[Surgical management of primary aldosteronism--progress in localization studies and operative treatment].

Primary aldosteronism due to an adrenocortical adenoma is commonly known as surgically correctable hypertension. Forty-three cases of primary aldosteronism were treated operatively at our Department between 1960 and 1985. Forty of them were operatively, found to have aldosteronoma with a mean size of 1.88 cm and weight of 2.67 g. The patients average age was 39.4 years old and the male to female ratio was 1 to 3. Adrenal phlebography, selective adrenal venous sampling, adrenal scintigraphy, CT and echography were used to preoperatively locate the tumor, and their diagnostic value was 83, 78, 76, 87 and 60%, respectively. Unilateral adrenalectomy or unilateral partial adrenalectomy was performed extraperitoneally with lumbar oblique incision in 32 of the 43 operated patients. Since the application of the preoperative diagnosing studies above mentioned, tumor localization has been clearly demonstrated in about 90% of the patients and the unilateral extraperitoneal approach is proved as a safe operative method with minimal blood loss of less than 200 ml and performed within two hours. Unilateral adrenalectomy by the flank approach has now become the operation of choice for the treatment of an aldosterone-producing adrenal adenoma.

[Pheochromocytoma: review of 22 patients and 23 operations].

Twenty-two cases of pheochromocytoma encountered at our department between 1965 and 1986 are reviewed. Eighteen of the patients were adults (26 to 62 years old) and 4 were children (9 to 13 years old). Thirteen were males and 9 were females. In a total of 23 operative procedures, 31 tumors were resected excluding lymph node metastasis. The preoperative accuracy of the diagnostic methods for locating the tumor was as follows. KUB and intravenous pyelography with tomography in 12 of 31 tumors (39%), aortography and/or selective arteriography in 21 of 27 (78%), selective venous sampling for catecholamine level in 4 of 4 (100%), computed tomography in 10 of 10 (100%), ultrasonography in 4 of 6 (67%), and iodine-131 metaiodobenzylguanidine scintigraphy in 4 of 6 (67%) tumors. The tumors resected were cases, as followings, a single unilateral adrenal tumor in 14 (right 6, left 8), bilateral adrenal tumors, bilateral adrenal plus extraadrenal tumors and unilateral adrenal plus extraadrenal in each, one case and extraadrenal tumor in six cases (single 5, multiple 1). Either lumbar or thoracolumbar surgery methods were adapted in 9 of 14 cases with single adrenal and one of 5 with single extraadrenal tumors, and in the remaining 13 cases transperitoneal methods were selected. One patient died on the 6th day after operation from gastrointestinal bleeding. Thus the surgical mortality rate in our series was 4.4%. Of the 3 patients with malignant pheochromocytoma, two (9 and 13 years old) are alive each 192 and 67 months after operation. The former is in a tumor-free condition, however in the latter multiple bone metastasis was detected 37 months after operation. The third patient (29 years old) died 62 months after operation from metastatic hemorrhagic tumors in the liver.

Arteriovenous fistula following central venous catheterization.

We surgically obliterated arteriovenous fistulas in three cases that developed after subclavian catheterization. The first patient presented with heart failure two years after the catheterization, and the other two presented with asymptomatic continuous bruits. The intervals between the removal of the catheter and the appearance of the bruit were 1.5 years, three days, and two months. It took another six months for the appearance of heart failure in the first case. The feeder of the fistula was a branch of the subclavian artery in all cases. In previously reported cases, there was usually some interval between the removal of the catheter and detection of the fistula, and the feeder was much more commonly one of the branches of the subclavian artery than the subclavian artery itself.

Ca content of human erythrocytes. What is the true value?

It has been generally accepted that the Ca content of washed erythrocytes is in the range of 1 - 3 X 10(-5) mole/l cells (1-10). These values were obtained after samples were treated in various way for Ca measurement. However, by using a simplified careful preparation technique to avoid Ca contamination together with inductively coupled argon plasma emission spectroscopy, we have found that the Ca content of erythrocytes washed with nominally Ca2+-free solution, is one order of magnitude lower than the reported values, in the range of 1 X 10(-6) moles/l cells.

4-Methylumbelliferyl lipase in human and mouse brain: a possible localization in myelin.

4-Methylumbelliferyl (4-MU) lipase activity in human and mouse brain, measured with 4-MU palmitate, was characterized with respect to effects of pH and detergents, and subcellular and myelin localization. Purified myelin isolated by Norton's procedure [J. Neurochem. 21, 749-757 (1983)] contained higher specific activity of 4-MU lipase, particularly in alkaline pH. Myelin lipase activity was markedly affected by the addition of different types of detergents, the amount of detergents added, and substrate. The optimal pH in myelin was bimodal--pH 4.5 and up to 8.0, respectively. These data indicate that myelin possesses 4-MU lipase activity at alkaline pH, with lower levels at acidic pH.

Bladder inhibition by electrical stimulation of the perianal skin.

Transcutaneous electrical stimulation was applied to the perianal skin of 25 patients with frequency, urgency or incontinence. Repeated cystometrogram during this stimulation disclosed suppression of detrusor activity, inhibition of detrusor instability in 4 of 8 patients and increase of maximum cystometric capacity in 5 of 25 patients. Electromyographic activity of the anal sphincter muscle increased in all the 25 patients. Poststimulation improvement was observed clinically in 9 of 12 patients.

A case of Takayasu's disease with ruptured carotid aneurysm.

Recently, a number of cases of Takayasu's disease having dilatative or aneurysmal lesions have been reported. Such lesions have come to be considered important manifestations of Takayasu's disease. A case, whose right common carotid artery perforated spontaneously and became a pseudoaneurysm, without any other stenotic lesion is presented with a review of the literature. Surgical treatment was performed successfully.

[The limit in differential diagnosis of renal angiomyolipoma from renal cell carcinoma on computerized tomography: a case report].

Recent reports have described that the accurate preoperative diagnosis of renal angiomyolipoma is possible with computerized tomography (CT). There are, however, some cases of angiomyolipoma which are indistinguishable from renal cell carcinoma on CT. Herein we report a case of renal angiomyolipoma which CT failed to reveal preoperatively. In a 71-year-old Japanese male, a left renal tumor was incidentally suspected on excretory urogram. Then CT demonstrated a homogeneous mass in the upper part of the left kidney, 6 cm in diameter, which showed a positive attenuation coefficient, slightly denser than renal parenchyma. Selective left renal arteriogram showed hypervascularity without aneurysmal formation or arteriovenous fistula. These radiologic findings strongly suggested renal cell carcinoma and radical left nephrectomy was performed. Histological examination confirmed angiomyolipoma composed mainly of smooth muscle with thick-walled vessels and immature fat cells. We reviewed the literature and discussed the computerized tomographic-pathologic correlation of angiomyolipoma.

[Recent progress and problems in bladder-sparing operations in bladder tumors].

Retrospective analysis of the bladder tumor patients was performed to reveal the clinical results of partial cystectomy or transurethral resection (TUR). The observed 3-, 5-, 10-year survival rates after 143 partial cystectomies indicated for the first tumors were 66.2%, 57.1% and 41.5% respectively. To obtain satisfactory results, however, the operation should be indicated for pT2 or G0-G1. Some of the G2 tumors could also be the candidates for this procedure, but the stage of the disease must be below T2. At present, all these tumors could be well controlled by TUR, and this statement was confirmed by the study of clinical results obtained by TUR. The analysis also revealed a poor outcome in 6% of the low grade or low stage tumors and it increased to 25% in high grade and pT2. The complete cure of the high stage or high grade tumor is still difficult, but to improve the survival rate, radical surgery should be employed more positively instead of partial cystectomy. The reasons for this conclusion are also discussed.

NGF-dependent and -independent growth of neurites from sympathetic ganglion cells of the aged human in a serum-free culture.

Small pieces of tissue isolated from abdominal sympathetic ganglia in aged male patients were cultured in a chemically defined, serum-free medium. The growth of neurites from pieces of ganglia in cultures with and without 50 ng/ml mouse 2.5S nerve growth factor (NGF) was compared. The NGF stimulated significantly greater regeneration of neurites, causing the growth of long fibers from the ganglion pieces. Many short neurites grew, even in the absence of the NGF, but these were generally short, except for long neurites generated in several nerve cells. A method was devised for the evaluation of NGF-dependent growth of neurites in the culture. The rate of the NGF-dependent growth of neurites, which was calculated by the difference in the total lengths of the NGF-dependent neurites between 2- and 4-day-old cultures, was approximately 160 micron/day. The results indicate that although the growth of neurites from some sympathetic nerve cells of the aged human become independent of the NGF, most of the nerve cells remain dependent on the NGF, even in the stage of senescence.

Cytotoxic activity of peripheral blood lymphocytes grown with interleukin 2 against autologous cultured tumor cells in patients with renal cell carcinoma: preliminary report.

To examine the use of interleukin 2 as a therapeutic agent for human renal cell carcinoma the peripheral blood lymphocytes derived from patients with renal cell carcinoma were grown in interleukin 2 and tested in a 4-hour 51chromium release cytotoxicity assay against autologous cultured tumor cells. Peripheral blood lymphocytes, which were cultured separately in medium alone, mitomycin C-treated tumor cells, 20 per cent interleukin 2 and tumor cells plus interleukin 2, were used as effector cells. In all patients neither peripheral blood lymphocytes in medium alone nor in mitomycin C-treated tumor cells could lyse their own tumor cells. However, in 5 of 7 patients peripheral blood lymphocytes grown in interleukin 2 regardless of the presence of mitomycin C-treated tumor cells were capable of causing 5.7, 9.4, 12.7, 36.1 and 52.5 per cent lysis at effector/target cell ratios of 25, 50, 50, 50 and 25, respectively. However, more significant cytotoxicity was not obtained by peripheral blood lymphocytes cultured with tumor cells and interleukin 2. These results suggest that interleukin 2 might become a useful agent for patients with renal cell carcinoma.

Reversion to bacillary forms of Serratia marcescens spheroplasts induced by carbenicillin. Scanning electron microscopic study.

Bacterial cells of Serratia marcescens were easily induced to form spheroplasts in liquid medium by the addition of carbenicillin. The spheroplasts were unable to divide, but they were able to revert to the bacillary forms in liquid medium not containing carbenicillin. Four phases of the reversion sequence could be differentiated by scanning electron microscopy. (1) After 3 hr of incubation in carbenicillin-free medium, some projections arose out of the spheroplasts, and grew and elongated. (2) Their elongation resulted in a morphological change in the spheroplasts from spherical bodies to long irregular bacillary forms. (3) Further incubation caused several constricted areas in the bacillary form. (4) The long bacillary forms split along the constricted areas to become the parent bacillary forms of S. marcescens. When the long bacillary form that developed during the reversion was retreated with carbenicillin, it was immediately induced to become a spheroplast again.
