[Megaureters in adults].

Megaureters are common in children but are rarely found in adults, probably due to the scarcity of clinical symptoms. Reconstruction surgery in adults has been performed only exceptionally up to a few years ago. We encountered 7 adults with 7 megaureters in the recent 6 years. The underlying pathologic entities responsible for the megaureters were non-peristaltic lower segment, ectopic ureter and ureterovesical junction stenosis. Six adults with 6 megaureters were treated by complete surgical reconstruction and reimplantation of the ureter. The outcome of all the reported cases was excellent.

Hepatic artery aneurysm. Report of a case and review of the literature.

One hundred and nineteen cases, including the one reported here, of hepatic artery aneurysm were reviewed. The clinical features and surgical management in four etiologic groups, consisting of arteriosclerosis, cholecystolithiasis, trauma and, specific diseases, were investigated. Interruption of the hepatic artery, entailing ligation of the hepatic artery, excision of the aneurysm without vascular reconstruction, and obliterative aneurysmorrhaphy, was done in 50 patients. Its high mortality rate (26%) was principally ascribed to delayed diagnosis or inappropriate site of interruption. If conditions such as shock, anoxia and portal thrombosis are avoided, interruption of the hepatic artery seems to be a safe and useful operative method for hepatic artery aneurysms, especially accompanied by severe local infection.

Further studies on inhibition of Epstein-Barr virus release by bovine serum: partial purification and characterization of the inhibitor.

The antivirus release factor (AVRF) was extracted from adult bovine serum and was partially purified in three steps by 1) ammonium sulfate precipitation, 2) Sephadex G-200 chromatography, and 3) DEAE-Sephadex chromatography, procedures that together increased specific activity 160 times. In molecular sieve chromatography, AVRF was found in fractions close to IgM. The AVRF activity was sensitive to KIO4 (0.04 M) but not to trypsin (0.1%), UV irradiation, ethyl ether, and heating at 56 degrees C for 30 minutes. The AVRF neither neutralized infectivity of Epstein-Barr virus (EBV) from the P3HR-1 cell line nor inhibited intracellular virus yields at concentrations that inhibited release of virus into culture fluid. Cellular release of two other strains of EBV (QIMR-WIL and B95-8) was also inhibited by AVRF. Electron microscopic examination of P3HR-1 cells cultured in the presence of AVRF revealed peculiar interacytoplasmic vacuoles containing many mature virus particles with tail-like structures.

Annual fluctuation of Schistosoma japonicum infection in field rats, Rattus rattus mindanensis, in Dagami, Leyte, Philippines.

The annual fluctuation of Schistosoma japonicum infection in field rats, Rattus rattus mindanensis, in Leyte, the Philippines was investigated. One hundrend live-traps were set for one week every month from February, 1978 to February, 1979, in and around Central Elementary School II, Diagami, Leyte. Out of 199 rats, 163 (81.9%) were infected. The prevalence rate of S. japonicum infecton in rats was high during the relatively dry season, April to July, 1978. Meanwhile, the prevalence rate of schistosome cercariae in Oncomelania quadrasi of the study area increased during the relatively rainy season, October, 1978 to February, 1979. It is supposed that miracidia hatched from eggs discharged in the feces of rats during the relatively dry season would encounter snails, following which the emerging cercariae would penetrate the mammalian hosts, such as humans and rats, during the relatively rainy season. This might be the transmission dynamics of schistosomiasis in the endemic areas of Leyte. It is also suggested that this annual prevalence rate could be used as a parameter to formulate and evaluate control works. No infection was established in the youngest age group, the weanling. Then, the prevalence increased gradually by age. All rats of the older age group were infected. According to age composition, the field rat had no definite breeding season in Leyte. COP reaction was weak in sera of infected rats, but strongly exhibited in sera from rats possessing eggs in the spleen. Consequently, field rats not only maintain the infection as reservoir hosts, but also are an important source of infection as definitive hosts in the endemic areas of Leyte Island. Specifically, the rat role must be stressed, should human schistosomiasis ever be controlled.
