A case of blue rubber bleb nevus syndrome with coronary artery fistula to left ventricle.

A 69-year-old female was admitted to the hospital because of tarry stool and dyspnea on exertion. Multiple hemangiomas were noticed on the tongue, right arm and abdomen. Gastrointestinal examination revealed multiple polypoid lesions in the stomach and small intestine. In addition, coronary artery-left ventricular fistula formation and pulmonary arteriovenous malformation were shown by angiography. Compared with other reported cases of blue rubber bleb nevus syndrome, the present case was unique because of the coronary artery-left ventricular fistula, heart failure and the familiar incidence of heart failure.

Quantitative gait evaluation of hip diseases using principal component analysis.

The measurement of five gait parameters, namely, joint angular displacement of lower extremities, floor reaction forces, trajectory for a point of force application, temporal factor and distance factor has been performed with ease and high speed using mini-computer on-line real-time processing. Gait data of 211 patients with hip diseases was normalized, quantified and summarized by the principal component analysis. A 'gait evaluation plane' was formed according to the results obtained by the principal component analysis. The gait evaluation using the plane was compared with clinical conditions of patients, and it was evident that this system can evaluate the recovery of the gait by treatment.

Effect of hypoxia on acetic acid ulcer of the stomach in rats with or without coenzyme Q10.

As one of the factors contributing to the intractability of chronic gastric ulcer, the effect of hypoxia is examined in this paper, based upon the experimental acetic acid ulcer in the stomach of Wistar rats. Results indicate that hypoxia has a harmful influence on the healing process of chronic, gastric ulcer in rats, and also that this effect of hypoxia can be prevented by administration of Coenzyme Q10 in diet.

Endoscopic studies on the minute structures of colonic mucosa in the follow-up observation of ulcerative colitis.

Thirty cases of ulcerative colitis were examined endoscopically by means of magnifying colonoscope with dye spraying method (the combined method), and their minute mucosal structures were classified into four categories endoscopically, which correlated well with the histological findings. By this procedure, it was easy to detect the subsidence of inflammation of the mucosa in the quiescent phase or to reveal the active inflammatory involvement of the bowel. Moreover, inspecting the minute changes of the colonic mucosa detected by the combined method, remission was more correctly decided and rigid control is contributing to the decrease in the recent rate of recurrence of ulcerative colitis.

Use of magnetization transfer contrast to improve single breath-hold three-dimensional MR-portography with bolus injection of gadopentetate dimeglumine: a preliminary report.

We investigated whether better visualization of the intrahepatic portal system could be obtained by adding magnetization transfer contrast (MTC) to three-dimensional MR portography (3D-MRP). In 15 healthy volunteers, 3D fast low-angle shot images combined with an off-resonance pulse were obtained after bolus injection of gadopentetate dimeglumine during one period of breath-holding. Within 1 week after injection, MR images without MTC were obtained using the same scanning condition. MRPs with and without the MTC pulse were compared quantitatively and qualitatively. The contrast-to-noise ratio of the portal vein-liver was significantly higher in the group imaged with the addition of MTC. Qualitatively, the use of MTC improved peripheral portal branch depiction on maximum-intensity projection images. Even when the MTC pulse was added to the conventional method of 3D data collection, it was possible to obtain images within one period of breath-holding. In conclusion, the use of MTC can substantially enhance the quality of 3D-MRPs.

Hepatocellular carcinoma exhibiting a concentric structure of different histologic grades: evaluation by chondroitin sulfate iron colloid-enhanced MR imaging.

A patient with hepatocellular carcinoma exhibiting a concentric structure of different histologic grades was examined with chondroitin sulfate iron colloid (CSIC)-enhanced MR imaging. After CSIC injection, the advanced component was enhanced in comparison with the surrounding liver, and the well-differentiated component was not enhanced. CSIC-enhanced MR imaging was helpful in evaluating histologic grade on the basis of reticuloendothelial function.

Use of three-dimensional segmented FLASH sequence with magnetization transfer contrast to improve Gd-DTPA-enhanced intrahepatic MR portography.

Twenty healthy volunteers underwent gadopentetate dimeglumine (gadolinium diethylenetriaminepentaacetic acid (Gd-DTPA))-enhanced MR angiography (MRA) using three-dimensional-segmented fast low angle shot images (FLASH) with magnetization transfer contrast (MTC) pulses. MRA was obtained at 75 seconds (early phase) and 135 seconds (late phase) after bolus injection of Gd-DTPA (MTC+ group) during one period of breath-holding. Within 1 week. MRA without MTC was performed under the same scanning conditions. Visualization of intrahepatic portal branches with these methods was compared in both phases. Portal vein-liver contrast-to-noise ratios were significantly higher in the MTC+ group in both phases. For third- and fourth-order portal branches, visualization was significantly better in the MTC+ group in both phases. Use of three-dimensional-segmented FLASH shortened acquisition time and facilitated imaging during breath-holding and also reduced whole-body average specific absorption rate values. Visualization of intrahepatic portal vein branches was improved by MTC pulses, and effective imaging time was prolonged.

Comparison of various techniques of iron oxide-enhanced breath-hold MR imaging of hepatocellular carcinoma.

The purpose of our study is to compare qualitatively and quantitatively the abilities of various superparamagnetic iron oxide (SPIO)-enhanced breath-hold magnetic resonance imaging (MRI) techniques to detect hepatocellular carcinoma (HCC). Eight patients with HCCs were imaged. The images were obtained with conventional T2-weighted spin-echo imaging (CSE), half-Fourier single-shot turbo spin-echo (HASTE), single-shot gradient-echo type echo planar imaging (GE-EPI), and single-shot spin-echo type echo planar imaging (SE-EPI) before and after SPIO administration. The liver signal-to-noise ratios (SNRs) in CSE and each EPI sequence were significantly decreased after SPIO administration. GE-EPI had the highest decrease ratio (DR) of liver SNR, followed by SE-EPI (TE=98), SE-EPI (TE=28), CSE, and HASTE in this order. The relative contrasts with GE-EPI and SE-EPI (TE=98) were significantly higher than that with CSE after SPIO administration. On receiver operating characteristic (ROC) analysis, diagnostic accuracy did not differ significantly among the pulse sequences after SPIO administration. GE-EPI and SE-EPI (longer TE) were useful for SPIO-enhanced breath-hold MRI performed to detect HCC.

Subtracted synthetic images in Gd-DTPA enhanced MR.

The evaluation of Gd-DTPA contrast enhancement (CE) in high intensity tissues on T1-weighted magnetic resonance images, such as fat and bone marrow, is quite difficult. In this study, we used subtraction and subtracted color images in an attempt to show more clearly the Gd-DTPA CE. In addition, we also carried out sequential post-Gd-DTPA imaging to assess the changes in enhancement of lesions with time. Twenty patients were studied with these methods and our results are illustrated.

Chondroitin sulfate iron colloid as an MR contrast agent for the hepatic reticuloendothelial system.

OBJECTIVE: Chondroitin sulfate iron colloid (CSI), a therapeutic agent for asiderotic anemia, is widely used in the clinical field, and its safety has been established in Japan. We studied the potential of this agent for MRI of the hepatic reticuloendothelial system. Chondroitin sulfate iron colloid has a paramagnetic trivalent iron ion and is effective in shortening T2 relaxation time because it is taken up rapidly in the hepatic reticuloendothelial system after intravenous injection. MATERIALS AND METHODS: Hepatic MR images were obtained before and 1 h after the intravenous injection of 26.9 mumol Fe/kg of CSI in rats with experimentally induced tumors and of 18-30 mumol Fe/kg in human subjects. RESULTS: The signal intensities of normal hepatic tissue were decreased in all cases, whereas those of tumors were unchanged after CSI injection in these two series. Consequently, good contrast was observed in each pulse sequence. CONCLUSION: Therefore, CSI seems to be clinically useful as a contrast agent for hepatic MRI.

Use of iron colloid-enhanced MRI for study of acute radiation-induced hepatic injury.

We present a case with acute radiation-induced hepatic injury using chondroitin sulfate iron colloid (CSIS)-enhanced MRI. Uptake of CSIC was decreased in the irradiated portion of the liver. CSIC-enhanced MRI is useful for obtaining information on the function of the reticuloendothelial system and demarcates between irradiated and nonirradiated zones.

A segmental hyperintensity area in the liver shown by delayed MR images with Mn-DPDP.

This is a report of a case with metastatic liver tumors in which a segmental hyperintense area was demonstrated by delayed liver MRI with manganese (II) N,N' -dipyridoxylethylenediamine-N,N'-diacetate-5,5'-bis(phosphate) (Mn-DPDP). No signal abnormality except for tumors was distinct either before or 30 min after administration of Mn-DPDP. The region with hyperintensity appeared 24 h after administration.
