid loss of BFU-E was prevented with IL-3; however, PKE alone did not prevent the disappearance of BFU-E. These results suggest that PKE possesses a BPA-like activity which is considered an enhancement of BFU-E.

Properties of colony promoting activity in procine kidney extract.

Aqueous extracts prepared from procine kidneys (PKE) possess colony-promoting activity (CPA) which increases the number of granulocyte and macrophage colonies in semi-solid cultures of mouse bone marrow cells (BMC) in the presence of colony-stimulating factor (GM-CSF). PKE was totally inactivated by 15 mM N-ethylmaleimide, but was resistant to 5 mM dithiothreitol (DTT), 50 mM sodium metaperiodate and a mixture of diisopropylether-n-BuOH (3:2). The proportion of deoxyribonucleic acid (DNA)-synthesizing cells of the PKE-responsive cells was about one-half in comparison to those of CSF-responsive cells, as estimated using the hydroxyurea (HU) suicide method. Upon marrow preincubation with PKE in liquid culture for 24 h, the suicide rate of the colony forming unit in culture (CFU-C) by HU increased to 3 times compared to that of the control. Since cyclophosphamide (CY) induces a change in the number of CFU-C, the effects of PKE on BMC obtained from CY injected mice were investigated. On day 1, the number of PKE-responsive cells significantly increased by about 2.3 times in comparison with that of control, whereas the number of CFU-C per 1 x 10(4) cells significantly decreased to about one-eighth of that of control. These results suggest that a sulfhydryl group(s) is required for the appearance of the colony-promoting activity of PKE, and glycoproteins, glycopeptides or hydrophobic components are not required; they also suggest that PKE may act on immature granulocyte/macrophage progenitors, which are younger than CSF-responsive CFU-C.

[Therapeutic efficacy of testolactone (aromatase inhibitor) to oligozoospermia with high estradiol/testosterone ratio].

To our knowledge, the action of estradiol which is produced from testosterone by aromatase on human spermatogenesis has not been fully clarified. In oligozoospermia, with high values of E2/T ratio, it is considered that the role of estradiol is suppressive to spermatogenesis. In this study, alteration of spermatogenesis was evaluated when serum estradiol levels were decreased by suppression of aromatase activity. Nine male infertile patients were treated with testolactone (Teslac: 1.0 g/day, for 3 months), one of the aromatase inhibitors. Four of them had an increase in sperm count (more than 10 x 10(6)/ml relative to base line). In endocrinological findings, serum estradiol levels and E2/free T ratio were significantly decreased after treatment. Serum free testosterone levels were significantly increased in all cases, presumably from decreased sex hormone binding globulin (SHBG) levels. These findings suggested the effectiveness of the administrated aromatase inhibitor. In particular four patients whose sperm counts were improved after testolactone treatment had high values of basal serum estradiol levels and E2/free T ratio before treatment, and these values were normalized after treatment. In conclusion we suggest that an aromatase inhibitor may be effective to male infertile patients with high serum estradiol levels.

[Study on frequency of prostate carcinoma and benign prostatic hypertrophy by mass screening in Hokkaido].

We conducted mass screenings for prostate diseases on male subjects over fifty years of age in three separate areas in Hokkaido. Prostate carcinoma and benign prostatic hypertrophy were searched in the 1,764 participants. Histopathologically proven prostate carcinoma was found in twenty-two (1.25%) of the 1,764 participants. This frequency of carcinoma was higher than any other carcinoma found in the mass screenings for gastric, uterine, breast and lung carcinoma in Hokkaido. Of the 22 prostate carcinomas found, 68% were in the early stage (stage B). This stage distribution was clearly distinct from that of prostate carcinoma found on the hospital visit, most of which had already progressed to an advanced stage. These results indicate that mass screening for prostate carcinoma on greater than or equal to 50 year old-male subjects is efficient in finding carcinoma of all stages but, in particular, carcinoma of early stage, when compared with mass screening for other carcinomas. BPH, defined as a moderately or markedly enlarged prostate on rectal palpation, was found in 10% of the participants. Questionnaire on subjective symptoms of voiding disturbance in the participants has confirmed that these symptoms, mainly elicited by BPH, become manifested in fifties and more frequent with age. Of thirteen patients with prostate carcinoma who received both rectal examination and prostate-related markers measurement in serum at the time of mass screening, three without induration of the prostate were diagnosed as having carcinoma from an abnormal value of the serum markers. This result suggests that the marker(s) is one of the useful screening tests for detecting carcinoma.(ABSTRACT TRUNCATED AT 250 WORDS)

[Malfunction of the Omniscience valve in mitral position: a case report].

A 55-year-old woman who underwent implantation of a 27 mm Omniscience valve in mitral position 5 and a half years ago, consulted our institution complaining reappearance of signs of mitral stenosis. In preoperative examinations, the maximal opening angle of the Omniscience valve was only 32 degrees and PCWP was elevated to 20 mmHg. Her symptoms disappeared after mitral valve re-replacement with a Medtronic Hall 27 mm prosthesis. The important point is that no foreign body such as thrombus or overgrown pannus interfering motion of the Omniscience valve was observed at the time of reoperation. The cause of insufficient opening seemed to be valve mechanism itself.

[Evaluation of [troponin-T] as a new biochemical marker for acute myocardial infarction].

Troponin (Tn) and tropomyosin (Tm) are located in the thin actin-containing filaments. Tn, a complex of three subunits (Tn-C, Tn-I and Tn-T), plays an important role in muscle contraction via calcium metabolism. In the present study, we evaluated the usefulness of a serum Tn-T assay in patients with acute myocardial infarction. The method, based on the sandwich immunoassay, was developed for the determination of serum Tn-T. The analytical precision data were good enough for routine analysis (CVs of repeatability and day-to-day reproducibility were between 3.2 to 6.2%). The values obtained on 60 healthy adults were below 0.2 ng/ml. Physical activity (mountain climbing and golf) increased serum total CK and myoglobin, but did not increase Tn-T. In myocardial infarction, serum Tn-T was up to 20 times higher than the cut off value at admission, and had a large peak at around 14 hr and another small peak at 40 to 55 hr after the onset. The ratio of Tn-T at around 14 and 32 hr (14/32) was an excellent marker for successful reperfusion. All cases of successful PTCA, had a ratio higher than 1.0.

[Clinical study on total intravenous anesthesia with droperidol, fentanyl and ketamine--12. Effects on plasma complement and immunoglobulin concentrations].

Complements and immunoglobulins in the plasma are the important humoral factors to maintain immunity. As there is no study on immune response to total intravenous anesthesia with droperidol, fentanyl and ketamine (DFK), twelve patients who underwent abdominal, neck dissection, or plastic surgery were studied to determine plasma concentrations of complements and immunoglobulins. In five patients of isoflurane group, anesthesia was induced with intravenous thiopental 5 mg.kg-1 and succinylcholine 0.8-1 mg.kg-1 and maintained with 1-2% isoflurane in nitrous oxide (50%) and oxygen (50%). The remaining seven patients of the DFK group received intravenous droperidol 0.25 mg.kg-1, fentanyl 1-2 micrograms.kg-1, ketamine 1-1.5 mg.kg-1 and succinylcholine 0.8-1 mg.kg-1 for the induction of anesthesia, and then they were given a total dose of fentanyl 5-15 micrograms.kg-1, ketamine 2 mg.kg-1.hr-1 and oxygen (30%) for the maintenance of anesthesia. Vecuronium was given intravenously as needed. Lactated Ringer's solution was used for intraoperative fluid replacement. A total of 40 ml of arterial blood was drawn on four occasions, just before the induction of anesthesia, at the recovery from anesthesia, on the third and tenth post-operative days. Plasma concentrations of complements (C3.C4) and immunoglobulins (IgG.IgA.IgM.IgD) were measured by immuno-turbidimetry. C3 concentrations in the plasma decreased significantly when the patients recovered from anesthesia, but they increased significantly on the third and tenth post-operative days in the isoflurane group. In the DFK group, they increased significantly on the tenth post-operative day only. No significant difference in the C3 concentrations was detected between two groups at any time of measurement.(ABSTRACT TRUNCATED AT 250 WORDS)

[Clinical study on total intravenous anesthesia with droperidol, fentanyl and ketamine--13. Application for pediatric patients].

Total intravenous anesthesia with droperidol, fentanyl, and ketamine (FK) was administered to 56 pediatric surgical patients ranging in ages from 5 to 15 years to evaluate their hemodynamics during anesthesia, post-operative hepatic as well as renal functions, and post-operative sedation as well as analgesia. These data were compared with those of the patients who underwent almost the same surgical procedures under enflurane-N2O anesthesia. The post-operative s-GOT, s-GPT, BUN, creatinine levels were not elevated significantly as compared with pre-operative levels in the FK group. As compared with those patients who received enflurane anesthesia, the blood pressure in the FK groups was higher by 15-30 mmHg, but it was stable during anesthesia without any complications. Their post-operative sedation and analgesia were better in the FK group than in the enflurane group and the complications such as nausea and vomiting were observed less frequently in the FK patients than in the patients who received anesthesia with ketamine alone reported in literatures. The data described above suggest that this method of anesthesia deserves further detailed clinical trials for pediatric patients.

[Simultaneous bilateral renal pelvic tumors: a case report].

A case of simultaneous bilateral renal pelvic tumors is reported. A 64-year-old man with the chief complaint of gross hematuria and left flank pain was admitted. Clinical investigations revealed a tumor in the right pelvis and ureter, and another tumor in the left renal pelvis. The right ureteral tumor had invaded the bladder. Right nephroureterectomy, total cystectomy, left partial pyelectomy and ureterocutaneostomy were performed. By pathological examination, right renal pelvic and ureteral tumors were non-papillary transitional cell carcinoma, grade 3, pT4, and the left renal pelvic tumor was papillary transitional cell carcinoma, grade 2, pT1. To our knowledge, this is the 16th case of simultaneous bilateral urothelial tumors of the upper urinary tract in Japan.

[Clinical significance of creatine kinase isoform assay by a new reagent based on immunoinhibition method].

Clinical significance of creatine kinase (CK) isoform assay by a new diagnostic reagent based on immunoinhibition method was evaluated. In the method, a monoclonal antibody (CKM-G 01), which inhibits MM3 but not MM1 isoform, was used. Serial serum samples from 143 acute myocardial infarction (AMI) patients were examined in hospitals. Isoform ratio (I.R.: defined as follows; inhibited CK activity/uninhibited CK activity) increased most rapidly (whose peak appeared 8.5 h after onset of AMI), compared with total CK (18.3 h) and CK-MB (16.1 h). In 61 patient samples collected within 4 h from onset, I.R. in 25 samples (41%) exceeded reference interval, whereas total CK in only 17 samples (28%) did it. Reference interval of the I.R. was 0.42 +/- 0.33 (mean +/- SD) in 1,246 normal subjects. These results show that the assay of CK isoforms by the reagents based on immunoinhibition method is useful for earlier detection of AMI.
