[Initial trial of tumor imaging using 123I-BMIPP: evaluation in soft tissue tumors].

To evaluate the usefulness of 123I-15-(p-iodophenyl)-3 (R, S)-methyl-pentadecanoic acid (123I-BMIPP) as a radiopharmaceutical for tumor imaging, six patients with liposarcomas and one patient with malignant fibrous histiocytoma were studied with 123I-BMIPP. 123I-BMIPP accumulated in the myxoid components of liposarcoma, while the well-differentiated components showed no increased uptake of 123I-BMIPP. 123I-BMIPP accumulated in viable parts of the tumor, while there was no uptake in the necrotic parts in one patient with malignant fibrous histiocytoma. This study indicated that 123I-BMIPP may be a special radiopharmaceutical for tumor imaging.

TEL and KIP1 define the smallest region of deletions on 12p13 in hematopoietic malignancies.

Unbalanced translocations as well as interstitial deletions of the short arm of chromosome 12 [del(12p)] are found as recurring chromosomal changes in a broad spectrum of hematopoietic malignancies. These changes result in the hemizygous deletion of genetic material from 12p. We mapped a yeast artificial chromosome containing the TEL gene, a cosmid contig containing part of TEL and a P1 contig containing the KIP1 gene to 12p13. These probes were used for fluorescence in situ hybridization to analyze samples from 47 patients with various hematologic malignancies who had unbalanced translocations (25 patients) leading to loss of 12p or deletions (22 patients) involving 12p13. The patients had acute lymphoblastic leukemia (8 cases), myelodysplastic syndrome (MDS; 11 cases), acute myeloid leukemia (AML; 10 cases), myeloproliferative disorders (4 cases), therapy-related MDS or AML (7 cases), non-Hodgkin's lymphoma (2 cases), and other hematopoietic malignancies (5 cases). All three probes were hemizygously detected in 26 cases and were completely retained in only 9 cases. In 12 cases probes for one of the two genes were deleted, allowing us to map the smallest region of overlap of these deletions to a small genomic region that is bordered on the telomeric side by the TEL gene and on the centromeric side by KIP1. The genomic distance between TEL and KIP1 is estimated to be about 1 to 2 Mbp.

Asymptomatic membranous obstruction of the inferior vena cava forming intrahepatic collateral pathways.

Intrahepatic and/or extrahepatic collateral pathways result from the membranous obstruction of the inferior vena cava. These collaterals are usually insufficient to prevent Budd-Chiari syndrome. We reprot an unusual case of asymptomatic membranous obstruction of the inferior vena cava in which marked intrahepatic collateral pathways were formed. Although the inferior vena cava terminated above the orifice of the right hepatic vein, the middle and left hepatic veins were patent above the membrane, without narrowing. Blood from the inferior vena cava drained into the right atrium via the intrahepatic collaterals between the right and middle hepatic veins without resistance.

Three-dimensional display of the pelvic structure of anorectal malformations based on CT and MR images.

Three-dimensional images of the pelvic structure of patients with anorectal malformation (ARM) were constructed by computer graphics based on radiographic computerized tomography (CT) and magnetic resonance (MR) images. Organ contour data from CT images and raw MR image data were transferred to a personal computer and to a graphic workstation respectively. On MR image processing, organs were extracted semiautomatically by thresholding enclosed areas. After several steps of image processing, three-dimensional anatomy of each anomaly was visualized with emphasis on position and shape of the muscle complex. In control patients without an anomaly, images showed that the rectum is supported by the levator muscle from behind and descends along with the urethra. In the male patient, the anal canal separates from the urethra and penetrates through the middle of the sphincter complex to reach the orifice. In those with low-type anomaly with a fistula opening to the perineum or the vestibule, images showed the fistula descending through the anterior portion of the sphincter complex. Images of those with a rectourethral fistula could show the muscle complex behind the rectum and at the region where the external sphincter should be. In those with cloacal anomalies, anatomical position and the shape of three different viscera were easily recognized, and the muscle complex was shown like that of rectourethral-type anomalies. This study is a new approach to the anomaly to facilitate understanding it and can assist a surgeon in planning a procedure. This kind of application would make it possible for a surgeon to consider the strategy on a display screen before the real surgery.

Accumulation of 123I-IMP in extrahepatic metastases of hepatocellular carcinoma.

Twenty patients with 58 extrahepatic metastases of hepatocellular carcinoma (HCC) (41 bone, 5 lung, 6 peritoneum, 4 adrenal glands, 1 brain, 1 lymph node) underwent 123I-iodoamphetamine (123I-IMP) and gallium-67 (67Ga) scintigraphy. The rate of positivity with 123I-IMP planar imaging was 44% above the level of the diaphragm and 76% below the diaphragm, with an overall sensitivity of 62%. The combination of 123I-IMP scintigraphy and single photon emission tomography enabled positive visualization of an additional six metastatic foci: three in the adrenal glands, one in the peritoneum, one in the cranium and one in a rib. The rate of positivity with 67Ga planar imaging was 75% above the level of the diaphragm and 45% below the diaphragm, with an overall sensitivity of 58%. Our findings suggest that 123I-IMP is effective for detecting extrahepatic metastases of HCC, particularly those below the diaphragm.

A case with dicentric translocation between chromosome 9 and 18: confirmation by fluorescent in situ hybridization on metaphase spread.

A female child with dicentric translocation between chromosome 9 and chromosome 18 presented non-specific minor anomalies with laryngomalacia. Chromosomal analyses were performed by the G-banding method and a fluorescence in situ hybridization (FISH) technique with a specific probe for the centromeric region of chromosome 18 and the painting probe for the chromosomes 9 and 18. Her full karyotype was confirmed as 45,XX,tdic(9;18)(p24;p11). This is the first case of dicentric translocation between chromosomes 9 and 18. The FISH technique is an important tool in chromosome diagnostics.

Dual contrast magnetic resonance imaging with combined use of positive and negative contrast agent in human hepatocellular carcinoma.

Dual contrast magnetic resonance imaging (DCMR) with combined use of a negative contrast medium, chondroitin sulfate iron colloid (CSIC), and a positive contrast medium, Gd-DTPA, was attempted in 20 cases of hepatocellular carcinoma. Spin echo T1 weighted and T2 weighted images (T1WI, T2WI), and T1 weighted images 15 min after intravenous injection of Gd-DTPA (0.1 mmol kg-1) were obtained. Within 1 week, these MR studies were repeated within 1 h of intravenous injection of CSIC (23.6 mumol Fe kg-1) under similar conditions. DCMR and the other five imaging techniques were visually evaluated and compared in terms of tumour detectability, tumour spread and qualification of tumours (depiction of inner structure). DCMR was significantly better than Gd-DTPA enhanced T1WI in tumour detectability, and better than Gd-DTPA enhanced T1WI or CSIC enhanced T1WI in depicting tumour spread. In the qualification of tumours, DCMR was significantly better than all the other five imaging techniques. None of the patients in this study showed adverse reactions or significant changes in biochemistry. DCMR is an imaging technique which is able to utilize the characteristics of these contrast agents collectively, and exhibits advantages in grasping the inner structure of tumours, especially in hepatocellular carcinoma.

Technical note: dynamic analysis of the pharynx during swallowing using Turbo-FLASH magnetic resonance imaging combined with an oral positive contrast agent--a preliminary study.

Serial images of the swallowing motions of 10 healthy volunteers were taken using the Turbo-fast low angle shot (FLASH) magnetic resonance (MR) imaging sequence, while the subject swallowed an oral contrast agent containing ferric ammonium citrate. In all cases, the flow of the contrast agent and the motion of the organs in the oral cavity and the pharynx during swallowing could be observed. High-speed MR imaging combined with an oral positive contrast agent has the potential to evaluate accurately swallowing motion.

[Clinical value of rapid clearance in resting sestamibi cardiac SPECT in patients with acute myocardial infarction].

Resting 99mTc sestamibi (MIBI) SPECT and exercise-reinjection thallium-201 (T1) SPECT were performed in fourteen patients with acute myocardial infarction (AMI). MIBI SPECT were obtained 90 min (MIBI-90) and 300 min (MIBI-300) after injection of 370 MBq of MIBI at rest. MIBI-90 and MIBI-300 were compared with exercise T1 imaging (T1-EX) and T1 reinjection imaging (REINJ). Each SPECT image was divided into 22 segments and myocardial uptake was scored visually. Abnormal perfusion defects were observed in 94 myocardial regions. Worsening of the score was observed in 79 segments (84%) on MIBI-300 compared with MIBI-90. Total MIBI-300 uptake score per person was significantly greater than that at MIBI-90 (14.8 +/- 8.6 vs. 7.7 +/- 7.9, p = 0.001). The concordance rate of defect score between MIBI-90 and REINJ was significantly higher than that between MIBI-300 and REINJ (55% vs. 17%, p = 0.001). In nine patients without recanalization of an infarct-related artery, perfusion defects were seen in 74 segments. The concordance rate of defect scores between MIBI-300 and T1-EX was significantly higher than that between MIBI-90 and T1-EX (45% vs. 16%, p = 0.001). In conclusion, rapid clearance of MIBI was observed frequently in patients with AMI. MIBI-90 and MIBI-300 may reflect myocardial viability and areas at risk for AMI, respectively.

Correlation between chondroitin sulfate iron colloid-enhanced MR imaging and the histological grade of hepatocellular carcinoma.

The association between contrast enhancement by chondroitin sulfate iron colloid (CSIC) and the histological grade of hepatocellular carcinoma (HCC) was evaluated in 24 patients diagnosed by histological examination of surgical specimens (26 nodules: 11 well-differentiated and 15 poorly-moderately-differentiated nodules). In the well-differentiated HCC nodules, the tumor-liver contrast to noise ratio (CNR) was not significantly increased after i.v. CSIC injection on both T1-weighted and T2-weighted images. In the moderately-poorly-differentiated HCC, CNR was significantly increased after CSIC administration on both T1-weighted and T2-weighted images (p < 0.01). MR imaging using CSIC may be useful for diagnosing the degree of HCC differentiation.

[Preliminary report on the usefulness of 123I-IMP scintigraphy in distant metastases from hepatocellular carcinoma: except for bone and lung metastases].

N-isopropyl-p-[123I] Iodoamphetamine (IMP) scintigraphy was performed in 10 lesions of metastatic hepatocellular carcinoma. Five of 10 lesions (50%) were detected with planar images, which was superior to the 2 of 8 lesions (25%) detected with 67Ga-citrate (Ga) scintigraphy. The positive ratio was increased to 83% (5/6) with SPECT. Thus our findings suggested that IMP could compensate for Ga scintigraphy in detecting distant metastases of hepatocellular carcinoma.

The potential use of I-123 IMP scintigraphy for pelvic bone metastases in hepatocellular carcinoma. A comparison with Ga-67 scintigraphy.

Conventional bone scintigraphy is often inconclusive in detecting bone metastases of hepatocellular carcinoma, suggesting the necessity of alternative scintigraphic agents. In this study, the diagnostic value of I-123 IMP scintigraphy was evaluated in 8 patients with 12 pelvic bone metastases from hepatocellular carcinoma, then compared with Ga-67 scintigraphy. Eleven of 12 lesions (91.7%) were detected by I-123 IMP scintigraphy; Ga-67 scintigraphy visualized 8 lesions (66.7%) in the same patient population. I-123 IMP scintigraphy is useful for detecting pelvic bone metastases of hepatocellular carcinoma with less physiologic accumulation in the intestine as compared with Ga-67 scintigraphy.

A method for simultaneous detection of fluorescent G-bands and in situ hybridization signals.

Improved techniques for detecting in situ hybridization signals on fluorescent G-bands are described. Prior to hybridization of biotinylated DNA probes, chromosomes with replication G-bands were treated with formaldehyde and denatured with an alkaline solution. These steps preserve chromosome morphology well, and enable us to simultaneously demonstrate fluorescein hybridization signals and G-banded chromosomes stained with propidium iodide. This method is useful for precise chromosomal localization of DNA markers.

Single breath-holding three-dimensional magnetic resonance portography with bolus injection of Gd-DTPA in subjects with normal liver: a comparison with two-dimensional time-of-flight technique.

Two-dimensional time-of-flight (2D-TOF) magnetic resonance (MR) portography tends to produce general image degradation owing to the phase discrepancy caused by multiple breath-holding. To solve this problem, three-dimensional (3D)-MR portographies were reconstructed from image data acquired with a bolus injection of Gd-DTPA with single breath-holding in 20 subjects with normal liver function. 2D-TOF MR portographies were obtained beforehand for the comparison. All the images were displayed in a cine mode for the two MR angiographic techniques. Visualization was assessed as positive when the vessel was visualized without disruption. 3D-MR portography produced a positive rate of over 80% for all but the left first-order branch. With the 2D-TOF techniques, however, only the portal trunk and left second-order branches were visualized at this rate. The positive rate for most portal branches was significantly higher with 3D-MR portography than with the 2D-TOF technique. In conclusion, 3D-MR portography makes it possible to depict the portal venous system without disruption using a short measurement time with single breath-holding and has the potential to become a powerful method for abdominal MR angiography.

Chondroitin sulfate iron colloid-enhanced MR imaging in patients with hepatocellular carcinoma. Comparison with CT during arterial portography.

Chondroitin sulfate iron colloid (CSIC) was used as an MR contrast agent for the detection of hepatocellular carcinoma (HCC). The findings of 25 surgically confirmed HCCs in 19 patients were retrospectively analyzed. T1-, T2- and proton density-weighted spin echo MR images were obtained before and after i.v. injection of 23.6 microM Fe/kg of CSIC. Unenhanced and CSIC-enhanced MR images and images obtained by CT during arterial portography (CT-AP) were correlated with surgical pathology findings. The sensitivities of CSIC-enhanced and unenhanced MR imaging, and CT-AP were 92%, 80%, and 88%, respectively. No significant differences were noted. Portal flow abnormalities demonstrated by CT-AP did not affect the detection of HCC by CSIC-enhanced MR imaging. CSIC-enhancement at MR imaging was a disadvantage in the detection of lesions less than 1 cm in diameter. CSIC-enhanced MR imaging is a supplemental method for the detection of HCC.
